Początkowo do oceny punktów kostnych i skórnych stosowano analizę konwencjonalną (ręczną), z czasem wprowadzono liczne analizy komputerowe. Cel. Ocena standardowej niepewności pomiaru ANB, ML-NL, 1+:1-, WITS, kąta nosowo-wargowego w obu metodach pomiarów (konwencjonalnej i komputerowej). Materiał i metody. Do badań wykorzystano 40 telerentgenogramów różnych pacjentów. Zostały one wykonane w jednej pracowni aparatem cyfrowym, zapisane 1 2 3 4 Abstract A cephalometric analysis is an examination based on an X-ray scan. At the beginning, a conventional (manual) analysis was used to determine skeletal and skin points, but with time numerous computer analyses were created. Aim. To assess standard measurement uncertainty for: ANB, ML-NL, 1+:1-, WITS, nasolabial angle in both methods of measurement (conventional and computer). Material and methods. 40 teleroentgenograms from various patients were used for analyses. They were taken in one laboratory with a digital camera, recorded on a CD and printed on a film. Oceniano pomiary kątowe wg analizy Segnera-Hasunda (ANB, kąt międzyszczękowy ML-NL, kąt międzysieczny 1+:1-, kąt nosowo-wargowy) oraz pomiar liniowy WITS. Wszystkie badania zostały wykonane trzykrotnie przez tę samą osobę w odstępach tygodniowych. Otrzymane wyniki poddano analizie statystycznej w programie Statistica 10. Wyniki. Najmniejsza średnia standardowa niepewność pomiaru wystąpiła przy pomiarze ANB (0,13 dla analizy komputerowej i 0,16 dla analizy konwencjonalnej). Średnia standardowa niepewność pomiaru dla kąta międzyszczękowego (ML-NL) i WITS zawierała się w przedziale od 0,29 do 0,46 dla obu metod pomiaru. Największą średnią standardową niepewność pomiaru zaobserwowano dla kąta międzysiecznego (1+:1-) i kąta nosowo-wargowego – wartości zawierały się w przedziałach od 0,95 do 1,09 dla obu metod pomiaru. Średnie standardowe niepewności pomiaru dla wszystkich pomiarów były niższe w przypadku analizy komputerowej. Otrzymane różnice nie były istotne statystycznie. Wnioski. Obie metody pomiarów mogą być stosowane zamiennie w praktyce ortodontycznej, jednak analiza wykonana metodą komputerową pozwala na bardziej precyzyjne wyznaczanie punktów. Największą średnią standardową niepewność pomiaru stwierdzono dla kąta międzysiecznego i nosowo-wargowego, należy więc zwrócić większą uwagę przy ich wyznaczaniu. Angular measurements used in the Segner-Hasund analysis (ANB angle, ML-NL interjaw-base angle, 1+:1- interincisal angle, nasolabial angle) and WITS linear measurement were analysed. All examinations were performed three times by the same person at one-week intervals. The results were subject to a statistical analysis using the Statistica 10 software. Results. The minimum mean standard measurement uncertainty was observed for the measurement of ANB (0.13 for a computer analysis and 0.16 for a conventional analysis). The mean standard measurement uncertainty for the interjaw-base angle (ML-NL) and WITS was in the range between 0.29 and 0.46 for both measurement methods. The largest mean standard measurement uncertainty was observed for the interincisal angle (1+:1-) and nasolabial angle; the values were in the range between 0.95 and 1.09 for both measurement methods. Mean standard measurement uncertainties for all measurements were lower in a computer analysis. The differences obtained were not statistically significant. Conclusions. Both methods of measurements can be used interchangeably in the orthodontic practice; however, a computer method allows to determine points with a higher precision. The largest mean standard measurement uncertainty was observed for the interincisal angle and nasolabial angle, therefore more attention should be paid during determination of these angles. Już w XV w. Leonardo da Vinci rozpoczął badania nad proporcjami ciała ludzkiego. Do naszych czasów zachowały się ryciny przedstawiające czaszkę męską z naniesionymi liniami symetrii. W XVIII w. Pieter Camper wprowadził kąty twarzowe. W XIX w. Broca zastosował kraniostat, aby udoskonalić techniki pomiarowe. Podczas XIII Kongresu Towarzystwa Niemieckich Antropologów we Frankfurcie nad Menem zaakceptowano płaszczyznę