Kaczmarek Streszczenie Wprowadzenie: u chorych z cukrzycą, zwłaszcza niewyrównaną, oprócz wielu patologicznych zmian ogólnoustrojowych, występują zmiany w jamie ustnej spowodowane podwyższonym poziomem glukozy w krwi. Zmniejszenie ilości wydzielanej śliny, a także wzrost stężenia glukozy we krwi i ślinie, sprzyja wzrostowi liczby patogennych bakterii w jamie ustnej, co może powodować stosunkowo dużą zapadalność na próchnicę pacjentów z cukrzycą, mimo małej kariogenności pożywienia. Cel pracy: porównanie występowania próchnicy zębów u pacjentów z cukrzycą typu 1 i 2 w zależności do poziomu hemoglobiny glikozylowanej i czasu trwania choroby. Materiał i metody: badaniem objęto 156 osób, w tym 34 z cukrzycą typu 1 i 59 z cukrzycą typu 2 oraz 63 zdrowe osoby stanowiące grupę kontrolną. Materiał rozpatrywano w aspekcie typu choroby, poziomu hemoglobiny glikozylowanej oraz czasu trwania choroby. Wyniki: wartość PUW/Z była wyższa u pacjentów z cukrzycą typu 2 w porównaniu do pacjentów z cukrzycą typu 1, jak i osób zdrowych. Podobne zależności zaobserwowano w podgrupach pacjentów z cukrzycą typu 2 niezależnie od wartości hemoglobiny glikozylowanej i czasu trwania choroby. Z kolei u pacjentów z cukrzycą typu 1 we wszystkich badanych podgrupach wskaźnik PUW/Z był niższy niż u osób z cukrzycą typu 2 i u osób zdrowych. Podsumowanie: pomimo niekariogennych nawyków żywieniowych i uregulowanego trybu życia, pacjenci chorzy na cukrzycę są narażeni na rozwój próchnicy, dlatego powinni być objęci szczególnym programem profilaktycznym. is c op y is for pe rso na lu se on ly - dis tr ibu Summary Introduction: Diabetics, especially those with poor glycemic control, have systemic complications as well as local, oral cavity complications of the disease. They are mainly caused by hyperglycemia. Decrease of saliva production and high concentration of glucose in saliva favour growth of pathogenic bacteria in the oral cavity that can lead to relatively high prevalence of caries in diabetics despite low cariogenicity of their diet. Aim of the study: To study the incidence of dental caries in patients with type 1 and 2 diabetes as related to glycated hemoglobin level and the duration of the disease. Material and methods: A total of 156 patients were examined including 34 type 1 diabetics, 59 type 2 diabetics, and 63 healthy volunteers as the control group. The material was analyzed with regard to the type of diabetes, glycated hemoglobin level and the duration of the disease. Results: DMF index was higher in type 2 diabetics as compared with type 1 diabetic patients and healthy individuals. Similar results were observed in subgroups of patients with HbA1c ≤ 8.5% and HbA1c ≥ 8.5% as well as in patients with longer (>10 years) and shorter (<10 years) duration of diabetes. In all analyzed subgroups of patients with type 1 diabetes the DMF index was lower as compared with type 2 diabetics and the control group. Conclusion: Despite noncariogenic dietary habits and a healthy lifestyle patients with diabetes are particularly prone to caries. A special caries prophylaxis programme should cover this group of patients. Absolute insulin deficiency is caused by the impairment of its biosynthesis and excretion of pancreatic islets of Langerhans by beta cells, while relative insulin deficiency is the result of the impairment of the reaction of cell, tissue and peripheral organs to insulin activity. The World Health Organisation has distinguished four basic types of the disease, of which the first two are the most frequent to occur. Type 1 diabetes mellitus generally manifests itself in childhood or in adolescence, and accounts for approximately 10% of all cases of the disease. It is caused by autoimmunological damage to β cells of pancreatic islets with accompanying environmental factors that have not been identified as yet. Type 2 diabetes mellitus on the other hand occurs in individuals older than 30 years of age and is a rare occurrence in children and adolescents. Clinically, it is characterized by hyperglycemia and, in some cases, by the presence of