MATERIAŁ I METODA: Materiał stanowiła dokumentacja 5797 pacjentów leczonych w Katedrze i Zakładzie Ortopedii Szczękowej AM w Lublinie w latach 1995-2005. Dokonano podziału na trzy grupy wiekowe: grupa 1 – dzieci do 13 roku życia w pełni uprawnione do leczenia bezpłatnego aparatami zdejmowanymi (4222 osoby), grupa 2 - dzieci i młodzież od 13 do 18 lat, którzy utracili prawo do leczenia bezpłatnego (1145 osób), grupa 3 - powyżej 18 roku życia (430 osób). Materiał podzielono na osoby płci żeńskiej - 3201 i męskiej – 2596 osób. Uzyskane wyniki poddano analizie statystycznej. WYNIKI: Aparaty zdejmowane wybierali głównie pacjenci z grupy 1. W grupie 2 od 1998 roku zaobserwowano spadek zainteresowania leczeniem aparatami zdejmowanymi wśród pacjentek a rok później wśród pacjentów. W badanym okresie AIM: The aim of the study was to determine the relation between patient’s rights for free treatment and the choice of treatment method. MATERIAL AND METHOD: The material for the study were dental histories of 5797 patients treated at the Department of Jaw Orthopedics in Lublin in the years 1995-2005.The patients were divided into three age groups: Group 1 – young children under 13 fully entitled to free treatment with removable appliances under the national health care system (4222 patients); Group 2 – children age 13-18 who were no longer entitled to free treatment (1145 patients), Group 3 – over age 18 (430 patients). The patients were divided according to sex: 3201 females and 2596 males. Obtained results were statistically analysed. RESULTS: Removable orthodontic appliances were mainly chosen by patients from Group 1. W grupie 3 pacjenci od roku 2001 decydowali się wyłącznie na leczenie aparatami stałymi. WNIOSKI: W okresie 1995-2005 wzrosło zainteresowanie leczeniem ortodontycznym pacjentów płci męskiej. Wybierali oni chętniej aparaty stałe. Leczenie aparatami zdejmowanymi stosowano głównie w grupie 1. Pacjenci z grupy 2 i 3 najczęściej decydowali się na leczenie aparatami stałymi. CONCLUSIONS: In the years 1995-2005 an interest with orthodontic treatment increased among male patients, who preferred fixed appliances. Treatment with removable appliances was chosen mainly in Group 1. Patients from Group 2 and group 3 most often chose treatment with fixed appliances. Od stycznia 1999 r. zaczęły funkcjonować Kasy Chorych, przekształcone w 2003 r. w Narodowy Fundusz Zdrowia. Leczenie ortodontyczne opłacane przez Kasy Chorych obejmowało wyłącznie aparaty zdejmowane, przysługujące pacjentom do 13 roku życia z bezpłatnymi wizytami kontrolnymi do lat 15 (1). NFZ finansuje leczenie ortodontyczne do 12 roku życia, a kontrole aparatów do lat 13. Leczenie aparatami stałymi zawsze wiązało się z kosztami ponoszonymi przez pacjenta. Wybierali tę metodę pacjenci, którzy utracili prawo do leczenia bezpłatnego oraz osoby przekonane o większej skuteczności tej techniki ortodontycznej. W świetle obserwacji praktyki codziennej można sądzić, że pacjenci ortodontyczni stali się bardziej wymagający i aktywni. Interesuje ich plan leczenia, jego cele i osobisty wkład w ich realizację (2). Brakuje w krajowej literaturze doniesień czy i jak zmieniało się podejście pacjentów do wyboru metody leczenia. Przeprowadzone badanie przedstawia te zależności w przeciągu ostatnich 11 lat. Social and economic changes in Poland in the 1990s resulted in the need for a change in the system of health care (1). The health scheme called the Sick Fund, founded in 1999, was converted into the National Health Fund (NFZ) in 2003. Orthodontic treatment paid by the Sick Fund included only removable appliances, free of charge for patients under 13, and free orthodontic control visits until age 15 (1). NFZ pays for orthodontic treatment until age 12, and orthodontic control visits until age 13. The treatment with fixed appliances was always paid by the patient. It was chosen by the patients who were not entitled to the free treatment and patients who were convinced about the higher effectiveness of this orthodontic technique. In the view of clinical practice it seems that orthodontic patients became more demanding and co-operative. They are interested in the treatment plan, its aim and their personal contribution to the treatment (2). There are no reports in Polish literature presenting whether and how the attitude of patients to the choice of treatment method has changed. This study presents these relations within the last 11 years. Materiał podzielono na osoby płci żeńskiej - 3201 i męskiej – 2596 osób. Dane dotyczące wieku, płci i rodzaju aparatu zostały wprowadzone do programu Excel Office. Obliczono procent wybieranych przez pacjentów aparatów stałych i