Aby ocenić wskaźniki dojrzałości kręgosłupa (ośrodki kostnienia) badano drugi, trzeci, czwarty i piąty krąg szyjny. Oceniano następujące cechy: – Obecność dolno-przednich i dolno-tylnych wtórnych ośrodków kostnienia kręgów szyjnych – Pomiar wklęsłości dolnej na trzonach kręgów szyjnych – Pomiar przedniej i tylnej wysokości trzonów kręgów szyjnych – Kąt dolno-przedni i dolno-tylny między podstawą trzonu i stycznymi do jego ściany przedniej i tylnej – Wartości wszystkich pomiarów były porównywane i oceniane statystycznie. Wyniki: ośrodki kostnienia kręgów szyjnych okazały się istotnymi parametrami w określaniu wieku kostnego. Wnioski: istnieje możliwość określania wieku kostnego na bocznych radiogramach głowy w oparciu o obecność wtórnych ośrodków kostnienia trzonów kręgów szyjnych, co oszczędza dodatkowego napromieniowania, jakie łączy się z wykonywaniem zdjęć radiologicznych nadgarstka. Results: The ossification centers of the cervical vertebras, resulted in significant parameters for the determination of the skeletal age. Conclusions: it is possible to determine on lateral headfilms the skeletal age by the presence of the secondary ossification centers on the cervical vertebral bodies, with the avoidance of additional radiation that comes from a hand-wrist X-Ray. Przy rozpoczynaniu leczenia, wybór zarówno ortodontycznej jak ortopedycznej metody powinien opierać się na znajomości wieku kostnego (1). Znając wiek kostny, można prognozować wynik leczenia, jak również rodzaj i czas trwania fazy retencyjnej. Z powodu indywidualnej zmienności terminu, czasu trwania i nasilenia wzrostu, ocena kostna ma zasadnicze znaczenie w formułowaniu racjonalnego planu leczenia ortodontycznego (2). Proces dojrzewania można ocenić tylko na podstawie kostnienia (3). Wygląd ośrodków kostnienia, ich zmiany morfologiczne oraz ich zespolenie z kością w rejonie nasady, okazały się jasnym i prostym narzędziem diagnostycznym do określania wieku kostnego pacjenta (4). Jednym z ważnych narzędzi używanych obecnie do ustalenia, czy skok wzrostowy rozpoczyna się, właśnie trwa, czy został zakończony, jest ocena radiologiczna nadgarstka. Björk (5), Greulich The estimation of the skeletal age is very important in the planning of orthodontic treatment during adolescence. When to begin treatment, as well as the choice of the orthodontic or orthopedic therapy should be based on the skeletal age (1). Based on the skeletal age we can obtain information about the prognosis for the treatment, as well as the type and duration of the retention phase. Because of individual variations on timing, duration and velocity of growth, skeletal assessment is essential in formulating viable orthodontic treatment plans (2). The maturity process can only be determined by ossification (3). The appearance of the ossification centers, their morphological changes and their fusion to the bone in the epiphyseal region, have been demonstrated to be a clear and simple diagnostic tool when defining one patient’s skeletal age (4). One important tool currently used in determining whether the pubertal growth has started, is occurring or has finished is the hand wrist radiographic evaluation. W literaturze opisano ocenę dojrzewania kręgów szyjnych jako sposób określania wieku kostnego. Autorzy badań stwierdzili, że wiek kostny można rozpoznać na bocznym radiogramie głowy. Pierwsze badania były wykonane przez Bencha (10) w 1963 r. i dotyczyły porównania wzrostu żuchwy ze wzrostem kręgów szyjnych i kości gnykowej. Następnie Lamparski w 1972 r. (11) stwierdził, że kręgi szyjne są ważnym parametrem w badaniu wieku kostnego i zauważył zmiany morfologiczne kręgów szyjnych związane z wiekiem. W oparciu o badania Lamparskiego, Hassel i Farman (2) opracowali w 1995 r. wskaźnik zmian morfologicznych w kręgach szyjnych zwany Wskaźnikiem Dojrzewania Kręgów Szyjnych (Cervical Vertebrae Maturation Index – CVMI), który zawierał sześć stadiów rozwojowych. Inną metodę obliczania wieku kostnego wynalazł Nanda (12), który mierzył odcinek Sella-Gnation i uznał go za wskaźnik wzrostu. Ruf i Pancherz (13) zaproponowali badanie zmian morfologicznych zatok czołowych jako ważnego wskaźnika wieku kostnego. Przedstawiany przez nas sposób badania opiera się na obserwacji wtórnych ośrodków kostnienia kręgów szyjnych. Poprzednio opierano się na zmianach morfologicznych