§1
Przedmiotem umowy jest określenie wzajemnych praw i obowiązków Uczelni oraz
Doktoranta podejmującego niestacjonarne studia doktoranckie w Uniwersytecie Łódzkim.
§2

§3
1. Uczelnia oświadcza, że:
a) spełnia warunki, w tym kadrowe i związane z niezbędnym wyposażeniem do
prowadzenia kształcenia na studiach określonych w §2,
b) podjęte przez Doktoranta studia kończą się uzyskaniem świadectwa ukończenia studiów
doktoranckich,
c) planowany okres studiów, określonych w §2 wynosi …… semestrów,
2. Uczelnia zobowiązuje się do realizowania swoich obowiązków wobec Doktoranta w
zakresie określonym w Regulaminie Studiów Doktoranckich.
§4
1. Doktorant oświadcza, że znany mu jest Regulamin Studiów Doktoranckich obowiązujący
w Uczelni.
2. Doktorant zobowiązuje się postępować zgodnie ze złożonym ślubowaniem oraz
przestrzegać przepisów obowiązujących w Uczelni.
3. Doktorant zobowiązuje się uiszczać na rzecz Uczelni opłaty za kształcenie w terminach
określonych w niniejszej umowie.
4. Doktorant może wybrać formę płatności spośród wariantów określonych w załączniku do
niniejszej umowy (harmonogram opłat za studia).
5. Opłata za rok studiów, z zastrzeżeniem zasad określonych w ust. 6 i 7, wynosi .............................zł

6. W przypadku uiszczenia przez Doktoranta opłaty z góry za cały rok studiów w terminie do 15
października, Uczelnia udziela bonifikaty w opłacie w wysokości 5 %. W przypadku uiszczenia
opłaty z góry za cały semestr studiów w terminie do 15 października (za semestr zimowy) lub do
25 lutego (za semestr letni) bonifikata wynosi 2,5 % za każdy semestr, za który jednorazowa
opłata została uiszczona.
7. Doktorant może ubiegać się o przyznanie ulg w opłatach za zajęcia dydaktyczne na zasadach
określonych przez Senat Uczelni.
§5
1.
Za datę uiszczenia opłaty uważa się datę jej wpływu na wskazane konto Uniwersytetu Łódzkiego.
2. Nieuiszczenie opłaty za zajęcia dydaktyczne w terminie określonym w niniejszej umowie
rodzi obowiązek zapłaty odsetek ustawowych za każdy dzień opóźnienia.
3. W razie uzyskania przez Doktoranta zgody na odroczenie terminu uiszczenia opłaty za
zajęcia dydaktyczne, nieuiszczenie opłaty w nowym terminie powoduje skreślenie
Doktoranta z listy doktorantów, bez wezwania do zapłaty.
4. Skreślenie z listy doktorantów nie zwalnia Doktoranta od obowiązku uiszczenia zaległych
opłat wraz z odsetkami.
§6
Terminy uiszczania opłat za zajęcia dydaktyczne w Uniwersytecie Łódzkim:
a. przy opłacie jednorazowej za semestr:
do 15 października za semestr zimowy i do 25 lutego za semestr letni.
b. przy opłacie jednorazowej za rok:
do 15 października.
c. przy opłatach ratalnych:
do 15 października,
do 15 listopada,
do 15 grudnia,
do 25 lutego,
do 15 marca,
do 15 kwietnia,
do 15 maja.
§7
1. Doktorant, przyjęty na studia po 1 października, powinien uiścić opłatę w pełnej wysokości
przewidzianej za dany semestr/rok akademicki. Opłata obejmująca również raty przypadające na
okres przed jego immatrykulacją, uiszczana jest w najbliższym terminie płatności.
2. Od opłaty dokonanej w terminie określonym w ust. 1 odsetek nie pobiera się.
3. W razie dokonania w terminie określonym w ust. 1 opłaty za cały semestr/rok studiów,
doktorantowi przysługuje bonifikata, zgodnie z zasadami określonymi w § 4 ust. 6 umowy.
§8
1.
Osoba, która uiściła opłatę za studia, a studiów nie podjęła (tj. nie była immatrykulowana lub nie
złożyła ślubowania) może, na swój wniosek, otrzymać zwrot wpłaconej kwoty. Wniosek, wraz z
potwierdzonym przez kwesturę UŁ dowodem uiszczenia opłaty za studia, należy złożyć do
Rektora za pośrednictwem właściwego dziekanatu w terminie 30 dni od rozpoczęcia roku
akademickiego.

