Materiały i metody: Dokonano przeglądu piśmiennictwa dotyczącego górnych i dolnych trzecich zębów trzonowych w czasopismach z dziedziny ortodoncji i chirurgii stomatologicznej dostępnych w bazach internetowych PubMed oraz Medline. Na podstawie publikacji analizowano zagadnienia dotyczące ich rozwoju, wyrzynania, etiologii zatrzymania w kości, wskazań do ekstrakcji, a także wpływu leczenia ortodontycznego na późniejsze ich losy. Oceniono metody pomiarowe wykonywane na rentgenogramach służące do Aim: To compare views found in a literature concerning wisdom teeth, either in a maxilla or in a mandible, and to determine uniform protocol of proceeding at children and adults during a planning orthodontic treatment. Materials and methods: There has been performed the review of a literature concerning upper and lower third molars published in orthodontic and dental surgery journals, available in internet bases of PubMed and Medline. Development of the wisdom teeth, their eruption, etiology of impaction in the bone, indications to extraction and also an influence of orthodontic treatment on their later fate were considered. Wyniki: 39 artykułów odpowiadało kryteriom tematycznym tej pracy przeglądowej. Zwrócono szczególną uwagę na zagadnienia, które mogą być pomocne podczas planowania leczenia pacjenta z wadą zgryzu. Wnioski: Trzecie zęby trzonowe charakteryzują się dużą zmiennością w trakcie rozwoju. Część z nich pozostaje zatrzymana w kości i nie wykazuje żadnych objawów przez całe życie, ale stanowią one potencjalne źródło problemów. Metody określania dostępności przestrzeni zatrzonowcowej w żuchwie oraz tylnej okolicy guzowatości szczęki są niepewne, dlatego istnieje duża grupa przeciwników profilaktycznej germektomii w wieku 8-9 lat. Długotrwale stosowana retencja do czasu zakończenia wzrostu oraz monitorowanie rozwoju trzecich zębów trzonowych w oparciu o rentgenogram wykonywany w odstępach 2-3 lat jest rutynowym postępowaniem w przypadku każdego pacjenta leczonego ortodontycznie. Special attention has been paid to problems which might be helpful during planning the treatment of a patient with a malocclusion. Conclusions: Third molars characterise of a great variability while developing. A part of them remains impacted in bone and does not indicate any symptoms the whole life but, they constitute a potential source of problems. The methods to determine the accessibility of retromolar space in a mandible and posterior outline of the maxillary tuberosity are questionable. There is a great opposing group of prophylactic germectomy at the age 8 to 9. Long lasting retention till the end of growth and monitoring of the development of third molars with the radiographs performed at the two to three years intervals is a routine proceeding in a case of every orthodontic patient. Received: 30.03.2012 Accepted: 27.06.2012 Wstęp Introduction Po raz pierwszy trzecie zęby trzonowe mogą zostać rozpoznane na radiogramie już w wieku 5 lat lub w późniejszym okresie, w 16 rż. (1-5). Jeżeli w wieku 10 lat nie można stwierdzić ich obecności, prawdopodobieństwo rozwoju ocenia się na mniej niż 50%. Na jednym zdjęciu możemy obserwować zawiązki w różnych etapach rozwojowych. Średni wiek wyrzynania przypada na 20.5 lat, natomiast w wieku 24 lat 95% trzecich trzonowców jest gotowych do wyrznięcia. Niezależnie od populacji są ostatnimi zębami, które pojawiają się w całkowicie rozwiniętych łukach zębowych (1-3). Duża rozwojowa autonomia trzecich trzonowców w porównaniu z innymi zębami powoduje, że zachowują one zdolności do ciągłych zmian przez długi okres czasu aż do późnej dorosłości (5). Określenie obecności trzecich zębów trzonowych, ich pozycji oraz ewentualnego zatrzymania w kości stanowi dopełnienie diagnozy i całościowego planu terapeutycznego pacjenta ortodontycznego. Mimo zakończonego aktywnego leczenia, każdy pacjent powinien pozostawać pod stałą kontrolą dopóki te zęby nie zostaną usunięte lub nie wyrzną się. Zatrzymane zęby mądrości odnotowano prawie u 50% pacjentów leczonych z powodu wady zgryzu (1). Przyczyny zatrzymania, przewidywanie ich wyrzynania oraz wpływ na stłoczenia siekaczy były przedmiotem wielu badań (1-14). Jednak nadal wiele kontrowersji budzi zagadnienie For the first time molars can be indicated on radiographs as early as at the age of 5 and as late as 16 (1-5). If at the age of 10 their presence cannot be established, the probability of their development is assessed at less than 50%. At one of the X-rays, we can see buds at different stages of development. The average age of eruption is estimated as the age 20.5, but at the age of 24, 95% of third molars is ready to erupt. Irrespective of the population they are the last teeth which appear in a totally developed dental arches (13). The large developmental autonomy of third molars, compared with other teeth, causes that they retain the ability to constantly