11.Streszczenie Liczne badania sugerują, że zmniejszenie stężenia endogennych estrogenów po menopauzie jest przyczyną dolegliwości menopauzalnych i może być krytycznym czynnikiem ryzyka chorób nasilających się w tym okresie życia, w tym chorób układu krążenia. Nadal trwają badania nad znalezieniem genetycznych czynników ryzyka dolegliwości i chorób nasilających się po menopauzie. Mimo, iż znaleziono wiele potencjalnych genów kandydatów to nadal niejasna jest w tym zakresie rola genów uczestniczących w działaniu hormonów płciowych. Z tych przesłanek zrodziła się potrzeba podjęcia się problematyki w poniższym badaniu. Celem głównym pracy była ocena związku polimorfizmu receptora estrogenowego α z wybranymi aspektami przebiegu naturalnej menopauzy, ze szczególnym uwzględnieniem zespołu menopauzalnego i występowania czynników ryzyka chorób układu krążenia. Cele szczegółowe to: 1) Analiza związku polimorfizmu receptora estrogenowego α z objawami zespołu menopauzalnego w badanej grupie kobiet; 2) Analiza związku polimorfizmu receptora estrogenowego α ze stężeniem estradiolu w badanej grupie kobiet po menopauzie; 3) Analiza związku polimorfizmu receptora estrogenowego α z BMI, WHR, obwodem talii badanych kobiet; 4) Analiza związku polimorfizmu receptora estrogenowego α z lipidogramem badanych kobiet; 5) Analiza związku polimorfizmu receptora estrogenowego α z subiektywną oceną przebiegu menopauzy u badanych kobiet; 6) Analiza związku interakcji pomiędzy stężeniem estradiolu i polimorfizmem receptora estrogenowego α a objawami zespołu menopauzalnego badanych kobiet; 7) Analiza związku interakcji pomiędzy stężeniem estradiolu i polimorfizmem receptora estrogenowego α a lipidogramem badanych kobiet; 8) Analiza związku interakcji pomiędzy stężeniem estradiolu i polimorfizmem receptora estrogenowego α a BMI, WHR, obwodem talii badanych kobiet. Hipotezy badawcze: 1) Nasilenie objawów zespołu menopauzalnego zależy istotnie od polimorfizmu receptora estrogenowego α Xba1 i PvuII; 2) BMI, WHR, lipidogram u kobiet po menopauzie zależą istotnie od polimorfizmu receptora estrogenowego α Xba1 i PvuII; 3) Stężenie estradiolu u kobiet po menopauzie zależy istotnie od polimorfizmu receptora estrogenowego α Xba1 i PvuII; 4) Interakcja stężenia estradiolu z objawami zespołu menopauzalnego istotnie zależy od polimorfizmu receptora estrogenowego α Xba1 i PvuII; 5) Interakcja stężenia estradiolu i lipidogramu u kobiet po menopauzie zależy istotnie od polimorfizmu receptora estrogenowego α Xba1 i PvuII; 6) Interakcja stężenia estradiolu z BMI, WHR, obwodem talii kobiet po menopauzie istotnie zależy od polimorfizmu receptora estrogenowego α Xba1 i PvuII; 7) Wysokość RR i jego zmiana po menopauzie zależą istotnie od polimorfizmu receptora estrogenowego α Xba1 i PvuII. Badanie zostało przeprowadzone w 2013 r. Kryteria włączenia do badania: wiek 50-65 lat; wykształcenie co najmniej pełne podstawowe; minimum 2 lata od ostatniej miesiączki; FSH > 30 mU/ml. Kryteria wykluczenia z badania: przewlekłe choroby w okresie ostatnich 5 lat, w tym zdiagnozowana choroba tarczycy; choroby psychiczne w wywiadzie; uzależnienia od leków i alkoholu; zdiagnozowana jednostka chorobowa z objawami demencji. Ostatecznie do badania zakwalifikowano 266 kobiet. Badanym została pobrana krew do oznaczenia stężenia estradiolu, FSH, cholesterolu całkowitego (CHOL), HDL-cholesterolu, LDL-cholesterolu, trójglicerydów (TG). Kobiety zakwalifikowane do badania zostały zważone, został zmierzony ich wzrost, oraz obwód talii (W) i obwód bioder (R). Zmierzono również ciśnienie tętnicze. Na podstawie uzyskanych pomiarów został obliczony wskaźnik BMI (kg/wzrost w m2) i WHR. Polimorfizm receptora estrogenowego α oznaczony został metodą multiplet PCR. Uczestniczki badania wypełniały ankietę składającą się z: 1. części