ketoacidosis. Frequently, it is associated with abdominovisceral obesity. In the course of diabetes the following complications present the most serious problems: microangiopathy (diabetic nephropathy and retinopathy), macroangiopathy (atherosclerosis, especially of the heart, the brain and the lower limbs) as well as peripheral neuropathy. The occurrence of complications is, among others, the consequence of lack of control of the disease leading to long periods of hyperglycemia. For that reason, maintaining a longterm optimal metabolic control of the disease allows the reduction of the risk of complications. Natomiast cukrzyca typu 2 występuje u osób powyżej 30 roku życia, zaś rzadko zdarza się u dzieci i młodzieży. Klinicznie charakteryzuje się hiperglikemią oraz niekiedy występowaniem kwasicy ketonowej. Często natomiast jest połączona z otyłością brzuszno-trzewną. W przebiegu cukrzycy największy problem stanowią powikłania w postaci mikroangiopatii (nefropatia i retinopatia cukrzycowa), makroangiopatii (miażdżyca tętnic, szczególnie serca, mózgu i kończyn dolnych) oraz neuropatii obwodowej. Występowanie powikłań jest między innymi następstwem braku wyrównania choroby, prowadzącej do długich okresów hiperglikemii. Dlatego też utrzymanie długotrwałego optymalnego wyrównania metabolicznego choroby umożliwia redukcję zagrożenia wystąpienia powikłań. Pomiar zawartości hemoglobiny glikozylowanej w krwi (HbA1c) jest podstawowym wskaźnikiem wyrównania cukrzycy. Hemoglobina glikozylowana powstaje przez cały czas życia krwinki czerwonej i jej stężenie jest proporcjonalne do czasu trwania hiperglikemii oraz stężenia glukozy w osoczu. Poziom HbA1c odzwierciedla średnie stężenie glukozy w ciągu ostatnich 2-3 miesięcy. Za idealną uważa się wartość poniżej 6,1%, jednak u pacjentów z cukrzycą jest ona trudna do osiągnięcia ze względu na możliwość wystąpienia hipoglikemii. Dlatego należy dążyć do osiągnięcia poziomu w przedziale od 6,5 do 7%. Występuje wprost proporcjonalna zależność pomiędzy poziomem HbA1c a ryzykiem wystąpienia powikłań. Przyjmuje się, że przy wartości HbA1c >6,5% istnieje duże ryzyko rozwoju miażdżycy, a przy wartości HbA1c >7,5% duże ryzyko wystąpienia mikroangiopatii.1-5 U chorych z cukrzycą, zwłaszcza niewyrównaną, oprócz wielu patologicznych zmian ogólnoustrojowych, występują zmiany w jamie ustnej, spowodowane podwyższeniem poziomu glukozy we krwi. Przyjmuje się, że przyczyną powikłań jest hiperglikemia, która powoduje powstawanie zaawansowanych glukozowanych produktów końcowych (ang. advanced glycation end-products – AGEs) gromadzących się w tkankach i plazmie. Zwiększają one wrażliwość komórek śródbłonka i monocytów na bodźce indukujące wytwarzanie mediatorów procesów zapalnych, co np. w tkankach przyzębia objawia się wzrostem przepuszczalności naczyń, rozpadem kolagenu, przyspieszoną destrukcją tkanki łącznej i kostnej. Z wielu badań wynika, że u pacjentów z cukrzycą występują częściej choroby przyzębia o cięższym przebiegu, infekcje oportunistyczne zwłaszcza grzybicze, suchość jamy ustnej i zespół pieczenia jamy ustnej.6-8 Zaobserwowano is c op y is for pe rso na lu the progression of the already present chronic complications. Testing for glycated hemoglobin content in blood (HbA1c) is the basic marker of diabetes control. Glycated hemoglobin is produced during the whole lifespan of the red blood cell, and its concentration is proportional to the duration of hyperglycemia and to the concentration of glucose in blood serum. The level of HbA1c reflects the mean concentration of glucose in the previous 2-3 months. The ideal value is accepted to be below 6.1%; it is, however, difficult to attain in patients with diabetes due to the risk of hyperglycemia. It is, therefore, essential to aim for 6.5 to 7% range. There is a directly proportional correlation between the level of HbA1C and the risk of complications. It is believed that at HbA1c>6.5% there is a high risk of sclerosis, while at HbA1c>7.5% there is a high risk of microangiopathy.1-5 In diabetes, especially when uncontrolled, oral lesions occur