Proces lateralizacji uznawany jest za jeden z aspektów rozwoju motorycznego. Większość prowadzonych badań określa stopień zróżnicowania między takimi narządami, jak oko, ucho, ręka, noga w kontekście częstotliwości lub precyzji ruchu. Są to jednak metody bardzo skomplikowane, często dyskusyjne i nie zawsze jednoznaczne w interpretacji wyników. Zapis EMG i rejestracja video wspomagają analizę istniejących różnic, a ponadto korelują z opisem błędów w technice wykonywanych czynności. Badania tego typu mogą zastać wykorzystane w protetyce lub rehabilitacji osób po amputacjach kończyn. Celem badań była próba określenia asymetrii czynnościowej za pomocą zapisu EMG. Materiał i metody. W prowadzonych badaniach autor określił różnicę napięcia mięśniowego między kończynami górnymi w rzucie piłką baseballową, wykorzystując do tego analizę zapisu EMG oraz obrazu video z kamery wysokiej częstotliwości (250 klatek/s). Badaniu podano napięcie mięśnia triceps oraz pectoralis major, które mają znaczący udział w momencie rzutu. Analizie poddano moment wyrzutu piłki baseballowej oraz moment największego napięcia mięśniowego w trakcie fazy wyrzutu w okresie 0,5 s przed i po wyrzucie. Wyniki. Zdecydowanie mniejsze napięcia mięśnia oraz mniejsze zróżnicowanie między prawą i lewą ręką wystąpiło w przypadku mięśnia pectoralis major. Zróżnicowany obraz napięcia mięśniowego między maksymalnym napięciem a napięciem w momencie wyrzutu, świadczy o braku zależności badanych elementów. Procentowe zróżnicowanie sygnału EMG ręki lewej do prawej ukazuje rozpiętość od 55% do 76% w fazie maksymalnego napięcia oraz od 27% do 93% w fazie wyrzutu. Wnioski. W rzucie baseballowym moment maksymalnego napięcia badanych mięśni nie pokrywa się z momentem wyrzutu piłki, natomiast widoczna jest istotna osobniczo różnica w asymetrii czynnościowej między obiema kończynami. Summary Word count: Tables: Figures: References: 3350 1 4 20 Background. The process of laterality is considered to be one of the motor development aspect. Most of researches define the degree of diversity between such organs as eye, ear, arm, leg in the context of frequency and precision of the movement. These methods are complicated, debatable and the interpretation of the results are not always explicit. EMG recording and video recording facilitate the analysis of existing differences, furthermore they correlate with the mistake description of the activity technique. This type of researchers can be used in prosthetics or rehabilitation after extremity amputation. The purpose of this researches was the trial to define functional asymmetry by means of EMG recording. Material and methods. In this researches the author defines the disparity of muscle tone between upper extremities during baseball throw. The analysis of the EMG recording and the video recording from the camera with high frequency (250 cages/second) was used. Triceps muscle tone and pectoralis major, which has a significant participation in the moment of throwing, was put through an examination. The moment of baseball ball throwing, as well as the moment of the tightest muscle tone during the phase of throwing in the period of 0,5 seconds before and after throwing, was analysed. Results. In case of pectoralis major, the muscle tone was much lesser as well as the diversity between right and left hand. The diverse image of the muscle tone between maximal tension and the tension in the moment of throwing, proves that there is a lack of interrelation between these elements. Percentage diversity of the left hand EMG signal to the right hand, shows the span from 55% to 76% in the maximal tension phase and from 27% to 93% in the throwing phase. Conclusions. In baseball throw the moment of maximal muscle tone do not correspond with the moment of throwing the ball, whereas the disparity in functional asymmetry between both extremities is evident. Określenie asymetrii czynnościowej za pomocą zapisu EMG Wstęp Background Procesy czynnościowe związane z asymetrią funkcjonalną ludzkiego ciała są bardzo skomplikowane i nie w pełni poznane. Pomimo bardzo dużej liczby publikacji o stronności, nadal niewyjaśniona jest jej etiologia. Poglądy specjalistów o jej powstawaniu i kształtowaniu u człowieka można podzielić na dwie grupy. Jedni są zwolennikami czynników endogennych powstawania lateralizacji [1,2,3,4,5], upatrując to zjawiska w zróżnicowaniu półkul mózgowych, warunkach rozwoju