change over a long period until late adulthood (5). Determining the presents of third molars, their position and possible impaction in the bone, completes the diagnosis and an overall therapeutic plan of an orthodontic patient. Despite the completion of the active treatment, every patient should be under permanent medical control until those teeth are extracted or erupted. Impacted wisdom teeth have been reported among almost 50% patients treated because of malocclusion (1). The causes of impaction, predicting of their eruption and their influence on incisor crowding have been the subject of a lot of research (1-14). Celem pracy jest porównanie istniejących skrajnych poglądów spotykanych w piśmiennictwie dotyczących postępowania z zębami mądrości zarówno w szczęce jak i w żuchwie oraz usystematyzowanie informacji na temat metod prognozowania erupcji, wskazań ortodontycznych do ekstrakcji tych zębów oraz negatywnych skutków związanych z ich zatrzymaniem. Określono jednolity protokół postępowania u dzieci i dorosłych w aspekcie planowanego leczenia ortodontycznego. extraction of unerupted third molars in the adolescence is still controversial. The goal of this paper is to compare the existing points of view found in the literature, discussing wisdom teeth in a maxilla as well as mandible. We also analyse the information of methods to predict their eruption, determining orthodontic indicators to extract those teeth and negative implications of impaction. There have been determined homogeneous protocols for children and adults when it comes to planning orthodontic treatment. Materiały i metody Materials and methods W realizacji celów pracy skorzystano z artykułów opublikowanych w latach 1970-2012 głównie w anglojęzycznych czasopismach specjalistycznych z dziedziny ortodoncji, chirurgii stomatologicznej oraz szczękowotwarzowej udostępnianych w bazach internetowych PubMed i Medline. Słowami kluczowymi były: third molars, prediction of eruption, prophylactic extraction, orthodontic treatment. We analyzed thirty articles published between 19702012 mostly in English professional journals connected with the field of orthodontic, dental and maxillofacial surgery accessible on internet databases of PubMed and Medline. The key words were: third molars, prediction of eruption, prophylactic extraction, orthodontic treatment. Wyniki Results Rozwój i etiologia zatrzymania trzecich zębów trzonowych Początkowo miejsca rozwoju dolnych trzecich zębów trzonowych zaznaczone są jako mało widoczne ogniska przejaśnienia, które pojawiają się na powierzchni kości, a w miarę wapnienia zanurzają się wewnątrz gałęzi żuchwy (4,15). We wczesnym etapie rozwoju dolne trzonowce wykazują mezjalną inklinację, ale progresywnie podczas wzrostu żuchwy na długość przesunięcie ich korzeni ku przodowi umożliwia zębom pionizację do 25 rż. (1,3,4,6,17,18). Mezjalne nachylenie jest zatem pozycją rozwojową i nie powinno być traktowane jako zatrzymanie do 15 rż. Richardson określiła jego średnią wartość na około 40° (mierzone w stosunku do płaszczyzny żuchwy). Szanse na ich wyrznięcie są ograniczone, gdy mezjalna inklinacja przewyższa 30°. Mimo zakończonego rozwoju korzeniowego zachowują potencjał erupcyjny z możliwością dalszej zmiany pozycji w wymiarze strzałkowym (5,7,17-19). Badając pozycję dolnych trzecich trzonowców w płaszczyźnie poprzecznej stwierdzono, że powierzchnia okluzyjna skierowana jest dojęzykowo z wierzchołkami korzeni zwróconymi dopoliczkowo (6,17). Bardziej policzkowe ułożenie tych zębów przypisuje się uwarunkowaniom anatomicznym takim jak: wewnętrzna linia skośna, szew skrzydłowo-żuchwowy czy przyczep mięśnia żwacza (1). W przypadku górnych zębów trzonowych mówimy Development and the etiology of impaction of third molars At the beginning, the places of the development of lower third molars are marked as radiolucency at the surface of the bone and during calcification they immerse inside of the mandible ramus (4,15). At the early stage of development lower third molars have mesial inclination but progressively, during mandible growth in length, they become more upright up to the age of 25 (1,3,4,6,17,18). A mesial inclination is a developmental position and it should not be treated as in impaction up to the age of 15. Richardson estimated its average value at around 40° (measured according to mandibular plane). The chances for their eruption are limited when a mesial inclination excels 30°. Despite the completed crown formation, they retain an eruptional potential with a possibility of further position change in a sagittal dimension (5,7,1719). Studying the position of lower third molars in a transversal plane, it has been found, that occlusal surface is pointing lingually with root apices pointing buccally (6,17). More buccal position of those teeth is connected with anatomical conditions such as: an internal oblique ridge, a