Sprawność stóp uzależniona jest od budowy morfologicznej zwłaszcza od wysklepienia podłużnego, poprzecznego, ustawienia palucha i palca V. Budowie stóp poświęcono wiele publikacji. Oceniano związki wysklepienia podłużnego z cechami morfologicznymi. Brakuje badań związków wysklepienia poprzecznego, ustawienia palucha i V palca z cechami somatycznymi. Celem pracy była ocena związków między wysklepieniem podłużnym, poprzecznym stóp, ustawieniem palucha, palca V, a cechami somatycznymi kobiet. Materiał i metody. Przebadano 67 pracownic w wieku 30-40 lat Szpitala Kolejowego w Wilkowicach-Bystrej, NZOZ Rehabilitacja w Wodzisławiu Śląskim, Zespołu Dziennych Ośrodków Rewalidacyjno-Edukacyjno-Wychowawczych i Rehabilitacyjnych w Radzionkowie. Średnia wieku‒36.5 lat. Plantograficznie oceniono wysklepienie podłużne, poprzeczne stóp, ustawienia palucha i palca V. Zmierzono masę, wysokość ciała, obwód talii, bioder i obliczono BMI oraz WHR. Wyniki. Stwierdzono korelację wysklepienia podłużnego z cechami budowy ciała na poziomie p<0.05; p<0.01; p<0.001 u kobiet. Wysklepienie poprzeczne, ustawienie palucha, palca V nie koreluje z budową ciała. Należy kontynuować badania związków budowy stóp z cechami somatycznymi. Wyodrębnienie, najistotniejszych cech może podnieść efektywność terapii przeciw deformacjom stóp ludzi dorosłych. Wnioski. Wysklepienie podłużne koreluje z masą ciała, wskaźnikiem BMI, WHR dlatego pykniczna budowa ciała sprzyja płaskostopiu. The function and performance of the feet depends on their morphology, and particularly on the longitudinal and transverse arch, and the position of the hallux and 5th toe. A number of publications have been devoted to foot morphology, mostly concerned with the relation between the longitudinal arch and morphological features. However, there is a dearth of studies of correlations between the transverse foot arch, and position of the hallux and the 5th toe and body build features. The aim was to determine relationships between longitudinal and transverse arches, hallux and 5th toe positioning and body features in females. Material and methods. Sixty-seven female workers aged 30-40 years from the Railway Hospital in Wilkowice-Bystra, “Rehabilitation” Non-Public Health Care Institution, and Social Rehabilitation-Education-Rehabilitation Centre in Radzionków were evaluated. The mean age of the participants was 36.5 years. Body weight, height, BMI, and WHR were measured. Plantography was used to determine the Clarke angle, Wejsflog index, and α and β angle. Results. The longitudinal arch correlated with BMI and WHR, p<0.05; p<0.01; p<0.001. The transverse arch, hallux, and 5th toe did not correlate with body build. Investigations of relationships between foot morphology and body build need to be continued. By selecting the most significant features, therapies to counteract foot deformities in adults can be enhanced. Conclusion. The longitudinal arch of the feet correlates with the BMI and WHR indices. Therefore, a pyknic body build promotes flat foot. The transverse arch, hallux and 5th toe do not correlated with body build; thus somatic features do not influence the quality of the front part of the feet. BACKGROUND The function and performance of the feet depends on their morphological features, and particular on a normal longitudinal and transverse arch and the position of the hallux, 5th toe and heel. The anatomy and performance of the feet under static or dynamic conditions has been investigated in numerous studies. These studies have focused on different aspects and made use of a variety of technical solutions to enable objective assessment of correlations between foot morphology and body build features [1,2,3, 4,5,6,7,8,9]. Most of these studies have been evaluated correlations between the longitudinal arch of the foot and body build features. At the same time, there is a dearth of publications