poziomą, zwaną płaszczyzną frankfurcką, jako powszechną metodę do oceny czaszek. Przełomem okazało się odkrycie w 1895 r. promieni rentgenowskich, które zapoczątkowało nową erę w medycynie. W 1931 r. dwaj dentyści (Hofrath w Dusseldorfie i Broadbent w Cleveland) Key words: computer cephalometric analysis, statistics, cephalometric measurements For many centuries variability of the structure of the human skull has been gaining much attention. As early as in the 15th century Leonardo da Vinci started his studies on the proportions of the human body. Figures presenting a male skull with lines of symmetry survived until our times. In the 18th century Pieter Camper introduced facial angles. In the 19th century Broca used a craniostat to improve measuring techniques. During the 13th Congress of the German Anthropological Society in Frankurt-on-Main a horizontal plane called the Frankfurt plane was approved as a common method to measure human skulls. The discovery of X-rays in 1895 turned out to be a breakthrough and it started a new era in medicine. Pozwoliło to przejść na nowy poziom kraniometrii i na rozwój cefalometrii (1, 2). Wśród metod antropometrycznych została wydzielona kraniometria (zajmująca się bezpośrednim pomiarem czaszek) i cefalometria (zajmująca się pomiarami głowy na zdjęciach radiologicznych). Wiek XX to dalszy rozwój i doskonalenie metod stosowanych do pomiarów radiologicznych. W tym okresie powstaje wiele nowych analiz wykorzystywanych do opisu zdjęć telerentgenowskich, na przykład Sassouniego, Björka, Arnetta (3, 4). Jest to okres ciągłych badań nad wzrostem i rozwojem. Szybki postęp w informatyce pozwolił na szersze zastosowanie komputerów w praktyce ortodontycznej. Poza analizą konwencjonalną (ręczną) wprowadzono liczne analizy komputerowe. Początkowo wykorzystywały one digitalizację, czyli wprowadzanie współrzędnych z kliszy do komputera (5, 6). Następnie cyfrowe aparaty pozwalały na zapis obrazu na płycie CD. Powstały programy komputerowe, które nie tylko mierzyły, ale również same wyznaczały punkty (7). Ostatnie lata to rozwój stożkowej tomografii komputerowej, który dał możliwość bardziej precyzyjnej analizy. designed devices to position the head in relation to a radiation source and a film. Consequently, craniometry achieved a new level and it was possible to develop cephalometry (1, 2). With regard to anthropometric methods craniometry (associated with direct measurements of the skull) and cephalometry (associated with measurements of the head on X-ray scans) were distinguished. The 20th century is associated with further development and perfection of methods used for radiological measurements. In this period many new analyses used to describe teleroentgenograms, such as methods by Sassouni, Björk, Arnett, are developed (3, 4). This period is associated with continuous studies on the growth and development. Thanks to rapid advances in the field of information technology it was possible to use computers in the orthodontic practice. Apart from conventional (manual) analysis numerous computer analyses were introduced. At the beginning such analyses were associated with digitalisation, namely coordinates were transferred from films into a computer (5, 6). Later, images were recorded on CDs by digital cameras. Then, computer software that was able not only to measure but to determine appropriate points was designed (7). Recent years are associated with development of cone beam computed tomography that allows for a more precise analysis. Do badań wykorzystano 40 telerentgenogramów różnych pacjentów. Wszystkie zdjęcia pochodziły z jednej pracowni i posiadały wysoką jakość obrazu. Zostały wykonane aparatem cyfrowym KODAK 9000 C i zapisane na płycie CD. Do wydruku na kliszy użyto drukarki laserowej. Ekspozycja przy wykonywaniu takich zdjęć trwa 1 sekundę, co zmniejsza ryzyko powstania artefaktów na skutek poruszenia się pacjenta. Pomiary przeprowadzono dwoma metodami: konwencjonalną, używając negatoskopu, kalki, ołówka i przyrządów pomiarowych (linijek, kątomierza) oraz posługując się programem komputerowym Ortodoncja 7. W badaniu uwzględniono wybrane pomiary kątowe z