W przypadku rezygnacji ze studiów w trakcie roku akademickiego (poprzez złożenie w
dziekanacie pisemnego oświadczenia), Doktorant może wystąpić, za pośrednictwem właściwego
dziekanatu, do Rektora o zwrot uiszczonej opłaty pomniejszonej proporcjonalnie do okresu
odbytej nauki. Wysokość opłaty podlegającej zwrotowi oblicza się następująco: opłatę
semestralną albo roczną dzieli się przez liczbę tygodni w semestrze albo roku i mnoży się przez
liczbę tygodni nauki pozostałych Doktorantowi do zakończenia semestru albo roku.
3. W wyjątkowych przypadkach, Rektor może podjąć decyzję o zwrocie całości albo części
uiszczonej opłaty za studia.
§9
Doktorant ma obowiązek pisemnego powiadamiania Uczelni o zmianie jego danych
osobowych zawartych w niniejszej umowie. Skutki zaniechania wykonania tego obowiązku
obciążają Doktoranta.
§ 10
Umowa obowiązuje przez okres trwania studiów, o których mowa w §2, a wszelkie jej
zmiany wymagają zachowania formy pisemnej pod rygorem nieważności.
§ 11
W sprawach nieuregulowanych w niniejszej umowie obowiązują przepisy ustawy Prawo o
szkolnictwie wyższym oraz przepisy wykonawcze do tej ustawy, przepisy wewnętrzne
obowiązujące w Uczelni, w szczególności Statut UŁ i Regulamin Studiów w UŁ, a także
przepisy kodeksu cywilnego.
Statut UŁ, Regulamin Studiów Doktoranckich, uchwały Senatu UŁ oraz zarządzenia Rektora
UŁ znajdują się na stronie: www.uni.lodz.pl.
§ 12
Wszelkie sprawy sporne wynikające z niniejszej umowy rozstrzygane będą przez właściwe
sądy.
§ 13
Umowę sporządzono w dwóch jednobrzmiących egzemplarzach, po jednym dla każdej ze
stron.
W przypadku podejmowania studiów w języku obcym, umowa sporządzona zostaje w
czterech egzemplarzach – dwa w języku polskim i dwa w języku angielskim, po jednym
egzemplarzu w każdym języku dla strony umowy.

§1
The subject matter of the Agreement is to determine the mutual rights and obligations of the
University and the Doctoral Student who takes up extramural doctoral studies at the University of
Lodz.
§2
The agreement is made for the period of the doctoral study programme held
at the Faculty of …………………………………………………………………………