on correlations between the transverse arch and the position of the hallux and 5th toe with features of body build in adults. Previous studies of the relationship between the longitudinal foot arch and body build features have been mostly conducted among children and adolescents. These investigations proved very difficult and did not provide unequivocal conclusions due to instability and variability of body build in the course of ontogenesis. For this reason, adulthood appears to be the best period for solving this problem as adults demonstrate stable body build features. Accordingly, the possibility of serious measurement error is greatly reduced, entailing an objective verification of the relationship between the longitudinal and transverse arches of the foot and the anterior support region vs. functional and somatic body build features in adults. These facts prompted a study to determine correlations between the longitudinal and transverse foot arches and the anterior support area and selected body build features in adult females. The results of the study and conclusions may offer a valuable complement to the current state of knowledge in this area and contribute to further research on the function and performance of the feet in adults. The aim of this publication is to determine relationships between longitudinal and transverse arches, hallux and 5th toe positioning and selected body build features, such as: body mass, body height, BMI (Body Mass Index) and WHR (Waist to Hip Ratio) in females. In order to verify the purpose of the study, the following research questions and hypotheses were posed: is c op y is Pytania 1. This copy is for personal use only - distribution prohibited. - WSTĘP Sprawność i wydolność stóp uzależniona jest od ich budowy morfologicznej, a zwłaszcza od prawidłowego ukształtowania wysklepienia podłużnego, poprzecznego, ustawienia palucha, V palca i pięty. Zagadnieniu budowy i wydolności stóp w warunkach statycznych i dynamicznych poświęcono wiele oryginalnych prac naukowych. Badania prowadzono w różnych aspektach, uwzględniających rozmaite rozwiązania techniczne ukierunkowane na obiektywną ocenę związków budowy stopy z cechami budowy ciała [1,2,3,4,5,6,7,8,9]. Większość z nich koncentrowała się na ocenie zależności wysklepienia podłużnego stóp od cech budowy ciała, natomiast brakuje publikacji na temat związków wysklepienia poprzecznego stóp, ustawienia palucha, V palca z cechami budowy ciała osób dorosłych. Dotychczasowe badania związków wysklepienia podłużnego stóp z cechami budowy ciała prowadzono zwłaszcza wśród dzieci i młodzieży. Okazały się one niezwykle trudne i nie dały jednoznacznych rozwiązań, ze względu na labilność i zmienność budowy ciała w ontogenezie. Dlatego, właśnie okres dojrzałości wydaje się być najkorzystniejszy dla ostatecznego rozwiązania powyższego problemu, ponieważ osoby dorosłe wykazują stabilność cech morfologicznych budowy ciała. Stąd bardziej prawdopodobne staje się popełnienie mniejszego błędu pomiarowego, a co za tym idzie obiektywne zweryfikowanie zależności pomiędzy wysklepieniem podłużnym, poprzecznym stóp oraz ich przedniej strefy podparcia, a parametrami somatycznymi i funkcjonalnymi budowy ciała dorosłych. Biorąc pod uwagę przedstawione fakty autor podjął badania, których celem jest ocena występowania związków wysklepienia podłużnego, porzecznego stóp oraz przedniej strefy podparcia z wybranymi cechami budowy ciała kobiet. Uzyskane wyniki i wyciągnięte wnioski mogą stać się w opinii autora cennym uzupełnieniem obecnego stanu wiedzy na powyższy temat i być przyczynkiem do dalszych badań naukowych w zakresie oceny sprawności i wydolności stóp dorosłych. Celem pracy jest ocena występowania związków między wysklepieniem podłużnym, poprzecznym stóp, ustawieniem palucha i V palca, a wybranymi cechami budowy ciała, takimi jak: masa ciała, wysokość ciała, BMI (Body Mass