analizy SegneraHasunda: kąt ANB, międzyszczękowy ML-NL, międzysieczny 1+:1-, kąt nosowo-wargowy oraz pomiar liniowy WITS. Przy wyznaczaniu pomiaru WITS w analizie ręcznej posłużono się płaszczyzną zgryzową według Steinera (tzn. przechodzącą przez największą liczbę guzków zębów trzonowych i przedtrzonowych). Wszystkie badania metodą konwencjonalną i komputerową dla każdego pomiaru były przeprowadzane trzykrotnie przez tę samą osobę w odstępach tygodniowych. All images were taken from one laboratory and their image quality was high. They were taken with the KODAK 9000 C digital camera and recorded on a CD. A laser printer was used to print films. When taking such images an exposure lasts only 1 second, therefore the risk of artefacts due to patient’s movement is low. Measurements were taken with two techniques: a conventional one with a negatoscope, tracing paper, pencil and measuring devices (rulers, protractor) and the other one with the Ortodoncja 7 computer software. Selected angular measurements used in the Segner-Hasund analysis were included in the study: ANB angle, ML-NL interjaw-base angle, 1+:1- interincisal angle, nasolabial angle and WITS linear measurement. In order to determine the WITS measurement in a manual analysis the Steiner occlusal plane (the plane drawn through the largest number of cusps of molars and premolars) was used. All examinations using a conventional and manual method were performed three times by the same person at one-week intervals, for each measurement. Powyższe postępowanie przy wyznaczaniu punktów miało na celu wyeliminowanie ewentualnych błędów wynikających ze zmęczenia wzroku badającego. Następnie dla każdego z 40 zdjęć i każdej metody policzono za pomocą średniej arytmetycznej średni pomiar i standardową niepewność pomiaru na podstawie serii trzech pomiarów. W przeprowadzonej analizie cechą statystyczną była standardowa niepewność pomiarów ANB, ML-NL, 1+:1-, WITS, kąta nosowo-wargowego. Otrzymane wyniki poddano analizie statystycznej w programie Statistica 10. Wykorzystano statystyki opisowe dla standardowych niepewności pomiarów, test normalności Shapiro-Wilka, test kolejności par Wilcoxona oraz test t-studenta. Przyjęto poziom istotności α = 0,05, zatem rozpatrywane różnice są istotne statystycznie dla p mniejszego od 0,05. the same time, with one-hour break after each analysis of ten teleroentgenograms. Then, an arithmetic mean was used to calculate the mean measurement and standard measurement uncertainty based on a series of three measurements for each out of 40 photographs and each method. In this analysis a statistical parameter included standard measurement uncertainty for: ANB, ML-NL, 1+:1-, WITS, nasolabial angle. The results were subject to a statistical analysis using the Statistica 10 software. Descriptive statistics for standard measurement uncertainties, Shapiro-Wilk test of normality, Wilcoxon’s signed-rank test and t-student test were used. The significance level was α = 0.05, therefore discussed differences are statistically significant for p below 0.05. Wyniki The following descriptive statistics were obtained for a computer (kp) and conventional (kn) measurement method for standard measurement uncertainties for the number of analysed photographs n = 40: mean standard measurement uncertainty (M); standard deviation of the mean standard measurement uncertainty (SD); coefficient of variation of the standard measurement uncertainty (V); minimum standard measurement uncertainty (Min); median standard measurement uncertainty (Me); maximum standard measurement uncertainty (Max). These values are presented in the table 1. Standard deviation of the mean standard measurement uncertainty (SD) and coefficient of variation of the standard measurement uncertainty (V) were higher in a computer method for the interincisal angle and WITS measurement (tab. 1.). It means that variability of the standard measurement uncertainty expressed as standard deviation and coefficient was higher in a computer analysis compared to a conventional analysis. In a computer analysis standard measurement uncertainties were less bunched around the mean compared to