TOMASZA WOLAŃCZYKA Klinika Psychiatrii Wieku Rozwojowego Warszawskiego Uniwersytetu Medycznego Warszawa 2016 STRESZCZENIE Wprowadzenie: Współistnienie problemów gastroenterologicznych u pacjentów chorujących na jadłowstręt psychiczny jest powszechnie znanym zjawiskiem. W początkowym okresie hospitalizacji większość pacjentów zgłasza nieprzyjemne dolegliwości zarówno ze strony górnej, jak i dolnej części przewodu pokarmowego. Ze względu na brak ścisłych wytycznych dotyczących kwalifikacji do poszerzania diagnostyki gastrologicznej, w praktyce klinicznej stanowi to znaczny problem, którego rozwiązanie zależy od subiektywnej oceny lekarza. Istnieje stosunkowo niewiele doniesień poruszających oba te zagadnienia, a opublikowane wyniki znacząco różnią się od siebie. Jak dotychczas nie ukazały się badania prowadzone w grupie pacjentów chorujących na jadłowstręt psychiczny wykorzystujące w diagnostyce współistniejącej choroby refluksowej najnowocześniejszą metodę wielokanałowej impedancji przełykowej z pH-metrią. Cele pracy: Ocena zaburzeń motoryki przewodu pokarmowego u nastoletnich pacjentek chorujących na jadłowstręt psychiczny tj. wykazanie częstości, charakteru zaburzeń oraz związanych z nimi dolegliwości. Ocena korelacji pomiędzy wiekiem, czasem trwania choroby, stopniem wyniszczenia, stosowanymi ograniczeniami dietetycznymi, zgłaszanymi dolegliwościami, wywiadem gastrologicznym oraz stanem psychicznym a rodzajem i nasileniem zaburzeń motoryki przewodu pokarmowego w celu stworzenia charakterystyki pacjentek szczególnie narażonych na wystąpienie zaburzeń motoryki przewodu pokarmowego. Ocena przydatności wielokanałowej impedancji przełykowej z pH-metrią, jako metody diagnostycznej choroby refluksowej na podstawie analizy poszczególnych epizodów refluksu i związanych z nimi dolegliwości. Materiał i metody: Badaniem objęto dziewczęta w wieku 12-18 lat chorujące na jadłowstręt psychiczny będące pacjentkami Oddziału Psychiatrii Dziecięcej. Projekt realizowano w początkowym okresie rehabilitacji żywieniowej. W pierwszym etapie badania użyto kwestionariusz uwzględniający wiek, niedobór masy ciała, czas chorowania, występujące dolegliwości, stosowaną w czasie choroby dietę, obciążenia gastrologiczne oraz aktualny stan psychiczny. Następnie przeprowadzono wysokiej rozdzielczości manometrię przełyku oraz wielokanałową impedancję przełykową z pH-metrią. Ostatni etap badania stanowiła ocena czasu pasażu jelitowego metodą radiologiczną. Wyniki: Do badania zakwalifikowano 24 dziewczęta, u których przeprowadzono badanie manometryczne. Ze względu na wycofanie zgody na udział w dalszych badaniach pHimpedancję zrealizowano u 23 pacjentek, natomiast czas pasażu jelitowego u 16 chorujących. Średnie BMI przy przyjęciu wynosiło 14 kg/m2, a średni wiek pacjentek zakwalifikowanych do badania 15,4 lat. Dolegliwości ze strony przewodu pokarmowego zgłaszało 87,5% badanych. Najczęściej zgłaszanymi objawami była pełność poposiłkowa (19/24), wzdęcia (17/24) oraz ból w nadbrzuszu (16/24). Rozpoznanie zaburzeń motoryki przełyku diagnozowane badaniem manometrycznym postawiono u 25% badanych, natomiast refluks żołądkowo – przełykowy rozpoznano u 26,1% pacjentek. 63,3% uwidocznionych refluksów stanowiły refluksy niekwaśne, z czego 83,3 to epizody słabo kwaśne, a 16,7% słabo zasadowe. Normy dotyczące refluksów kwaśnych nie zostały przekroczone w żadnym przypadku. Nieprawidłowy czas pasażu jelitowego zdiagnozowano u 50% badanych, z czego u 62,5% był on wydłużony, a u 37,5% przyspieszony. Nie znaleziono korelacji pomiędzy czasem trwania choroby, obciążeniami gastrologicznymi w przeszłości oraz wiekiem badanych a motoryką górnej części przewodu pokarmowego. Wykazano dodatnie korelacje jedynie pomiędzy dwoma parametrami mierzonymi w badaniu manometrycznym (DCI, duże przerwy w ciągłości fali perystaltycznej), a stopniem niedoboru masy ciała. Odkryto szereg ujemnych korelacji pomiędzy dolegliwościami ze strony górnej części przewodu pokarmowego a ciśnieniem w obrębie LES/UES. Dodatnie korelacje wykazano jedynie pomiędzy obecnością bólu w klatce piersiowej i nadbrzuszu a zaburzeniami motoryki przełyku w badaniu manometrycznym (p=0,005, p=0,048). Nie znaleziono korelacji pomiędzy objawami ze strony dolnej części przewodu pokarmowego a czasem pasażu jelitowego. Wykazano natomiast istotną statystycznie ujemną korelację pomiędzy częstotliwością wypróżnień oraz dodatnią