opisującej cechy społeczno-demograficzne; 2. części zdrowotnej, dotyczącej wywiadu ginekologicznego oraz nasilenia objawów menopauzalnych, ocenianych wskaźnikiem Kuppermana i Greene’a wraz z subiektywną oceną przebiegu menopauzy w zakresie aktywności zawodowej, fizycznej i społecznej. Statystyczną analizę zebranych danych wykonano w statystycznym pakiecie komputerowym STATISTICA. Przyjęto poziom istotności α= 0,05. Średnia wieku badanych kobiet wynosiła 56,4±3,4 lat. Średnie stężenie estradiolu w surowicy krwi wynosiło 23,7±20,22 pg/ml. Większość badanych miała stężenie estradiolu w normie - 88,72%, a u 11,28% stężenie estradiolu przekraczało normę dla kobiet po menopauzie. Prawie połowa badanych kobiet miała genotyp ERα Xba1 AG, około 35% to nosicielki genotypu Xba1 AA, a około 17% - GG. Polimorfizm ERα PvuII najczęściej występował w genotypie TC (45% badanych), zaś pozostałe dwa genotypy tego polimorfizmu występowały z podobną częstością (28% miało TT i 27% CC). Objawy menopauzy trwały od 2 do 5 lat u prawie połowy badanych kobiet. Wykazano że, czas trwania objawów menopauzy u badanych kobiet zależał od posiadanych genotypów polimorfizmu ERα Xba1 oraz PvuII. Kobiety z genotypem AA Xba1 miały istotnie rzadziej objawy menopauzy trwające do 2 lat i częściej objawy menopauzy trwające od 2 do 5 lat (2/3 kobiet), niż badane z pozostałymi genotypami tego polimorfizmu (AG i GG). Kobiety o genotypie TT PvuII miały objawy menopauzy trwające do 2 lat istotnie rzadziej niż badane z pozostałymi genotypami tego polimorfizmu. Natomiast kobiety z genotypem CC miały częściej objawy menopauzy trwające od 2 do 5 lat niż kobiety z pozostałymi genotypami polimorfizmu PvuII. Dodatkowo, kobiety o genotypie TT i TC PvuII miały najmniejsze, a kobiety CC PvuII największe nasilenie objawów naczynioruchowych w skali Greene’a. Stwierdzono zależność między stężeniem estradiolu a nasileniem zespołu menopauzalnego, mierzonego wskaźnikiem Kuppermana. Kobiety bez zespołu menopauzalnego miały istotnie niższe stężenie estradiolu niż kobiety z zespołem menopauzalnym. Ciężki zespół menopauzalny częściej dotyczył kobiet ze stężeniem estradiolu w normie, niż kobiet ze stężeniem estradiolu powyżej normy. U badanych kobiet zarówno ciśnienie skurczowe, jak i rozkurczowe, zależały od posiadanych genotypów polimorfizmu Xba1 i PvuII. Kobiety z genotypem GG Xba1 miały istotnie niższe ciśnienie skurczowe i rozkurczowe, niż kobiety z pozostałymi genotypami tego polimorfizmu. Kobiety z genotypem CC PvuII miały najniższe ciśnienie skurczowe i rozkurczowe, nieco wyższe – kobiety z TT, a najwyższe – z TC. Wzrost ciśnienia tętniczego po menopauzie wystąpił istotnie rzadziej u kobiet z genotypem GG Xba1 i CC PvuII niż u kobiet z pozostałymi genotypami tych polimorfizmów. W badanej grupie kobiet 86,84% miało cholesterol całkowity powyżej normy, 43,23% kobiet miało LDL powyżej normy, 83,83% - HDL poniżej normy. Wykazano istotną zależność stężenia cholesterolu całkowitego od Xba1 oraz zależność stężenia trójglicerydów od polimorfizmów Xba1 i PvuII. Badane kobiety z genotypem GG Xba1 miały istotnie wyższe stężenie całkowitego cholesterolu niż kobiety z pozostałymi genotypami tego polimorfizmu. Najwyższe stężenie trójglicerydów stwierdzono u kobiet z genotypem GG Xba1 oraz genotypem CC PvuII. Niższe stężenia trójglicerydów wystąpiły u kobiet z genotypem AA Xba1 oraz genotypem TT PvuII, a najniższe stężenia stwierdzono u kobiet z genotypem AG Xba1 oraz genotypem TC PvuII. Badane kobiety ze stężeniem trójglicerydów w normie miały istotnie wyższe stężenie estradiolu niż kobiety ze stężeniem trójglicerydów powyżej normy. Wyższym stężeniom estradiolu towarzyszyły niższe stężenia trójglicerydów. Stężenia HDL, LDL i trójglicerydów korelowały różnokierunkowo ze stężeniem estradiolu w zależności od posiadanych genotypów polimorfizmu ERα. Wyższemu stężeniu estradiolu u kobiet o genotypach: a) GG Xba1 i TC PvuII odpowiadało wyższe stężenie HDL; b) TT PvuII odpowiadało niższe stężenie HDL; c) TC PvuII odpowiadało niższe stężenie LDL; d) AG Xba1 i TC PvuII odpowiadało niższe stężenie trójglicerydów. Średnia wartość BMI u badanych kobiet wynosiła 26,40 kg/m2. 