Rules of the Programme at ENEL-MED Medical Centres

Rules of the Programme at ENEL-MED Medical Centres:
1. ENEL-MED shall grant a permanent 10% (say: ten per cent) discount on its services to customers who
make payments using a MasterCard credit or debit card.
2. The services that ENEL-MED grants discounts for are:
- medical consultations, outpatient procedures, laboratory examinations,
- diagnostics: MRI, CT, ultrasound, Doppler ultrasound, X-ray, ECG, echocardiogram, spirometry,
Holter, and others,
- dentistry: conservative, paediatric, orthodontics, prosthetics, implantology, periodontology, dental
surgery, oral hygiene room,
- surgeries at ENEL-MED hospitals (excluding the costs of anaesthesia and hospital stay).
3. None of the discounts referred to in paragraph 2 applies to the cost of materials.
4. The discount referred to in paragraph 1 shall not be combined with other discounts offered by ENEL-MED
as at the date of sale or order placement. The customer shall have the right to choose the discount or
promotion that will be used.
5. The discounts are granted when the payment is made by means of the following cards:
- MasterCard Gold;
- MasterCard Platinum;
- MasterCard World;
- MasterCard World Signia;
- MasterCard World Elite;
- MasterCard Business;
- MasterCard Professional.
6. The discounts can be used at ENEL-MED own centres and hospitals.

7. If the Bank receives a complaint concerning ENEL-MED’s permanent or periodic failure to grant the
discount to the customer, the Bank has the right to report this complaint to ENEL-MED. ENEL-MED shall
examine the complaint within 14 days of it being reported by the Bank.
8. If the complaint is accepted, ENEL-MED will return to the customer the difference between the amount
paid by the customer and the price with the discount. The reimbursement will take place after presenting
the receipt at the ENEL-MED centre where the payment was made.
9. All complaints and objections related to granting discounts at ENEL-MED centres, in particular as regards
the type of products covered by the discount, shall be considered by ENEL-MED. Any objections concerning
the payment and debiting of the MasterCard card shall be considered by Boson Michał Wieteska. A
complaint lodged by a Bank customer shall be forwarded to ENEL-MED if the scope of the complaint relates
to the type of product. If the complaint concerns the payment for the service, the entity competent to
examine the complaint shall be the Bank. If the complaint is related both to the price and the type of
product, it shall be first examined by ENEL-MED, and then by the Bank. The answer to the customer
complaint shall be drawn up by the Bank and forwarded to ENEL-MED for information.
10. The Bank shall not be liable for ENEL-MED’s failure to grant the discount or its improper settlement.
