I mplant dentistry has developed further over the last several years as a result of patient demand for aesthetic treatment. Augmentation of hard and soft tissue is increasingly necessary to achieve good aesthetic results and to follow a precise backward treatment plan. Nowadays, guided bone regeneration, the use of a barrier membrane that will separate and protect the grafting material covered by a flap, is the preferred technique among all augmentation procedures in the treatment of alveolar ridge defects. At aesthetics ONE dental studios, we recently conducted a study based on our clinical cases. The purpose was to determine whether a ridge augmentation procedure using a resorbable collagen membrane deliberately left open to heal, that is not covered by a lap (open healing), could be an alternative for better bone preservation and new bone formation. We also wanted to establish whether it would increase patient comfort in daily practice compared with the classic guided bone regeneration protocol. For the study, radiographs, CBCT scans and measurements were taken |Dr Alecsandru Ionescu, Romania. |Dr Alecsandru Ionescu, Rumunia. preoperatively, immediately postoperatively, as well as after six and 12 months, in order to determine the bone preservation and formation at each point and to integrate the results with the clinical outcome. We found that the bone volume was preserved in height and width for an ideal second-stage lapless implant placement after six months with bone preservation of 98.9 ± 0.7 % compared with initial measurements. 1a AD plantach jest odtworzenie optymalnych warunków klinicznych poprzez zastosowanie zabiegów regenerujących utracone tkanki twarde i miękkie. Preferowaną techniką, stosowaną w zabiegach uzupełniania ubytków kostnych w zakresie wyrostków zębodołowych, jest sterowana regeneracja kości (GBR) z wykorzystaniem biomateriałów i membran oddzielających i chroniących materiał kościozastępczy. Miejsce regeneracji w klasycznym postępowaniu całkowicie pokrywa się płatem tkankowym. Celem badania było sprawdzenie, czy procedura uzupełniania ubytków kostnych w zakresie wyrostka zębodołowego z użyciem membrany kolagenowej w warunkach otwartego gojenia (bez zespalania brzegów rany) może być alternatywnym rozwiązaniem, zapewniającym lepsze zachowanie kości i wytwarzanie nowej tkanki kostnej. Chcieliśmy także stwierdzić, czy opisywana metoda zapewni pacjentowi 1b klinicznymi. Wyniki badania potwierdziły, że powstała szerokość i wysokość kości umożliwiły idealne, bezpłatowe umieszczenie implantu. Objętość kości (wysokość i szerokość) po sześciu miesiącach w porównaniu do początkowych pomiarów została zachowana w 98,9 % +/- 0,7 %. We wszystkich przypadkach zabieg został przeprowadzony bez odwarstwiania okostnej i mobilizacji płata. Uzupełniany obszar pokryty został wyłącznie wchłanialną membraną kolagenową, którą przyszywano do brzegów rany w celu jej stabilizacji. Zastosowano takie samo postępowanie zarówno w zabiegach estetycznych, jak i w przypadkach dotyczących odcinków bocznych. Z powodu wyjątkowych właściwości, materiał szewny PTFE jest materiałem z wyboru w opisywanej metodzie. In all of the cases we performed bone grafts without deperiostation and lap mobilisation, covering the augmented area from the beginning with only a resorbable collagen membrane and suturing it to the wound edges in order to ix it in a stable position using a PTFE suture (Corelon, Implacore). We followed the same procedure for both aesthetic and posterior zones. Owing to its unique properties, the PTFE suture contributed to adequate tissue healing with minimal micro-damage reaction and bacterial colonisation. It also increased patient comfort by very good adaptation to the volume of tissue in all its healing stages. We therefore consider the use of a continuous suture with Corelon the gold standard in the open healing protocol. Podstawowym warunkiem uzyskania estetycznych uzupełnień protetycznych opartych na im- większy komfort pozabiegowy w porównaniu do klasycznego protokołu GBR. Przed zabiegiem chirurgicznym i zaraz po nim oraz po 6 i 12 miesiącach od zabiegu, wykonano badania rentgenowskie oraz skany CBCT, aby określić w każdym przypadku zachowanie i tworzenie się nowej tkanki kostnej i aby powiązać otrzymane wyniki z rezultatami etapach gojenia oraz ograniczona do minimum reakcja na mikrouszkodzenia przyczyniły się do prawidłowego gojenia rany. Miękkość i gładkość materiału zapewniła pacjentom znacznie większy komfort pozabiegowy.
