Celem eksperymentu jest próba oceny ewentualnych różnic w zakresie ruchomości w stawie ramiennym w zależności od fazy oddychania. W proces oddychania zaangażowane są mięśnie, które równocześnie biorą udział w ruchach stawów obwodowych. Wykazanie związku zakresu ruchu w stawie ramiennym z fazą oddychania może zasadniczo wpływać na skuteczność zabiegów fizjoterapeutycznych lub osiągnięcia zawodników sportowych. Materiał i metody. Osoby objęte eksperymentem zostały poddane badaniom, które polegały na ocenie zakresu ruchu rotacji wewnętrznej w stawie ramiennym w kończynie górnej prawej i lewej. Dla każdej z osób badanie przeprowadzono trzykrotnie w następujacej kolejności: pomiar w czasie spoczynku, pomiar na szczycie maksymalnego wdechu, pomiar na bezdechu (po wykonaniu maksymalnego wydechu przez badaną osobę). Wyniki. Przeprowadzona analiza wykazała istotne statystycznie różnice pomiędzy pomiarami dokonanymi w trakcie spoczynku i na szczycie maksymalnego wdechu. Wyniki zarejestrowane w badaniu na szczycie maksymalnego wdechu osiągnęły większą wartość, niż odnotowane w trakcie pomiaru w spoczynku, zarówno w pomiarach dokonanych na kończynie górnej prawej, jak i lewej. Wnioski. Na podstawie otrzymanych wyników można przypuszczać, że istnieje zależność pomiędzy zakresem ruchu rotacji wewnętrznej w stawie ramiennym a fazą cyklu oddechowego. The aim of the experiment is an attempt to assess possible differences in the range of movements in the glenohumeral joint depending on the respiratory phase. The respiratory process involves the activity of the muscles which at the same time participate in movements of the peripheral joints. Showing the relationship between the range of movement and the respiratory phase can significantly affect the effectiveness of physiotherapeutic procedures or results in sport. Material and methods. The participants were subjected to the examination involving assessment of the range of internal rotation in the glenohumeral joint of the right and left upper limb. For each subject, the examination was carried out thrice in the following sequence: measurement at rest, measurement during peak inspiration and measurement after peak expiration. Results. The analysis showed statistically significant differences between the results of the measurements performed at rest and during peak inspiration.The values recorded during peak inspiration were higher than those noted during the measurement at rest, both of the right and the left upper limb. Conclusions. The obtained results suggest dependence between the range of movement of internal rotation in the humeral joint and the respiratory cycle phase. Diagnostyka stawu ramiennego Wstęp Background Ocena zakresu ruchu jest jednym z podstawowych badań przedmiotowych. Uzyskany wynik wpływa na planowanie i ocenę skuteczności ćwiczeń w trakcie treningu w przypadku np. elongacji przykurczonych mięśni czy oceny skuteczności zabiegów fizjoterapeutycznych np. po endoplastyce stawu. W metodyce pomiaru zakresu ruchu dokładnie opisane są pozycje wyjściowe do badania, miejsce przyłożenia osi goniometru czy płaszczyzna ruchu. Określone są normy zarówno dla ruchów biernych, jak i czynnych [1]. Dotychczas nie zwracano uwagi na fakt, iż wynik pomiaru zakresu ruchu może być zależny od tego, w której fazie cyklu oddechowego dokonujemy pomiaru. W proces oddychania zaangażowane są mięśnie, które biorą także udział w ruchach stawów obwodowych i stabilizacji kręgosłupa [2,3]. Fakt ten może wpływać na zmianę zakresu ruchu w poszczególnych stawach w zależności od fazy oddychania. Wykazanie zależności pomiędzy fazą oddychania a zakresem ruchu może mieć znaczenie zarówno w parcy trenera, jak i fizjoterapeuty. Celem eksperymentu jest próba oceny ewentualnych różnic w zakresie ruchomości w stawie ramiennym w zależności od fazy oddychania. Wykazanie związku zakresu ruchów w stawie ramiennym z fazą oddychania będzie miało znaczenie w sporcie np. w dyscyplinach wytrzymałościowych, gdzie wraz ze wzrostem zmęczenia może dochodzić do spłycania oddechu, co w konsekwencji może prowadzić do ograniczania rotacji wewnętrznej w stawie ramiennym. Natomiast każde ograniczenie musi sprowokować organizm do kompensacji zaburzonej funkcji, zwiększając w ten sposób ryzyko wystąpienia kontuzji. Assessment of the range of movement is one of the basic medical examinations. The obtained