a conventional method. The differences were not statistically significant. Dla komputerowej (kp) i konwencjonalnej (kn) metody pomiaru otrzymano następujące statystyki opisowe dla standardowych niepewności pomiarów przy liczbie analizowanych zdjęć n = 40: średnia standardowa niepewność pomiaru (M); odchylenie standardowe średniej standardowej niepewności pomiaru (SD); współczynnik zmienności standardowej niepewności pomiaru (V); najmniejsza standardowa niepewność pomiaru (Min); wartość środkowa standardowej niepewności pomiaru (Me); maksymalna standardowa niepewność pomiaru (Max). Te wartości znajdują się w pierwszej tabeli. Najmniejsza średnia standardowa niepewność pomiaru (M) wystąpiła przy pomiarze ANB (0,13 dla analizy komputerowej i 0,16 dla analizy konwencjonalnej). Średnia standardowa niepewność pomiaru (M) dla kąta międzyszczękowego (ML-NL) i WITS zawierała się w przedziale od 0,29 do 0,46 dla obu metod pomiaru. Największą średnią standardową niepewność pomiaru (M) zaobserwowano dla kąta międzysiecznego (1+:1-) i kąta nosowo-wargowego wartości zawierały się w przedziałach od 0,95 do 1,09 dla obu metod pomiaru (tab. 1.). Średnie standardowe niepewności pomiaru dla wszystkich pomiarów były niższe w przypadku analizy komputerowej (ryc. 1.). Odchylenie standardowe średniej standardowej niepewności pomiaru (SD) oraz współczynnik zmienności standardowej niepewności pomiaru (V) przyjął wyższe wartości w metodzie komputerowej dla kąta międzysiecznego i pomiaru WITS (tab. 1.). Oznacza to, że zmienność standardowej niepewności pomiaru, wyrażona odchyleniem standardowym i współczynnikiem, była większa w metodzie komputerowej w porównaniu z metodą konwencjonalną. Różnice nie były istotne statystycznie. Zbadano, czy można uważać, że wartości standardowych niepewności pomiarów pochodzą z populacji o rozkładzie normalnym w poszczególnych metodach pomiaru (tab. 2.). Na mocy testu normalności Shapiro-Wilka w standardowych niepewnościach pomiarów ANB i ML-NL, otrzymanych metodą konwencjonalną, oraz parametrów 1+:1- i WITS, otrzymanych metodą komputerową, odrzucono hipotezę o normalności rozkładu wartości standardowych niepewności pomiarów. Dla pozostałych przypadków nie było podstaw do odrzucenia hipotezy o normalności ich rozkładów. Dla pomiaru kąta nosowo-wargowego porównano testem t-studenta (dla prób powiązanych i zależnych) średnie standardowe niepewności pomiarów otrzymane metodą komputerową i konwencjonalną. Natomiast dla pomiarów ANB, ML-NL, 1+:1- i WITS do porównania rozkładów standardowych niepewności pomiarów w obu metodach pomiaru zastosowano nieparametryczny test kolejności par Wilcoxona (dla prób powiązanych). It was tested whether values of standard measurement uncertainties might be considered as coming from a population with a normal distribution in individual methods of measurement (tab. 2.). Using the Shapiro-Wilk test of normality for standard measurement uncertainties for ANB and ML-NL obtained with a conventional method and 1+:1- and WITS parameters obtained with a computer method a hypothesis of a normal distribution of values of standard measurement uncertainties was rejected. There were no grounds to reject a hypothesis of a normal distribution for other cases. With regard to the measurement of the nasolabial angle the t-student test (for paired and dependent samples) was used to compare mean standard measurement uncertainties obtained with a computer and conventional method. On the other hand, for measurements of ANB, ML-NL, 1+:1- and WITS the Wilcoxon non-parametric signed-rank test (for paired samples) was used to compare distributions of standard measurement uncertainties in both measuring methods. Dyskusja W pracy oceniano standardową niepewność pomiaru dla powszechnie stosowanych w praktyce ortodontycznej kątów z analizy cefalometrycznej oraz pomiaru WITS. Badania wykonano metodą konwencjonalną i komputerową. Kąt ANB i pomiar WITS określają położenie żuchwy w relacji do szczęki i są kluczowe przy planowaniu leczenia ortodontycznego. Kąt międzyszczękowy (ML-NL wyraża stopień nachylenia żuchwy w stosunku do podstawy szczęki), kąt międzysieczny (1+:1- określa wzajemne