Wyniki uzyskane w badaniu porównano z danymi dotyczącymi oceny potrzeby i zapotrzebowania na leczenie ortodontyczne w wybranych krajach Europy, które pozyskano z piśmiennictwa dostępnego w bazie informacji naukowej PubMed, Medline oraz w zasobach Bibliotek Medycznych Uniwersytetu Jagiellońskiego i Śląskiego Uniwersytetu Medycznego. Najczęstszym powodem dla którego pacjenci podejmują decyzję o rozpoczęciu leczenia ortodontycznego jest zaburzona estetyka uzębienia i poczucie braku atrakcyjności. Atrakcyjny wygląd jest jednak pojęciem względnym i każda osoba może inaczej go rozumieć i postrzegać. Z drugiej jednak strony sprawne i skuteczne działanie systemu opieki zdrowotnej wymaga określenia jasnych zasad kwalifikacji pacjenta do leczenia ortodontycznego. Powinno się jej dokonywać na podstawie jednoznacznych kryteriów. Rozpoznanie wady zgryzu nie jest równoznaczne z potrzebą leczenia ortodontycznego, ponieważ niektóre odstępstwa od normy zgryzowej nie wymagają korekty lub można ją odłożyć na późniejszy czas. Niejednokrotnie bywa, że lekarz ortodonta stwierdza dużą wadę zgryzu, ale sam pacjent nie jest zainteresowany leczeniem. Zdarzają się również sytuacje odwrotne, kiedy to pacjenci zauważają u siebie problem ortodontyczny (najczęściej z powodu wysokich wymagań estetycznych odnośnie swojego wyglądu), natomiast lekarz nie widzi bezwzględnych wskazań do leczenia. Czasami pacjent w ogóle nie dostrzega u siebie istniejącej wady lub nie podejmuje leczenia z powodów np. finansowych. Leczenie ortodontyczne zaczyna się wówczas, gdy ortodonta stwierdza wskazania, aesthetics, and a comparison of researchers’ indications two dentists realizing orthodontics specialization within indications made on the basis of responses provided by the parents. The results obtained in the study were compared with data regarding an evaluation of the need and demand for orthodontic treatment in selected European countries taken from the literature available via the PubMed and Medline databases and from the resources of Medical Libraries of the Jagiellonian University and the Medical University of Silesia. Introduction The results of recent research on the effects of malocclusion on oral health are varied and ambiguous (1). The most common reason why patients decide to start orthodontic treatment is impaired aesthetics and a feeling of lack of attractiveness. However, attractive looks is a relative term and each person may understand and perceive it in a different way. On the other hand, though, an efficient and effective operation of the health care system requires determination of clear rules for a patient’s eligibility for orthodontic treatment. This should be done on the basis of some explicit criteria. The diagnosis of malocclusion is not synonymous with the need for orthodontic treatment because some deviations from the norms do not require occlusal adjustment or it can be delayed for a later time. It often happens that an orthodontist diagnoses a severe malocclusion, but the patient himself is not interested in treatment. There are also opposite situations when a patient himself/herself notices an orthodontic problem (mostly due to high aesthetic demands as regards their appearance) and the physician does not see absolute indications for treatment. Sometimes a patient himself/herself does not perceive a defect at all or he/she does not take up treatment for financial reasons. Gdy w ortodoncji przy podejmowaniu przez pacjentów decyzji o leczeniu ortodontycznym zaczęto doceniać znaczenie estetyki, pojawiła się potrzeba unormowania i zobiektyzowania zasad ułatwiających podejmowanie tych decyzji. Udaną próbę ich opracowania podjęli w Wielkiej Brytanii w 1989 roku Brook i Shaw, wprowadzając na bazie kryteriów stosowanych przez Swedish Dental Health Board wskaźnik ortodontycznych potrzeb