Index) oraz WHR (Waist to Hip Ratio) kobiet. MATERIAL AND METHODS This cross-section study involved sixty-seven female workers aged 30-40 years from the Railway Hospital in Wilkowice-Bystra, “Rehabilitation” Non-Public Health Care Institution, and the Social Rehabilitation-Education-Rehabilitation Centre in Radzionków. The mean age of the participants was 36.5 years. The participants declared no disease or past injuries of the musculoskeletal system, including the lower limbs. The study was carried out in May 2008. The methodology was based on plantographic measurement of features of the plantar surface under static conditions with no additional load. The foot prints were used to determine the Clarke angle, Wejsflog index, and α and β angle. The Clarke angle is the angle between the tangent at the medial margin of the foot print (a-b) and the line connecting the greatest depression and the point at which the medial tangent crosses the margin of the front foot (B-C). The degree of the longitudinal foot arch can be determined to an accuracy of 1º (Fig. 1). Measurement of transverse foot arch (Wejsflog’s W index). This the ratio of foot print length to foot print width, where D is the distance from the furthest point on the heel to the bulb of the longest toe, and S is the distance between the farthest anterior points of the 1st and 5th toe metatarsus bones. This copy is for personal use only - distribution prohibited. - MATERIAŁ I METODY Badaniami przekrojowymi objęto 67 kobiet w wieku 3040 lat. Średnia wieku w badanej grupie wynosiła (͞x) 36.5 lat. Badania przeprowadzono wśród pracowników: Szpitala Kolejowego w Wilkowicach Bystrej, NZOZ Rehabilitacja w Wodzisławiu Śląskim, Zespołu Dziennych Ośrodków Rewalidacyjno-Edukacyjno-Wychowawczych i Rehabilitacyjnych w Radzionkowie. Osoby nie deklarowały żadnych chorób i przebytych urazów narządu ruchu, w tym kończyn dolnych. Badania przeprowadzono w maju 2008 roku. Podstawową metodą badawczą była plantograficzna ocena podeszwowej powierzchni stóp w warunkach statycznych w obciążeniu własną masą ciała. Z uzyskanych odbitek otrzymano: kąt Clarke’a, wskaźnik „W” Wejsfloga, kąt koślawości palucha (α), szpotawości V-go palca (β). Kąt Clarke’a (Cl) – jest zawarty między styczną przyśrodkowego brzegu odbitki (a-b) a linią łączącą punkt największego wgłębienia i zetknięcia stycznej przyśrodkowej z brzegiem przodostopia (B-C). Kąt Clarke’a określa poziom wysklepienia łuku podłużnego stopy z dokładnością do 1º (Ryc. 1). Obliczono średnią arytmetyczną (͞x), odchylenie standardowe (S), współczynnik zmienności (V). Dla wykazania, czy istnieją związki między cechami podeszwowej powierzchni stóp, a parametrami budowy ciała wykorzystano korelację liniową „r” Pearsona [10]. Poniżej w Tab. 1 przedstawiono podstawowe charakterystyki statystyczne mierzonych cech u badanych kobiet. Statistical design The basic descriptive statistics were used, such as means (͞͞x), standard deviations (s) and variability coefficients (V). The evaluation of relationships between longitudinal and transverse foot arches, and hallux and 5th toe position and body build features used Pearson’s r linear correlation [10]. The results show the selected body build features (BMI, WHR and body weight) correlated with the longitudinal foot arch in the women, but did not correlate with the transverse arch, because the correlation coefficients were low (from r=-0.025 to r=0.202) and did not achieve the required level of statistical significance of p<0.05, for which r=0.250 (Tab. 2). lu W Tab. 2. zamieszczono uzyskane wartości współczynników korelacji między wybranymi cechami budowy ciała, a wysklepieniem podłużnym i poprzecznym stóp kobiet. The WHR index correlated with Clarke angle values for the left and right foot (r=-0.450 and r=-0.436, p<0.001) (Tab. 2). Body weight correlated with the Clarke angle of the left and right foot (r=0.406, p<0.01 and r=-0.289, p<0.05) (Tab. 2). No correlations were found between the Clarke angle and Wejsflog’s W index vs. height, because the respective correlation coefficients were low and did not achieve the level of statistical significance (r=0.014 and r=0.202, p>0.05) (Tab. 2). No relationships were found between hallux (α angle) and 5th toe (angle β) position and body build features in the females. All correlation coefficients were low did not achieve the level of statistical significance, i.e. p<0.05 (Tab. 3). The BMI index did not correlate with the position of the hallux (angle α) in the left and right foot (r=-0.175 and r=-0.132, p>0.05) (Tab. 3). Similarly, the BMI did not correlate (β angle) with the position of the 5th toe in the left and right foot (r=-0.228 and r=-0.135, p>0.05) (Tab. 3). The correlation coefficients between the WHR index and the α angle of the left and right foot also were not statistically significant (r=0.105 and r=0.070, p>0.05) (Tab. 3). The WHR index did not also correlate with the β angle (position of the 5th toe) in the left and right foot in the females (r=0.049 and r=0.114, p>0.05) (Tab. 3). The correlation coefficients between body weight and the α and β angles of the left and right feet varied between r=-0.150 and r=-0.161, p>0.05 and from r=-0.146 to r=-0.041, p>0.05, respectively (Tab. 3). Wskaźnik BMI wykazuje związek z wysklepieniem podłużnym stopy lewej i prawej (kąt Clarke’a) (r =-0.415, p<0.001 i r=-0.269, p<0.05) (Tab. 2). Wskaźnik WHR koreluje z kątem Clarke’a stopy lewej i prawej (r=-0.450 i r=-0.436, p<0.001 (tab. 2). Masa ciała wykazuje związek z wysklepieniem podłużnym stopy lewej i prawej (r=0.406, p<0.01 i r=-0.289, p<0.05) (Tab. 2). Nie stwierdzono związków między wysklepieniem podłużnym i poprzecznym stóp, a wysokością ciała kobiet, ponieważ uzyskane współczynniki korelacji są niskie, wahają się od r=0.014 do r=0.202 i nie osiągają najniższego zakładanego poziomu istotności statystycznej p<0.05 (Tab. 2). Nie stwierdzono związków ustawienia palucha (kat α), V palca (kąt β) z wybranymi cechami budowy ciała kobiet. Współczynniki korelacji we wszystkich przypadkach są niskie i nie osiągają najniższego zakładanego poziomu istotności p<0.05 (Tab. 3). Wskaźnik BMI nie koreluje z ustawieniem palucha (Tąt α) stopy lewej i prawej (r=-0.175 i r=-0.132, p>0.05) (Tab. 3). Podobna tendencja występuje w przypadku korelacji między BMI, a ustawieniem V palca (kąt β) w stopie lewej i prawej (r=-0.228 i r=-0.135, p>0.05) (Tab. 3). Współczynniki korelacji między wskaźnikiem WHR, a kątem α stopy lewej i prawej są także nieistotne (r=0.105 oraz r=0.070, p>0.05) (Tab. 3). Wskaźnik WHR również nie koreluje z ustawieniem V palca stopy lewej i prawej (r=0.049 i r=0.114, p>0.05) (Tab. 3). The correlation coefficients between body height and α and β angles of the left and right foot were not statistically significant (r=0.028 and r=-0.091, p>0.05 and r=0.167 and r=0.212, p>0.05, respectively) (Tab. 3). DISCUSSION Assessments of relationships between the undersurface of the feet and body build features have been conducted in people of different ages and in different environments. Most reports have focused on causal relations between the longitudinal foot arch and body build features. At the same time there is a dearth of publications between the relation of transverse foot arches, and the position of the hallux and 5th toe with body build features. Lizis and Puszczałowska-Lizis [4] ascertained a correlation between the Clarke angle vs. body weight and the BMI index in Polish league basketball players. According to the authors, increased static load on the feet reduces the longitudinal foot arch. Micle et al [5] studied obese preschool children and concluded that a higher BMI directly influenced the presence of a low longitudinal foot arch and, when persistent, could lead to permanent structural