PRODUCT REGULATIONS
‘For the love of a happy smile’ DENTISTRY gift card (PLN 100 and PLN 200 variants)
§ 1. Definitions.
The terms used in these Regulations shall have the following meanings:
Medicover, ORGANIZER – Medicover Sp. z o.o., with its registered office in Warsaw
PARTICIPANT – a natural person, who uses the Product at the Organizer’s establishments
throughout Poland, based on the ‘for the love of a healthy smile’ DENTISTRY gift card bought by
him or bought by a third party for him of a denomination of PLN 100 and PLN 200.
THIRD PARTY - natural person who bought the PARTICIPANT a ‘For the love for a healthy smile’
gift card.
PRODUCT - dental services (full range assigned to the “For the love of a healthy smile”
DENTISTRY gift card for PLN 100 or PLN 200. If the dental service exceeds the volume of the
card, the Participant shall pay the difference in price.
GIFT CARD – a document authorizing the use of the Product, the use on the given service / given
services.
§ 2. Place of use of the Product.
The product can be used in dental surgeries, at selected Medicover Dentistry sites:

The Product can only be used at the Medicover Dentistry dental surgeries specified above.
§ 3. Conditions of use of the Product.
The condition for using the Product is to have a (purchased or received) ‘For the love of a healthy
smile’ gift card, as well as read these Regulations and sign its conditions legibly, giving the
forename, surname and date.
§ 4. Fulfilment of the Product
1. The individual dental services within the Product may be performed during the period
assigned to the Gift Card, at an amount equal to or greater than specified on the card.
2. If a service is performed that exceeds the volume of the card, the Participant shall pay the
difference in the price.
3. The Scope of the Product is described in § 5.
§ 5. Scope of the product.
The Product consists of the following consultations and diagnostic examinations:

§ 6. Principles of the sale
1. A gift card may be purchased for personal use or for a third party.
Details of the person authorized to use the dental card:

2. The buyer provides the personal details of the third party at his own risk. Medicover Sp. z
o.o. processes personal data in order to provide health services, bill for these services and
keep, store and provide medical records on the basis of the provisions of the Act on medical
activity. Therefore, Medicover Sp. z o.o. declares that it is the data controller in the meaning
of the Personal Data Protection Act with respect to the PARTICIPANT’s personal details given
to it by the PARTICIPANT / THIRD PARTY
3. Medicover Sp. z o.o. agrees to process the PARTICIPANT’s personal data in accordance with
the Personal Data Protection Act and to ensure the protection of such data in accordance
with the applicable law.
4. The THIRD PARTY declares that he is providing the PARTICIPANT’S personal data with his
consent.
5. The THIRD PARTY undertakes to immediately notify the PARTICIPANT of:
a. the registered address and full name of Medicover Sp. z o.o. as the data controller of
the PARTICIPANT’S personal data provided to it by the THIRD PARTY,
b. the objective and scope of collection of the data referred to in clause 1 above by
Medicover Sp. z o.o.,
c. the PARTICIPANT’S right of access to his data which is being processed by Medicover
Sp. z o.o. as the data controller and the right to correct it,
d. the rights arising from Article 32, para. 1 items 7 and 8 of the Personal Data
Protection Act.
6. Medicover Sp. z o.o. declares that, in accordance with Article 43, para. 1 item 5 of the
Personal Data Protection Act, it is released from the obligation to register the data set on the
people using Medicover Sp. z o.o.’s medical services.
7. Medicover Sp. z o.o. declares that the personal data may be provided to courier companies
or other data controllers, if this is necessary for fulfilling the order. The personal data may
be provided to authorized entities.
§ 7. Product price.
Price of the ‘For the love of a happy smile’ DENTISTRY gift card – PLN 100 and PLN 200 variants.
§ 8. Form of payment.
1. The buyer shall pay for the product at the reception of the selected Medicover Centre:

2. The buyer can make payment in cash or using a non-cash method (payment card or credit
card).

§ 10. Submission of reservations.
Any reservations on the Product and its fulfilment can be reported at [email protected] or by
telephone at 500 900 500.
§ 11. Final provisions.
1. Medicover reserves the right to change these Regulations.
2. Any changes in these Regulations must be made in writing, otherwise being null and void,
and will be effective after 7 days from the date of their announcement at the locations
specified in § 2.
3. All appendices to these Regulations constitute its integral part.
