dụng luật ȃn xá, nộp đơn xác nhận rằng
bạn có thể sống hợp pháp ở Ba Lan
trong thời gian qui định. Ngày cuối cùng
để bạn nộp đơn ȃn xá vào ngày 2
tháng 7 năm 2012.
Komentarz [r3]: Nộp cùng với đơn là
bốn hình màu (cỡ 35 * 45 mm). Hình
không được quá 6 tháng từ khi bạn nộp
đơn. Hình phải thấy vổ mắt và mặt,
không đội gì trên đầu, không đeo kính
đen, và không được mở miệng. Mặt
chiếm 70-80% diện tích tấm hình.

Jestem świadomy, że złożenie wniosku lub dołączenie dokumentów zawierających nieprawdziwe dane osobowe lub
fałszywe informacje, a także zeznanie w postępowaniu o udzielenie zezwolenia na zamieszkanie na czas oznaczony
nieprawdy, zatajenie prawdy, podrobienie, przerobienie dokumentu w celu użycia za autentyczny lub używanie takiego

1.Hộ chiếu còn hạn hoặc tài liệu khác
chứng minh là bạn-3 bản sao.
2. Nộp cùng với đơn là bốn hình màu
(cỡ 35 * 45 mm). Hình không được quá
6 tháng từ khi bạn nộp đơn. Hình phải
thấy vổ mắt và mặt, không đội gì trên
đầu, không đeo kính đen, và không
được mở miệng. Mặt chiếm 70-80%
diện tích tấm hình.

Tất cả cải tài liệu bạn làm bản sao,
nhưng nhớ là khi bạn nộp đơn phải đưa

POUCZENIE / NOTES / INSTRUCTION
1) Należy wypełnić wszystkie wymagane rubryki. W przypadku braku niezbędnych danych wniosek może być
pozostawiony bez rozpatrzenia.
All required fields should be completed. In the case of a lack of necessary data the application might not be reviewed.
Il faut remplir toutes les cases demandées. En cas d’absence des données nécessaires, la demande pourrait ne pas être
instruite.
2) Kwestionariusz należy wypełnić czytelnie, drukowanymi literami wpisywanymi w odpowiednie kratki.
The application should be completed eligibly, in printed letters entered into the appropriate boxes.
Le formulaire doit être remplie lisiblement, en majuscules inscrites dans les cases correspondantes.
3) Części A, B, C, D, E i H wypełnia wnioskodawca.
Parts A, B, C, D, E and H should be completed by the applicant.
Les parties A, B, C, D, E et H sont à remplir par le demandeur.
4) Część F wypełnia organ przyjmujący wniosek, jeżeli wniosek został złożony u konsula. Część G wypełnia
organ rozpatrujący sprawę.
Part F shall be completed by the authority accepting the application, if the application was filed with the consul. Part
G should be completed by the authority reviewing the case.
La partie F est à remplir par l’organe qui reçoit la demande, si la demande est déposée auprès du consul. La partie G
est à remplir par l’organe qui instruit le dossier.
5) W części A, w rubryce „płeć” wpisać M – dla mężczyzny, K – dla kobiety; w rubryce: „stan cywilny” należy
użyć sformułowań: panna, kawaler, mężatka, żonaty, rozwiedziona, rozwiedziony, wdowa, wdowiec.
In Part A in the „sex” field „M” should be entered for a male and „K” for women; in „marital status” fields the
following terms should be used: unmaried, married, divorced, widow, widower.
Dans la partie A, rubrique „sexe” – inscrire: „M” pour un homme, „K” pour une femme; rubrique „état civil”, utilisez
une des formulations suivantes: célibataire, marié, mariée, divorcée, divorcé, veuve, veuf.
6) W części H podpis nie może wychodzić poza ramki.
In Part H the signature should be within the box.
Dans la partie H, la signature ne doit pas dépasser l’encadrement.
7) W przypadku gdy wniosek ma obejmować dzieci i inne osoby wpisane do dokumentu podróży
wnioskodawcy należy dodatkowo wypełnić części A i D wniosku dla każdej z tych osób, podając stopień
pokrewieństwa, a objęte wnioskiem osoby potrafiące pisać wypełniają także dodatkowo część H.
In the case of when the application is supposed to also include children and other individuals entered into the
applicant’s travel document, in addition parts A and D should be completed for each of these individuals, stating the
degree of kin and the individuals covered under the application, those that can write should also complete part H.
Si l’invitation doit englober les enfants et autres personnes inscrites au document de voyage du demandeur, les parties
A et D doivent être remplies séparément pour chacune de ces personnes, avec l’indication du degré de parenté ; les
personnes englobées par la demande qui savent écrire remplissent aussi la partie H.
