This agreement is concluded under the Framework of ZKZEiT Project, carried out by the Sending
Institution, financed by the European Community from the European Social Fund (Operational
Programme Human Capital, Task 4.1.1), Contract No. UDA-POKL.04.01.01-00-073/14
Internship is understood as a temporary work done by the Beneficiary, appointed by the Sending
Institution at the Host Institution, under the specific conditions described in this agreement. The
internship is aimed at obtaining new knowledge and skills development.

§2
To assist and support the Beneficiary, the Internship supervisor is appointed. The Beneficiary is
obliged to complete the commands of the Internship supervisor as well as the appointed employees.

§3
The Parties agree that duration of the internship will be about 40 working days (320 hours; 8 hours
for a day). The internship will be carried out between the …………. and ………., according to the agenda
agreed between the Hosting Institution and the Beneficiary.

Sending Institution will cover the travel, accommodation and per diem costs under the conditions
described in a separate agreement concluded between Sending Institution and the Beneficiary.
§6
Hosting Institution obligations:

to host the Beneficiary between the 19st June 2015 and 31st August 2015 in

to organize the workplace for the Beneficiary,
to inform the Beneficiary on the working conditions and regulations, provide necessary
training on the workplace designed for the Beneficiary,
to develop the specific practice agenda, in a close cooperation with the Beneficiary,
to delegate the Beneficiary tasks according to the practice agenda,
to appoint the Beneficiary Internship supervisor, responsible for Beneficiary support,
assistance and control,
to inform immediately the Sending Institution on any problems and irregularities in Practice
realization as well as in situation of internship termination,
to issue a confirmation on internship as well as the opinion of the Beneficiary on his / her
inquiry.
§7
Hosting Institution is entitled to dismiss the Beneficiary in case of his violation of work discipline or if
the internship agenda is not carried out.
§8
Hosting Institution on request of Beneficiary, will effectively support Beneficiary in
obtaining/acquirement of accident and death insurance for the whole period of Practice.
§9
Hosting institution is obliged to undergo control in the previously agreed dates. The control scope
will cover the obligations enumerated in this agreement.
§ 10
In case that Hosting Institution is not fulfilling the obligations enumerated in this agreement, which
could cause that the internship realization will not be possible, the Sending Institution has the right
to terminate the agreement.

In case of the practice termination by the Beneficiary personal reasons, disputes are settled by the
ZKZEiT Project Coordinator.
§12
In matters not covered in this Agreement the Civil Code will be applied. Any disputes arising under
this Agreement, the Parties shall submit to the jurisdiction of the Civil Court on Gliwice.
§ 13
This Agreement is concluded in 2 copies; one for Sending Institution and one for Hosting Institution.