leczniczych, tzw. IOTP (ang. Index of Orthodontic Treatment Priority). Został on następnie zmodyfikowany przez Richmonda (1992) i Lunna (1993) i nazwany IOTN (ang. Index of Orthodontic Treatment Need). Z biegiem lat stał się jednym z bardziej znanych i częściej stosowanych kryteriów oceny potrzeb leczniczych zarówno Europie, jak i wielu innych krajach świata. Wskaźnik IOTN składa się z dwóch składowych często nazywanych również w literaturze komponentami. Pierwsza składowa, tzw. komponenta zdrowotna DHC (ang. Dental Health Component), opisuje stopień ciężkości wady zgryzu i ocenia się według niej (wykorzystując 5-stopniową skalę) wadę zgryzu pod kątem zagrożeń morfologicznoczynnościowych dla narządu żucia (3). Druga składowa określana mianem komponenty estetycznej AC (ang. Asthetic Component) ocenia estetykę uzębienia i potrzeby lecznicze w zakresie estetycznym na podstawie odniesienia do 10 fotografii wewnątrzustnych przedstawiających uzębienie o pogarszającej się estetyce – ryc. 1 (4). Fotografie od 1. do 4. przedstawiają przykładowe przypadki niewymagające leczenia lub jego znikomą potrzebę. Fotografie od 5. do 7. to przypadki umiarkowane, które ewentualnie mogą wymagać leczenia. Natomiast zdjęcia od 8. do 10. to przypadki wymagające podjęcia leczenia (3). and the patient is appropriately motivated and interested in the treatment (2, 3). When the aesthetic factor in orthodontics first began to be appreciated when patients made a decision concerning orthodontic treatment, a need for standardization and objectivization of the rules facilitating such decision-making process appeared. A successful attempt to develop such rules was made in 1989 in the United Kingdom by Brook and Shaw who, based on the criteria used by the Swedish Dental Health Board, introduced the Index of Orthodontic Treatment Priority (IOTP). It was later modified by Richmond (1992) and Lunna (1993) and called the Index of Orthodontic Treatment Need (IOTN). Over the years it has become one of the best known and frequently used criteria for assessing treatment needs both in Europe and in many other countries around the world. The IOTN index consists of two components. The first component called the Dental Health Component (DHC) describes the severity of the malocclusion and estimates (using a 5-grade scale) such malocclusion in terms of morphological and functional risks for the masticatory organ (3). The other component known as the Aesthetic Component (AC) evaluates the aesthetic features of the dentition and the needs for aesthetic medical treatment based on the reference to 10 intrabuccal photographs presenting dentition with deteriorating aesthetics - Fig. 1 (4). Photos from 1 to 4 show the sample cases that do not require any treatment or possibly a slight treatment need. Photos from 5 to 7 show the moderate cases that may or may not require treatment. However, images from 8 to 10 represent the cases that definitely require treatment (3). Cel pracy Aim Materiały i metody Materials and methods Celem pracy była ocena potrzeb leczenia ortodontycznego u dzieci w wieku 8–12 lat w województwie małopolskim, weryfikacja użyteczności komponenty AC w szacowaniu poczucia estetyki uzębienia oraz porównanie wskazań osób badających: dwóch lekarzy dentystów w trakcie specjalizacji z ortodoncji ze wskazaniami uzyskanymi na podstawie odpowiedzi udzielonych przez rodziców. Grupę badaną stanowiło 74 uczniów w wieku 8–12 lat, w tym 31 dziewczynek i 43 chłopców pochodzących z Krakowa oraz pobliskich miasteczek i wsi z terenu województwa małopolskiego. Subiektywną ocenę wyglądu uzębienia ustalono na podstawie ankiety przygotowanej przez lekarzy, która była wypełniana przez rodziców dzieci zgłaszających się do poradni ortodontycznej po raz pierwszy. Przy ankietowaniu, w celu ułatwienia badanym oceny, posługiwano się barwną