płodu, lub niesymetrycznym położeniu organów wewnętrznych. Grupa autorów preferujących czynniki egzogenne powstawania lateralizacji ma przeświadczenie, że podstawowym determinantem tego zjawiska mogą być czynniki środowiskowe wynikające z codziennej pracy i wychowania w społeczeństwie lub presji społecznej [6,7,8,9]. Opisując asymetrię funkcjonalną wielu autorów odnosi ją do uprzywilejowania ruchu kończyny prawej na niekorzyść lewej (względnie odwrotnie) podczas wykonywania codziennych czynności [10,11]. Określając asymetrię czynnościową można stosować wiele narzędzi i metod badawczych, które z większym lub mniejszym skutkiem prezentują poziom stronności, lecz nadal będą to tylko przypuszczenia a nie paradygmaty. Stosując porównanie napięcia mięśniowego za pomocą analizy sygnałów EMG możemy dokładniej określić różnice jakie występują w momencie wykonywania określonej czynności ruchowej. Badanie tego zjawiska w kontekście wielu czynności życia codziennego, może pomóc w tworzeniu doskonalszych protez dla ludzi po amputacji, natomiast określenie zróżnicowania napięcia może być punktem wyjścia w usprawnianiu po urazach neurologicznych. The functional processes connected with functional asymmetry of human body are very complicated and they are not fully examined. Despite the fact that there is a big amount of publication about laterality, its etiology is still unexplained. There are two different specialists’ opinions about how the laterality has arisen and developed. The first group of specialists is the supporter of endogenous factor of laterality arisen [1,2,3,4,5]. They surmise these phenomenon in diversity of cerebral hemispheres, in conditions of development of a fetus or in internal organ location. The second group of specialists prefers exogenous arise of laterality. In their opinion the basic determinant of this phenomenon could be the environment factors, which result from everyday work, education in community or social pressure [6,7,8,9]. Many authors, when describing functional asymmetry, ascribe it to privileged right extremity movement to left disadvantage (relatively on a contrary), during everyday activities [10,11]. Many tools and research methods can be used when defining functional asymmetry. These methods show a level of laterality with different effects, but the results will be only an assumptions, not paradigms. To compare the muscle tone by means of the analysis of EMG signals, we can define, what differences appears in the moment of performing particular movement more precisely. In the context of everyday life, to research this phenomenon can be helpful in producing better artificial limbs. What is more, the definition of the tension diversity can be a starting point when healing after neurological trauma. Materiał i metody Material and methods W badaniach przeprowadzonych w 2014 r. wzięło udział dziesięciu najdalej rzucających chłopców w wieku 12 lat ze szkoły o profilu sportowym z Opola. Chłopcy zostali poinformowani o przedmiocie badań, natomiast prawni opiekunowie wyrazili pisemną zgodę. Badania zostały przeprowadzone w hali sportowej Wydziału Wychowania Fizycznego i Fizjoterapii Politechniki Opolskiej. Celem badania było określenie różnicy maksymalnego napięcia mięśnia triceps i pectoralis major lewej i prawej strony ciała w trakcie rzutu piłką baseballową. Analizie poddano dwa aspekty pomiaru napięcia mięśniowego. Pierwszy aspekt dotyczył momentu/ wyrzutu piłki z ręki i został określony literą „A”. Drugi aspekt dotyczył maksymalnego napięcia jakie wystąpiło w okresie 0,5 s przed wyrzutem i 0,5 s po wyrzucie sprzętu. Badany aspekt został określony w niniejszej pracy literą „B”. Każdy z badanych miał wykonać rzut na odległość z maksymalną siłą. Rzut był wykonywamy z miejsca, by uniknąć błędów związanych z nieprawidłowa pozycją wyrzutu, która często występuje w tej grupie wiekowej a dotyczy nieprawidłowego ustawienia nogi w momencie wyrzutu. Rzuty powtarzano trzykrotnie naprzemian ręką prawą i następnie ręką lewą, a wynik został uśredniony. U badanych nie określano wcześniej lateralizacji za pomocą innych testów, natomiast z przeprowadzonego wywiadu wynikało, że u wszystkich badanych występuje dominująca prawa ręką. In researches, conducted in 2014, ten, aged 12 years old boys took part. They had the furthest throw and all of them were attended to sport school in Opole. The boys were informed about the researches, whereas their guardians