due to increased level of glucose in the blood beside a number of systemic pathologies. Hyperglycemia is believed to be the cause of these complications that provokes advanced glycation endproducts (AGEs) to collect in tissues and plasma. They increase the sensitivity of endothelial cells and monocytes to stimuli, which induces the production of mediators of inflammatory processes. In periodontal tissues, for example, this process manifests itself by increased permeability of vessels, collagen disintegration, accelerated destruction of osseous and connective tissue. Many research studies indicate that diabetics more frequently succumb to severe periodontal disease, opportunistic, especially fungal, infections, xerostomia, and stomatopyrosis.6-8 Moreover, the destruction of salivary glands has been reported, which diminishes the flow of saliva leading to lesions within hard and Th This copy is for personal use only - distribution prohibited. Spowodowane jest to prawdopodobnie złożonym związkiem między cukrzycą i próchnicą zębów, zależnym od wielu różnych czynników, m.in. od typu, czasu trwania i wyrównania cukrzycy, stopnia redukcji wydzielania śliny i higieny jamy ustnej, które nie we wszystkich analizach były uwzględniane.11,13 dis tr ibu tio np roh ibit ed . soft oral tissues. Decreased salivary secretion and increased concentration of glucose in blood and saliva favour the growth of oral pathogenic bacteria, in turn resulting in high prevalence of caries in the diabetic population despite low cariogenicity of their diet.9-12 The outcome of research on the influence of diabetes on the development of caries is not explicit. This may be due to the fact that the correlation between caries and diabetes is complex, and depends on many factors such as the type, duration and control of diabetes, the extent of salivary flow reduction and oral hygiene. These factors have not been taken into consideration in every study.11,13 op y is for pe The study involved 156 patients (tab. 1) including 34 patients with type 1 diabetes (group C1), 59 with type 2 diabetes (group C2) and 63 healthy individuals as the control (K0). The patients’ age in group C1 ranged from 21 to 57 years, in C2 from 45 to 79 years, and in K0 from 21 to 79 years. The patients were additionally subdivided according to the type of diabetes and the level of control (subgroups A1, A2, B1, B2) and the duration of the disease (subgroups D1, D2, E1, E2). Każdy pacjent został szczegółowo zbadany stomatologicznie w sztucznym oświetleniu z użyciem lusterka stomatologicznego i sondy SOZ 626. W ocenie uwzględniono stanu uzębienia, liczbę zębów i powierzchni dotkniętych procesem próchnicowym, wypełnionych i usuniętych (PUW/Z, PUW/P). Analizy materiału dokonano w aspekcie typu choroby, poziomu jej wyrównania (HbA1c ≤8,5% i HbA1c >8,5%) oraz czasu trwania choroby (do 10 lat i powyżej 10 lat). Detailed intra-oral examination was performed for each patient in artificial light by means of a dental mirror and a WHO 626 probe. The following were evaluated: dental status, the number of teeth and surfaces affected by the carious process, teeth filled and missing (DMF/T, DMF/S). The material was analysed with regard to the type of the disease, level of control (HbA1c ≤8.5% and HbA1c >8.5%) as well as the duration of the disease (less than 10 years or more than 10 years). Also, the number of teeth missing due to caries was statistically significantly higher in this group of patients than in the other two (p<0.001). Natomiast nie zauważono statystycznie istotnych różnic w wartości wskaźników PUW/Z i jego składowych pomiędzy pacjentami chorującymi na cukrzycę typu 1 i osobami zdrowymi. W grupie chorych na cukrzycę typu 1 wartości obu wskaźników PUW były niższe (różnice były statystycznie istotne względem pacjentów z grupy C2) oraz niższa liczba zębów z próchnicą i zębów usuniętych. Kolejna analiza dotyczyła intensywności próchnicy w zależności od poziomu hemoglobiny glikozylowanej w krwi i typu choroby (tab. 3). U chorych z poziomem HbA1c poniżej 8,5% stwierdzono, że wartości PUW/Z