result affects the planning and assessment of the effectiveness of exercises during training, e.g. in case of muscle elongation after contracture and assessment of the effectiveness of physiotherapeutic procedures, e.g. after joint endoplasty. Methodology of measuring the range of movement involves description of baseline positions for examination, the body area where the goniometer axis is placed and the plane of motion. Norms are defined, both for passive and active movements [1]. So far, no attention has been paid to the fact that the result of the measurement of the range of movement can depend on the “respiratory cycle” phase, in which the measurement is taken. Muscles are involved in the process of respiration, participating in movements of the peripheral joints and spinal stabilization [2,3]. This may result in changes in the range of movement of individual joints, depending on the respiratory phase. The proven dependence between the respiratory phase and the range of movement can be important, both for a trainer and a physiotherapist. The aim of the experiment was an attempt to assess possible differences in mobility range of the glenohumeral joint, depending on the respiratory phase. Showing the relationship between the range of movement in the humeral joint and the respiratory phase is important in sports, e.g. the endurance disciplines which can result in shallow breathing, leading in consequence to limitation of internal rotation in the glenohumeral joint. Each limitation, in turn, provokes the body to compensate the impaired function, increasing the risk of contusion. Materiał i metody Material and methods Badaniami objęto 34 piłkarki ręczne i 9 piłkarzy ręcznych. Wiek badanych zawierał się w przedziale 17-19 lat. W przeprowadzonym eksperymencie uczestniczyły tylko osoby praworęczne, co jednak nie było założeniem metodologicznym badań. Osoby objęte eksperymentem zostały poddane badaniom, które polegały na ocenie biernego zakresu ruchu rotacji wewnętrznej w stawie ramiennym. Pomiarów dokonywano za pomocą goniometra. Osoba badana przyjmowała pozycję leżenia tyłem z wyprostowanymi kończynami dolnymi. W trakcie dokonywania pomiaru zakresu ruchu kończyna górna była odwiedziona do kąta prostego w stawie ramiennym i zgięta do kąta prostego w stawie łokciowym. W pozycji 0°, od której rozpoczynano pomiar, przedramię badanej kończyny ustawione było wertykalnie. Pomiaru zakresu ruchu dokonywano w kończynie górnej prawej i lewej. Dla każdej z osób badanie przeprowadzono trzykrotnie dla kończyn górnej prawej i lewej. Pomiary przebiegały w następujacej kolejności: – pomiar w czasie spoczynku, – pomiar na szczycie maksymalnego wdechu, – pomiar na bezdechu (po wykonaniu maksymalnego wydechu przez badaną osobę). Rezultaty przeprowadzonych badań poddano analizie statystycznej z wykorzystaniem programu STATISTICA wersja 10.0 firmy StatSoft. W celu określenia ewentualnych różnic w zakresie ruchomości The sample comprised 34 female and 9 male handball players. The subjects’ age ranged from 17 to 19 years. Only right-handed individuals participated in the experiment, which was not, however, the methodological concept of the study. The participants were subjected to examinations involving assessment of passive range of internal rotation movement in the glenohumeral joint. The measurements were taken using a goniometer. The subject assumed a supine position with straightened lower extremities. During the measurement, the upper limb was abducted to the straight angle in the humeral joint and flexed to the straight angle in the elbow joint. The measurement was started in 0° position with the forearm of the examined limb in vertical extension. The measurement of the range of movement was taken in the right and the left lower limb. For each subject the measurement was taken thrice for the right and the left upper limb. The sequence of the measurements was as follows: • measurement at rest, • measurement at peak inspiration, • measurement after peak expiration. The results of the study were statistically analysed using StatSoft STATISTICA 10.0 program. Przyjęto p=0,05 jako graniczny poziom istotności dla wszystkich zastosowanych testów. the respiratory phase, variance analysis for repeated measurements was carried out. The borderline significance level was accepted at p=0.05. Wyniki Results W Tabelach 1 i 