kręgów szyjnych, za które wtórne ośrodki kostnienia są odpowiedzialne. Podobny proces można zobaczyć w nadgarstku z pojawieniem się trzeszczki, która również jest wtórnym ośrodkiem kostnienia używanym jako wskaźnik dojrzałości. Kręgosłup szyjny składa się z siedmiu kręgów. Wszystkie mają 3 pierwotne ośrodki kostnienia; jedno na trzonie i dwa na łukach. Dwa wtórne ośrodki kostnienia pojawiają się na górnej i dolnej powierzchni trzonu kręgu (również na wyrostku poprzecznym i wyrostku kolczystym), zmieniając morfologię kręgu z powodu zwiększającej się wklęsłości w dolnej części trzonu kręgu i zmiany jego kształtu. Nasady kręgów mają przednią i tylną płytkę. Zaczynają one kostnieć począwszy od 8 roku życia przyjmując kształt kolisty i ulegają zlaniu w 18 roku życia, zgodnie z doniesieniami podręcznikowymi na ten temat (14). W porównaniu z innymi sposobami określania wieku kostnego, proponowana przez nas metoda ma następujące zalety: w stosunku do oceny nadgarstka zmniejszona jest liczba radiogramów i koszt badania, a ponadto dokładność oceny jest wysoka. Celem badań było określenie wieku kostnego na podstawie kręgów szyjnych ocenianych na bocznym radiogramie głowy. Jako wskaźników wzrostu użyto wtórnych ośrodków wzrostu kręgów szyjnych. Innym celem było sprawdzenie, czy boczne radiogramy głowy są wiarygodną dokumentacją do badania wieku kostnego zamiast radiogramu nadgarstka. Pomogłoby to uniknąć dodatkowego napromieniowania pacjenta i ortodonty, jak też zbędnych kosztów ponoszonych przez system opieki zdrowotnej i przez pacjenta. FORUM ORTODONTYCZNE Greulich and Pyle (6), Tanner and Whitehouse (7), Fishman (8), Grave and Brown (9). Cervical vertebras have been described in literature as tools for determining bone age. Some authors affirm that the skeletal age is capable of being identified on the lateral X-Ray. The first studies were carried out by Bench (10) in 1963, who compared mandibular growth with cervical vertebrae and hyoid bone growth. It was later that Lamparski, 1972 (11) found that the cervical vertebras were a valid parameter for identifying skeletal age. He made some observations based on the morphological changes of the cervical vertebras correlated to age. Based on Lamparski’s study, Hassel and Farman in 1995 (2) developed an index of the morphological changes that happen to the cervical vertebrae called: (Cervical Vertebrae Maturation Index – CVMI), which contains six developmental stages. Other methods for estimating skeletal age were developed by Nanda (12), by measuring the Sella-Gnathion distance and using this as a growth indicator. Ruf and Pancherz (13) proposed in their study that the morphological changes in the frontal sinus, should be valid skeletal age indicators. The method that we are presenting is based on the observation of the secondary ossifcation centers of the cervical vertebras. The methods listed before are based on the morphology changes that occur on the cervical vertebrae. Responsible for those morphological changes are the secondary ossification centers. We could see a similar process happening on the hand wrist, with the appearance of the sesamoid bone, also a secondary ossification center, used as a maturity indicator. The cervical column consists of 7 vertebras. All of them have 3 primary ossification centers; one on the body and the other 2 on the arches. Two secondary ossification centers appear on the upper and lower surface of the cervical vertebral body (also on the transverse process and the spinous process), being responsible for the morphology changes on the vertebrae, such as concavity increase on the lower part of the vertebral body and shape changes. The vertebrae epiphysis has an anterior and posterior plaque. They begin to ossify from the 8th year of life in a circular shape and they fuse at an age of 18 years, according to handbook information on the subject (14). Compared to other methods, used for the determination of the skeletal age, proposed by us method in relation to the hand-wrist assessment shows following advantages: the accuracy is high and lower number of X-Rays results in cost reduction. The aim of our study is to determine from a lateral headfilm the skeletal age on cervical vertebras. As growth indicators, the secondary growth centers on the cervical vertebrae were used. Another goal of this investigation is to evaluate if the lateral X-Ray is a reliable diagnostic record for the determination of the skeletal age, in order to avoid a hand-wrist X-Ray. This could prevent additional radiation to the patient and the treating orthodontist, as well as unnecessary costs for the health system and the patient. 