i PUW/P były wyższe u pacjentów z cukrzycą typu 2 (podgrupa A2), a niższe u chorych z cukrzycą typu 1 (podgrupa A1), zaś istotne statystycznie różnice wystąpiły pomiędzy podgrupą A1 i A2 oraz A1 i K1- grupą kontrolną (p<0,05). Pacjenci z cukrzycą typu 2 (podgrupa A2) mieli istotnie wyższą liczbę zębów usuniętych w stosunku do chorych z cukrzycą typu 1 (p<0,001), jak i do podgrupy kontrolnej K1 (p<0,05). There were statistically insignificant differences in DMF/T index value and its parameters between type 1 diabetics and healthy individuals. In type 1 diabetics the values of both DMF indices were lower (the differences were statistically significant as compared with the C2 group of patients). The number of teeth decayed or missing was also lower. Another analysis concerned the intensity of caries as related to the glycated hemoglobin level in blood and the type of the disease (tab. 3). In patients with HbA1c below 8.5% DMF/T and DMF/S indices were higher in patients with type 2 diabetes (subgroup A2), and lower in patients with type 1 diabetes (subgroup A1), with statistically significant differences between A1 and A2 subgroups, and A1 and K1 control (p<0.05). Patients with type 2 diabetes (subgroup A2) had a significantly higher number of teeth missing as compared with type 1 diabetics (p<0.001) or the K1 control subgroup (p<0.05). U pacjentów chorujących na cukrzycę typu 2 zaobserwowano istotnie większą liczbę zębów usuniętych (p<0,05) niż w pozostałych podgrupach. W podgrupie kontrolnej K4 zaobserwowano najwięcej zębów z próchnicą i z wypełnieniami. Istotne statystycznie były różnice w wartościach P/Z między pacjentami z cukrzycą typu 2 (podgrupa E2) a podgrupą kontrolną K4 (p<0,05) oraz W/Z między osobami z cukrzycą typu 1 ( podgrupa E1) i podgrupą kontrolną K4 (p<0,001) Intensywność próchnicy zębów była również analizowana w zależności od stopnia wyrównania oraz od czasu trwania choroby, niezależnie od typu cukrzycy. Wszyscy chorzy zostali podzieleni w zależności od poziomu HbA1C na dwie podgrupy – G1 (HbA1c ≤ 8,5%) i G2 (HbA1c>8,5%). Dokonując oceny wartość PUW/Z i PUW/P w podgrupach G1 i G2 stwierdzono, że były one wyższe w podgrupie G2. W podgrupie G2 zaobserwowano również nieco wyższą liczbę zębów usuniętych i wypełnionych (Fig. 1). Chorych niezależnie od typu cukrzycy podzielono również w zależności od czasu trwania choroby na podgrupy H1 (chorujących krócej niż 10 lat) i H2 (chorujących dłużej niż 10 lat). Stwierdzono, że wartości wskaźników PUW/Z, PUW/P i U/Z były wyższe u pacjentów dłużej chorujących na cukrzycę (podgrupa H2), natomiast w podgrupie osób krócej chorujących stwierdzono wyższą liczbę zębów wypełnionych i z ubytkami próchnicowymi. All subjects were divided into two subgroups according to the HbA1C level: G1 (HbA1c≤8.5%) and G2 (HbA1c>8.5%). While analyzing DMF/T and DMF/S indices values in the G1 and G2 subgroups it was noted that they were higher in the G2 subgroup. The same subgroup also had a slightly higher number of teeth missing or filled (fig. 1). As for the duration of the disease, the patients were subdivided into H1 (shorter than 10 years) and H2 (longer than 10 years) irrespective of the type of diabetes. It was found that DMF/T, DMF/S and D/T indices values were higher in patients who suffered from diabetes for a longer period (H2 subgroup), whereas in the subgroup of patients with shorter history of diabetes the number of decayed and filled teeth was higher. On the one hand, patients with diabetes should have noncariogenic dietary habits, being obliged to eliminate carbohydrates from their diet; they also have to take their food at regular hours and in small quantities. Moreover, they are aware of the risks if they failed to follow their dietary recommendations. On the other hand, however, there are some specific characteristics connected with the progress of diabetes that predispose to caries. Z jednej strony pacjenci z cukrzycą powinni mieć niekariogenne nawyki żywieniowe, gdyż są zobligowani do wyeliminowania ze swojej diety węglowodanów, a także muszą spożywać pokarmy