gave the permit. The researches were conducted in the sports hall in the Faculty of Physical Education and Physiotherapy in Technical University in Opole. The purpose of the researches was to define the disparity of maximal triceps muscle tone and pectoralis major in left and right side of the body during baseball ball throwing. Two aspects of muscle tone measurements was analysed. The first aspect concerned the moment of throwing the ball from a hand, and was called the letter: “A”. The second aspect concerned maximal tension in the period of 0,5 second before throwing and 0,5 second after throwing. The aspect, which is researched, was called the letter “B”. Each boy should have thrown the ball with the maximal strength. The throw was executed straight away to avoid mistakes connected with a wrong position that is very common in this age group, and it concerns a bad leg position in the moment of throwing. The throws was repeated threefold, alternate right and then left hand. The results was averaged. By means of another tests the lateralization was not affirmed in examined boys, yet an earlier interview follows that the right hand was their dominant. Define of functional asymmetry by means of EMG recording Do badań wykorzystano aparat EMG MyoTrace 400 (Ryc. 1) oraz kamerę wysokiej częstotliwości (250 klatek/s), dzięki której dokładnie określono moment wyrzutu. Wyniki badań poddano analizie w oprogramowaniu NORAXON, natomiast istotność różnice między przeprowadzonymi próbami zostały ocenione analizą wariancji (ANOVA). second) was used in researches, what facilitated to define the moment of throwing very precisely. The results was put through an analysis in NORAXON software, whereas the crucial differences between attempts was estimated by the variation analysis (ANOVA). Wyniki Results Z przeprowadzonych analiz wynika, że istnieje duże indywidualn zróżnicowanie w napięciu mięśniowym 12-latków, zarówno w mięśniu pectoralis major jak i triceps. Ze względu na tak duże rozbieżności trudno jest interpretować średnie zbiorowe wszystkich badanych i dlatego autor zdecydował się zaprezentować wszystkie dziesięć wyników poszczególnych chłopców (Ryc. 2). Przeprowadzony zapis EMG bardzo dobrze ilustruje zróżnicowanie zarówno między mięśniami triceps i pectorlis major oraz stronami ciała badanych mięśni. Zróżnicowanie w każdym z badanych przypadków przekracza kilkaset mikrowoltów zarówno w momencie wyrzutu piłki jak i w momencie największego napięcia w badanym przedziale czasowym. Zdecydowanie mniejsze napięcie oraz mniejsze zróżnicowanie między prawą i lewą stroną wystąpiło w przypadku mięśnia pectoralis major. Rozbieżności w zapisie EMG badanych momentów rzutu świadczą o braku zależności między wyrzutu sprzętu a największym napięciem badanych mięśni. Procentowe zróżnicowanie zapisu EMG ręki lewej do prawej wyniosło od 55% do 76% w przypadku maksymalnego napięcia mięśni oraz od 27% do 93% napięcia w momencie wyrzutu piłki (Tab. 1). According to analysis there are a big individual diversity in 12 aged boys muscle tone in both, pectoralis major and triceps muscle. Considering a big discrepancy, it is hard to interpret collective average of all examined boys, hence the author decided to present all ten results of particular boys (Fig. 2). EMG recording illustrates the diversity between triceps, pectoralis major muscle and body sides examined muscles. The diversity in every case exceeds a few hundred microvolts in the moment of throwing the ball, as well as in the moment of the biggest tension in examined temporal period. Definitely much less tension and less diversity between right and left side appeared in case of pectoralis major muscle. Discrepancies in EMG recording of examined moments of throw, show that there is a lack of interrelation between throwing the ball and the highest tension of muscles. Percentage diversity in EMG recording shows that left hand to right hand was from 55% to 76% in the case of maximal muscle tension and from 27% to 93% tension in the moment of throwing the ball (Tab. 1). Lack of statiscally crucial differences appeared only in tension of pectoralis major in the aspect concerning the moment of throwing a ball. The percentage difference between the tension left and right triceps muscle Dyskusja Discussion Próba podjęcia badań nad lateralizacją z użyciem EMG została podjęta przez autorów, ponieważ ani badania archeologiczne [10] ani badania