pterygomandibular raphe or an attachment of the buccinators muscle (1). In the case of upper molars, we talk about an initial distal angulation. Informacje na temat jej zmiany są ograniczone, jednak można powiedzieć, że istnieje podobne zróżnicowanie jak w przypadku ich dolnych antagonistów (18,20). Występowanie mezjalnego nachylenia oraz większego niż 30° dystalnego, może świadczyć o późniejszym zatrzymaniu górnych zębów (8). Badając mineralizację górnych i dolnych zębów mądrości określono zależność między ich rozwojem a wiekiem szkieletowym w oparciu o radiogram nadgarstka i dłoni. W fazie PP2 większość badanych osób prezentowała całkowitą mineralizację korony i początek mineralizacji korzenia, DP3u to w pełni wykształcony korzeń, a z kolei w fazie Ru 1/3 pacjentów całkowicie zakończyła rozwój zęba, 1/3 miała w połowie wykształcony korzeń, pozostali wykazywali koniec formowania korony. Można zatem wysunąć wniosek, że mimo zakończonego wzrostu żuchwy część trzecich trzonowców nadal się rozwija (21,22). Gdy zawiodą czynniki dostosowawcze podczas pionizowania trzonowców, dochodzi do zatrzymania ich w kości bądź erupcji ektopowej (9,20). Odnotowuje się, że stanowią one od 55% do 98% wszystkich zatrzymanych zębów, co głównie wiąże się z ich późnym rozwojem w porównaniu z resztą uzębienia (16). Elsey podaje, że co najmniej 73% dorosłych ma jedną zatrzymaną ósemkę (15). Dotyczy to częściej zębów w żuchwie niż w szczęce, jednakże niezadowalająca pionizacja podczas końcowej fazy rozwoju korzenia jest ich wspólną cechą (3,20,23,24). Wśród górnych zębów mądrości 25% wykazuje zatrzymanie w inklinacji dystalnej, podczas gdy 43% dolnych w mezjalnej. Zwiększenie tego nachylenia zdarza się częściej w żuchwie niż w szczęce. W przypadku nadmiernej pionizacji („przekorygowanie”) mówimy o mezjalnym nachyleniu w szczęce - 12% i dystalnym w żuchwie - 6% (3,18). Etiologia zatrzymania trzecich zębów trzonowych nie jest do końca wyjaśniona (4,7,13,24). Bjork wykazał zależność między zatrzymaniem dolnych zębów mądrości, a brakiem przestrzeni w łuku zębowym między drugim zębem trzonowym a wznoszącą się ku górze gałęzią żuchwy. Artun i wsp. podaje, że z każdym powiększeniem przestrzeni zatrzonowcowej prawdopodobieństwo zatrzymania zęba w kości maleje o 29% (5). Z niewystarczającą przestrzenią współdziałają 3 szkieletowe czynniki takie jak: • pionowy kierunek wzrostu kłykcia określany przez kąt żuchwy, co skutkuje zmniejszoną resorpcją przedniej krawędzi gałęzi żuchwy oraz doprzednią rotacją żuchwy - zwiększony kąt beta, niskie wartości kąta żuchwy (10,11,13,20,24), • zmniejszona długość żuchwy mierzona od głowy kłykcia do punktu bródki (25), • dystalny wzorzec wyrzynania zębów określany poprzez stopień wyrostkowej inklinacji (2,5,20,25). Połączenie ze sobą tych głównych czynników wyjaśniło 80% przypadków z zatrzymanymi dolnymi zębami of lower antagonists (18,20). The occurrence of mesial tilt and more of than 30% of the distal one, can indicate later impaction of upper teeth (8). Studying mineralization of upper and lower wisdom teeth, the dependence between their development and skeletal age based on a radiograph of a wrist and a hand, has been estimated. In the phase PP2, the most of the examined patients, presented with the total mineralization of crown and the beginning of mineralization of a root. The phase DP3u is a fully formed root. In the phase Ru a third of patients had totally finished tooth development, a third had a half formed root, and the rest of patients has indicated the end of crown formation. In conclusion, despite the end of mandible growth, a part of third molars is still evolving (21,22). The failure of the adjustment factors during the uprighting of the molars may cause their impaction in a bone or an ectopic eruption (9,20). It was reported, that they constitute from 55-98% of all impacted teeth, and this is mainly connected with their later development in comparison with the rest of teeth (16). Elsey adds, that at least 73% of adults have an impacted third molar (15). It occurs more often in the mandible than in the maxilla, but an unsatisfactory uprighting during the final phase of a root formation is their common feature (3,20,23,24). Among upper wisdom teeth 25% have an impaction in a distal inclination, while 43% of lower ones are impacted in a mesial inclination. Increase of this inclination appears more often in the mandible than in the maxilla. In the case of over uprighting (overcorrection) we can talk about a mesial tipping in the maxilla - 12% and a distal one in a mandible -6% (3,18). An etiology of third molars impaction is not completely explained (4,7,13,24). Bjork demonstrated a relationship between an impaction of lower wisdom teeth and a lack of space in a dental arch between the second molar and an ascending mandible ramus. Artun et al reported that with each expansion of retromolar