planszą z klasyczną 10-stopniową skalą AC (ryc. 1). The research group was represented by 74 school-children aged 8-12 years, including 31 girls and 43 boys from Cracow and nearby towns and villages of from the Lesser Poland province. A subjective evaluation of teeth appearance was based on a research questionnaire prepared by the physicians, which was completed by parents of children brought to an orthodontic outpatient clinic for their first orthodontic visit. In order to facilitate evaluation to the respondents when the questionnaire questions were asked, a multi-colored classical ForumOrthodontic Ortodontyczneforum 175 Badania kliniczne / Clinical research D. Niedziejko, M. Pniok et. al użyto retraktorów wargowych i lusterka. Kluczowym punktem ankiety była prośba (skierowana do rodzica), zgodnie z sugestią Lunna i in. (5): „na podstawie przedstawionych fotografii o malejącej estetyce w skali 1–10 proszę o przyporządkowanie do fotografii uzębienia swojego dziecka”. Równocześnie lekarz dokonywał własnej oceny stanu uzębienia dziecka według przyjętych kryteriów, zgodnie z komponentą DHC oraz AC. Rodzice dzieci, jako pierwsi, porównywali stan uzębienia swojego dziecka z fotografiami według 10-stopniowej skali AC. Następnie lekarze oceniali wskaźnik IOTN, zarówno DHC oraz AC bezpośrednio podczas badania klinicznego. Wyniki badań własnych porównano z danymi dotyczącymi oceny potrzeb i zapotrzebowania na leczenie ortodontyczne w wybranych krajach Europy pozyskanymi z piśmiennictwa dostępnego w bazach PubMed, Medline oraz w zasobach Bibliotek Medycznych Uniwersytetu Jagiellońskiego i Śląskiego Uniwersytetu Medycznego. Wyniki Uzyskane wyniki przedstawiono w tabelach 1–2. Dla komponenty zdrowotnej DHC badania wykazały: wadę o dużym lub bardzo dużym nasileniu wskazujące na potrzebę leczenia u 18,92% badanej populacji, wadę o umiarkowanym stopniu nasilenia wskazującą na potrzebę leczenia u 36,49% populacji oraz wadę o niewielkim stopniu nasilenia wskazującą na niewielką potrzebę leczenia lub też jej brak u 44,59% populacji. Z kolei dla komponenty ACa badania wykazały: 41,89% populacji bez lub z małą potrzebą leczenia, 44,60% z umiarkowaną potrzebą leczenia oraz 13,51% z dużą potrzebą leczenia. Natomiast dla komponenty ACb: stwierdzono 55,41% populacji bez lub z małą potrzebą leczenia, 41,89% z umiarkowaną potrzebą leczenia oraz 2,70% z dużą potrzebą leczenia. 10-grade AC board was used (compliant with Fig. 1). For auxiliary purposes lip retractors and mirrors were used. The key point of the questionnaire was a request (addressed to the parent) as suggested by Lunn et al. (5) - "on the basis of a 1-10 scale presenting photographs with decreasing aesthetic looks, please assign a photo of your child's dentition." At the same time the physician performed his/her own evaluation of the appearance of the child's dentition in accordance with the adopted criteria compliant with the DHC and AC components. The children’s parents were asked to be the first to compare the dentition condition of their own child with the photographs according to the 10-grade AC scale. Next, directly during the clinical trial the physicians assessed the IOTN INDEX both in terms of the DHC and AC. The results of their own research were compared with data concerning the evaluation of the needs and demands for orthodontic treatment in selected European countries taken from the literature available via the PubMed database, Medline database and the resources of the Medical Libraries of the Jagiellonian University and the Medical University of Silesia. Results The results obtained are shown in Tables 1-2. For the dental health component (DHC) the research demonstrated a malocclusion defect of severe or very severe intensity degree indicating a need for treatment in 18.92 % of the population under study, a