regularnie o ustalonych porach i w małych ilościach. Dodatkowo są oni świadomi zagrożeń jakie płyną z nie stosowania się do zaleceń dietetycznych. U chorych z cukrzycą stwierdzono patologiczne zmiany w obrębie gruczołów ślinowych, powodujące zmniejszone wydzielanie śliny. Ponadto wzrost stężenia glukozy we krwi i ślinie przy równoczesnych zmianach w składzie flory bakteryjnej, może wpływać na przebieg procesu próchnicowego.13-17 Jednakże niektórzy autorzy nie stwierdzili wzrostu częstości próchnicy u pacjentów z cukrzycą, ani też nie zanotowali różnic w intensywności próchnicy między pacjentami z cukrzycą a pacjentami zdrowymi.18-20 Bacic i wsp.21 badając dorosłych pacjentów z cukrzycą typu 1 i 2 nie stwierdzili istotnych statystycznie różnic w wartościach PUW między chorymi i zdrowymi, nie zaobserwowali istotnej różnicy w liczbie zębów wypełnionych, natomiast zanotowali u chorych istotnie wyższą liczbę zębów usuniętych z powodu próchnicy. Porównując chorych w zależności od typu cukrzycy, stwierdzili istotnie większą liczbę zębów usuniętych u pacjentów z cukrzycą typu 2 i istotnie większą liczbę zębów z wypełnieniami u pacjentów z cukrzycą typu 1. Moor i wsp.16 zbadali poziom próchnicy korony i korzenia dorosłych pacjentów z cukrzyca typu 1. Odnotowali istotnie wyższe wartości PUW/P i istotnie wyższą liczbę zębów usuniętych niż u osób zdrowych. Natomiast wartości składowych PW/P i P/P u pacjentów z cukrzycą były nieistotnie statystycznie wyższe. Zaobserwowali także większą zachorowalność na próchnicę korony zęba starszych kobiet z nefropatią cukrzycową, ponadto stwierdzili, że wzrost stężenia glukozy w ślinie, wiek i choroby przyzębia predysponowały do rozwoju próchnicy korzenia. Bakhshandeh i wsp.22 rozpatrywali wpływ różnych czynników (wiek, poziom wykształcenia, typ cukrzycy, czas trwania choroby, poziom HbA1c, obecność powikłań cukrzyc) na intensywność choroby próchnicowej. Przeciwnie is c op y is for pe rso na lu within bacterial microflora may affect the carious process.13-17 However, some authors have not reported the increased incidence of caries among diabetics, nor have they noted any differences in the intensity of caries among diabetics and healthy individuals.18-20 Bacic et al.,21 who examined adult patients with type 1 and 2 diabetes, did not report statistically significant differences in the DMF value between healthy and sick individuals. There was no significant difference in the number of teeth filled, either. However, sick patients had a significantly higher number of teeth missing due to caries. When they compared diabetic patients with regard to the type of the disease the reported a significantly higher number of teeth missing in type 2 diabetics, and a significantly higher number of teeth filled in type 1 diabetics. Moor et al.16 examined the degree of coronal and root caries in type 1 adult diabetics. They reported significantly higher DMF/ S values and a significantly higher number of teeth missing than in healthy individuals. However, the DF/S and D/S components in patients with diabetes were statistically insignificantly higher. They also noted that elderly women with diabetic nephropathy had a higher incidence of coronal caries; they also observed that increased concentration of glucose in saliva, patient’s age and periodontal disease are predisposing factors in the development of root caries. Bakhshandeh et al.22 studied the influence of a variety of factors (age, education, type of diabetes, duration of the disease, the HbA1c level, presence of diabetic complications) on caries intensity. Syrjala i wsp.,23 podobnie jak w niniejszej pracy, nie stwierdzili bezpośredniego związku między nieprawidłowym wyrównaniem cukrzycy (HbA1c >8,5%) a wzrostem zachorowań na próchnicę, natomiast zaobserwowali, że stopień wyrównania choroby jest czynnikiem modyfikującym i ma wpływ na zmianę składu śliny, które mogą zwiększać podatność na próchnicę. U pacjentów z nieprawidłowym wyrównaniem choroby stwierdzono istotnie wyższe miano Streptoccocus mutans i Lactobacillus, powodujące wzrost liczby powierzchni