space, the probability of impaction of teeth in the bone decreases by 29% (5). There are three skeletal factors cooperating with insufficient space, such as: • a vertical direction of condylar growth is specified by the mandibular angle, what results in a little resorption of an anterior border of the mandibular ramus and the forward mandibular growth rotation - increased beta angle, low value of the mandibular angle (10,11,13,20,24); • a reduced mandibular in length measured as the distance from the condylar head to the chin point (25); • a distal pattern of eruption of the mandibular dentition, determined by the degree of alveolar inclination (2,5,20,25). Putting together all those main factors explained 80% of the cases with impacted lower wisdom teeth. W przypadku górnych trzecich trzonowców brak kompensacyjnej apozycji w tylnym zarysie guzowatości szczęki może doprowadzić do zaburzonego wyrzynania tych zębów (8). Wśród istotnych czynników wymieniano także opóźniony rozwój szkieletowy twarzoczaszki oraz zwiększoną inklinację zęba (25). Istnieje harmonijna relacja między wzrostem żuchwy a rozwojem zębów. Żuchwa rośnie niezależnie od rozwoju zębów. Odwrotnie niekoniecznie musi być to prawda. W badaniu porównującym pacjentów z zatrzymanymi (grupa I) i wyrzniętymi (grupa II) zębami ósmymi odnotowano w grupie I wyższy odsetek osób z II klasą szkieletową (13). W opozycji do przedstawionego badania pozostaje odkrycie Alhaja, który odnotował wyższy odsetek zatrzymanych dolnych trzecich trzonowców u pacjentów z III klasą szkieletową. Zwiększonym wymiarom żuchwy towarzyszyła jednak zmniejszona odległość między drugim trzonowcem a gałęzią żuchwy oraz niekorzystna angulacja zatrzymanych zębów (24).Wyrzynanie zatem, co najmniej w części będzie zależeć od prawidłowego wzrostu kości. Małe tempo wzrostu żuchwy, wczesne dojrzewanie płciowe, późna mineralizacja, wielkość zęba, konfiguracja korzeni, czynniki genetyczne, nawyki żywieniowe oraz odmienność rasowa mogą również odgrywać rolę w etiologii zaburzonego wyrzynania (10,12,13,26). Przedmiotem wielu badań była ocena wpływu ekstrakcji w łuku zębowym na możliwości wyrzniecia zębów mądrości. Wśród wielu doniesień dominuje teoria, że usunięcie przedtrzonowców zwiększa dostępność przestrzeni zatrzonowcowej oraz okolicy guzowatości szczęki poprzez mezjalny dryf segmentów policzkowych. W trakcie zamykania luk poekstrakcyjnych krypty ósemek migrują do dołu i przodu, a w przypadku zębów z zakończonym rozwojem dochodzi do korzystnej zmiany w angulacji z następczą ich pionizacją. Niepodważalne są wyniki badań, wykazujące większą częstotliwość zatrzymania zębów mądrości u pacjentów leczonych metodą nieekstrakcyjną, u których doszło do dystalizacji oraz przechylenia pierwszego i drugiego zęba trzonowego (20,26-29). Wysoki odsetek pacjentów ekstrakcyjnych z obecnymi trzecimi trzonowcami (94%) i fakt, że stosując jednostronne usunięcie doszło do pojawienia się go po stronie łuku zębowego z utraconym zębem, zdaje się jeszcze potwierdzać te doniesienia (2,16). Lekarz planując jednak ekstrakcje musi pamiętać, że przy dużej dyskrepancji łuku zębowego, luka niemalże całkowicie zamykana jest w celu leczenia dysproporcji zębowo-wyrostkowej pozostawiając mało miejsca lub w ogóle dla ruchu doprzedniego trzonowców, a indywidualna zmienność ruchu tylnych zębów wynika z różnego stopnia retrakcji kłów powiązanego z prostowaniem siekaczy i zastosowaną mechaniką. Dlatego poziom tego przemieszczenia dobrze jest określić przed leczeniem (12,29). Staggers i in. uważała, że leczenie ortodontyczne nie of upper third molars the lack of compensatory periosteal apposition at the posterior outline of the maxillary tuberosity can lead to disorder of the eruption of those teeth (8). Among important factors of impactions a retarded maturation of a facial skeleton and increased tooth inclination were mentioned (25). There is a harmonious relation between growth of the mandible in length and a development of teeth. The mandible grows regardless of maturing teeth. The opposite does not have to be truth. In the study, comparing patients with impacted (group 1) and erupted (group 2) third molars it was noticed, that in the group 1 there was a higher percentage of people with the second skeletal class (13). In opposition to the present study is the finding of Alhaja, who marked a higher percentage of impacted lower third molars in patients with third skeletal class. However increased dimensions of the mandible accompanied reduced the distance between the second molar and the mandibular ramus and unfavorable angulation of impacted teeth (24). That is why an eruption, at least partially will depend on a correct growth of a bone. A low rate of mandible growth, early puberty, late mineralization, a size of a tooth, a root configuration, genetic factors, eating habits and race diversity can also play a role in