ENEL-MED GIFT CARD – Terms and Conditions

1) ISSUER OF THE GIFT CARD
The Issuer of the ENEL-MED GIFT CARD (hereinafter referred to as the “Card”) is Centrum
Medyczne ENEL-MED S.A., with its registered office in Warsaw, Słomińskiego 19/524, 00-195
Warszawa, entered into the National Court Register maintained by the District Court for the capital
city of Warsaw in Warsaw, 12th Business Department of the National Court Register under number
KRS 0000275255, share capital (paid-up) of PLN 23,566,900.00, Tax Identification Number (NIP):
5242593360.
2) SALE OF ENEL-MED GIFT CARDS
Cards can be purchased at Centrum Medyczne ENEL-MED S.A. own centres listed in point
5(k) of these Terms and Conditions, through the Sales Department of Centrum Medyczne
ENEL-MED S.A. or by placing the order on the website at sklep.enel.pl.
3) PERSONS ENTITLED TO USE ENEL-MED GIFT CARDS
The person entitled to use a Card is the person who received the Card and presents it at the
reception desk before scheduling the appointment.
After the Entitled Person starts using the services covered by the Card, the Card cannot be
transferred to third persons. The Card can be used only by one Entitled Person.
4) TERMS OF USE OF ENEL-MED GIFT CARDS
a) In order to use medical services based on the Card, the Entitled Person has to present the Card
before using each service covered by the Card.
b) The Card entitles to use medical services included in one of the packages referred to in point 4(j)
of Terms and Conditions as set out herein.
c) As a part of the package that the Entitled Person uses based on the Card, the Entitled Person can
use every medical service included in the package only once.
d) The User shall not be entitled to any claims against the Issuer due to loss, destruction or theft of
the Card. In the aforementioned instances, the Issuer shall not issue a copy of the Card.
e) The Card cannot be returned after it is purchased.
f)
If the User decides not to use one or several services covered by the Card, the User shall not be
entitled to request any reimbursement.
g) The services covered by the Card do not include consultations with professors of medicine and
assistant professors of medicine.

h) The “Healthy Child” and “Five Consultations” Cards are also available to children (0-18 years of
age). In this case, the services can be carried out by paediatric specialists. Other Cards are
intended for adults (above 18 years of age).
i)
In order to use the medical services covered by the Card, it is necessary to previously schedule an
appointment directly at a Centrum Medyczne ENEL-MED S.A. centre or by calling the national
Call Centre at 22 23 07 007.

Scope of Card packages:
I.
The “Preventive Care” package includes: ultrasound of the abdominal cavity, complete blood
count, general urine examination, TSH, glucose in serum, creatinine, lipid profile,
transaminases (ALT), and consultation with a specialist in internal medicine.
II. The “Healthy Heart” package includes: resting ECG, lipid profile, ultrasound, echocardiogram,
and a cardiologist consultation.
III. The “Five Consultations” package includes: any 5 specialist consultations, including individual
psychological consultations. (do not include dental consultations).
IV. The “Healthy Woman” package is dedicated to women up to 45 years of age and it covers
breast ultrasound, transvaginal ultrasound, and a gynaecological consultation with
(vaginal) gynaecological cytology.
The services covered by the Card should be used after a menstrual period between the
5th and the 9th day of the cycle, unless the doctor recommends otherwise.
V. The “Healthy Man” package includes: testicular ultrasound (or prostate ultrasound), automated
complete blood count, general urine examination, TSH, glucose in serum, creatinine in
serum, lipid profile, transaminases (ALT and AST), total PSA, uric acid in serum, and a
consultation with a specialist in internal medicine.
VI. The “Healthy Spine” package covers 5 spine massages lasting 30 minutes each.
VII. The “FIT” package covers a consultation with a dietician and includes preparation of an
individual diet for 14 days.
VIII.
The “Healthy Child” package includes: three paediatric or paediatric specialist
consultations and a package of 15% discounts on laboratory examinations (the discounts
apply during the term of use of the Card).
IX. The “Beautiful Smile” package includes dental check-up performed by a restorative dentist and
oral hygiene procedures, including scaling, sandblasting and fluoridation performed by a
qualified dental hygienist.
X. The whiteCARD package includes a consultation with a dentist and teeth whitening using the
Opalescence – Boost in-office method.

k) Medical services covered by the Card can be used only at the centres listed below, provided that
the particular service is available at the particular centre.
Medical services included in the “Beautiful Smile” package can be used only at Centrum
Medyczne ENEL-MED S.A. centres that offer dental services. Medical services included in
the “whiteCARD” package are available only at the Atrium Centre in Warsaw, Al. Jana
Pawła II 29.
l)
Selecting the card for Warsaw or “other cities” determines the place of using the services based
on the card – cards, in the case of which the “other cities” option was selected, will be used at a
selected centre outside Warsaw.

6. VALIDITY OF THE ENEL-MED GIFT CARD
a) Services included in the package subject to the particular GIFT CARD should be used within one
year of the date of purchase of the Card. The only exception is the whiteCARD that should be used
within 6 months of the date of purchase of the Card.
b) If the Card expires, the entitled person has no right to demand reimbursement of the amount paid to
the Issuer of the Card.
7. FINAL PROVISIONS
a) Terms and Conditions are published on the website at www.enel.pl. Terms and Conditions are
binding since the day of their publication.
b) Any discounts related to the Card cannot be combined with other promotions and discounts
granted by Centrum Medyczne ENEL-MED S.A.
c) Medical services covered by the Card are provided without a referral.

d) Any complaints concerning GIFT CARDS can be sent to the address [email protected].
Complaints will be examined within 14 working days.