malocclusion defect of moderate intensity degree indicating a need for treatment in 36.49 % of the population, and a malocclusion defect of low intensity degree indicating a small need for treatment or a lack thereof in 44.59 % of the population. As regards the ACa component, the research demonstrated that in 41.89% of the population there was no treatment need or a low treatment need; in 44.60% of the population - a moderate treatment need, and in 13.51% of the population - a high treatment need. Odzwierciedla on stopień liniowej zależności pomiędzy dwoma zestawami danych – wskazaniami lekarzy i rodziców pacjentów – który wyniósł 0,6. Tabele nr 3 i 4 prezentują zbiorcze wyniki badań wskaźnika IOTN w wybranych krajach Europy, które uzyskano na podstawie przeglądu piśmiennictwa. Odniesiono je do wyników uzyskanych w ramach badań własnych autorów. Based on the results obtained for the ACa component (evaluated by the physicians) and for the ACb component (evaluated by patients’ parents) respectively, Pearson's linear correlation r coefficient was calculated. The coefficient reflects the linear dependence between two data sets: indications made by physicians and those made by patients’ parents, which was 0.6. Tables 3 and 4 present the collective results of IOTN index study in selected European countries obtained from the literature review. These results were referred to the results obtained in the authors' own research. Dyskusja Discussion David Manzanera i in. ocenili ortodontyczne potrzeby lecznicze w grupie 363 hiszpańskich 12-latków (5). Oceny potrzeb leczenia dokonywało 6 lekarzy, używając zarówno komponenty zdrowotnej DHC, jak i komponenty estetycznej AC wskaźnika IOTN. Bardzo dużą i dużą potrzebę leczenia (skala 4–5) stwierdzono odpowiednio u 5,8% i 15,4% populacji; umiarkowaną i średnią potrzebę leczenia (skala 2–3) stwierdzono odpowiednio u 31,1% i 31,7% populacji, natomiast niewielką potrzebę lub jej brak (skala 1) stwierdzono u 15,4% populacji. Przy czym znacznie częściej potrzebę leczenia odnotowano u dziewczynek. Autorzy dokonali również oceny komponenty estetycznej AC na wybranej grupie badawczej. Stwierdzono, że 85,4% badanych nie wymaga leczenia lub też istnieje znikoma potrzeba leczenia (skala 1–4 AC); 10,2% populacji wymaga umiarkowanej potrzeby leczenia (skala 5–7 AC), natomiast w przypadku 4,4 % badanych istnieje bardzo duża potrzeba leczenia. W Turcji Alev Aksoy Dogan i in. przebadali 208 dzieci w wieku 9–18 lat, które zgłaszały się do lekarza ortodonty David Manzanera et al evaluated orthodontic treatment needs in a group of 363 of Spanish 12-year-olds (6). Treatment needs were evaluated by six physicians who used both the aesthetic component (AC) and dental health component (DHC) of the IOTN index. A very high and high degree treatment need (4-5 degrees) was found in 5.8% and 15.4% of the population, respectively; a moderate and medium treatment need (2-3 degrees) was found in 31.1% and 31.7% of the population, respectively, and a low or null treatment need (1 degree) was found in 15.4% of the population. The need for treatment was much more frequent in the group of girls. The authors evaluated also the aesthetic component (AC) in a selected research group. It was found that 85.4% of the patients did not require any treatment, or there was a minimum treatment need (1-4 degrees AC scale), 10.2% of the population had a moderate treatment need (5-7 degrees AC scale), while in the case of 4.4% of the patients there was a very high treatment need. In Turkey, Alev Aksoy Dogan et al studied a group of 208 children aged 9-18 years visiting orthodontists for the first ForumOrthodontic Ortodontyczneforum 178 Badania kliniczne / Clinical research The Index of Orthodontic Treatment Need (IOTN) in a group of school-aged children... po raz pierwszy (6). W badaniu