etiology of a failed eruption (10,12,13,26). The influence of extraction in a dental arch on an eruption of wisdom teeth, was a subject of many studies. There is a theory, that an extraction of premolars increases the availability of a retromolar space and makes the area of maxillary tuberosity bigger by mesial drift of the buccal segments. During the process of orthodontic space closure, the third molar’scrypts migrate downward and forward and in the case of the teeth with completed development, there are beneficial changes in angulation with the following uprighting. There are undeniable reports, that demonstrate a higher frequency of wisdom teeth impaction in patients treated with non-extractional method, who experienced a distalization and tilt of the first and second molar (20,26-29). These reports seem to be confirmed by a high percentage of extractional patients with the presence of third molars (94%) and the fact that using a unilateral extraction third molar appears on the side of the tooth arch with the lost tooth (2,16). A doctor, planning an extraction must remember, that with a large discrepancy of the tooth arch, the space is almost totally closed in order to treat dento alveolar disproportion, leaving too little or no space for the forward movement of molars, and an individual variation of posterior teeth movement depends on a various extent of canine retraction related to uprighting of incisors and applied mechanism. It is beneficial to determine the level of that movement before the treatment (12,29). Natomiast powiększenie dostępności przestrzeni w łuku górnym i dolnym nie było wystarczające by mogło dojść do ich wyrznięcia. Dlatego ekstrakcja dolnych pierwszych zębów przedtrzonowych u pacjentów „granicznych” z niewystarczającą przestrzenią zatrzonowcową, doprzednią rotacją wzrostową żuchwy i mezjalną inklinacją zębów ósmych, nie jest zalecana (26,30). Wśród niektórych autorów panuje pogląd, że jedynie ekstrakcja zębów trzonowych może mieć pozytywny wpływ na dalsze losy zębów mądrości. Na skutek ekstrakcji drugiego trzonowca, w czasie gdy korzenie trzecich trzonowców nie zakończyły swojego rozwoju doszło nawet do 100% wyrznięcia się tych zębów. Jednak sprzyjająca zmiana w nachyleniu osi, niekoniecznie może oznaczać ich wyrznięcie w dobrej inklinacji, z właściwym kontaktem z pierwszym trzonowcem i zadowalającą okluzją (16,32,33). Ze względu na istotną rolę jaką odgrywają zęby trzonowe w całym układzie stomatognatycznym, ortodonci skłaniają się raczej do ekstrakcji przedtrzonowców. Wyniki tych badań jedynie mogą pomóc podjąć decyzję w przypadku złej prognozy leczenia próchnicy w pierwszych zębach trzonowych. Metoda szybkiego poszerzenia szczęki (RME), podobnie jak ekstrakcje, jest często stosowanym postępowaniem w terapii pacjentów ortodontycznych. Sokoru podaje, iż zwiększeniu szerokości międzykłowej i międzytrzonowcowej towarzyszy natychmiastowa pionizacja górnych trzecich trzonowców, a w późniejszym okresie retencji ich wyrznięcie. Zmiany w położeniu trzecich górnych zębów trzonowych na skutek ekspansji w wymiarze poprzecznym wymagają jednak dalszego badania (31). Metody prognozujące wyrzynanie Istniejąca metoda pomiarowa w oparciu o rentgenogramy czy modele diagnostyczne wykonywane rutynowo każdemu pacjentowi ortodontycznemu, pomogłaby z pewnością ustrzec się nie tylko powikłań związanych z wyrzynaniem tych zębów, ale też prawidłowo zaplanować leczenie pacjenta z wadą zgryzu, szczególnie gdy rozważamy terapię ekstrakcyjną. W tym celu wielu badaczy oceniało najczęściej dwie zmienne: przewidującą przyszłą dostępność przestrzeni oraz zmianę angulacji trzecich zębów trzonowych. Ricketts opierał swoje badania na analizie rentgenogramu cefalometrycznego bocznego u 8- oraz 9-latków mierząc odległość od punktu Xi (punkt konstrukcyjny wyznaczony na gałęzi żuchwy) do dystalnej powierzchni drugiego trzonowca wzdłuż płaszczyzny okluzyjnej (Rycina 1). Jeśli odległość ta wynosiła 30 mm lub więcej, wskazywało to na dostateczną przestrzeń. Przy wyniku 20 mm albo mniej, przestrzeń określana była jako niewystarczająca (9,10,12). Olive i Basford obliczali współczynnik przestrzeń/ szerokość (później zmodyfikowany przez Ganss i wsp., znany jako stosunek Ganss), który jest odległością od not able to improve angulation of those teeth in spite of using the extracting therapy. Whereas extending the space availability in upper and lower arch was not satisfactory to let them erupt. That is why the extraction of premolars in ‘borderline’ patients with insufficient retromolar space, forward mandibular growth rotation and mesially angulated mandibular third molars should be avoided (26,30). Some authors have an opinion that only extraction of molars can have a positive influence on further faith of wisdom teeth. As a result of the extraction of the second molar, when the roots of third molars have not completed