osiowe nachylenie zębów siecznych górnych i dolnych) oraz kąt nosowowargowy (kąt oceniany na tkankach miękkich twarzy, nie zależy od wieku ani płci tylko od kształtu nosa i pozycji wargi górnej) odgrywają zasadniczą rolę w estetyce twarzy (8). Wielkość powyższych kątów jest uwzględniana przy planowaniu leczenia ekstrakcyjnego. Najbardziej precyzyjnie spośród analizowanych kątów, zarówno metodą komputerową jak i konwencjonalną, wyznaczano kąt ANB. Podobne wyniki uzyskali inni badacze (9, 10). Nie stwierdzono istotnych statystycznie różnic w zależności od metody pomiaru, co znalazło potwierdzenie w piśmiennictwie (10, 11, 12). Różnice w wartościach kąta ANB inni badacze wiązali z wyznaczaniem punktu A. Tłumaczyli to trudnym w diagnostyce rejonem na zdjęciu. Uysal jako problematyczny przy wyznaczaniu podawał punkt Nasion (12). Na powyższe wyniki mogła wpłynąć jakość zdjęć, które były wykonane na kliszy, a następnie skanowane. Zdjęcia analizowane w przedstawionych badaniach wykonywano cyfrowym aparatem rentgenowskim, a następnie nanoszono na kliszę drukarką laserową. WITS był jedynym pomiarem liniowym analizowanym w badaniu. Ważne jest, aby płaszczyznę okluzji zawsze wyznaczać tą samą metodą. Jankowska w swojej pracy doktorskiej (Kąt ANB i pomiar WITS: wiarygodność i wpływ na postępowanie ortodontyczne w przypadkach wad przednio-tylnych) udowodniła, że ma to istotny wpływ na wartość pomiaru (13). W obu metodach analizy pomiaru WITS uzyskano zbliżone wartości średniej standardowej niepewności pomiaru. W badaniach Chena i współpracowników stwierdzono istotne różnice statystyczne, porównując obie metody pomiaru WITS (10). Mogła na to wpłynąć technika skanowania zdjęć. W badaniu własnym największa średnia standardowa niepewność pomiaru wystąpiła przy kącie międzysiecznym i kącie nosowo-wargowym. Przy pozostałych pomiarach średnia standardowa niepewność pomiaru oscylowała pomiędzy 0,13–0,46 w obu metodach analizy. Tu wyraźnie wzrosła do wartości 0,95–1,09. Chen i Nagasaka uważają, że im bliżej siebie są położone punkty tworzące badany kąt, tym większe będą różnice w jego wartości przy kolejnych próbach wyznaczania (10, 15). Innym wytłumaczeniem jest trudność przy odnajdywaniu wierzchołków zarówno górnych jak i dolnych siekaczy, nawet przy zdjęciach bardzo dobrej jakości oraz możliwości powiększenia obrazu na monitorze i zmiany kontrastu. Siekacze Discussion The paper assessed the standard measurement uncertainty for angles measured in a cephalometric analysis used commonly in the orthodontic practice and WITS measurement. The tests were performed with a conventional and computer method. The ANB angle and WITS measurement determine the position of the mandible in relation to the maxilla and are vital when planning orthodontic treatment. The interjaw-base angle (ML-NL, indicates a degree of inclination of the mandible in relation to the maxillary base), the interincisal angle (1+:1-, indicates mutual axial inclination of the upper and lower incisors) and the nasolabial angle (an angle measured on facial soft tissues, does not depend on the age or sex but on the shape of the nose and a position of the upper lip) play significant roles in the facial aesthetics (8). Values of these angles are taken into account when planning extraction treatment. The ANB angle was the most precisely determined of all analysed angles, both in a computer and conventional method. Similar results were obtained by other authors (9, 10). There were no statistically significant differences observed between various measurement methods, and it was also confirmed in the literature (10, 11, 12). Differences in values of the ANB angle were considered to be related to the determination of the A point by other authors. They associated this problem with difficulties diagnosing this region in photographs. According to Uysal the Nasion point was associated with problems during determination (12). Results as above might have been affected by the quality of photographs as they were taken on a film, then scanned. Photographs analysed in these studies were taken with a digital X-ray camera, then transferred onto a film with a laser