changes in foot anatomy in the future. Villaroya et al. [8] found a correlation between the longitudinal arch of the feet and the BMI index as well as body weight in Spanish children from Saragossa aged 9-16 years. The authors found that people with normal body weight had high longitudinal foot arches and obese people had low longitudinal arches. In other words, obese people had flatfoot. They found a correlation between the longitudinal foot arch and BMI as well as body weight. At the same time, the authors did not discover any relationships between the longitudinal foot arch and body height. Wearning et al [9] found a correlation between body fat and increased load on the plantar surface of the feet under dynamic conditions in adult people, which, according to the authors, means that increased static load leads to a reduction in the longitudinal foot arch. Dowling et al [11] ascertained, in a dynamic evaluation on the tensometric platform, that a BMI index above average values was associated with excessive load pressure on the plantar surface of the feet, directly increasing their contact with the ground, increased foot length and width and a concomitant reduction in the longitudinal foot arch. Leung et al [12] evaluated relationships between the longitudinal foot arch and body weight under dynamic conditions on the tensometric platform in a Chinese population. They found that increased load pressure on the plantar surface of the feet was accompanied by a reduction of the longitudinal arch of the feet. Accordingly, they posit that excessive weight has a negative influence on foot function and performance under dynamic conditions. Politowski et al. [13], in a study of people working in the standing position, found that the longitudinal arch of the feet decreased as the Rohrer index increased. Ślężyński and Rottermund [14] made plantographically evaluated feet in medium-aged and elderly women. Lizis i Puszczałowska-Lizis [4] stwierdzili u koszykarzy I ligi polskiej korelację między kątem Clarke’a, a masą ciała i wskaźnikiem BMI, zwracając uwagę, że wzrost statycznego obciążenia stóp wpływa negatywnie na poziom ich wysklepienia podłużnego. Micle i wsp [5] badali otyłe dzieci przedszkolne, dochodząc do wniosku, że wzrost BMI wpływa bezpośrednio na niski poziom wysklepienia podłużnego stóp i taki długotrwały stan może w przyszłości prowadzić do trwałych zmian strukturalnych w ich budowie anatomicznej. Villaroya i wsp [8] analizowali związki wysklepienia podłużnego stóp z masą ciała, wysokością ciała, wskaźnikiem BMI hiszpańskich dzieci w wieku 9-16 lat z terenu Saragossy. Autorzy stwierdzili, że osoby o prawidłowej masie ciała posiadają wysoki poziom wysklepienia podłużnego stóp, osoby z nadwagą posiadają przeciętne wysklepienia, a otyli charakteryzują się płaskostopiem. Jednocześnie cytowani autorzy stwierdzili korelacje między wysklepieniem podłużnym stóp, a wskaźnikiem BMI i masą ciała. Nie stwierdzili związków między wysokością ciała, a poziomem wysklepienia podłużnego stóp. Wearning wsp [9] stwierdzili u osób dorosłych w warunkach dynamicznych korelację między ogólną masą tkanki tłuszczowej, a wzrostem obciążenia podeszwowej powierzchni stóp. W rezultacie uzyskane wyniki wskazują, że u osób otyłych wzrost obciążenia statycznego powoduje obniżenie się poziomu wysklepienia podłużnego stóp. The results of the present study are consistent with the results presented by the above authors. We found a negative correlation between the longitudinal foot arch (Clarke angle) and body weight, BMI, and WHR in females. The results indicate that increased static load on the feet leads to a reduction of the longitudinal foot arch in women, confirming the study hypothesis. Therefore, pyknic (obese) persons more frequently have longitudinal flat feet and static and dynamic insufficiency of the feet than people with normal body weight. There