their growth yet, there have been even up to 100% of eruptions of those teeth. However, the favourable change in an inclination of the axis does not have to mean they will erupt in the right inclination, with the right contact with the first molar and with satisfying occlusion (16,32,33). Because of a crucial role of molars in the whole stomatognathic system, orthodontists tend to extract premolars. The results of that research, can help make decisions, in case of poor prognosis in treatment of the caries of first molars. The method of rapid maxillary expansion (RME), as well as extractions, is a procedure often used in the treatment of orthodontic patients. Sokoru reports, that the increase in intercanine and intermolars width are accompanied by immediate uprighting of upper third molars, and their eruption in later retention period. Changes in the position of the upper third molars as a result of transversal expansion, require further investigation (31). The methods to predict the eruption An existing measurement, method based on radiographs and study casts, performed as a routine for every orthodontic patient, could help to prevent not only from complications related to eruption of those teeth, but also could help to plan the treatment of a patient with malocclusion, especially when we consider an extraction therapy. In this case, there are two variables that need to be determined: predicting the future availability of space for third molars and predicting the changes in the angulation of third molars. Ricketts based his research on an analysis of lateral cephalometric projection of 8 and 9- year-olds measuring the distance from point Xi (constructional point appointed on the ramus of the mandible) to the distal surface of the second molar along the occlusial surface (Figure 1). If its length was 30mm or more, it indicated enough space. When the result was 20mm or less, it meant that there was not enough space (9,10,12). Wartość poniżej 120% wskazywała na wysokie prawdopodobieństwo zatrzymania w przyszłości (Rycina 2). Ganss prowadząc pomiary na rtg cefalometrycznym oraz pantomogramie określił optymalne wartości przestrzeni zarówno dla dolnych jak i górnych zębów. Współczynnik Ganss ≥1 oraz odległość miedzy punktem Xi a dolnym drugim trzonowcem ≥25 mm oraz płaszczyzną PTV a górnym pierwszym trzonowcem ≥25 mm zwiększały szansę na wyrznięcie tych zębów (5,8-10). Optymizmu powyższych autorów nie podzielała Richardson, stwierdzając że nie jest możliwe przewidzenie tego procesu u tak młodych pacjentów. W wieku 8-9 lat bardziej prawdopodobne wydają się być pomiary dla szczęki, w której procesy wzrostowe już zakończyły się, podczas gdy dla żuchwy trwają nadal do 18 rż. Dopiero po 20 rż., powiększanie przestrzeni zatrzonowcowej jest wątpliwe, a mimo zmian pozycji zębów ósmych, nie ma pewności ich wyrznięcia (1,5,10,13,21,34). Warto podkreślić także znaczenie policzkowo-językowej szerokości wyrostka zębodołowego. Bliskie położenie drugich zębów trzonowych w stosunku do gałęzi żuchwy widoczne np. na bocznym zdjęciu cefalometrycznym nieraz jest mylne, gdyż w rzeczywistości może oddzielać je szeroka strefa widoczna na szczycie wyrostka zębodołowego (6). W ostatnim czasie Begtrup i wsp. badając zależność między zdjęciem cefalometrycznym a panoramicznym stworzyli prostą i niezawodną metodę przewidywania wyrznięcia trzeciego zęba trzonowego w żuchwie za pomocą pantomogramu. Pomiary zostały przeprowadzone u osób dorosłych (średnia wieku to 22 lata). Poprzez porównanie wielkości kąta gonion, odległości olr-id (odległość od przedniej granicy gałęzi żuchwy do punktu infradentale) z mezjodystalną szerokością dolnego drugiego zęba trzonowego (w oparciu o badania Lee) graficznie zilustrowano prawdopodobieństwo wyrznięcia trzeciego trzonowca - Rycina 3 (11). Ekstrakcja czy obserwacja? Wśród klinicystów możemy wyodrębnić dwie grupy prezentujące odmienne poglądy na temat profilaktycznej ekstrakcji trzecich zębów trzonowych. Zwolennicy profilaktycznych ekstrakcji podkreślają zalety wczesnej germektomii, wśród których wyliczają zmniejszenie ryzyka śród- i pooperacyjnego związanego z zabiegiem chirurgicznym (3,9,25). Zatrzymane zęby traktują jako stan nieprawidłowy, który choć bezobjawowy, może z czasem stać się źródłem potencjalnych komplikacji: nawrotowe zapalenia okołokoronowe, choroby przyzębia, próchnica, resorpcja drugich zębów trzonowych, torbiele oraz łagodne i złośliwe nowotwory (3,9,10). Pozostawienie ich w żuchwie lub usuwanie w późniejszym terminie może również skutkować zniszczeniem przyczepu przyzębia the third molar’s crown. The result below 120% indicated a high probability of impaction in the future (Figure 2). Conducting measurements in a cephalometric projection and panoramic radiograph, Ganss estimated optimal values of space for lower as well as upper teeth. A Ganss ratio ≥1 and the distance between