§3
1. The University represents that:
a)
It complies with the requirements, including those related to staff resources and necessary
teaching facilities, for the provision of education in the study programme specified in §2,
b)
The study programme in which the Student has enrolled leads to the doctoral studies leaving
certificate,
c)
the planned period of the study programme chosen by the Student is …………………….
semesters.
2. The University undertakes to fulfill its responsibilities towards the Student, as determined by the
Rules for Doctoral Studies.
§4
1. The Doctoral Student represents that he/she is familiar with the Study Regulations in force at the
University.
2. The Doctoral Student undertakes to act in accordance with the Oath taken, as well as to obey the
internal regulations of the University.
3. The Doctoral Student undertakes to pay the study fees within time limits specified in the present
Agreement.
4. The Doctoral Student can choose the method of payment from the options listed in the Annex (the
Study Fees Schedule) to the present Agreement.
5. The amount of the annual fee for a degree programme has been set at ……………….., (in words,
……………………………….) except for cases stipulated in part 6 and 7. The amount of fee for
one semester has been set as half the amount of the annual fee.
6. Should the Student make payment of annual fee in advance, and not later than 15 October, the
University shall give a 5% discount for the payment. Should the Student make payment of fee for
one semester in advance, and not later than 15 October (if for Winter semester) or 25 February (if
for Summer semester), a 2,5% discount shall be given for the payment for each semester.
7. The Student is entitled to apply for fees payment relief on conditions defined by the University
Senate.
§5
1. Payment date shall be defined as the date of arrival of the amount at the specified bank
account of the University of Lodz.
2. A failure to make payment of study fees in good time shall impose on the Student an
obligation of statutory interest payment for each day of delay.
3. Should the Doctoral Student be given permission for a prolongation of payment, a failure
to make the study fees payment by the new deadline shall result in removal, without a
notice of pay, from the University Register of Doctoral Students.
4. Removal from the University Register of Doctoral Students shall not free the Doctoral
Student from the obligation to make outstanding payment together with interest thereon.
§6
Deadlines for study fees payments at the University of Lodz:
a. For payment of semester fee covered in one amount:
15 October for Winter semester, and 25 February for Summer semester
b. For payment of semester fee covered in one amount:

§7
1. A Doctoral Student enrolled later than 1 October should pay the fee in full amount set for the
semester / academic year. The amount of fees, also including installments due to time before the
Doctoral Student’s matriculation, shall be paid in the earliest possible time.
2. No interest shall be collected on the fees paid in the time stipulated in Part 1.
3. Should the Doctoral Student make payment of annual/semestral fee in the time stipulated in Part 1,
they shall be given a discount for the payment in accord with rules stipulated in §4 Part 6.
§8
1. A person who has paid the study fees yet did not begin the course, (i.e. had not
matriculated, or did not take the Oath), is entitled to reimbursement for the fees on his/her
own request. The request, together with the study fee payment receipt confirmed by the UL
Bursar’s Office, is to be submitted to the Rector through the agency of the appropriate
Faculty Dean’s Office, not later than 30 days from the date of start of the academic year.
2. In the event of the Doctoral Student’s renouncement from the study programme in the
course of the academic year (through the means of waiver submitted in the appropriate
Faculty Dean’s office), the Doctoral Student can apply to the Rector, through the agency of
the appropriate Faculty Dean’s Office, for reimbursement of the study fees, reduced
proportionally to the period of attendance. The amount of funds reimbursed shall be
calculated as follows: the annual/semester fee is divided by the number of weeks in an
academic semester/year, and, subsequently, multiplied by the number of weeks remaining
from the date of renouncement to the end of the academic semester or year.
3. The Rector is entitled, in exceptional cases, for a decision on the reimbursement of the
entire, or partial amount of study fees.
§9
The Doctoral Student undertakes to notify the University in writing about any changes to his/her
personal data specified in this Agreement. Any consequences of the failure to comply with this
obligation shall be borne by the Doctoral Student.
§ 10
The Agreement shall be valid during the period of studies referred to in §2, and all amendments to the
present Agreement shall require written form, under pain of nullity.
§ 11
Any matters not provided for by this Agreement shall be governed by the provisions of the Act of 27
July 2005 – Law on Higher Education (Journal of Laws – Dz. U. No 164, item 1365, with subsequent
amendments) and its enforcement provisions, University internal regulations, especially including the
UL Statutes, and the UL Study Regulations, as well as, to the extent not governed therein, the
provisions of the Polish Civil Code.

§ 12
Any disputes that might arise from or out of this Agreement shall be settled by appropriate courts of
law.
§ 13
This Agreement has been drawn up in two identical copies, of which each Party has one copy.
In the case of doctoral studies instructed in a foreign language, this Agreement shall be drawn
up in four copies, two in English, and two in Polish, of which each Party shall have one copy
of each language version.
