Streszczenie Wstęp Wzrost znaczenia sportu w ostatnich latach spowodował, że coraz częstsze stają się uszkodzenia aparatu więzadłowego stawu kolanowego. Więzadło krzyżowe przednie (ACL) jest najczęściej zrywanym, a co za tym idzie najczęściej rekonstruowanym więzadłem tego stawu. Do rekonstrukcji ACL w Polsce najczęściej stosuje się ścięgna mięśnia półścięgnistego ST i smukłego GR. Zdecydowanie mniej popularne są materiały syntetyczne, takie jak LARS. Przekłada się to na liczbę doniesień na temat funkcjonowania stawu kolanowego po rekonstrukcji ACL przy pomocy materiałów syntetycznych. Cel pracy Głównym celem pracy jest ocena funkcji stawu kolanowego po rekonstrukcji więzadła krzyżowego przedniego przy zastosowaniu metody LARS i przeszczepu autogennego. Materiał i metodyka Materiał badań stanowiło 96 pacjentów, 76 mężczyzn i 20 kobiet operowanych w latach 2010-2012 w Szpitalu Specjalistycznym im. Świętej Rodziny w Rudnej Małej k. Rzeszowa, u których wykonana została rekonstrukcja zerwanego więzadła krzyżowego przedniego. Grupę badaną stanowiło 44 osoby operowane metodą LARS. Grupę kontrolną stanowiło 52 osoby, u których wszczepiono autogenny przeszczep ST/GR. Badania kliniczne, ocenę jakości życia i ankietę własną u wszystkich chorych przeprowadzono jednokrotnie, średnio 4 lata po zabiegu operacyjnym. Wyniki badań odniesiono do wcześniej opisanych wyników innych autorów oraz norm. Porównano wyniki otrzymane dla kończyny operowanej i zdrowej. Wyniki Nie ma podstaw do stwierdzenia, iż istnieje różnica w ocenie funkcji stawu kolanowego za pomocą skali Lysholma. W obydwu badanych grupach pacjentów uzyskano wynik na poziomie bardzo dobrym. W przeprowadzonych badaniach jakości życia za pomocą skali SF36v2 pacjenci wysoko ocenili swoją jakość życia na średnim poziomie dla obydwu grup 83,9/100 pkt. Najlepiej zostały ocenione podskale dotyczące ograniczeń roli ze względu na problemy fizyczne i psychiczne. Wyniki w tych skalach przekraczały wyraźnie próg 90 pkt. Najgorzej oceniona została jakość życia w kategorii zdrowie ogólne 62,5 pkt. oraz samopoczucie 71,1 pkt. Nie stwierdzono istotnych statystycznie różnic pomiędzy zbadanymi grupami pacjentów z grupy LARS i ST/GR pod względem jakości ich życia. Badanie siły mięśniowej na stanowisku do oceny izokinetycznej w trybie pracy koncentrycznej (con/con) wykazało, że wartości uzyskiwane przez zginacze i prostowniki kończyny operowanej były znacznie niższe niż te same parametry w kończynie zdrowej. Badani ze wszczepionym więzadłem LARS uzyskali niższe wyniki dla zginaczy i prostowników w obydwu kończynach w porównaniu do badanych z grupy ST/GR. Różnice te były jednak nieistotne statystycznie za wyjątkiem siły zginaczy w kończynie operowanej. Wyraźne zaburzenia pomiędzy silą mięśniową poszczególnych grup mięśniowych uwidaczniają się dopiero podczas testowania ich w trybach, w których wykonują pracę ekscentryczną (tryb ecc/con i con/ecc). Nie zaobserwowano różnic pomiędzy kończyną zdrową i operowana, jak było w przypadku badania con/con. Widać natomiast, bardzo wyraźną różnicę siły mięśniowej zginaczy i prostowników pomiędzy pacjentami z grup LARS i ST/GR. Parametrami ocenianymi podczas badania na platformie stabilometrycznej były średnie wychylenie COP w osi X i Y w testach z oczami otwartymi (OO), zamkniętym (OZ) oraz średnie obciążanie kończyn dolnych. Różnice pomiędzy badanymi grupami zaobserwowano w wychyleniach COP z uwzględnieniem kierunku. Pacjenci z grupy autogennej mieli tendencje do większych wychyleń COP w tył, a pacjenci z grupy LARS w przód. Wychylenia te zwiększyły swoją amplitudę podczas próby z zamkniętymi oczami. Wykonano również testy dystrybucji obciążeń kończyn dolnych, jednak nie stwierdzono różnic w wynikach tego parametru dla obu zbadanych grup pacjentów. Wnioski 1. Rodzaj zastosowanego przeszczepu nie ma wpływu na poziom oceny funkcji stawu kolanowego. 2. Jakość życia pacjentów nie zależy od rodzaju przeszczepu zastosowanego do rekonstrukcji..ACL 3. Poziom aktywności fizycznej u pacjentów po rekonstrukcji ACL obniża się niezależnie od rodzaju zastosowanego wszczepu. 4. Siła mięśniowa zginaczy i prostowników stawu kolanowego jest niższa w kończynie operowanej niezależnie od zastosowanego rodzaju przeszczepu. 5. Pacjenci, u których wykonano przeszczep autogenny z ST/GR mają lepsze parametry pracy ekscentrycznej zginaczy i prostowników stawu kolanowego. 6. Rodzaj wszczepionego więzadła nie wpływa na dystrybucje obciążeń kończyn dolnych. 7. Ocena równowagi na statycznej platformie stabilometrycznej nie wnosi istotnych informacji o zaburzeniach równowagi pacjentów po rekonstrukcji ACL w okresie 4 lat od operacji. 