Terms and Conditions of THE ENEL-MED PATIENT CARD
1) THE ISSUER AND OWNER OF THE ENEL-MED PATIENT CARD
The issuer and owner of the ENEL-MED PATIENT CARD is Centrum Medyczne ENEL-MED S.A. with
its registered office in Warszawa at ul. Słomińskiego 19/524, 00-195 Warszawa, entered into the
National Court Register maintained by the District Court for the capital city of Warszawa in Warszawa,
12th Commercial Department of the National Court Register under No. KRS: 0000275255, with the
share capital of PLN 23.566.900,00, Tax Identification Number NIP: 5242593360.
2) ENEL-MED PATIENT CARD USER
The user of the card is a person who meets the conditions for being granted the card and accepts
these rules.
3) PERSONS ENTITLED TO USE THE ENEL-MED PATIENT CARD
The person entitled to use the card is its user who has received the card from Centrum Medyczne
ENEL-MED S.A.
4) TERMS OF USE OF THE ENEL-MED PATIENT CARD
a) In order to use the card (discount), the cardholder has to present the card before making payment
for the service.
b) The








user of the card is entitled for:
10% discount on medical consultations,
15% discount on laboratory examinations,
15% discount on diagnostic examinations (ultrasound, X-ray, and others),
10% discount on MRI,
15% discount on CT,
20% discount on mammography,
50% discount on dental consultations and check-ups with a dental panoramic X-ray,
15% discount on dental hygiene procedures.
c) The discount does not apply to the costs of materials.
d) The ENEL-MED Patient Card can be used only at the centres and laboratories of the Issuer and
Owner of the Enel-Med Patient Card, i.e.

6. VALIDITY OF ENEL-MED PATIENT CARD DISCOUNTS




The discounts indicated above apply from 01 January 2011 until further notice.
The scope and validity of the discounts may be modified during the term of validity.
ENEL-MED undertakes to inform patients of changes in the scope of the discounts via the
www.enel.pl website and at the centres and laboratories referred to in point 4d.
The Issuer has the right to revoke the card if the general rules for granting the discount are
changed by the Management Board of the Company.
7. FINAL PROVISIONS
a) The rules for granting and using the ENEL-MED PATIENT CARD are made publicly available at
www.enel.pl. The Issuer has the right to change the rules or the privileges related to a given type of
cards at any time without specifying the reasons.
b) The rules come into force on the day of their publication.
c) Discounts related to the ENEL-MED PATIENT CARDS cannot be combined with other promotions
and discounts granted by Centrum Medyczne ENEL-MED S.A.