were no causal relationships between the transverse foot arch (Wejsflog’s W index) and body build features in the women. The absence of correlations indicates that the body build features investigated in this study do not have a major influence on the transverse foot arch. Therefore, the hypothesis that body build features have an important influence on Wejsflog’s W index should be rejected. No correlations were also found between body build features and the position of the hallux (α angle) or the 5th toe (β angle) in the left and right foot in the females. These results probably indicate that body build features also are not directly associated with the performance of the forefoot support area and lead to the rejection of the hypothesis that higher static load leads to hallux valgus and 5th toe varus deformity. Perhaps, attention should be drawn to bony anatomy of the foot, and especially to genetic determinants of skeletal development, which are hardly modifiable. We are also quite powerless against the accumulating macro- and microscopic structural changes in bone tissue over time, which appear to be more significant for the prevention of untoward age-related structural changes in the feet. The results and conclusions presented in this publication indicate, in the opinion of the present author, a need to continue extensive research on the relationships between the transverse foot arch and the position of the hallux and 5th toe, and body build features under static and dynamic conditions using state-of-the-art technical solutions in the form of tensometric platforms. Uzyskane wyniki badań własnych są zbieżne z rezultatami cytowanych powyżej autorów. Stwierdzono, bowiem występowanie ujemnych korelacji między wysklepieniem podłużnym stóp (kąt Clarke’a), a masą ciała, wskaźnikiem BMI, WHR kobiet. Przedstawione wyniki wskazują, że wraz ze wzrostem statycznego obciążenia stóp spowodowanego nadmierną masą ciała, zbyt dużym wskaźnikiem BMI i WHR dochodzi do obniżenia się poziomu wysklepienia podłużnego stóp kobiet, co potwierdza założoną w niniejszej publikacji hipotezę badawczą. W związku z tym osoby z tendencją do pyknicznego typu budowy ciała (otyłe) są częściej narażone na płaskostopie podłużne i niewydolność statyczno-dynamiczną stóp, niż osoby szczupłe. Nie stwierdzono związków przyczynowo-skutkowych między poziomem wysklepienia poprzecznego stóp (wskaźnik „W” Wejsfloga), a cechami budowy ciała kobiet. Brak korelacji wskazuje, że analizowane cechy budowy ciała nie mają pierwszorzędnego wpływu na wysklepienie poprzeczne stóp kobiet, a hipotezę zakładającą, że cechy budowy ciała wpływają na wskaźnik „W” Wejsfloga należy odrzucić. Nie stwierdzono również związków między cechami budowy ciała, a ustawieniem palucha (kąt α) i V palca (kąt β) w stopie lewej i prawej. Spostrzeżenie to prawdopodobnie wskazuje, że parametry budowy ciała również nie mają bezpośredniego wpływu na wydolność przedniej strefy podparcia stóp, a przyjętą hipotezę zakładającą, że nadmierne obciążenie statyczne prowadzi do koślawości palucha i szpotawości V palca należy odrzucić. Może, więc należy zwrócić uwagę na układ kostny stopy, szczególnie na genetyczne uwarunkowania rozwoju kośćca, na którego przebieg w nikłym tylko stopniu możemy wpłynąć, a zupełnie bezsilni jesteśmy wobec nasilających się z czasem zmian w mikro i makroskopowej strukturze tkanki kostnej, które wydają się bardziej istotne z punktu widzenia zapobiegania niekorzystnym zmianom w budowie stóp z wiekiem. Przedstawione wyniki badań oraz wyciągnięte wnioski, w opinii autora wskazują na potrzebę prowadzenia dalszych, szeroko zakrojonych badań naukowych dotyczących oceny związków wysklepienia poprzecznego, ustawienia palucha, V palca z cechami budowy ciała w warunkach statycznych i dynamicznych z zastosowaniem nowoczesnych rozwiązań technicznych w oparciu o wykorzystanie platform tensometrycznych.
