Orthodontic Clinic, NZOZ Ortho-Dent in Mielec Streszczenie Abstract Zębiak jest wadą rozwojową twardych tkanek zęba. Powstaje na skutek zaburzeń podziału listewki zębowej. Najczęściej jest to skupisko małych zębopodobnych tworów zlokalizowanych śródkostnie. Obecność zębiaków może powodować zaburzenia w wyrzynaniu zębów, nieprawidłowe ustawienie lub zatrzymanie zębów w kości. W pracy opisano dwa przypadki występowania zębiaków o identycznej lokalizacji u matki i syna. Badano modele diagnostyczne i zdjęcia rentgenowskie dwojga spokrewnionych pacjentów: matki i syna. W obu przypadkach zębiaki zlokalizowane były w rejonie siekacza przyśrodkowego lewego w szczęce. U syna stwierdzono przetrwały ząb mleczny, zatrzymany ząb stały oraz współistniejącą wadę zgryzu klasy II. Leczenie polegało na usunięciu przetrwałego zęba mlecznego 61 oraz zębiaka, a następnie na wprowadzeniu zatrzymanego siekacza stałego 21 do łuku podczas leczenia ortodontycznego. U matki stwierdzono zatrzymany ząb stały 21 oraz braki zębowe w odcinkach bocznych. Leczenie polegało na usunięciu zębiaka i zatrzymanego zęba stałego oraz Odontoma is a developmental defect of dental hard tissue. It forms as a result of a pathological division of dental lamina. It is usually comprised of an enosseous cluster of small tooth-like structures. The presence of odontoma may lead to disturbances in the tooth eruption process, incorrect tooth positioning or the retention of teeth in the bone. This paper describes two cases of odontoma located in exactly the same position in a mother and son. The documentation of two related patients (mother and son) included diagnostic casts and X-rays of the patients. In both cases the odontoma was located in the area of the right upper central incisor. In son a persistent deciduous incisor, an impacted permanent incisor and co-existing Class II div. 2 malocclusion was found. The treatment consisted in the extraction of persistent deciduous tooth 61 and removal the odontoma, followed by orthodontic movement of permanent incisor 21 into the correct position in the dental arch. In the case of the mother an impacted permanent tooth (21) and lost teeth in the posterior segments. A clinically detected missing permanent tooth, a retained deciduous tooth, and a distended alveolar ridge should all be regarded as indicators of possible pathology and therefore need to look for the origin of the problem. The occurrence of dental anomalies located in identical positions in the related patients, in this case in a mother and son, suggest that the origin of this problem was hereditary. Received: 25.09.2011 Accepted: 21.11.2011 Wstęp Introduction Zębiak jest wadą rozwojową twardych tkanek zęba (1). Ze względu na budowę wyróżniamy zębiaki złożone i mieszane. Mogą one być zlokalizowane śródkostnie, wyrzynać się do jamy ustnej lub pozostawać pod błoną śluzową. Najczęściej sa to skupiska małych zębopodobnych tworów zlokalizowanych śródkostnie w przednim odcinku szczęki (2). Guzy te często rozwijają się powoli, nie dają objawów klinicznych, dlatego najczęściej wykrywane są przypadkowo podczas badań rentgenowskich (3). Istnieją jednak objawy, które mogą sugerować obecność guza. Są to: przetrwałe zęby mleczne, zatrzymane zęby stałe, nieprawidłowe ustawienie zębów, wygórowanie na wyrostku zębodołowym. Sugeruje się, że przyczyną powstawania zębiaków są zaburzenia na etapie podziału lub rozwoju listewki zębowej. Stwierdzono, że niektóre nieprawidłowości zębowe jak hipodoncja, zaburzenia wielkości, kształtu i położenia zębów mogą mieć charakter dziedziczny (4, 5, 6). Często obserwowane w praktyce jest rodzinne występowanie np. szczątkowych siekaczy bocznych, hipodoncji, zębów zatrzymanych czy nadliczbowych (5, 7, 8). Dziedziczność wydaje się być jednym z najważniejszych czynników etiologicznych w powstawaniu nieprawidłowości zębowych. Odontoma is a developmental defect of dental hard tissue (1). Morphologically complex and compound odontomas were differentiated. They can be located in the bone, extrude into the oral cavity or stay under the mucous membrane. Most frequently they represent enosseous clusters of small tooth-like structures located in the anterior segment of the maxilla (2). These tumors develop slowly without any clinical symptoms, therefore they are usually detected by chance during radiographic examinations (3). There are, however, a number of symptoms that might indicate the presence of a tumor. These are the following: persistent deciduous teeth, impacted permanent teeth, incorrect tooth position, extrusions on the alveolar ridge. It has been suggested that the reason for the formation of odontomas are irregularities during the process of dental lamina division and development. It has been found that some dental pathologies, such as hypodontia, anomalies in size, shape and position can be hereditary in character (4, 5, 6). In clinical praxis family-related peg laterals, hypodontia, impacted or supernumerary teeth were frequently observed (5, 7, 8). Heredity factors appear to be among the most important etiological reasons responsible for the development of dental anomalies. The objective of this paper was to describe a familyrelated occurrence of odontoma located in identical areas, i.e. around tooth 21, based on the documentation of 2 patients – mother and son. Pacjenci zgłosili się w latach 2006-2008 do Poradni Ortodontycznej NZOZ Ortho-Dent. Analizowano modele diagnostyczne, karty stomatologiczne, dane z wywiadu lekarskiego, zdjęcia rentgenowskie pantomograficzne i zębowe pacjentów wykonane przed, po i w trakcie leczenia. Oceniano objawy kliniczne sugerujące obecność guza: przetrwałe zęby mleczne, braki zębów stałych w łuku, zmiany grubości wyrostka zębodołowego. The study material comprised the documentation of 2 patients (mother and son) in both of whom odontoma was found in identical location. The patients received treatment at the NZOZ Ortho-Dent orthodontic clinic in the years 2006-2008. The following were analysed: diagnostic casts, dental cards, data from the patients’ medical history, pantomographic and dental X-rays of the patients taken prior, during and after the course of the treatment. The following clinical symptoms indicating the presence of a tumor were evaluated: persistent deciduous teeth, missing permanent teeth in the arch, changes in the thickness of the alveolar ridge. Wyniki Results Pacjent w wieku 14-5 zgłosił się w październiku 2006 roku do Poradni Ortodontycznej wraz z matką celem konsultacji. Matka pacjenta była zaniepokojona nieprawidłowym wyglądem zębów przednich u syna. W badaniu klinicznym stwierdzono przetrwały ząb mleczny 61, niewielkie pogrubienie wyrostka nad zębem 61, zęby 14, 24, 34, 44 o nieprawidłowej budowie (zmniejszony wymiar koron i korzeni zębów) oraz wadę zgryzu klasy drugiej, grupy drugiej z pogłębionym nagryzem pionowym w odcinku przednim. W wywiadzie uzyskano informację o przebytej chorobie nowotworowej wątroby (hepatoblastoma) we wczesnym dzieciństwie. Wykonano modele diagnostyczne i przeprowadzono badania rentgenowskie: pantomograficzne, cefalometryczne i celowane zdjęcie zębowe na okolicę korzenia zęba 61. Na zdjęciach rentgenowskich stwierdzono: przetrwały ząb 61, zatrzymany ząb 21 oraz rzutujący się na koronę zęba 21 nieregularny guz o wysyceniu zbliżonym do zębów sąsiednich i średnicy około 5 mm (ryc. 1). Po przeanalizowaniu zgromadzonych dokumentów podjęto decyzję o leczeniu ortodontycznym. Analiza dojrzałości kręgów na zdjęciu cefalometrycznym wykazała stadium CS 4. Ze względu na występowanie wady klasy II (tyłozgryz całkowity) oraz zakończony skok wzrostowy podjęto decyzję o usunięciu zębów 14 i 24, które miały zmniejszony wymiar koron i nieprawidłową budowę korzeni. Po założeniu aparatu stałego w łuku górnym usunięto przetrwały ząb 61 oraz nieregularny guz. W