zdejmowanych w latach 1995-2005 z uwzględnieniem płci. Uzyskane wyniki badań scharakteryzowano przy pomocy liczności i odsetka. Do porównania rozkładu częstości w analizowanych grupach użyto testu jednorodności chi - kwadrat. Przyjęto 5% błąd wnioskowania i związany z nim poziom istotności p<0,05 wskazujący na istnienie istotnych statystycznie różnic. treatment (1145 patients), Group 3 - over 18 (430 patients). The patients were divided according to sex into 3201 females and 2595 males. Data concerning age, sex and the used orthodontic appliance were introduced into Excell Office program. The percentage of chosen fixed and removable appliances in the years 1995-2005 was calculated taking into account the sex of the patient. The obtained study results were characterized by the amount and percentage. The homogeneity chi-square test was used to compare the distribution of frequency in the analysed groups. The inference error of 5% and connected with it level of significance p<0,05 indicating the presence of statistically significant differences was assumed. Wyniki Results Dane z pierwszego roku badania przyjęto za poziom wyjściowy. W badanym okresie leczenie aparatami zdejmowanymi wybrało 3473 (59,91%) pacjentów a aparatami stałymi 2324 (40,09%). W 1995 roku aparaty zdejmowane stanowiły aż 5,80%, podczas gdy aparaty stałe w tym samym czasie jedynie 2,17% wszystkich oddanych aparatów w ciągu 11 lat. Od 1997 roku różnica pomiędzy liczbą oddanych aparatów stałych i zdejmowanych zmniejszała się, co doprowadziło w 2003 roku do odwrócenia stosunku na korzyść częstszego leczenia aparatami stałymi niż zdejmowanymi. Na przestrzeni 11 lat nastąpił dwukrotny wzrost oddawanych aparatów stałych (Ryc.1). W każdym roku badania występuje istotny związek statystyczny pomiędzy wyborem aparatu przez osoby płci żeńskiej (p≤0,003) i męskiej (p≤0,002). Grupa 1 była najliczniejsza, stanowiła aż 72,83% pacjentów, grupa 2 - 19,75% pacjentów, grupa 3 - 7,42% pacjentów. W badanym okresie aparaty zdejmowane wybierali głównie pacjenci z grupy 1. Osoby płci żeńskiej stanowiły 55,22% ogółu badanych a płci męskiej 44,88% (Tab.1). Data from the first year of study were used as a baseline. In the study period the treatment with removable appliances was chosen by 3473 (59,91%) of patients while with fixed appliances 2324 (40,09%) patients. In 1995 removable appliances constituted 5,80%, while fixed appliances at the same time were only 2,17% of all appliances used within 11 years. Starting from 1997 the difference between used fixed and removable appliances steadily decreased, which resulted in 2003 in reversing the ratio for the sake of more frequent treatment with fixed appliances than with removable ones. Within 11 years the number of fixed appliances increased twice (Fig.1). In each year of the study there was a statistically significant relation between the choice of appliance by females (p≤0,003) and males (p≤0,002). Group 1 was the largest and included 72,83% of patients, Group 2 - 19,75% of patients, Group 3 - 7,42% of patients. In the study period removable appliances were chosen mainly by patients from Group 1. Female patients constituted 55,22% of the total number of patients and male patients 44,88% (Table 1). Tabela 1. Odsetek oddanych aparatów stałych u pacjentek wzrósł trzykrotnie z 0,48% do 1,71%, a u pacjentów czterokrotnie z 0,36% do 1,67%. W ostatnich trzech latach badania w grupie tej wzrosło zainteresowanie leczeniem aparatami stałymi a ich ilość zbliżyła się do ilości oddanych aparatów zdejmowanych (Tab.2). In Group 1 the choice of removable appliances by the female patients remained on the similar level througout the whole study period and was 26,23% (Table 2). The percentage of used fixed appliances in the female patients increased threefold from 0,48% to 1,71%, and among the male patients fourfold from 0,36% to 1,67%. In the last three years of study the interest in the treatment with fixed appliances increased in this group and their number was close to the number of removable appliances (Table 2). Tabela 2. Od 1998 roku zaobserwowano spadek zainteresowania leczeniem aparatami zdejmowanymi wśród pacjentek a rok później wśród pacjentów (Tab. 3). Najstarsza grupa wiekowa to w 4,66% kobiety, które leczyły się aparatami stałymi w zbliżonym odsetku przez wszystkie badane lata. Zmniejszyła się ilość oddanych aparatów zdejmowanych z 17 w 1995 roku do 0 w 2005 roku. Mężczyźni w tej grupie wiekowej również zrezygnowali z leczenia aparatami zdejmowanymi. W grupie 3 pacjenci od roku 2001 decydowali się wyłącznie na leczenie aparatami stałymi (Tab.4). In Group 2 there was an increasing interest in the treatment with fixed