A PROCEDURE FOR ENROLLMENT OF INTERNATIONAL STUDENTS TO THE I CYCLE STUDIES taught in English (or other foreign languages) at the UNIVERSITY of LODZ (starting from the academic year 2010/2011) (supported by the original documents) 1. The candidate downloads the application form from the International Student’s Office (ISO) website. 2. The candidate legalises his/her school leaving diploma and obtains confirmation of the right to apply for any type of higher education programme in the country where the candidate was awarded the school leaving diploma. In order to do so the candidate should contact the competent Polish authorities located in the country where the candidate has completed his/her secondary education. NOTICE: In order to recognise the above mentioned documents as equal to the Polish final exam (matura) undertaking the nostrification process will be necessary (details regarding the nostrification process are described in paragraphs 12. and 15. of this procedure). NOTICE: Should the country where the school leaving diploma was issued be a signatory of Convention of 5 October 1961 Abolishing the Requirement of Legalisation for Foreign Public Documents, then the legalization may be replaced by an apostille attached to the original document. The complete list of signatories of the Convention may be found at www.hcch.net/index_en.php. 3. Apart from what has been mentioned above the candidate has to collect the remaining documents. The complete list of documents (originals) that the candidate is obliged to send to the ISO is as follows: a. application form (appendix 1) that is legibly filled in using capital letters and signed – original; b. legalised school leaving diploma – original; c. confirmation of the right to apply for any type of higher education programme in the country where the candidate was awarded the school leaving diploma – original; d. copy of the passport page containing the candidates photo; e. three passport photographs; f. signed cover letter (detailed information regarding the cover letter can be found in appendix 2) – original; Opracowanie: dr Maciej Turała (Wydział Zarządzania UŁ); Karolina Korytkowska-Ogrodowczyk; Dominika Bursa (ISO BWZ UŁ) g. certificate confirming the candidate's proficiency in the language of instruction; this could be e.g. Certificate in Advanced English (CAE), Certificate of Proficiency in English (CPE), International English Language Testing System (IELTS), and Test of English as a Foreign Language (TOEFL) (or any other document that can certify whether a candidate’s high school instruction was conducted in this particular language) – original; h. confirmation that a non-returnable application fee of 200 EUR has been paid to the University of Lodz account; i. statutory obligation where the candidate commits to complete the nostrification process no later than until the end of the first semester of studies, provided that the candidate has been successfully enrolled (appendix 3) – original; j. completed and signed accommodation form (appendix 9). 4. If necessary the ISO asks the candidate to arrange for a legal translation of the legalised school leaving diploma to Polish (or, if that would not be possible, to English) and to send it to ISO. 5. A set of documents defined in paragraph 3 together with a translated copy described in paragraph 4 is then sent by ISO to the IO at the appropriate Faculty. 6. IO in compliance with the recruitment procedures and requirements for the particular academic year, makes a decision on whether the total score requirements have been fulfilled by the candidate. The IO then prepares a recommendation to the Dean in charge of international cooperation. 7. The Dean in charge of international cooperation inserts a decision onto the application form of the candidate. 8. In the event of negative decision made by the Dean, the IO sends the set of candidate's documents to ISO which, in turn, returns them to the unsuccessful candidate. At this stage, the enrolment procedure comes to an end (note: the application fee of 200 EUR is not returned). 9. In the event of positive decision made by the Dean: a. the IO sends the set of candidate's documents to ISO where the Rector makes the final decision about the candidate’s enrolment; b. ISO informs the candidate about the Rector’s decision and asks the candidate to pay for the first year of studies to the University account. 10. The candidate sends a confirmation of payment to ISO which verifies whether the payment was done. Upon the candidate’s request ISO may prepare and send to the candidate a document confirming his/her acceptance to the chosen study programme at University of Lodz. Upon arrival to Lodz the candidate should report to the ISO in order to provide additional documents: a. health certificate stating that there are no medical objections for undertaking studies in the chosen field; b. certificate of health insurance (foreigners may be enrolled for study programmes only if they hold an appropriate health and accidents insurance valid for the duration of their stay in Poland); c. proof of payment for a Student ID and an Index; d. copy of a visa, residence card or any other document confirming the right to reside in Poland; e. statement where the candidate provides his/her correspondence address and other contact details, such as phone number or email together with a formal obligation to update this information in case of any changes. 