40% badanych kobiet miało BMI w normie, 40% miało nadwagę, 19% miało otyłość, a 6 kobiet niedowagę. U większości badanych kobiet nastąpił wzrost masy ciała po menopauzie (59%), u 34% - masa ciała nie zmieniła się, zaś u 7,5% - zmniejszyła się. Wykazano zależność wyników w przedziałach BMI od polimorfizmu PvuII. Badane kobiety o genotypach TT i CC PvuII miały najczęściej masę ciała w normie, podczas gdy kobiety z genotypem TC tego polimorfizmu miały najczęściej nadwagę. Około połowa kobiet z TT i CC miała masę ciała poniżej normy lub w normie, zaś większość kobiet z TC – nadwagę lub otyłość. Stwierdzono że, większy obwód talii był związany z niższym stężeniem estradiolu. Stwierdzono istotnie niższe stężenie estradiolu u kobiet otyłych w porównaniu z kobietami z nadwagą i masą ciała w normie. U badanych kobiet z genotypem AA polimorfizmu Xba1 oraz z genotypem TT polimorfizmu PvuII wykazano, że im wyższe było stężenie estradiolu, tym miały one mniejszy obwód talii i mniejszy stosunek obwodu talii do obwodu bioder. U badanych kobiet nie wykazano zależności między polimorfizmem Xba1 i PvuII a subiektywną oceną wpływu menopauzy na obniżenie jakości życia. Stwierdzono istotnie niższe stężenie estradiolu u kobiet z lekkim obniżeniem jakości życia po menopauzie niż u kobiet ze średnim lub silnym obniżeniem jakości życia i u kobiet nie deklarujących obniżenia jakości życia. Na podstawie przeprowadzonego badania wyciągnięto następujące wnioski: 1. Objawy menopauzalne dłużej utrzymywały się u kobiet z polimorfizmami ERα XbaI AA i PvuII TT. Objawy naczynioruchowe były najsilniej wyrażone u nosicielek polimorfizmu PvuII CC. 2. Genotyp ERα XbaI AG i PvuII TC był związany z wyższymi wartościami ciśnienia skurczowego i rozkurczowego oraz ze wzrostem RR po menopauzie. 3. Genotyp ERα PvuII TC był związany z częstszym występowaniem nadwagi i otyłości u badanych kobiet. 4. Polimorfizm ERα XbaI AA i PvuII TT wpływał na korzystny efekt oddziaływania estradiolu na rozmieszczenie tkanki tłuszczowej po menopauzie. 5. Genotyp ERα XbaI GG i PuvII CC związany był z wyższymi wynikami cholesterolu całkowitego i trójglicerydów. 6. Genotyp ERα XbaI i PuvII wpływa na efekt oddziaływania estradiolu na stężenie lipidów w badanej grupie: • w podgrupie XbaI GG oraz PvuII TC wyższe stężenie estradiolu było związane z wyższym stężeniem cholesterolu HDL i niższym LDL • w podgrupie PvuII TT wyższe stężenie estradiolu było związane z niższym stężeniem cholesterolu HDL • w podgrupie XbaI AG oraz PvuII TC wyższe stężenie estradiolu było związane z niższym stężeniem trój glicerydów 7. Istnieje potrzeba badań wpływu terapii hormonalnej na występowanie objawów menopauzalnych w zależności od posiadanych polimorfizmów ERα, ponieważ nie stwierdzono oddziaływania posiadanych polimorfizmów ERα na związek stężenia endogennego estradiolu z nasileniem objawów menopauzalnych. 8.Na podstawie badań polimorfizmów ERα można zidentyfikować grupę kobiet bardziej zagrożoną na rozwój chorób układu krążenia. 9.Dalsze badania polimorfizmów ERα mogą ułatwić podejmowanie decyzji co do słuszności zastosowania terapii hormonalnej w profilaktyce pierwotnej chorób układu krążenia. 