Description of subject of the enquiry:
Development of technology for production of brass forgings with mechanical treatment (selection of:
machinery, design of manufacturing technology, design and execution of tooling, and preparation of
technology) to manufacture measurement valves and F.221 balancing valves. Forgings shall be in
accordance with the technical documentation after machining. The technical documentation is
available for inspection at the registered office of ZETKAMA S.A. It is possible, however, to obtain
the technical documentation after signing a confidentiality agreement.

Term of execution of the subject of the offer:
two full passages (DN) of all six passages – 31.07.2013
two next passages (DN) of all six passages – 31.08.2013
last two passages (DN) of all six passages - 30-09-2013
The most advantageous offer will be selected based on the following criteria:
price of service (technology development) – 20 points (20%)
forging unit price – 70 points (70%)
execution date – 10 points (10%)
The ordering party after evaluation of the submitted offers will send the tender results to all bidders,
then will propose a bidder, who received the highest number of points, to conclude an agreement on
execution of the subject of the purchase.
The offer must contain the following items
Full identity data of the bidder (name, address, NIP (tax id), KRS/EDG no. (no. at a registry of
entrepreneurs),
Bid preparation and validity dates,
Scope and detailed description of the offered services/products,
Reference to all offer selection criteria included in the enquiry,
Net and gross total price,
Terms and date of payment,
Date/period of execution of the subject of the offer,
Contact person details (full name, phone number, e-mail),
Signature of a person authorized to issue the offer.

Absence of any of the above items may result in rejection of the offer.

Offers submitted after the deadline will not be considered. The date of receipt of the offer by the
company matters.
Along with the offer the Ordering Party must receive a confirmation of receipt the enquiry by
the Bidder (Xerox copy/scan of the first page of this enquiry with a note „Received on …” and a
company stamp of the Bidder and signature of a person authorized by the Bidder).
As part of the application for financing the offers may be submitted for verification to an appropriate
public authority.