wzięli udział również rodzice, którym proponowano wskazanie fotografii komponenty estetycznej ich zdaniem najbardziej odpowiadającej uzębieniu ich dziecka. Z wyników, które otrzymali wynika, że 33,6% rodziców wskazało fotografię 8–10, uznając, iż dziecko bezwzględnie wymaga leczenia; 23,6% wskazało fotografię 5–7, a 43,8% uznało, że dziecko nie wymaga lub w minimalnym stopniu wymaga leczenia ortodontycznego, wskazując fotografię 1–4. W ocenie lekarzy wyniki dla komponenty AC wyglądały odpowiednio: 51,4% – fotografie 8–10; 18,3% – fotografie 5–7; 30,3% – fotografie 1–4. Procentowy rozkład komponenty DHC pokazał konieczność leczenia (4. i 5. stopień) u 74 % pacjentów, umiarkowaną potrzebę leczenia (3. stopień) – u 13,9%, a małą potrzebę lub jej brak (1–2 stopień) – u 12,1% badanych. We Francji Mourad Souames i in. przeprowadzili badania na 511-osobowej grupie dzieci w wieku 9–12 lat. Komponenta DHC wykazała ekstremalną i poważną potrzebę leczenia u 21,3% dzieci, graniczną potrzebę – u 28,6% badanych, a minimalną lub brak potrzeby – u 50,1% dzieci. Komponenta AC wykazała wysoce niezadawalającą estetykę w 7% przypadków, natomiast dla 75% przypadków stwierdzono brak lub minimalną potrzebę leczenia ze względów estetycznych (7). Letizia Perillo i in. przebadała w południowych Włoszech 703 dzieci w wieku 12–13 lat. Komponenta DHC wykazała dużą potrzebę leczenia w 27,3 % przypadków (stopień 5. i 4.), umiarkowaną potrzebę leczenia w 36,7 % (stopień 3) oraz małą potrzebę leczenia lub jej brak w 35,8% (stopień 2. i 1.). W zakresie oszacowania komponenty estetycznej AC (8) nie podjęto badań. Badania przeprowadzone na grupie 12–14-latków w Bośni i Hercegowinie przez Vildana Džemidžić i in. (9) wykazały, że w przypadku komponenty zdrowotnej DHC u 99 osób (33,6%) nie ma potrzeby leczenia ortodontycznego; 38 badanych (12,9%) wykazywało graniczną potrzebę leczenia ortodontycznego, a u 158 badanych (53,6%) stwierdzono dużą i bardzo dużą potrzeba leczenia ortodontycznego. W przypadku komponenty estetycznej AC stwierdzono brak potrzeby leczenia u 278 osób (94,2%, AC klasa 1–4), 6 osób (2,0%) wykazywało graniczną potrzebę leczenia (AC klasa 5–7), a 11 osób (3,7%) miało dużą i bardzo dużą potrzebę leczenia ortodontycznego (AC klasa 8–10). Na podstawie danych uzyskanych z badania wynika, że brak związku między zapotrzebowaniem na leczenie ortodontyczne a potrzebą leczenia ortodontycznego. U dzieci potrzeba leczenia jest dużo większa niż popyt na leczenie wśród ich rodziców. Na taki stan może mieć wpływ wiele czynników: świadomość osób dorosłych, wiek, płeć, status socjoekonomiczny, wykształcenie czy różnice kulturowe. Rodzice o wysokim poziomie świadomości potrzeby leczenia swojego dziecka bardziej zwracają uwagę na estetykę uzębienia. Inaczej jest w przypadku rodziców, których poziom świadomości jest time (7). The study also comprised their parents who were requested to indicate in the photographs an aesthetic component (AC) that in their opinion corresponded to their child’s dentition as closely as possible. The results that they obtained showed that 33.6% of the parents indicated photographs 8-10 deciding that the child urgently needed treatment; 23.6% of the parents indicated photographs 5-7, and 43.8% of the parents indicated photographs 1-4 decided that the child did not require any orthodontic treatment or only very slight treatment. The physicians’ AC component evaluation results were as follows: 51.4% of them indicated photographs 8-10; 18.3% - photographs 5-7, and 30.3% photographs 1-4, respectively. The percentage distribution of the DHC component demonstrated the need for treatment (4th and 5th degree) in 74% of patients, a moderate need for treatment (3rd grade) in 13.9% and low or null need (1st-2nd degree) in 12.1% of patients. In France Mourad Souames et al conducted a study