12.Summary Numerous studies suggest that the reduction in endogenous estrogen after menopause is the cause of menopausal symptoms and can be a fatal disease risk factor worsening in this period of life, including cardiovascular disease. Researches are still continued in order to find the genetic ailments and disease risk factors worsening after the menopause. Although many potential candidate genes were found, the role of the genes involved in the activity of female sex hormones is still unclear. From these premises was the need to take up the issues in this study. The main aim of this study was to evaluate the association of estrogen receptor α polymorphisms of selected aspects in the course of natural menopause, with particular emphasis on menopausal syndrome and risk factors for cardiovascular disease. The specific objectives are: 1) Analyzing the connection between estrogen receptor α polymorphism with menopausal symptoms of the women in the study group; 2) The analysis of estrogen receptor α polymorphism with estradiol concentration in the study group of postmenopausal women; 3) Analysis of the connection of estrogen receptor α polymorphism with BMI, WHR, waist circumference of the studied women; 4) Analyzing the connection of estrogen receptor α polymorphism with the lipid profile of the studied women; 5) Analysis of the connection of estrogen receptor α polymorphism with the subjective evaluation of the course of menopause in tested women; 6) Analysis of the connection of the interaction between the concentration of estradiol and estrogen receptor α polymorphism and the menopausal symptoms in studied women. 7) Analysis of the connection of interaction between the concentration of estradiol and estrogen receptor α polymorphism and the lipid profile of women studied; 8) Analysis of the connection of interaction between concentrated estradiol and estrogen receptor α polymorphism and BMI, WHR, waist circumference in women studied. Research hypotheses: 1) The severity of the symptoms of menopause are indeed dependent on polymorphism of the estrogen receptor α Xba1 and PvuII; 2) BMI, WHR, the lipid profile in postmenopausal women depend significantly on the polymorphism of the estrogen receptor α Xba1 and PvuII; 3) The concentration of estradiol in postmenopausal women materially depends on estrogen receptor α polymorphism Xba1 and PvuII; 4) Interaction of estradiol concentration with symptoms of menopause syndrome significantly depends on the polymorphism of estrogen receptor α Xba1 and PvuII; 5) Interaction of estradiol concentrations and the lipid profile in postmenopausal women significantly depends on the polymorphism of estrogen receptor α Xba1 and PvuII; 6) Interaction of estradiol concentration with BMI, WHR, waist circumference in postmenopausal women significantly depends on estrogen receptor α polymorphism Xba1 and PvuII; 7) The level of RR and its change after menopause significantly depend on the polymorphism of estrogen receptor α Xba1 and PvuII. The study was conducted in 2013. The inclusion criteria: women age 50-65 years; at least primary education level; a minimum of 2 years from the last menstrual period; FSH> 30 mU / ml. The exclusion criteria from the study: chronic illnesses in the last 5 years, including a diagnosed thyroid disease; history of mental illness; addiction to drugs and alcohol; diagnosed disease with dementia symptoms. The study group consisted of 266 women which fulfilled the above criteria. From the study group blood samples were taken to mark the concentration of estradiol, FSH, total cholesterol (CHOL), HDL-cholesterol, LDL-cholesterol, triglycerides (TG). The women qualified for the study were weighed, their height, waist (W) and hips (R) were measured. Their blood pressure was also measured. The achieved data was used to calculate BMI (kg/height/ in m2) and WHR. The estrogen receptor α polymorphism determined by multiplet PCR method. The participants were asked to fill in a two part questionnaire survey. Part 1 consisted of: sociodemographic characteristics;. Part 2 consisted of gynecological history and the intensification of the menopausal symptoms estimated by Kupperman i and Greene index along with the subjective evaluation in terms of physical, social and job activity. The gained data was statistically analyzed by computer software STATISTICA. The significance α = 0.05 level was assumed. The average age of women was 56.4 ± 3.4 years. The average estradiol concentration in blood serum was 23.7 ± 20.22 pg / ml. Most of the participants had normal levels of estradiol 88.72%. 