pomiędzy wiekiem pacjentki a czasem pasażu jelitowego (p=0,005, p=0,034). Wykazano szereg korelacji pomiędzy stosowaną w czasie choroby dietą oraz działaniami mającymi na celu redukcję masy ciała a motoryką zarówno górnego, jak i dolnego odcinka przewodu pokarmowego. Dodatnie korelacje wykazano pomiędzy wskaźnikiem IRP w manometrii przełyku a oceną nastroju, snu, koncentracji. Ujemne natomiast pomiędzy apetytem a występowaniem nudności i pełności poposiłkowej oraz nieprawidłowościami w zakresie fali perystaltycznej. Dodatnie korelacje wykazano pomiędzy stosowaniem leków p/depresyjnych a występowaniem nudności (p=0,044). Wnioski: Zaburzenia motoryki przełyku są częstym problemem u pacjentek chorujących na jadłowstręt psychiczny wynikającym z osłabionej mięśniówki przewodu pokarmowego i w konsekwencji nieefektywnej pracy; znaczenie kliniczne zjawiska nie jest jednak duże. Refluks żołądkowoprzełykowy wymagający stosowania farmakoterapii dotyczy 26% pacjentek, a w diagnostyce problemu powinno używać się badania pH-impedacyjnego. Objawy sugerujące chorobę refluksową nie są jej właściwym wskaźnikiem. Nieprawidłowy czas pasażu jelitowego dotyczy połowy chorujących. W ocenie pasażu jelitowego znaczenie ma częstotliwość wypróżnień, a nie zgłaszane objawy. U pacjentek chorujących na jadłowstręt psychiczny zaparcia wymagają potwierdzenia w oparciu o przyjęte kryteria. Brak istotnych korelacji pomiędzy wiekiem badanej, czasem chorowania, stopniem niedoboru masy ciała, czy obciążeniami gastrologicznymi a motoryką przełyku. Model odżywiania znacznie odbiegający od normy zarówno w ilościowym, jak i jakościowym zakresie prowadzi do nieefektywnej perystaltyki przełyku, a właściwa podaż płynów ma znaczenie dla zachowania prawidłowego pasażu jelitowego. Zgłaszane objawy ze strony górnej części przewodu pokarmowego związane są raczej z obniżonym ciśnieniem w obrębie zwieraczy przełyku i nie wymagają leczenia farmakologicznego. Wyrównany stan psychiczny pozytywnie wpływa na pracę przewodu pokarmowego. Pacjenci ze współtowarzyszącymi objawami depresyjnymi, bądź depresyjno – lękowymi wymagający leczenia farmakologicznego częściej zgłaszają dolegliwości ze strony górnej części przewodu pokarmowego. SUMMARY Gastrointestinal motility disorders in adolescent female patients with anorexia nervosa Introduction: Coexistence of gastrointestinal disorders with anorexia nervosa is a commonly known phenomenon. In the initial period of hospitalisation the majority of patients report unpleasant discomfort related to the upper and lower gastrointestinal tract. As strict guidelines regarding qualifications for expanded gastrology diagnostic tests are missing, it is a significant problem in the clinical practice and whether it can be solved or not depends on the physician’s subjective opinion. There are relatively few reports describing both these issues, and published results are often significantly contradictory. So far, there have been no studies in a group of patients with anorexia nervosa that would use the latest method of multichannel intraluminal impedance and pH- monitoring to diagnose a coexisting gastroesophageal reflux disease. Aim of the study: Assessment of gastrointestinal motility disorders in adolescent female patients with anorexia nervosa: evaluation of the incidence, nature of disorders and related complaints. Assessment of correlation between the age, duration of disease, degree of cachexia, applied diet limitations, reported complaints, gastrological anamnesis and the psychiatric condition as well as type and grade of gastrointestinal motility disorders in order to create characteristics of patients that are especially at the risk of gastrointestinal motility disorders. Assessment of the usefulness of multichannel intraluminal impedance and pH-monitoring as a diagnostic tool for gastroesophageal reflux disease based on the analysis of individual episodes of gastroesophageal reflux and related complaints. Materials and methods: The study included girls at the age of 12–18 years with anorexia nervosa who were patients at the Ward of Children Psychiatry. The project was conducted at the initial stage of nutritional rehabilitation. During the first stage of the study a questionnaire was used and it contained questions regarding the age, body weight loss, duration of disease, reported complaints, diet used during the disease, history of gastrointestinal