zębów dotkniętych przez próchnicę, w porównaniu z pacjentami z poziomem HbA1c < 8,5%. W badaniach własnych stwierdzono, że wartość PUW/Z była wyższa u pacjentów z cukrzycą typu 2 w odniesieniu do pacjentów z cukrzycą typu 1, jak i osób zdrowych. Podobne zależności zaobserwowano w podgrupach pacjentów z cukrzycą typu 2 niezależnie od poziomu hemoglobiny glikozylowanej oraz czasu trwania choroby. These correlations were more clearly marked in males than in females. Syrjala et al.,23 likewise in this paper, did not confirm a direct relationship between poor diabetic control (HbA1c >8.5%) and the increased incidence of caries; they noted, however, that the level of control is a mediating factor, and has an influence on changing saliva composition that can predispose to caries. In patients with poor control of the disease a significantly higher Streptoccocus mutans and Lactobacillus titre was reported resulting in the increased number of dental surfaces affected by caries, in comparison with patients whose HbA1c level was <8.5%. Our own research has revealed that the value of DMF/T was higher in patients with type 2 diabetes as compared with type 1 diabetics, or healthy subjects. Similar correlation was observed in subgroups of patients with type 2 diabetes irrespective of the level of glycated hemoglobin or the duration of the disease. In patients with type 1 diabetes the DMF/T index value was lower in all examined subgroups than in type 2 diabetics or healthy individuals. It is to be stressed that in all analyses the DMF/T index value was significantly lower in type 1 than in type 2 diabetics. Statistically significant differences in relation to healthy individuals were confirmed only in type 2 diabetics (C2 group) where the DMF/T index value was significantly higher, and in the subgroup of patients with type 1 diabetes and good control the DMF/T index value was significantly lower than in the control group. Natomiast największą liczbę zębów usuniętych stwierdzono u pacjentów z cukrzyca typu 2 we wszystkich badanych podgrupach. Liczba zębów usuniętych była istotnie wyższa u chorych z cukrzycą typu 2 względem osób z cukrzycą typu 1 i takie same zależności stwierdzono w podgrupach z wyrównaną i niewyrównaną chorobą a także u osób z różnym czasem jej trwania. Liczba zębów z wypełnieniami najwyższa była u pacjentów zdrowych względem osób chorych we wszystkich analizach, oprócz podgrupy D1 – pacjentów z cukrzycą typu 1 chorujących krócej niż 10 lat, gdzie zanotowano najwyższą wartość W/Z. U pacjentów z cukrzycą typu 2 wartość W/Z była istotnie statystycznie niższa względem osób zdrowych. Hintao i wsp.14 oceniali wpływ cukrzycy na rozwój próchnicy w zależności od jej lokalizacji (na powierzchni korony i korzenia zęba), a także próbowali określić czynniki, które dodatkowo przyspieszają i ułatwiają rozwój zmian próchnicowych. Zaobserwowali, podobnie jak w niniejszej pracy, istotnie zwiększoną podatność na próchnicę korzenia u pacjentów z cukrzycą typu 2, prawdopodobnie spowodowaną zwiększoną częstością występowania recesji dziąseł, która sprzyjała rozwojowi próchnicy cementu. Ponadto u tych chorych występowało niższe pH śliny, zmniejszona pojemność buforowa śliny, wyższy wskaźnik płytki PI (Plaque Index) oraz wyższy poziom bakterii S.mutans i Lactobacilli, które także wpływają na rozwój próchnicy. Hintao i wsp.14 nie zaobserwowali wpływu takich czynników jak: czas trwania choroby i poziom HbA1c >8,5% na zwiększoną podatność na próchnicę. Z kolei Hintao i wsp.24 badając mikrobiologiczny profil płytki nad i poddziąsłowej u pacjentów z cukrzycą typu 2 i jej wpływ na rozwój próchnicy stwierdzili, że podwyższonemu poziomowi glukozy w krwi towarzyszy wzrost stężenia glukozy w ślinie i w płynie dziąsłowym, powodując wzrost liczby bakterii kwasotwórczych w jamie ustnej, co stanowi ważny czynnik ryzyka rozwoju próchnicy (wzmożona przemiana glukozy przez bakterie zgromadzone w płytce z wytworzeniem dużej ilości kwasów może być przyczyną początkowo demineralizacji powierzchni szkliwa a następnie powstawania ubytków próchnicowych).
