Streszczenie Wstęp: Środowisko wodne stwarza korzystne warunki wielokierunkowego oddziaływania na organizm człowieka. Wykorzystując metody rehabilitacji w wodzie można stworzyć program usprawniania, podobnie jak w przypadku metod stosowanych w warunkach lądowych. Proces usprawniania metodami terapii w wodzie osób z dolegliwościami ze strony narządu ruchu, może wspomóc leczenie w efekcie wzmocnienia znacznych grup mięśni, poprawy zakresu ruchów w stawach, poprawy i doskonalenia reakcji równoważnych, obniżenia lub wzmocnienia napięcia mięśni, obniżenia dolegliwości bólowych, poprawy ogólnego stanu psychosomatycznego. Cel: Niniejsza praca jest próbą stwierdzenia, czy włączenie do standardowej gimnastyki w wodzie elementów metody Ai Chi jest korzystne pod względem terapeutycznym dla osób z dolegliwościami ze strony narządu ruchu. Materiał i metody: Materiał badań stanowili chorzy Oddziału Dziennego Pobytu Rehabilitacyjnego Szpitala Ortopedyczno-Rehabilitacyjnego im. Wiktora Degi w Poznaniu. W grupie 101 przebadanych osób znajdowało się 79 kobiet i 22 mężczyzn, w wieku od 27 do 85 lat. W przebadanej grupie chorych najczęściej występowały rozpoznania w zakresie zmian w obrębie kręgosłupa, takich jak kręgozmyk, dyskopatie, bóle kręgosłupa i choroby zwyrodnieniowe stawów. Badanych podzielono, w zależności od zastosowanej metody ćwiczeń i czasu trwania cyklu ćwiczeń, na cztery grupy: - grupę eksperymentalną zwaną grupą AiChi4 (okres 4 tygodni ćwiczeń z zastosowaniem metody Ai Chi), - grupę eksperymentalną zwaną grupą AiChi2 (okres 2 tygodni ćwiczeń z zastosowaniem metody Ai Chi), - grupę kontrolną zwaną grupą K4 (okres 4 tygodni standardowej gimnastyki w wodzie), - grupę kontrolną zwaną grupą K2 (okres 2 tygodni standardowej gimnastyki w wodzie). Zastosowano następujące metody badań: test Fullerton, pomiar progu wrażliwości dotykowej, koordynację wzrokowo-ruchową, pomiar czucia bólu, pojemność płuc, siłę mięśni zginaczy ręki, ocenę akceptacji choroby i ocenę satysfakcji z życia. Wyniki: Stwierdzono, że zastosowanie metody Ai Chi wpływa bardziej na poprawę sprawności fizycznej ćwiczących niż standardowa gimnastyka w wodzie w zakresie poprawy siły mięśni kończyny górnej i dolnej, gibkości oraz koordynacji wzrokowo-ruchowej. Pojemność płuc wyraźnie bardziej poprawiła się w grupach Ai Chi. Odczucie bólu zmniejszyło się w obu grupach ćwiczących przez cztery tygodnie, nieco bardziej w grupie AiChi4. Wrażliwość dotykowa poprawiła się, czasem istotnie, czasem nie, we wszystkich grupach i na wszystkich punktach pomiaru. Tylko w grupie K4 zaistniała statystycznie istotna poprawa akceptacji choroby. Grupa AiChi4 w początkowym badaniu miała najwyższy poziom akceptacji choroby. W efekcie zajęć osiągnęła poziom najwyższy spośród badanych grup, trudno jednak w tej sytuacji o statystycznie istotną poprawę. Wnioski: Żadna z metod badań nie ujawniła negatywnych skutków ćwiczenia w wodzie metodą Ai Chi. Duża grupa wyników badań potwierdziła lepsze działanie ćwiczeń w wodzie z elementami metody Ai Chi niż standardowa gimnastyka w wodzie. Na wyniki zastosowanych testów wpływać może wiele różnych czynników. Stwierdzono jednak, że różne choroby kręgosłupa, w większym stopniu niż inne czynniki, rzutują na poprawę badanych w niniejszej dysertacji cech. Wyniki badań potwierdziły znane zjawisko lepszych wyników badań u chorych aktywnych fizycznie. Ćwiczenia ruchowe w środowisku wodnym obniżyły ciśnienie krwi, co jest bardzo korzystne u starszych osób. Ćwiczenia ruchowe w środowisku wodnym poprawiają wrażliwość dotykową, szczególnie na stopach, co jest bardzo korzystne w wielu sytuacjach pogorszenia czucia dotyku, na przykład w cukrzycy, chorobach neurologicznych, reumatycznych i ortopedycznych. Using the method of rehabilitation in water it is possible to create a rehabilitation program, similar to the methods used in terrestial conditions. The process of rehabilitation with methods of water therapy in persons with musculoskeletal problems can help in their treatment by strengthen of muscle groups, improving of joint range, improving and perfecting of balance reactions, reduce and strengthen of muscle tension, reduce pain, improving the general psychosomatic condition. Objective: This study is an attempt to determine whether the inclusion of the standard water gymnastics elements of Ai Chi method is advantageous in terms of therapy for people with complaints from the musculoskeletal system. Material and methods: The research material consisted of patients from the Branch Day Care - Rehabilitation Hospital of Orthopaedics and Rehabilitation Wiktor Dega in Poznan. In the group of 101 tested people were 79 women and 22 men, ranging in age from 27 to 85 years. In the tested group the most frequent diagnosis were changes within the spine, such as spondylolisthesis, discopathies, back pain and osteoarthiritis. Patients were divided according to the method of exercises and duration of the cycle of exercises, into four groups: - experimental group called the AiChi4 group (4 weeks of exercises using the Ai Chi method) - experimental group called the AiChi2 group (2 weeks of exercises using the Ai Chi method) - control group called the K4 group (4 weeks of the standard water gymnastics), - control group called the K2 group (2 weeks of the standard water gymnastics). The following test methods were used: Fullerton test, measurement of tactile sensitivity on the thresholds, eye-hand coordination, measurement of feeling pain, lung capacity, strength of flexor hand muscles, assessment of the disease’s acceptance and the evaluation of satisfaction with life. Results: It was found that the application of Ai Chi method affects more the improvement of physical fitness than the standard water gymnastics in improving muscle strength of the upper and lower limb, flexibility and hand-eye coordination. The capacity of lungs improved more in Ai Chi groups. Feeling pain reduced in both groups which trained for four weeks, a little bit more in the AiChi4 group. Tactile sensitivity improved, sometimes significantly, sometimes not, in all groups and at all points of measurement. Only in the K4 group occurred a statistically significant improvement in the illness acceptance. AiChi4 group at the beginning of the study had the highest level of illness acceptance. As a result, the course has reached the highest level among the groups studied, but it is difficult in this situation, with a statistically significant improvement. Conclusion: None of the test methods showed negative effects of water exercises with the Ai Chi method. A large group of test results confirmed an improvement of water exercises with elements of the Ai Chi method than the standard water gymnastics. The results of the assays used may be influenced by many factors. It has been found, however, that different diseases of the spine, to a greater extent than other factors impinge on improving investigated in this doctoral dissertation. The results confirmed a phenomenon known better test results in patients who are physically active. Physical exercises in water lowered blood pressure, which is very advantageous in the elderly. Physical exercises in water improve tactile sensitivity, especially on the feet, which is very advantageous in many situations the deterioration of sense of touch, for example, diabetes, neurological rheumatic and orthopedic diseases.