8. Czas i rodzaj rehabilitacji jaką przeszli pacjenci z obydwu grup jest niewystarczający do odbudowy siły zginaczy i prostowników stawu kolanowego. Summary Introduction The growing importance of sport in recent years has resulted in more and more frequent injuries of the knee ligament system. Anterior cruciate ligament (ACL) is most frequently torn, and thus the most often reconstructed ligament of that joint. The tendons from semitendinosus (ST) and gracilis (GR) muscles are most frequently used in ACL reconstruction in Poland. Synthetic materials, such as LARS are definitely less popular. This fact is reflected in the number of papers on the functioning of the knee joint after ACL reconstruction by means of synthetic materials. Aim The main aim of the study is to assess knee function after anterior cruciate ligament reconstruction by means of LARS method and autologous graft. Materials and Methods The research material consisted of 96 patients, 76 men and 20 women operated in 2010-2012 in Holy Family Specialist Hospital in Rudna Mała near Rzeszów, who underwent the reconstruction of the ruptured anterior cruciate ligament. The test group included 44 people operated by means of LARS method. The control group consisted of 52 subjects who underwent ST / GR autogenous graft. Clinical tests, quality of life assessment and the author’s questionnaire for all patients were performed once, on average, four years after the surgery. The test results were compared to previously described results of other authors and norms. The results obtained for the operated and healthy limb were compared. Results There is no basis to conclude that there is a difference in the assessment of the knee function in Lysholm scale. The results achieved in both groups of patients were very good. The quality of life was measured with SF36v2 scale, the patients rated their quality of life highly at an average level for both groups, 83.9 / 100 points. The best scores were in the subscales concerning restrictions in the role due to physical and mental problems. The results of these scales clearly exceeded the threshold of 90 points. Quality of life for general health 62.5 points and well-being - 71.1 points were assessed the worst. There were no statistically significant differences between the researched patients from LARS and ST / GR groups in terms of their quality of life. Muscle strength test conducted at the isokinetic evaluation site in concentric mode (con / con) showed that the values obtained by flexors and extensors of the operated limb were significantly lower than the same parameters in the healthy limb. The subjects with implanted LARS ligament obtained lower scores for flexors and extensors in both limbs compared to the control ST / GR group. However, these differences were not statistically significant except for flexor strength in the operated limb. Prominent disorders between muscle strength of individual muscle groups were apparent only when tested in eccentric modes (ecc/con and con/ecc mode). There were no differences between the healthy and the operated limb, as in case of con / con test. However, a very clear difference can be observed in muscle strength of flexors and extensors between patients from LARS and ST / GR group. The parameters assessed during the test on stabilometric platform were mean deflection of COP in X and Y-axis in tests with eyes open (OO), closed (OZ) and average load of the lower extremities. Differences between the groups were observed in deflections of COP, taking into account the direction. Patients treated with autologous graft have a tendency to higher COP deflection backwards and patients from LARS group to forward deflection. These deflection increased their amplitude during the test with eyes closed. Tests of load distribution in the lower limbs were also performed, but there were no difference in the results of this parameter for both examined groups of patients. Conclusions 1.The kind of applied graft had no effect on the assessment of the knee function. 2.The quality life of patients did not depend on the type of graft used for ACL reconstruction. 3. The level of physical activity in patients after ACL reconstruction was reduced regardless of the type of graft. 4. Flexor and extensor strength in the knee was lower in the operated limb regardless of the type of graft. 5. The patients, who underwent autogenous ST / GR transplant had better eccentric flexors and extensors parameters of the knee. 6. The kind of implanted ligament did not affect the distribution of the load of the lower limbs. 7. The assessment of balance on static stabilometric platform did not bring any new on balance disorders in patients after ACL reconstruction 4 years after the surgery. 8. Time and type of rehabilitation that the patients from both groups underwent was insufficient to restore the strength of flexors and extensors in the knee.