11.28% of tested women had exceeded the norm level of estradiol for women after the menopause. Almost half of the tested women had ERα Xba1 AG genotype, about 35% are carriers of AA Xba1 genotype, and about 17% - GG. ERα PvuII polymorphism occurred most frequently in the TC genotype (45%), while the other two genotypes of polymorphism occurred with similar frequency (28% had TT and 27% CC). The menopausal symptoms lasted from 2 to 5 years in nearly half of the studied group of women. It has been shown that the duration of menopausal symptoms depend on the type of polymorphism genotype ERα ,Xba1 and PvuII. The menopausal symptoms which lasted to 2 years were significantly less observed in the group of women with AA Xba1 genotype . In 2/3 of these women the symptoms lasted from 2 to 5 years than in those who had AG and GG polymorphism genotype. The women with genotype TT PvuII had menopausal symptoms lasting up to two years, significantly less than the women with other types of this polymorphism genotype. Women with the CC genotype had menopausal symptoms lasting from 2 to 5 years more often than those with other types of PvuII polymorphism genotypes. Additionally, women with TT and TC PvuII genotype had the lowest, and women CC PvuII the highest intensification of vasomotor symptoms on the Greene scale. The connection between estradiol concentration and the severity of menopausal syndrome, measured by the Kupperman index was found. Women without menopausal syndrome had significantly lower levels of estradiol than women with menopausal syndrome. Serious menopausal syndrome more frequently occurred to women with the concentration of estradiol in the standard, than to women with estradiol levels above normal. In the studied group of women both systolic and diastolic blood pressure dependeded on their Xba1 and PvuII polymorphism genotypes. Women with GG Xba1genotype had significantly lower systolic and diastolic blood pressure than women with other genotypes of this polymorphism. Women with CC PvuII genotype had the lowest systolic and diastolic blood pressure, slightly higher - women with TT, and the highest - with TC. Increase in blood pressure after the menopause occurred significantly less frequently in women with GG genotype CC Xba1 and PvuII than in women with other genotypes of these polymorphisms. In the studied group of women 86,84% had total cholesterol above the norm, 43.23% of the women had LDL above the norm, 83.83% - HDL below normal. There was a significant relationship between total cholesterol levels and the Xba1 and the relationship of triglycerides levels from Xba1 and PvuII polymorphisms. The women with GG Xba1 genotype had significantly higher total cholesterol than women with other genotypes of this polymorphism. The highest concentration of triglycerides was observed in women with the GG Xba1genotype and CC PvuII genotype. Lower concentrations of triglycerides occurred in women with AA Xba1 genotype and TT PvuII genotype, and the lowest levels were found in women with AG Xba1 genotype and TC PvuII genotype. The studied group of women with normal triglyceride levels had significantly higher levels of estradiol than women with triglyceride levels above normal. Higher concentrations of estradiol were accompanied by lower triglycerides. HDL, LDL and triglyceride levels correlated multidirectionally with the concentration of estradiol depending on their polymorphism ERα genotypes. Higher concentrations of estradiol in women with genotypes: a) GG Xba1 and TC PvuII corresponded to higher levels of HDL; b) TT PvuII corresponded to the lower concentration of HDL; c) TC PvuII corresponded to lower LDL; d) AG Xba1 and TC PvuII corresponded to lower triglyceride levels. The average value of BMI in tested women was 26.40 kg / m2. 