W kostnej postaci zgryzu otwartego stwierdza się wydłużenie zębów bocznych i wyrostka zębodołowego. Celem badania było określenie tylnej wysokości wyrostka zębodołowego szczęki i żuchwy u osób bez zaburzeń pionowych na cefalogramach bocznych głowy. Materiał stanowiło 300 cefalogramów bocznych głowy pacjentów z wadą klasy I i prawidłowymi proporcjami pionowymi twarzy. Wyodrębniono dwie grupy: 213 osób z uzębieniem mieszanym i 87 z uzębieniem stałym. Metoda: mierzono w milime- Inclination angle between mandibular plane (ML) and anterior cranial base plane (SN) affects anterior lower face. In skeletal open bite elongation of posterior teeth and alveolar process is observed. Aim: determination of posterior height of maxillary and mandibular alveolar process in subjects with normal vertical relationships of face in profile cephalographs. Material: consisted of 300 lateral cephalometric radiographs of patients with Class I malocclusion and correct vertical facial proportions. Two groups were distinguished: 213 subjects with mixed and 87 with permanent dentition. Otrzymane pomiary porównano z wynikami badań wykonanych w innych populacjach. Wyniki: u badanych wartość kąta SN/ML wahała się w granicach od 27 do 36º (średnio 32º ± 3,03 w uzębieniu mieszanym oraz 32º ± 3,14 w uzębieniu stałym). Odległość guzka pierwszego trzonowca górnego do płaszczyzny podstawy szczęki (ms–NL) rosła z wiekiem i w uzębieniu mieszanym wynosiła 20,0 mm, a w stałym 23,0 mm. Podobnie wzrastała odległość trzonowca dolnego do podstawy żuchwy od 28,5 mm w uzębieniu mlecznym do 31,0 mm w uzębieniu stałym. Wnioski: 1.Średnia wartość kąta SN/ML wynosiła 32º zarówno w uzębieniu stałym jak i mlecznym. 2. Pomiar ms-NL i mi-ML rósł wraz z wiekiem i średnio zwiększył się o 3 mm. Należy rozróżniać normy wysokości zębów bocznych i wyrostka zębodołowego dla uzębienia stałego i mieszanego. (Forum Ortod. 2008; 5: 5 - 10). Nadesłano: 5.01.2009 Przyjęto do druku: 20.03.2009 the tip of first upper permanent molar`s mesiobuccal cusp and its projection onto maxillary plane (ms- NL) was measured in millimetres, so was the distance between the tip of first lower permanent molar`s mesiobuccal cusp and mandibular plane (mi-ML). The measurements were compared with study results obtained from other populations. Results: SN/ML angle was 27º to 36º (mean 32º ± 3.03º in mixed and 32º ± 3.14º in permanent dentition). Distance between first upper molar`s cusp and maxillary plane (ms-NL) increased with age from 20.0 mm in mixed to 23.0 mm in permanent dentition. Distance between the lower molar and mandibular plane increased similarly and was 28.5 mm in deciduous and 31.0 mm in permanent dentition. Conclusions: 1. Mean SN/ML angle was 32 degrees in both permanent and deciduous dentition. 2. ms-NL and mi-ML measurement increased with age, approximately by 3 mm in population. Norms for posterior teeth and alveolar process hight should be assessed in mixed and permanent dentition. (Orthod. Forum 2008; 5: 5 10). Gdy wzrost pionowy wyrostka zębodołowego w okolicy trzonowców jest większy niż wzrost pionowy wyrostków kłykciowych, żuchwa rotuje do tyłu powodując zwiększenie przedniej wysokości twarzy. Towarzyszy temu prawidłowe lub kompensacyjnie nadmierne wyrznięcie zębów w odcinku przednim. Isaacson i wsp.(1) stwierdzili, że duża tylna wysokość wyrostka szczęki wywołuje dotylną rotację żuchwy i niekorzystny wzorzec wzrostu. Już w 1965 roku Schudy (2) zauważył diagnostyczne znaczenie pomiaru tylnej wysokości szczęki, ale ze względu na niedoskonałość techniki wykonywania zdjęć cefalometrycznych uznał za trudny do wykonania. Obecnie producenci aparatów rentgenowskich gwarantują minimalną różnicę pomiędzy obrazem rzeczywistym a radiologicznym oraz powtarzalny stopień powiększenia obrazu, dzięki czemu pomiary liniowe nabrały większego znaczenia diagnostycznego. Celem pracy było określenie tylnej wysokości wyrostka zębodołowego szczęki i żuchwy u pacjentów w okresie uzębienia mieszanego i stałego z prawidłową proporcją trzech pięter twarzy. It is well known that the posterior height of the maxillary alveolar process is increased in individuals with the skeletal open bite. When the vertical growth of the alveolar process in the molars` region exceeds the vertical growth of the