complaints and the current psychiatric condition. Then, high resolution manometry of the oesophagus and multichannel intraluminal impedance and pH-monitoring were performed. The last stage of the study included the assessment of colonic transit time with a radiological method. Results: 24 girls who had manometry performed were considered eligible for the study. Due to lack of consent for further tests pH-impedance was performed in 23 patients, and colonic transit time in 16 patients. The mean BMI on admission was 14 kg/m2, and the mean age of patients eligible for the study was 15.4 years. Gastrointestinal complaints were reported by 87.5% of subjects. The most common symptoms included postprandial fullness (19/24), bloating (17/24) and epigastric pain (16/24). The diagnosis of oesophageal motility disorders diagnosed with a manometry was made in 25% of subjects, and gastroesophageal reflux was diagnosed in 26.1% of subjects. 63.3% of all diagnosed reflux cases included non-acid reflux, and 83.3 of them were weakly acidic episodes, and 16.7% – weakly alkaline episodes. The reference range for acid reflux was not exceeded in any case. Abnormal colonic transit time was observed in 50% of subjects: in 62.5% it was prolonged and in 37.5% – accelerated. No correlation was observed between the duration of disease, history of gastrological complaints and the age of patients and the motility of the upper gastrointestinal tract. Positive correlations were demonstrated only between two parameters measured during a manometry (DCI, large intervals in the continuity of the peristaltic wave) and a grade of body weight loss. Several negative correlations between upper gastrointestinal complaints and pressure in the LES/UES were demonstrated. Positive correlations were only demonstrated between the presence of pain in the chest and epigastric and oesophageal motility disorders in the manometry (p=0.005, p=0.048). There was no correlation between lower gastrointestinal symptoms and colonic transit time. On the other hand, there was a statistically significant negative correlation between the frequency of defecation and a positive correlation between the patient’s age and colonic transit time (p=0.005, p=0.034). Several correlations between a diet used during disease and activities aimed to reduce the body weight and the motility of the upper and lower gastrointestinal tract were demonstrated. Positive correlations were demonstrated between the IRP index for oesophageal manometry and assessment of mood, sleep and concentration. There were negative correlations between appetite and nausea and postprandial fullness and abnormalities in the peristaltic wave. Positive correlations were demonstrated between the use of antidepressants and nausea (p=0.044). Conclusions: Oesophageal motility disorders are a frequent problem in patients with anorexia nervosa and they result from weakened muscles of the gastrointestinal tract, and consequently from their inefficiency; however, this phenomenon is not clinically significant. Gastroesophageal reflux disease combined with the need for pharmacotherapy is observed in 26% of patients, and pHimpedance tests should be used as part of diagnostic tests. Gastroesophageal reflux disease should not be diagnosed based on its relevant symptoms. Abnormal colonic transit time was observed in half of patients. The frequency of defecation and not symptoms reported are significant with regard to the assessment of colonic transit. In patients with anorexia nervosa constipation has to be confirmed based on the accepted criteria. There were no significant correlations between the patient’s age, duration of disease, grade of body weight loss or history of gastrological complaints and oesophageal motility. A nutrition pattern demonstrating abnormalities both with regard to the quantity and quality of food leads to ineffective oesophageal peristalsis, and appropriate supply of fluids is important in order to maintain appropriate colonic motility. Upper gastrointestinal symptoms reported are more associated with reduced pressure of the oesophageal sphincters and do not require pharmacotherapy. A balanced psychiatric condition has a positive effect on the gastrointestinal motility. Patients with coexisting depression or anxiety and depression symptoms who need pharmacotherapy report upper gastrointestinal symptoms more often.