2 przedstawiono wyniki badań biernego zakresu ruchu rotacji wewnętrznej stawu ramiennego zarejestrowane w kończynie górnej prawej i lewej wśród kobiet i mężczyzn w trakcie pomiarów dokonywanych w czasie spoczynku, na szczycie maksymalnego wdechu i na bezdechu (po wykonaniu maksymalnego wydechu przez badaną osobę). Przeprowadzona analiza wariancji wykazała istotne statystycznie różnice pomiędzy pomiarami dokonanymi w trakcie spoczynku i na szczycie maksymalnego wdechu (p=0,01). Wyniki zarejestrowane w badaniu na szczycie maksymalnego wdechu osiągnęły większą wartość, niż odnotowane w trakcie pomiaru w spoczynku (Ryc. 1). Tables 1 and 2 present the results of the measurements of the range of passive internal rotation movement in the glenohumeral joint of the right and left upper limbs, at rest, during peak inspiration and after peak expiration. The variance analysis showed statistically significant differences between the results of the measurements taken at rest and during peak inspiration (p=0.01). The results obtained from the measurement at peak inspiration reached higher values than those recorded at rest (Figure. 1). Tab. 1. [4] oceniali rozbieżności w pomiarach zakresu rotacji wewnętrznej i zewnętrznej stawu ramiennego. W swoich badaniach wykazali różnice rejestrowanych wyników wynikające z dodatkowej stabilizacji łopatki w porównaniu z grupą bez żadnej stabilizacji. W dostępnej literaturze można znaleźć doniesienia na temat rozbieżności w pomiarach zakresu rotacji w stawie ramiennym pomiędzy kończyną górną dominującą a niedominującą [5]. Inni autorzy zwracają uwagę na fakt, iż mimo podobnych wartości w pomiarach całkowitego zakresu ruchu rotacji w obu stawach ramiennych, różnice dotyczące zakresu rotacji wewnętrznej i zewnętrznej znacznie różnią się pomiędzy kończyną górną prawą i lewą [6]. Wynik pomiaru całkowitego zakresu ruchu może być także zależny od wieku badanej osoby [7]. Dotychczasowe badania pomiaru zakresu ruchomości rotacji kości ramiennej uwzględniały także wpływ pozycji klatki piersiowej, ustawienia odcinka piersiowego kręgosłupa czy pozycję łopatki, a także jej kinematykę [8-10]. Poszukiwano również różnic w pomiarach rotacji między zawodnikami a osobami nietrenującymi [11,12]. Brak jednak badań uwzględniających ewentualny wpływ fazy oddychania na wynik pomiaru zakresu ruchomości w stawie ramiennym. W przeprowadzonym eksperymencie analiza wyników zarejestrowanych w badaniu biernego zakresu ruchu rotacji wewnętrznej stawu ramiennego wykazała istotne różnice w pomiarach ze względu na fazę oddychania. Asymetryczna budowa ciała ludzkiego, która zauważalna jest już około 6 tygodnia życia [13], determinuje w konsekwencji typowy wzorzec posturalnej asymetrii [14-17]. Strukturalno-nawykowa orientacja tułowia (mostka i kręgosłupa) do strony prawej, na osi skośnej od lewego barku do prawego uda (serce – żołądek – wątroba) powoduje ustawienie żeber po prawej przedniej stronie w rotacji wewnętrznej, zaś po lewej przedniej w rotacji zewnętrznej [18]. Aby zbalansować rotacyjne ustawienie żeber do strony lewej, żebra 1-sze i 2-gie po stronie prawej unoszą się ku górze do rotacji zewnętrznej [19]. Powyższa pozycja tułowia i żeber wywołuje uniesienie panewki prawej łopatki do góry wokół osi strzałkowej, jak i ustawienie łopatki w rotacji wewnętrznej wokół osi pionowej. Podczas badania zakresu ruchu rotacji wewnętrznej w stawie ramiennym – ramię odwiedzione do kąta 90° – pozycja stawu ramiennego może wywołać Ellenbacker et al. [4] assessed the discrepancies in the measurements of internal and external rotation in the humeral joint. In their study, they showed differences in the recorded results which were due to additional stabilizing of the scapula, compared with the results obtained from the group which did not use any stabilization. In the available literature there are reports on the discrepancies in the results of the measurements of rotation range in the glenohumeral joint, between the dominant and the non-dominant limb [5]. Other authors emphasize the fact that, despite similar values obtained from the measurements of the total rotation movement range in both humeral joints, the differences in internal and external rotation range between the right and the left upper limb are significant [6]. The result of the measurement of the total range of movement may also depend on the subject’s