condylar process, the mandible rotates backwards causing the increase in the anterior facial height. This is accompanied by normal or compensatory excessive eruption of the teeth in the anterior segment. Isaacson et al. (1) claimed that increased posterior height of the maxillary process induces posterior rotation of the mandible and thereby an unfavourable growth pattern. In 1965 Schudy (2) noticed diagnostic significance of the posterior maxillary height measurement but, because of the shortcomings of cephalometric radiography at that time, he found the measurement hard to conduct. Nowadays X-ray machines ensure the minimum of discrepances between the real and radiographic pictures as well as reproducibility of image magnification, which has increased diagnostic significance of linear measurements. The aim of the study was determination of the posterior height of the maxillary and mandibular alveolar process in the patients with the mixed and permanent dentition who present correct proportions of the three facial parts. Kolejne kryterium doboru materiału stanowiła obecność całkowicie wyrzniętych pierwszych stałych trzonowców i siekaczy. Wyodrębniono dwie grupy: 213 osób z uzębieniem mieszanym i 87 z uzębieniem stałym. Pacjenci pochodzili z rejonu wschodniej Polski. U badanych mierzono wartość kąta podstawy czaszki do podstawy żuchwy (SN/ ML). Przed przystąpieniem do mierzenia tylnej wysokości wyrostka zębodołowego żuchwy i szczęki autorzy przeanalizowali w dostępnej literaturze technikę wykonywania tego pomiaru. W literaturze punkt referencyjny na pierwszym trzonowcu zaznaczano na szczycie guzka policzkowego mezjalnego lub dystalnego, a także na środku bruzdy, czy środku połączenia szkliwno-cementowego (3,4,5,6). Dane przedstawia Tabela 1 i 2. Zdecydowana większość autorów(7-17) mierzyła opisywany pomiar liniowy między wierzchołkiem guzka mezjalnego policzkowego pierwszego stałego górnego trzonowca a rzutem tego punktu na płaszczyznę podstawy szczęki (NL). Analogicznie wyko- The studied material consisted of 300 lateral cephalographic radiographs obtained from the patients with no vertical disturbances in the face. Another criterion for the study material selection was the presence of fully erupted first permanent molars and incisors. Two groups of patients were distinguished: 213 individuals with the mixed dentition and 87 individuals with the permanent dentition. All of them came from the eastern part of Poland. In the studied subjects the value of the angle between the cranial base and the mandibular base (SN/ML) was measured. Before the measurement of the posterior height of the mandibular and maxillary process was carried out, the authors had studied the techniques of taking this measurement presented in the literature. There the reference point on the first molar was marked at the tip of the mesiobuccal or distal cusp as well as at the midpoint of the intercuspal groove or at the midpoint of the CEJ (cemento-enamel junction) (3,4,5,6). The data are presented in Tables 1 and 2 . The landmarks selected by the majority of authors were accepted for the purpose of our study (Fig.1). Zakres wartości kątów był identyczny u pacjentów z uzębieniem mieszanym i stałym. Średnia wartość kąta wynosiła 32º przy odchyleniu standardowym ±3,03 w uzębieniu mieszanym i ±3,14 w uzębieniu stałym (Tab. 3.). The range of the angle value was identical in the patients with both the mixed and permanent dentition. The mean value of the angle was 32 degrees with the standard deviation of ± 3.03 degrees in the mixed dentition and 3.14 degrees in the permanent dentition (Table 3). Powiększenie wysokości wyrostka dla szczęki wynosiło 3 mm. Podobnie wzrastała odległość trzonowca dolnego od podstawy żuchwy. W uzębieniu mieszanym średnia wartość to 28,5 mm a 31,0 .mm w uzębieniu stałym (Tab.4.). Pomimo wzrostu pionowego wyrostków zębodołowych zarówno szczęki jak i żuchwy średnia wartość kąta SN/ML nie ulegała powiększeniu. cusp of the upper permanent first molar and the mandibular plane in the mixed dentition showed that the mean value was 20 mm, whereas in the permanent dentition it was 23.0 mm. The increase in the height of the alveolar process for the maxilla was 3 mm. The distance between the lower molar and the mandibular