the point Xi and the lower second molar ≥25mm and the PTV plane and the upper first molar ≥25mm made a probability of eruption of those teeth higher (5,8-10). Richardson was not as optimistic as the authors above were. She said that it was not possible to predict such a process at such young patients. At the age of 8-9, it is more likely to make the measurements of a maxilla, where the growing processes have been completed, whereas such processes in a mandible continue until the age of 18. Increasing of retromolarspace after the age of 20 is doubtful; nevertheless the third molars might not erupt, despite the change in their position (1,5,10,13,21,34). It is worth to highlight the meaning of bucco-lingual width of the alveolar process. A close position of the second molars in comparison to the mandible ramus seen e.g. on a lateral cephalometric, can be incorrect sometimes, because in fact, there can be a wide area seen on the top of the alveolar process (6). Recently Begtrup et al examined the relationship between a cephalometric and panoramic radiographs created a simple and reliable method of predicting the third molar eruption in the mandible by phantomography. Measurements were carried out in adults (average age 22 years). By comparing the value of the gonion angle, olrid distance (the distance from the anterior border of the mandibular ramus to the infradentale point), mesiodistal width of the lower second molar (based on Lee studies) graphically illustrates the probability of eruption of the third molar - Figure 3 (11). Extraction or observation? We can separate two groups among clinicians, presenting opposing views on a prophylactics extractions of third molars. The propagators of prophylactic extractions highlight advantages of early germectomy. According to them, there are less surgical and postoperative complications (3,9,25). They treat impacted teeth as an abnormal state, which although asymptomatic can become a source of potential complications in time: a recurrent pericoronitis, periodontitis, caries, resorption of the second molars, cysts and cancers (3,9,10). Leaving some mandibular third molars in, or taking them out too late, can result in damage of second molar’s periodontal attachments (13). Bruce et al. showed that patients younger than 25 years of age had a significantly shorter operation time, less postoperative complications and fewer postoperative visits. Bruce i wsp. wykazali, że u pacjenta młodszego niż 25 lat czas zabiegu jest znacząco krótszy, występuje mniej powikłań i pozabiegowych kontroli, a zaletami w tym czasie są: niekompletnie rozwinięte korzenie, szeroka przestrzeń około koronowa, oddalony nerw żuchwowy i zatoka szczękowa (10,3537). Jednak nadal problemem pozostaje określenie, które z zębów będą faktycznie narażone na zatrzymanie, a które wyrzną się jako funkcjonalna jednostka całego uzębienia. Przeciwnicy powtarzają, że profilaktyczne ekstrakcje nie są usprawiedliwione obawą przed wymienionymi powikłaniami ponieważ prawdopodobieństwo ich rozwoju jest małe, a może zdarzyć się w 1%-12%. Największym problemem związanym z zaburzeniem wyrzynania są stany zapalne (9,38). Dotyczą one 40% przypadków i to właśnie one pozostają nadal głównym powodem usuwania tych zębów. Wśród wskazań do ekstrakcji trzecich zębów trzonowych warto wymienić te powiązane ściśle z leczeniem ortodontycznym tj.: planowany zabieg ortognatyczny, leczenie wymagające dystalizacji pierwszego i drugiego zęba trzonowego, zaburzenie wyrzynania drugiego zęba trzonowego, oraz budzące wiele kontrowersji późne stłoczenie siekaczy (3,14,23,25,39). Oto sugerowane protokoły postępowania u pacjentów z obecnymi zębami mądrości zgłaszających się w celu leczenia wady zgryzu (23,37): • Wiek 9-16 lat Podczas rutynowego badania ortodontycznego na podstawie rentgenogramu panoramicznego określa się obecność lub brak trzecich trzonowców. Rozwój krypt oraz uwapnienie korony będzie dostrzegalne radiograficznie w większości przypadków, a u niektórych uwidaczniać się już może początek kształtowania korzeni. Gdy obecne są patologiczne objawy związane z ich obecnością zalecane jest usunięcie. Jeżeli planowane są procedury ortognatyczne lub ortodontyczna dystalizacja pierwszego lub drugiego trzonowca, należy zalecić ekstrakcje w określonym czasie. W pozostałych przypadkach odnotowujemy pozycję, rozwój korony oraz korzenia w celu oszacowania ich dojrzewania przy porównaniu z następnym rentgenogramem. Takie postępowanie możemy wytłumaczyć tym, że usuwanie asymptomatycznych zębów nie jest usprawiedliwione celami ortodontycznymi, a zawiązek który początkowo wykazuje niekorzystne usytuowanie, może poprawić swoją pozycję poprzez mechanizmy wyrzynania. Pacjenci i rodzice powinni zostać poinformowani o stadium rozwoju zębów oraz o konieczności oceny radiologicznej ich dojrzewania w przyszłości. • Wiek 17-24 lat Podczas rutynowego badania