12. At the same time the candidate is obliged to start the nostrification procedure for the school leaving diploma. In order to do so the candidate needsto fill a statement where he/she acknowledges the receipt of the above mentioned document from ISO. ISO makes a copy of this document and certifies it as a true copy. The original document is issued to the student. The certified copy is attached to the personal student's records. 13. ISO sends the complete set of documents (including documents outlined in paragraphs 3., 4., 11. and 12.) to the International Office at an appropriate faculty. The IO verifies whether the documents are complete and passes them on to the Dean's Office. 14. The candidate reports to the Dean's Office where he/she receives an index and a student ID card. 15. Once the nostrification procedure is complete, the student is obliged to report to the International Office at the appropriate faculty with the original school leaving diploma and a nostrification certificate. The IO passes these documents on to the Dean’s Office. In the event of the negative feedback on the nostrification in the legally specified term (e.g. the end of the first semester of studies) the candidate is removed from the student list. In such a case the tuition fee for the I year of studies is not returned. ISO informs the candidate about the Rector’s decision and asks the candidate to pay for the first year of studies to the University account. 10. The candidate sends a confirmation of payment to ISO which verifies whether the payment was done. Upon the candidate’s request ISO may prepare and send to the candidate a document confirming his/her conditional acceptance to the chosen study programme at University of Lodz (INVITING LETTER). Such a document may be helpful when applying for a Polish visa. 11. Upon arrival to Lodz the candidate should report to the ISO in order to provide additional documents: a. secondary school leaving certificate; b. confirmation of the right to apply for any type of higher education programme in the country where the candidate was awarded the school leaving diploma; c. certificate confirming the candidate's proficiency in the language of instruction; Additionally the candidate needs to provide additional documents: d. health certificate stating that there are no medical objections for undertaking studies in the chosen field; e. certificate of health insurance (foreigners may be enrolled for study programmes only if they hold an appropriate health and accidents insurance valid for the duration of their stay in Poland); f. proof of payment for a Student ID and an Index; g. copy of a visa, residence card or any other document confirming the right to reside in Poland; h. statement where the candidate provides his/her correspondence address and other contact details, such as phone number or email together with a formal obligation to update this information in case of any changes. 12. ISO verifies the documents listed in paragraph 11, points from a) to c). In case the documents provided by the candidate are in line with previously submitted copies or scans the Rector makes the final decision about the candidate’s enrolment. 13. The candidate is obliged to start the nostrification procedure for the school leaving diploma. In order to do so the candidate needsto fill a statement where he/she acknowledges the receipt of the above mentioned document from ISO. The original document is issued to the student. The certified copy is attached to the personal student's records. 14. The IO verifies whether the documents are complete and passes them on to the Dean's Office. 15. The candidate reports to the Dean's Office where he/she receives an index and a student ID card. 16. Once the nostrification procedure is complete, the student is obliged to report to the International Office at the appropriate faculty with the original school leaving diploma and a nostrification certificate. The IO passes these documents on to the Dean’s Office. The cover letter should: help you to clarify your own ideas on what you want to do and why over your period of studies; provide us with some knowledge on your interests, background and future plans; outline why you decided to choose this particular study programme. We highly recommend including the following information: 1. general information about an education history and any additional qualifications of the candidate; 2. information on language skills / competences; 3. employment history in brief; 4. additional information regarding the candidate's experience; 5. the name and address of at least one referee. Please be aware of the above mentioned points while constructing your letter. Załącznik 2 LIST MOTYWACYJNY List motywacyjny jest ważnym elementem składającym się na procedurę kwalifikacyjną na studia na Uniwersytecie Łódzkim. Od kandydatów na studia oczekujemy: 1. określenia własnych celów i ambicji oraz oczekiwań dotyczących studiów; 2. ukazania swoich zainteresowań i planów na przyszłość; 3. określenia przyczyn, które zdecydowały o wyborze uczelni oraz programu, na który aplikują.