printer. WITS was the only linear measurement analysed in the study. It is important to determine the occlusal plane using the same method in all cases. In her PhD dissertation (Kąt ANB i pomiar WITS: wiarygodność i wpływ na postępowanie ortodontyczne w przypadkach wad przednio-tylnych [ANB angle and WITS measurement: reliability and effects on orthodontic treatment in cases of anteroposterior defects]) Jankowska proved that it had a significant effect on the value of measurements (13). In both methods used to analyse the WITS measurement the values of the mean standard measurement uncertainty were similar. In the studies by Chen et al. there were significant statistical differences when both methods to measure WITS were compared (10). It might have been affected by a photo scanning technique. In our study the largest mean standard measurement uncertainty was observed for the interincisal angle and nasolabial angle. With regard to other measurements the mean standard measurement uncertainty was in the range 0.13–0.46 for both methods of analysis. Here, the value increased visibly to 0.95–1.09. According to Chen and Nagasaka when points forming a given angle are located close to each other, differences in the value of this angle will be greater during subsequent attempts of its determination (10, 15). Najdłuższy jest korzeń kła, który często przysłania i uniemożliwia odnalezienie wierzchołka siekacza dolnego (5). Podobnie duża średnia standardowa niepewność pomiaru wystąpiła przy kącie nosowo–wargowym. Sommer i współpracownicy również odnotowali duże rozbieżności przy wielokrotnych pomiarach powyższego kąta. Tłumaczono to gorszą jakością obrazu tkanek miękkich (14). W przeprowadzonym badaniu jakość obrazu badanych struktur była wysoka, a mimo to wystąpiły trudności w wyznaczeniu punktu subnasale (punkt najmniejszej krzywizny przy przejściu w wargę górną). Mogło to być spowodowane dużą zmiennością tej okolicy. Ponadto do wyznaczenia kąta były potrzebne trzy blisko siebie leżące punkty, w związku z czym nawet niewielki błąd w wyznaczeniu jednego z nich znacznie wpływał na zmianę wartości kąta. Z tego względu maksymalna standardowa niepewność pomiaru była czternaście razy większa od minimalnej. Kąt międzyszczękowy przyjmował zbliżone wartości średniej standardowej niepewności pomiaru w obu metodach analizy. Podobne wyniki uzyskali inni badacze (14). Średnia standardowa niepewność pomiaru dla wszystkich pomiarów była wyższa w analizie konwencjonalnej. Natomiast testy statystyczne nie wykazały istotnych różnic pomiędzy średnią standardową niepewnością pomiaru dla obu metod badania. Do podobnych wniosków doszli Uysal i Baysal (12). Zwrócili oni również uwagę na fakt, że na dokładność pomiarów ma wpływ doświadczenie lekarza. Chen i współpracownicy, wykorzystując mniej precyzyjny skaner, wykazali wyższość analizy ręcznej nad komputerową (16). Nowsze badania uznają metodę komputerową za bardziej precyzyjną (9, 10, 14, 17, 18). Wszyscy jednocześnie podkreślają, że stwierdzone różnice nie mają znaczenia klinicznego, chociaż Ghoneima zasugerował, że u pojedynczych pacjentów mogą one wpływać na planowanie leczenia (18). W publikacjach porównywano analizy komputerowe, w których automatycznie wyznaczano punkty, z konwencjonalną i półautomatyczną analizą. Analizy w pełni automatyczne były mało dokładne, co wykluczało ich stosowanie w codziennej praktyce (14, 19). Analiza CBCT (Cone beam computed tomography ) umożliwia najbardziej precyzyjne pomiary, jednak wykonywanie tego typu zdjęć wyłącznie w celu diagnostyki cefalometrycznej nie jest wskazane ze względu na dawkę promieniowania (17, 20, 21). Najlepszej jakości zdjęcie uzyskuje się przy użyciu aparatów cyfrowych, ponadto ta technika obrazowania pozwala na redukcję dawki promieniowania nawet o połowę (22). W powyższym badaniu wszystkie analizy zostały wykonane przez jedną osobę z długoletnim doświadczeniem. Zdjęcia zrobiono tym samym aparatem, więc ich techniczna jakość była zbliżona. Różnice w wartościach poszczególnych kątów i pomiaru WITS powstające w kolejnych próbach w obu metodach analizy wynikały głównie z trudności w precyzyjnym oznaczeniu punktu. Należy