na małpach [11] nie dowiodły jednoznacznie etiologii tego zjawiska. Wykorzystanie sygnału EMG w badaniu asymetrii nie jest jeszcze powszechnie wykorzystywane i nadal wielu autorów posługuje się tradycyjną formą testów zajmującą dużo czasu [12,13]. Zróżnicowanie wyników między ręką prawą i lewą w rzucie piłką tenisową na grupie badawczej o dużej rozpiętości wiekowej prezentuje Drabik [13]. Podobne wnioski uzyskał również w swych badaniach Konieczny [14]. Autor ukazuje występowanie dużego zróżnicowania w wynikach między ręką prawą a lewą w rzucie na odległość oraz narastanie tego zjawiska z wiekiem. Rozpatrując powyższe zagadnienia w kontekście wieku badanych osób Ljach [15] stwierdza, że asymetria prawostronna wyraźnie narasta od młodszego do średniego wieku. Wykorzystanie Sygnału EMG zdaje się tylko potwierdzać powyższe zjawisko. Podobną problematykę potwierdzającą stronne zróżnicowanie w mięśniu triceps z uwzględnieniem sugestii kończyny dominującej opisują również autorzy [16], chociaż nie są to czynności związane z rzutem. Zjawisko lateralizacji kończyn górnych z użyciem sygnału EMG próbują również wyjaśnić w swych badaniach Ballanger i Boulinguez [17] odnosząc się do specyfiki i precyzji ruchu. Autorzy podkreślają znaczenie użycia EMG w tego typu badaniach. Potwierdzenie zróżnicowania sygnału EMG pod kątem asymetrii obręczy barkowej prezentują autorzy [18] prezentujący to zagadnienie z uwzględnieniem rodzaju wykonywanej czynności w sporcie. Ibrahimy i wsp. [19] opisują zróżnicowanie w lateralizacji z użyciem EMG ale wykorzystują do tego celu również sieci neuronowe, co jeszcze bardziej podkreśla możliwości głębszej analizy zjawiska. Illyés i Kiss [20] potwierdzają zależność między precyzją ruchu a czasem wystąpienia maksymalnego napięcia mięśniowego u sportowców i amatorów. The authors decided to undertake the laterality researches with the usage of EMG because neither archaeological researches [10] nor experiments on monkeys [11] did not explicitly prove the etiology of this phenomenon. The usage of EMG signals in asymmetry research is not commonly used and there are still many authors who operate traditional and time-consuming test form [12,13]. Drabik [13] shows the diversity of results between right and left hand in tennis ball throw in a group of big age grade range. Konieczny [14] gained similar conclusions. The author claims that in results, there is a big diversity during throwing at a distance, between right hand and left hand. The growth of this phenomenon is visible with age. Considering preceding issue, in the context of the examined participants age, Ljach [15] states that the right-side asymmetry clearly increase from young to middle age. The usage of EMG signal supposes to confirm preceding phenomenon. Similar issue that confirms the diversity of lateralization in triceps muscle, taking into consideration a dominant extremity, was described by the authors [16], but the activities are not connected with a throw. In researches of Ballanger and Boulinguez [17], the authors also try to explain the phenomenon of upper extremities laterality, with EMG signals usage, and refer to specificity and precision of movement. The authors emphasize the importance of EMG usage in this kind of researches. Confirmation of diversity of EMG signals when examine it from a shoulder girdle asymmetry perspective, was presented by the authors [18]. They show this issue taking account of an activity type in sport. Ibrahimy and wsp. [19] describe the diversity of laterality with the EMG usage, but they use also biological neutral network, what emphasize the chance to analyse the phenomenon much more in depth. Illyes and Kiss [20] confirm interrelation between movement precision and the time of maximal muscle tension in athletes and amateurs. Wnioski Conclusion 1. W rzucie baseballowym moment maksymalnego napięcia badanych mięśni nie pokrywa się z momentem wyrzutu piłki. 2. Widoczna jest istotna osobniczo różnica w asymetrii czynnościowej między obiema kończynami. 1. Zapis EMG może wspomagać proces badań nad lateralizacją czynnościową, natomiast jednoczesna analiza zapisu EMG i wideo pozwala dokładnie opisać błędy i rozbieżności w technice rzutu. 3. EMG recording can be helpful in the process of functional laterality, whereas the simultaneous analysis of EMG recording and video recording allow to describe mistakes and discrepancy exactly, in the throw technique.