wyniku badań histo-patologicznych stwierdzono, że był to zębiak. Podczas zabiegu naklejono zaczep na koronę zęba 21 w celu wprowadzenia go do łuku. W następnym etapie założono aparat stały w łuku dolnym i kontynuowano leczenie ortodontyczne, mające na celu wprowadzenie zęba 21 do łuku oraz zamknięcie luk poekstrakcyjnych. W trakcie trwania leczenia ortodontycznego chłopca jego matka w wieku 36-6 lat wykonała zdjęcie rentgenowskie pantomograficzne przed planowanym In 2006 a boy age 14-5, and his mother came to the Orthodontic Clinic for a consultation. The patient’s mother was concerned by the incorrect appearance of her son’s anterior teeth. During the clinical examination the following were found: retained deciduous tooth 61, a slight thickening of the ridge above tooth 61, and teeth 14, 24, 34, 44 revealed improper morphology (smaller dimensions of crowns and roots) as well as malocclusion Class II division 2 with increased overbite. During the interview it was disclosed that the patient had suffered from neoplastic diseases of the lever (hepatoblastoma) in his early childhood. Diagnostic casts were made and a radiographic examination was performed in the form of pantomographic and cephalometric images, as well as targeted dental X-rays of the roots of tooth 61. The following was found on the X-rays: retained tooth 61, impacted tooth 21, and an irregular tumor with radiolucency similar to adjacent teeth and with a diameter of ca. 5 mm (Fig.1) projecting on the crown of tooth 21. After an analysis of the available documentation the decision was made to treat patient orthodontically. An examination of the maturity of the vertebrae on the cephalometric image showed stage CS 4. Due to the presence of malocclusion Class II and a completed growth spurt, the decision was made to extract teeth 14 and 24, which had underdeveloped crowns and irregular root morphology. Following the placement of a fixed appliance in the upper arch, both the retained tooth 61 and the irregular tumor were removed. A histo-pathological examination revealed that the tumour was an odontoma. During the treatment an attachment was bonded to the crown of tooth 21 with the purpose of ensuring its correct positioning in the arch. In the next phase, a fixed appliance was placed in the lower arch, and orthodontic treatment continued with the aim of achieving the correct alignment and levelling of tooth 21 and closing up post-extraction spaces. Leczenie miało polegać na wykonaniu uzupełnienia protetycznego w odcinku przednim od 11 do 22. Z wywiadu uzyskano informację, że ząb 21 nigdy się nie wyrznął. Na zdjęciu rentgenowskim stwierdzono zatrzymany ząb 21 oraz rzutujący się na jego koronę guz o wysyceniu podobnym do tkanek zęba o średnicy około 4 mm. Poza tym stwierdzono liczne braki zębowe w odcinkach bocznych (ryc. 2). Przyczyną powstania braków zębowych w odcinkach bocznych były ekstrakcje z powodu próchnicy i przewlekłych stanów zapalnych przyzębia wierzchołkowego. Po analizie zdjęć i omówieniu wariantów leczenia z pacjentką podjęto decyzję o ekstrakcji zęba zatrzymanego i zlokalizowanego w jego okolicy guza oraz późniejszym uzupełnieniu protetycznym braków zębowych. Decyzję taką podjęto ze względów ekonomicznych jak również z powodu obecności wcześniej oszlifowanych zębów sąsiadujących z luką, które wymagały odbudowy protetycznej. Dyskusja Discussion W opisywanych przypadkach zębiaki spowodowały zaburzenia w wyrzynaniu zębów stałych. U syna objawem był przetrwały ząb mleczny i pogrubienie wyrostka zębodołowego, u matki brak siekacza stałego. Identyczna lokalizacja zębiaków w okolicy lewego górnego siekacza szczęki u matki i syna sugeruje komponentę genetyczną tego zaburzenia. Przyczyny powstawania zębiaków nie są zbadane. Nie wyklucza się czynników genetycznych, urazów, stanów zapalnych. W piśmiennictwie opisywano podobne przypadki rodzinnego występowania nieprawidłowości zębowych (4, 5, 6). Browne (9) w 1970 i Yoda (10) w 1998 opisywali rodzinnie występujące zębiaki, sugerując wpływ czynników dziedzicznych. U chłopca stwierdzono obecność zębiaka w okolicy siekacza bocznego szczęki oraz nieprawidłowo zbudowanych pierwszych zębów przedtrzonowych szczęki. Dodatkowo uzyskano informację o przebytej chorobie nowotworowej wątroby w wieku 1-2 lat. W piśmiennictwie opisywano przypadki występowania nowotworów wątroby typu hepatoblastoma i zmian w kościach szczęk np. zębiaków w połączeniu ze zmianami polipowatymi jelita grubego, które ujawniały się w drugiej lub trzeciej dekadzie życia. Skojarzone występowanie opisanych zmian opisywano jako zespół Gardnera (11, 12). W analizowanym materiale należy podkreślić, że u syna stwierdzono występowanie nieprawidłowości dopiero w wieku 14-5 lat, u matki w wieku 36-6 lat. Pacjentka nie została zdiagnozowana wcześniej, mimo, że wykonywano u niej leczenie protetyczne w badanym obszarze. The symptoms in the son included a persistent deciduous tooth and a thickening of the alveolar ridge, and in the mother’s case a permanent incisor was missing. The identical location of odontoma in the area of the left upper central incisor in both the mother and the son suggests a genetic component in this anomaly. The reasons for odontoma formation are not clear. Possible causes that have been considered include genetic factors, trauma, and inflammatory processes. The literature contains descriptions of similar cases of familyrelated occurrences of dental irregularities (4, 5, 6). Browne (9) in 1970 and Yoda (10) in 1998 described cases of odontoma occurring in the same family, which suggests the influence of genetic factors. In a boy odontoma was found in the area of the upper lateral incisor and the first upper premolars exhibited improper morphology. In addition, it was disclosed that the patient suffered from neoplastic liver disease at the age of 1-2 years. The literature describes cases of hepatoblastoma-type liver tumors and anomalies in the maxillary bones in form of e.g. odontoma, with accompanying polypoid changes in the large intestine occurring in the second or third decade of the patient's life. The concomitant occurrence of such anomalies has been described as Gardner’s Syndrome (11, 12). With respect to the analysed material it should be stressed that the occurrence of anomalies in boy was only observed when he was 14-5 years old, and in his mother when she was 36-6 years old. The anomaly in the mother Patient’s mother age 36-6, before the planned prosthetic restoration, was referred to the Orthodontic Clinic for consultation and pantomogram X-ray was taken. The planned treatment consisted in the prosthetic restoration of the anterior segment (teeth 11 to 22). The patient’s dental history revealed that tooth 21 had never erupted. The X-ray showed retained tooth 21 and a tumor similarly mineralized to the adjacent teeth and a diameter of ca. 4 mm projecting on the crown of tooth 21. Numerous teeth were missing in the posterior segments (Fig. 2). The tooth loss in the posterior segments was due to extractions. After an analysis of the X-rays and a discussion with the patient of possible treatment modalities the decision was taken to extract the retained tooth and the tumor localized in its proximity and to restore the missing teeth prosthetically at a later date. This decision was taken due to financial reasons as well as because of the fact that teeth prepared for prosthetic restoration were already present in the existing spaces. This fact may indicate that no radiographic examination was performed on her during the course of the previous dental treatment. ryc. 1. Klinicznie stwierdzony brak zęba stałego, przy obecności zęba jednoimiennego, przetrwały ząb mleczny, rozdęcie wyrostka zębodołowego powinny nasuwać podejrzenie istnienia nieprawidłowości i skłaniać do poszukiwania przyczyny problemu.