age [7]. The performed to date measurements of the range of rotation movement in the glenohumeral humeral joint, also considered the effect alignment of the thorax, thoracic spine or the scapula as well as kinematics of the humerus [8-10]. Besides, differences were sought in the results of rotation range measurements between competitive athletes and nontraining individuals [11,12]. However, no study has dealt with a possible effect of respiratory phase on the result of measurement of the mobility range in the glenohumeral joint. In the reported experiment, analysis of the results obtained from the measurements of passive internal rotation range showed significant differences, depending on the respiratory phase. The asymmetric build of the human body, which can be noticed as early as around the sixth week of life [13], determines in consequence a typical pattern of postural asymmetry [14-17]. Structural and habitual orientation of the trunk (the sternum and the spine) to the right side on the oblique axis from the left shoulder to the right thigh (heart-stomach-liver) results in the alignment of ribs on the right front side in internal rotation and on the left front side in external rotation [18]. In order to balance the rotational alignment of the ribs from the left side, the first and second ribs on the right side are lifted in external rotation [19]. This position of the trunk and ribs results in lifting of the right scapular acetabulum around the sagittal axis and in scapular alignment in internal rotation around the vertical axis. 16 112 niedzielski1:Layout 1 2014-04-27 20:16 Strona 5 Niedzielski M. et al. Po stronie lewej, orientacja tułowia i żeber ogranicza bardziej odwiedzenie horyzontalne niż rotację wewnętrzną. Panewka lewej łopatki jest w odwrotnej pozycji niż po stronie prawej, a prawe zorientowanie mostka zwiększa napięcie lewego m. piersiowego większego i ogranicza odwiedzenie horyzontalne lewej głowy kości ramiennej na panewce. W przeprowadzonym eksperymencie wyniki zarejestrowane w badaniu na szczycie maksymalnego wdechu osiągnęły większą wartość, niż odnotowane w trakcie pomiaru w spoczynku. W wyniku maksymalnego wdechu, uniesienie żeber i zwiększona ich wypukłość stabilizuje wklęsłą łopatkę, dopasowując ją lepiej do tylnego śródpiersia. Zmiana ułożenia panewki wywołuje zmniejszenie „zaryglowania” stawu ramiennego prawego, zwiększając jego bierny ruch rotacji wewnętrznej. Dokładne omówienie zagadnienia zakresu ruchu w stawie ramiennym, z wyszczególnieniem rotacji wewnętrznej w związku z fazami oddychania, wymaga dalszych badań klinicznych, których dotychczasowa literatura nie objęła. During the measurement of internal rotation range in the glenohumeral joint with the arm in abduction to 90°, the position of the glenohumeral joint may result in “locking” of the articular surfaces in the posterosuperior portion of the joint. At the left side, orientation of the trunk and ribs limits rather the horizontal abduction than internal rotation. The acetabulum of the left scapula is inversely aligned compared with the right side and the right-sided orientation of the sternum increases the tension of the left pectoralis major muscle and limits horizontal abduction of the left humeral head on the acetabulum. In the reported experiment, the values obtained of the measurement at peak inspiration were higher than those noted during the measurement at rest. At peak expiration, lifting and the increased convexity of the ribs stabilize the concave scapula, resulting in its better adjustment to posterior mediastinum. The change in the acetabular alignment causes reduction of “locking” of the right glenohumeral joint, increasing its passive internal rotation movement. A detailed description of the movements in the glenohumeral joint, especially internal rotation depending on respiratory phases, requires further clinical studies which have not been reported so far in the literature on this topic. Wnioski Conclusions 1. Pomiar biernego zakresu ruchu rotacji wewnętrznej w stawie ramiennym dokonywany na szycie maksymalnego wdechu wpływa na zwiększenie osiąganych wartości w stosunku do pomiaru wykonywanego w spoczynku. 2. Na podstawie otrzymanych wyników można przypuszczać, że istnieje zależność pomiędzy biernym zakresem ruchu rotacji wewnętrznej w stawie ramiennym a fazą cyklu oddechowego. 1.