40% of these women had normal BMI, 40% were overweight, 19% were obese and 6 women were underweight. In the majority of tested women there was an increase in body weight after menopause (59%), in 34% body weight has not changed, and at 7.5% - has decreased. The dependence of the results in BMI ranges to PvuII polymorphism has been shown .The tested women with TT and CC PvuII genotypes had mostly normal weight, while women with the TC genotype of this polymorphism were mostly overweight. About half of the women with TT and CC had weight below the standard or normal, and the majority of women with TC were overweight or obese. It was alleged that, larger waist circumference was associated with a lower concentration of estradiol. It was alleged that there are significantly lower concentrations of estradiol in obese women compared to overweight women and women with normal weight. In the tested group of women with AA genotype Xba1 polymorphism and TT genotype PvuII polymorphism has been alleged that the higher the level of estradiol, the smaller was their waist circumference and lower the ratio of waist to hip circumference. The tested group of women showed no relationship between Xba1 and PvuII polymorphism and the subjective influence of menopause decreasing the quality of life. It was found that the women with significantly lower concentration of estradiol claimed a slight reduction in the quality of life after menopause than the women with moderate , high or no reduction in quality of life. Based on conducted studies the following conclusions were made: 1. Menopausal symptoms remained longer in women with ERα s XbaI and PvuII AA TT polymorphisms. Vasomotor symptoms were most intensely expressed in carriers PvuII CC polymorphism. 2. ERα PvuII XbaI AG and TC genotype was connected with higher systolic and diastolic blood pressure, and the increase of RR after menopause. 3. ERα PvuII TC genotype was connected with a higher incidence of overweight and obesity in women examined. 4. ERα ,XbaI and PvuII AA TT polymorphism had the beneficial effect of estradiol impact on the distribution of body fat after menopause. 5. XbaI GG ERα and PuvII CC genotype was related to higher results in total cholesterol and triglycerides. 6. ERα Xbal and PuvII genotype affect the result of the impact of estradiol on lipid levels in the study group: • in the subgroup XbaI and PvuII TC GG higher concentrations of estradiol were associated with higher levels of HDL and lower LDL • subgroup PvuII TT higher concentrations of estradiol was associated with lower levels of HDL cholesterol • subgroup XbaI and PvuII AG TC higher concentrations of estradiol was associated with lower serum triglycerides 7. There is a need for research on the impact of hormone therapy on menopausal symptoms depending on their ERα polymorphisms because there was no impact of ERα polymorphisms of the relationship of the concentration of endogenous estradiol with the severity of menopausal symptoms. 8. On the basis of ERα polymorphisms studies groups of women more at risk for developing cardiovascular disease can be identified. 9. Further studies of ERα polymorphisms can facilitate decision-making as to the advisability of the use of hormone therapy for the primary prevention of cardiovascular disease.