stwierdzić, że obie metody pomiarów mogą być stosowane zamiennie. Na korzyść komputerowej (przy photographs and even when it is possible to enlarge images on the screen and change contrast. The incisors and canines in the mandible are usually located in one line, therefore their roots overlap. The canine root is the longest and it often covers the apex of the lower incisor and makes it impossible to find (5). A similarly large mean standard measurement uncertainty was observed for the nasolabial angle. Sommer et al. also observed great discrepancies when they measured this angle many times. In their opinion it was caused by a worse image quality in case of soft tissues (14). In the study performed the image quality was high for tested structures, and despite this there were problems determining the subnasale point (the point of the smallest concavity along the distance to the upper lip). It might have been caused by high variability of this area. Additionally, three adjacent points were required to determine this angle, therefore even a small mistake associated with determination of one of them significantly affected changes in this angle. Consequently, the maximum standard measurement uncertainty was fourteen times larger than the minimum. The interjaw-base angle had similar values of the mean standard measurement uncertainty in both methods of analysis. Other authors obtained similar results (14). The mean standard measurement uncertainty for all measurements was higher in a conventional analysis. However, statistical tests did not demonstrate any significant differences between the mean standard measurement uncertainty for both test methods. Uysal and Baysal had similar conclusions (12). They also draw attention to the fact that physician’s experience affects precision of measurements. Chen et al. used a less precise scanner and demonstrated that a manual analysis was superior to a computer one (16). However, the latest studies consider a computer method to be more precise (9, 10, 14, 17, 18). All simultaneously emphasise that observed differences are of no clinical significance; however, Ghoneima suggested that they may affect planned treatment in individual cases (18). There are publications comparing computer analyses where points were determined automatically with conventional and semi-automatic analyses. Fully-automatic analyses were hardly precise, therefore they cannot be used in everyday practice (14, 19). The CBCT (Cone beam computed tomography ) analysis allows for the most precise measurements; however, it is not recommended to perform such imaging tests only for the purposes of a cephalometric diagnosis due to a radiation dose (17, 20, 21). Images of the best quality are obtained with digital cameras and additionally, this imaging method makes it possible to reduce a radiation dose even by half (22). In this study all analyses were performed by one person with long-term experience. Images were taken with the same camera, therefore their technical quality was consistent. Differences in values of individual angles and the WITS measurement observed during subsequent attempts in both methods of analysis were mainly associated with problems to determine points in a precise way. It should be stated that both methods of measurements can be used interchangeably. Zaletą jest łatwiejszy sposób archiwizacji oraz możliwość przesyłania danych w celach konsultacyjnych. Różnice w pomiarach poszczególnych kątów i wartości WITS uzyskane w obu metodach nie były istotne statystycznie i nie wpłynęły na plan leczenia pacjenta. Wynika to z dużej rozpiętości normy dla poszczególnych pomiarów w stosunku do wielkości wyliczonej średniej standardowej niepewności pomiaru. Benefits also include easier archivisation and a possibility to transfer data for consultations. Differences in measurements of individual angles and WITS values obtained in both methods were not statistically significant and did not affect a patient’s treatment plan. It is a result of the fact that the reference range for individual measurements in relation to the value of calculated standard measurement uncertainty is wide.