appliances among the male patients. Since 1998 less interest in the treatment with removable appliances was observed among the female patients and a year later among the male patients (Table 3). Group 3 – patients over 18 - included 4,66% of females treated with fixed appliances in similar percentage throughout all study years. The number of removable appliances decreased from 17 in 1995 to 0 in 2005. The male patients in this age group also resigned from the treatment with removable appliances. Wachlarz oferowanych metod diagnostycznych i leczniczych zaczął się zwiększać od 1989 roku, kiedy na szerszą The demand for treatment depends on social and economic situation and also on cultural background (3,4). Większa efektywność, mniejsze wymagania co do współpracy pacjenta i krótszy okres leczenia, zwiększyły zainteresowanie pacjentów aparatami stałymi. Wzrost zapotrzebowania na leczenie ortodontyczne przyczynił się do zwiększenia liczby specjalistów (5). W 1984 roku obserwowano lepsze wyniki lecznicze u pacjentek leczonych aparatami zdejmowanymi z powodu większej motywacji, systematyczności i pilności (6). Obecne badania wskazują na stopniowe wyrównywanie się liczby leczonych pacjentów płci męskiej i żeńskiej, choć w przeszłości autorzy tego nie zauważali (3,6). Przez cały okres badania wzrastała liczba osób płci męskiej i byli oni tak samo jak pacjentki zainteresowani poprawą swojego wyglądu. W kolejnych latach wzrastał odsetek pacjentów płci męskiej leczonych aparatami stałymi, którzy gorzej współpracowali podczas leczenia aparatami zdejmowanymi (1). Stwierdzono istotną zależność statystyczną pomiędzy wyborem aparatu w poszczególnych grupach wiekowych (p≤0,00000003). W pierwszej grupie wiekowej statystycznie częściej wybierane były aparaty zdejmowane (p≤0,00000001), w grupach powyżej 13 roku życia aparaty stałe (p<0,000006). W każdej grupie wiekowej leczyło się więcej osób płci żeńskiej, ale jednocześnie w kolejnych latach wzrastało zainteresowanie leczeniem ortodontycznym wśród osób płci męskiej. Do 1999 roku aparaty zdejmowane nie były objęte limitem wiekowym dopiero Kasy Chorych ograniczyły dostęp do bezpłatnego leczenia tymi aparatami (1). Nie wpłynęło to jednak na zwiększenie ilości oddanych aparatów zdejmowanych w grupie do 13 r. ż. Wnioski 1. W okresie 1995-2005 wzrosło zainteresowanie leczeniem ortodontycznym pacjentów płci męskiej. Wybierali oni chętniej aparaty stałe. 2. Leczenie aparatami zdejmowanymi stosowano głównie w grupie 1. 3. Pacjenci z grupy 2 i 3 najczęściej decydowali się na leczenie aparatami stałymi. 4. Na przestrzeni 11 lat nastąpił dwukrotny wzrost zainteresowania aparatami stałymi. in Lublin on a large scale. Higher effectiveness, lower requirements concerning co-operation of the patient and shorter lenght of treatment increased the interest of patients in fixed appliances. The increased demand for the orthodontic treatment contributed to the increase in the number of specialists (5). In 1984 better effectiveness of treatment was observed among female patients treated with removable appliances due to their higher motivation, regular wearing and conscientiousness (6). Current studies reveal gradual levelling out the number of treated male and female patients, although this fact was not noticed by the authors in the past (3,6). The number of male patients was constantly increasing during the whole period of study and they were interested in the improvement of their appearance in the same degree as female patients. In the consecutive years the percentage of male patients treated with fixed appliances was increasing, as they did not co-operate well during the treatment with removable appliances (1). A significant statistical dependence between the choice of the appliance in particular age groups (p≤0,00000003) was revealed. In Group 1 removable appliances were chosen statistically more often (p≤0,00000001), in Group 2 - fixed appliances (p<0,000006). In each age group more female patients were treated, but at the same time the interest in the orthodontic treatment among male patients gradually increased in successive years. Until 1999 removable appliances were not a subject to the age limit, but the Sick Fund restricted the access to free treatment with these appliances (1). However, this did not affect the increase in the number of removable appliances in Group 1 under age 13. Conclusions 1. In the years 1995-2005 the interest in orthodontic treatment among male patients increased. They usually chose fixed appliances. 2. The treatment with removable appliances was used mainly in Group 1. 3. Patients from Group 2 and 3 most often decided to be treated with fixed appliances. 4.