on a group of 511 children aged 9-12 years (8). The DHC component showed an extreme and serious need of treatment in of 21.3% of the children, border need in 28.6% of the patients, and a minimum or null need in 50.1% of the children. The AC component showed a highly unsatisfactory aesthetics in 7% of the cases, while in 75% of the cases null or minimal need for treatment for aesthetic reasons was found. Letizia Perillo et al investigated 703 school-children in the age of 12-13 years in southern Italy (9). The DHC component revealed a high need for treatment in 27.3% of the cases (5th and 4th degrees), a moderate need for treatment in 36.7% (3rd degree), and low or null need for treatment in 35.8% of the cases (2nd and 1st degrees ). No research was undertaken to estimate the AC aesthetic component (8). Research conducted on a group of 12-14-year-olds in Bosnia and Herzegovina by Vildana Džemidžić et al demonstrated that in terms of the DHC component, 99 people (33.6%) had a null need for orthodontic treatment, 38 examined people (12.9%) had a border need for orthodontic treatment, and 158 examined people (53.6%) had a high and very high need for orthodontic treatment (10). In the case of the aesthetic component (AC), no need for treatment was found in 278 people - 94.2% (AC Class 1 - 4), in 6 patients (2.0%) a border-line need for treatment was found (AC Class 5 - 7), and 11 patients (3, 7%) had high and very high need for orthodontic treatment (AC grade 8 - 10). Based on the data obtained from the research it is concluded that there is no connection between the demand for orthodontic treatment and the need for orthodontic treatment. The need for treatment in children is much higher than the demand for treatment shown by their parents. Such a situation can result from many factors such as adult people’s awareness degree,, age, gender, socioeconomic status, education or cultural differences. Parents with a high level of awareness of their child treatment need give more attention to dental esthetics. Pniok et. al niski, co może mieć bezpośredni wpływ na większy lub mniejszy popyt na leczenie ortodontyczne. Małe zapotrzebowanie ze strony rodziców może również wynikać z faktu, że wady zgryzu występujące w bocznych odcinkach uzębienia w ich ocenie nie zaburzają estetyki. W trakcie przeprowadzania badań autorzy pracy zauważyli wyższe zapotrzebowanie na leczenie dziewczynek (rodzice częściej wskazywali na potrzebę leczenia). Obecnie media, kreując obowiązujące kanony piękna, doprowadziły do wzrostu świadomości społeczeństwa w zakresie estetyki własnego wyglądu. Ładny uśmiech wpływa na dobre samopoczucie, akceptację w grupie, daje pewność siebie. Jest to bardzo ważne szczególnie u dzieci, które niejednokrotnie są bardzo podatne na wpływ otoczenia. Dlatego też głównym powodem dla którego rodzice przyprowadzają dziecko do poradni ortodontycznej jest poprawa estetyki uzębienia. Długi okres oczekiwania na leczenie ortodontyczne refundowane z NFZ oraz wysoki koszt leczenia ortodontycznego w gabinetach prywatnych mają również negatywny wpływ na popyt w zakresie takiego leczenia. Ocena estetyki uzębienia może zależeć od rasy i kultury w jakiej się wychował człowiek. To, co w jednej kulturze uważane jest za ładne i atrakcyjne, w innej kulturze już takie może nie być. Jednak jak wskazuje przegląd piśmiennictwa w zakresie badań prowadzonych w Europie na dzieciach w wieku 8–13 lat z wykorzystaniem wskaźnika IOTN, nie można dopatrzyć się znaczących odstępstw zarówno dla komponenty zdrowotnej DHC oraz dla komponenty estetycznej AC wskaźnika IOTN. W pracy porównano również wartości składowych DHC i AC, które uzyskano w czasie badań, z wartościami tych komponent w wybranych krajach Europy. Wyniki badań przeprowadzonych przez autorów w Polsce są podobne do wyników innych badaczy w