radiologicznego oceniamy pozycję zęba, uformowanie korony oraz korzenia. pericoronal space, great distance to the alveolar nerve or maxillary sinus (10,35-37). But still it is a problem to estimate which teeth might be exposed to an impaction, and which of them may erupt as a functional unit of entire dentition. The opponents state, that prophylactic extractions are not justified by the possibility of complications because probability of their development is low, and it ranges from 1-12%. The biggest problem related to eruption disorders could be inflammations (9,38). They account for 40% of the cases and they are the main reason of extractions of those teeth. Among indications to extract third molars are those closely related to orthodontic treatment i.e. planned orthognathic surgery, treatment requiringfirst and second molars distalization, disorders of eruption of the second molar and controversial late crowding of incisors (3,14,23,25,39). These are suggested protocols for patients with wisdom teeth who present to treat malocclusion (23,37): • Age 9 to 16 During a routine orthodontic record-taking on the basis of panoramic radiographs, one should describe thepresence or absence of third molars. The crypt development and crown mineralization will be noticed radiographically in most cases, and at some cases the beginning of root formation is observed. If a pathologic condition is detected, a removal is recommended. If there are adjunctive treatments, such as orthognathic procedures or orthodontic distalization of first or second molar, removal according to timing requirements is recommended. Otherwise, the position of the third morals and stage of crown and root development should be recoreded, in order to assess dental maturation at the time of future radiographs. Such protocol can be explained by the fact that removal of asymptomatic teeth is not justified for orthodontic purposes and tooth bud that appearsinitially poorly positioned may correct its position through normal eruption mechanisms. Patients and parents should be informed of the status of development of third molars and the necessity for future radiographic assessments. • Age 17 to 24 During a routine X-ray, one should evaluate tooth position, crown and root formation. If a pathologic condition is detected, you should recommend removal. If the root is one-half or two-thirds complete, the retromolar space should be determinated. If the inclination of the third molar is more than 60 degree to the occlusal plane of the first and second molarsand if it is in close proximity to the second molar roots or when you recognize ForumOrthodontic Ortodontyczneforum 69 Badania kliniczne Clinical research Tom 8, nr 2, 2012 Volume 8, no 2, 2012 W przypadku obecności procesu patologicznego zalecana jest ekstrakcja. Jeżeli korzeń jest w połowie albo w 2/3 rozwinięty możemy określić obecną przestrzeń zatrzonowcową. Przy inklinacji większej niż 60 stopni w stosunku do płaszczyzny okluzji pierwszego i drugiego trzonowca oraz bliskiej odległości do korzeni drugiego trzonowca lub gdy rozpoznajemy dystoangulację (w przypadku dolnych zębów) warto zalecić dalszą konsultację lub ich ekstrakcję. Ten okres jest decydującym momentem, w którym możemy rozważyć usunięcie, gdyż w tym czasie jest to stosunkowo prosty zabieg, obarczony mniejszym ryzykiem około- i pooperacyjnym. Jako optymalny czas ekstrakcji określono stadium, gdy korzenie osiągają 1/3 lub 2/3 długości. distoangular position (lower third molars), referral for consultation or removal should be recommended. This period is the most critical time to consider prophylactic removal, as it is relatively easy surgery with low risk of operative and postoperative complications. Wnioski Conclusions 1. Trzecie zęby trzonowe najczęściej ulegają zatrzymaniu w kości co można tłumaczyć późnym czasem ich rozwoju oraz różnorodnością położenia. 2. Określanie dostępnej przestrzeni zatrzonowcowej u dzieci jest niepewną wartością diagnostyczną w przewidywaniu wyrzynania zębów mądrości. 3. Profilaktyczne ekstrakcje nie są usprawiedliwionymi praktykami. 4. Długotrwale stosowana retencja do czasu zakończenia wzrostu oraz monitorowanie rozwoju trzecich zębów trzonowych w oparciu o rentgenogram wykonywany w odstępach 2-3 lat jest rutynowym postępowaniem w przypadku każdego pacjenta leczonego ortodontycznie. 1. Third molars are impacted in a bone most often, what can be explained by retarded maturation of the teeth and variety of their position. 2. An estimation of retromolar space at children has a questionable diagnostic value in predicting of third molars eruption. 3. Prophylactic extractions are not justified. 4. Long lasting retention until the end of growth and monitoring of the development of third molars based of the radiograph made in 2-3 years intervals is a routine practice in patients undergoing orthodontic treatment.