plane increased in a similar way. The mean value in the mixed dentition was 28.5 mm, whereas in the permanent dentition it was 31 mm (Table 4). Despite the vertical growth of the alveolar process, both maxillary and mandibular, the mean value of the SN/ML angle did not increase. Dyskusja Discussion Nadmierne wyrznięcie zębów bocznych zdaniem Bjorka powoduje rotację wyrównawczą żuchwy do dołu i do tyłu, czyli żuchwa wykonuje obrót ku tyłowi, co powoduje zwiększenia kąta nachylenia płaszczyzny żuchwowej do podstawy czaszki. U badanych średnia wartość kąta SN/ ML wynosiła 32º i była podobna do normy dla rasy kaukaskiej (18). W badanym materiale nie stwierdzono różnic u osób z uzębieniem mieszanym w porównaniu do osób z uzębieniem stałym w zakresie wartości kąta podstawy żuchwy do podstawy czaszki, czyli kąt ten był relatywnie stały pomimo wzrostu odległości guzków trzonowców od podstaw szczęki i żuchwy. Zwiększenie wysokości tylnych segmentów wyrostków zębowych nie wpływało na wydłużenie dolnego piętra twarzy dzięki jednoczesnemu wzrostowi gałęzi żuchwy i tylnej wysokości twarzy. Porównanie wyników własnych odległości szczytów guzków mezjalnych pierwszych trzonowców stałych szczęki i żuchwy do podstaw kostnych (ms-NL i mi-ML) u osób dorosłych z danymi populacji afroamerykańskiej, chińskiej, amerykańskiej oraz brazylijskiej (Tab. 5) wykazało, że największe różnice były między wynikami badań chińskich a własnymi, natomiast najmniejsze w porównaniu do wyników brazylijskich. Wartości (ms-NL i mi-ML) otrzymane z badań osób z uzębieniem stałym z rejonu wschodniej Polski były najmniejsze w porównaniu z innymi i to zarówno w szczęce jak i w żuchwie. FORUM ORTODONTYCZNE According to Bjork, excessive eruption of posterior teeth causes rotation of the mandible downward and backward, i.e. the mandible makes a turn backward, which results in the increased angles of inclination between the mandibular plane and the cranial base. In the studied groups the mean value of the SN/ML angle was 32 degrees and was similar to the norm for the Caucasian race (18). No difference in the values of the angle between the mandibular plane and the cranial base was found between the subjects with the mixed dentition in comparison with those with the permanent dentition, which is indicative of the fact that this angle was relatively constant even though the distance between the molars` cusps and the maxillary and mandibular planes increased. The increase in the height of the posterior segments of the alveolar processes did not affect elongation of the lower height of the face due to the concomitant growth of the mandibular ramus as well as the increase in the posterior facial height. The comparison of obtained results concerning the distances from the tips of mesial cusps of the permanent first maxillary and mandibular molars to the skeletal planes (ms-NL and mi-ML) in adult individuals with the data concerning the groups of Afroamerican, Chinese, American and Brazilian descent (Table 5) showed the greatest difference were found between the Chinese and Polish groups, whereas the smallest difference was observed in comparison with the Brazilian subjects. Podobnie wzrastała odległość trzonowca stałego dolnego do podstawy żuchwy (mi-ML) od 28,5 mm w uzębieniu mlecznym do 31,0 mm w uzębieniu stałym. Zwiększenie wysokości tylnych segmentów wyrostków zębowych nie wpływało na wydłużenie dolnego piętra twarzy dzięki jednoczesnemu wzrostowi gałęzi żuchwy i tylnej wysokości twarzy. Conclusions 1. The value of the SN/ML angle was in the range of 27-36 degrees (mean 32 degrees) and it was similar in the patients with the mixed and peramnent dentition. 2. The distance between the cusp of the first upper permanent molar and maxillary plane (ms-NL) in the mixed dentition was 20.0 mm, and 23.0 mm in the permenant dentition. The distance between the lower permanent molar and the mandibular plane (mi-ML) increased similarly and was from 28.5 mm in the deciduous dentition to 31.0 mm in the permanent dentition. The increase in the height of the posterior segments of the alveolar processes did not affect elonglation of the lower height of the face due to concomitant growth of the mandibular ramus followed by the increase in the posterior facial height.