Hiszpanii, Francji i we Włoszech (czyli w krajach Unii Europejskiej) oraz w Turcji i w Bośni, przy czym daje się zauważyć większe rozbieżności dla komponenty AC niż dla DHC. Może to świadczyć o innym kanonie piękna będącym wynikiem różnic kulturowych. a low level of awareness, which may directly translate into a higher or lower demand for orthodontic treatment. Parents’ low demand for treatment may also result from the fact that, in their opinion, malocclusions occurring in the lateral dentition sections do not disturb dental aesthetics. In the course of their tests, the authors noticed a higher demand for treatment of girls (parents more often indicated such treatment need). Currently, with the media creating the ideals of beauty, the public awareness of the aesthetics of physical appearance has increased. A nice smile increases one’s frame of mind, acceptance in a social group, and gives self-confidence. This is particularly important for children who are often very susceptible to environmental influence. Therefore, the main reason why parents bring their child to the orthodontic clinic is to improve the aesthetic look of its dentition. The necessity of having to wait for an extended period of time in order to be admitted for orthodontic treatment refunded by the NFZ (National Health Fund) and the high cost of orthodontic treatment in private clinics also h ave a n e ga t ive i m p a c t o n t h e d e m a n d fo r orthodontic treatment. The evaluation of dental aesthetics may depend on the race and cultural background in which a person was brought up. What is considered to be pretty and attractive in one culture it may not be so in a different culture. However, as the literature review regarding the research conducted in Europe on schoolchildren aged 8-13 years with the use of the IOTN index demonstrated, no significant deviations can be detected neither for the DHC nor the AC components of the IOTN index. This study also compared the values of DHC and AC components that were obtained during the research conducted with values of these components in selected European countries. The results of research conducted by the authors in Poland are similar to those carried out by other researchers in Spain, France and Italy (that is in European Union countries), Turkey and Bosnia, with greater discrepancies observed for the AC than for the DHC component, which may prove that a different ideal of beauty results from cultural differences. Wnioski Conclusions Rozpoznanie wady zgryzu nie jest równoznaczne z koniecznością podjęcia leczenia, gdyż zawsze istnieją odstępstwa od zgryzu idealnego (stopień 1. według wskaźnika IOTN). Wskaźnik IOTN powinien być w naszym kraju stosowany coraz powszechniej, ponieważ pomaga wyłowić z grupy pacjentów tych najbardziej wymagających leczenia, jak i tych, którym na leczeniu bardzo zależy. Wskaźnik ten może znaleźć duże zastosowanie w badaniach epidemiologicznych oraz mógłby w przyszłości, zwłaszcza w dobie coraz bardziej ograniczonych środków finansowych, które NFZ przyznaje na leczenie stomatologiczne, stanowić solidną podstawę do refundacji leczenia ortodontycznego. The diagnosis of malocclusion is not tantamount to the necessity of taking up treatment because there are always deviations from the ideal occlusion (grade 1 according to the IOTN index). The IOTN index should be used in our country more and more frequently because it helps to “fish” from a group of patients the ones who are in the highest need of treatment as well as those who care the most to be treated. This index may find a wide application in epidemiological studies and, in the future, especially in the era of scarcer and scarcer financial resources allocated for dental treatment by the National Health Fund (NFZ), might provide a solid basis for the refund of orthodontic treatment.