35 ORTHODONTIC FORUM Prace oryginalne Original papers Tom 4 nr 2 2008 Volume 4 no 2 2008 Materiał i metody Material and Methods Badanie należy do kategorii badań przekrojowych i retrospektywnych. Próba składała się z 280 bocznych radiogramów głowy pacjentów obu płci (żeńskiej 153 i męskiej 127) z Zakładu Ortodoncji Uniwersytetu Maimónidesa w Buenos Aires. Radiogramy pochodziły od pacjentów w wieku od 6 do 18 lat (średnia 12.3 lat) – Ryc.1. Wszyscy pacjenci byli zdrowymi Argentyńczykami rasy białej, którzy nie podjęli w przeszłości leczenia ortodontycznego ani nie byli leczeni w chwili wykonywania badania radiologicznego. Wykluczono osoby z wadami zespołowymi, zniekształceniami, zaburzeniami odżywiania, niedoborami wzrostu lub po przebytych urazach twarzy, głowy i szyi. Odrzucono radiogramy nieostre lub ze słabo widoczną okolicą kręgosłupa szyjnego. Wszystkie radiogramy były wykonywane na tej samej aparaturze (Orthophos© Siemens AG) przy takiej samej odległości od obiektywu i z użyciem cefalostatu. Filmy AGFA 24x30cm ustawiano pionowo w celu lepszej wizualizacji trzonów kręgów szyjnych. Wszystkie radiogramy były obrysowywane przez jednego ortodontę, który również wykonywał wszystkie pomiary. Aby ocenić wskaźniki dojrzałości, wzięto pod uwagę następujące pomiary: – Obecność dolnych-przednich i dolnych-tylnych wtórnych ośrodków kostnienia kręgów C2,C3,C4 i C5 – Ryc.2. – Wklęsłość dolna była mierzona po narysowaniu linii stycznej do dolnej krawędzi trzonu kręgu jako odległość w milimetrach do najniższego punktu krzywizny dolnej trzonu – Ryc. 3. – Przednia i tylna wysokość C3 i C4 – Ryc. 4. – Kąt dolny-przedni i dolny-tylny C3 i C4 – Ryc. 5. Poszczególne wartości pomiarów były obliczane i oceniane statystycznie za pomocą testu regresji, testu regresji logistycznej oraz testu Fishera. This is a retrospective and transversal study. The sample consists of 280 lateral X-Rays (n=280) of patients of both sexes (female 153, male 127) from the Department of Orthodontics, Maimónides University, Buenos Aires, Argentina. The X-Rays were of patients between the ages of 6 and 18 years. The average age was 12.3 years – Fig 1. All patients were healthy, caucasian Argentinians, who had neither started nor undergone any orthodontic treatment at the moment of the X-Ray. Patients that were suffering from syndromes, malformations, alimentary disorders, growth deficiencies or had a trauma history in the facial, head or neck region were excluded. Also excluded were X-Rays that were not well defined enough as well as those where the cervical region was difficult to observe. All X-Rays were taken using the same equipment (Orthophos© Siemens AG), with the same focus-object distance, and with the aid of a cephalostat. AGFA© 24x30cm films were used vertically in order to get a better visualization of the cervical vertebrae bodies. All X-Rays were traced and all values measured by the same orthodontist. To assess the maturity indicators the following measurements were evaluated: – Presence of the lower-anterior and lower-posterior secondary ossification centers of C2, C3, C4 and C5 – Fig. 2. – The lower concavity was measured tracing a tangent line to the lower edge of the vertebrae body and the distance to the deepest point on the lower curvature of the body was measured in mm. – Fig 3. – Anterior and posterior height of C3 and C4 – Fig. 4. – The lower-anterior angle and lower-posterior angle of C3 and C4 - Fig. 5. FORUM ORTODONTYCZNE 38 ORTHODONTIC FORUM Prace oryginalne Original papers Tom 4 nr 2 2008 Volume 4 no 2 2008 Wyniki Results Stwierdzono wysoką statystycznie korelację (p≤ 0,0001) między obecnością dolnych-przednich wtórnych ośrodków kostnienia na kręgach C2, C3, C4 i C5, a wiekiem chronologicznym u osób płci męskiej i żeńskiej – Ryc.6. Zauważono wcześniejsze pojawianie się ośrodków kostnienia u dziewcząt. Wyraźne zwiększanie występowania punktów kostnienia u dziewcząt przypadało na 10, a u chłopców na 12 rok życia. Oznacza to, że w tym wieku ośrodki kostnienia były widoczne na radiogramach, a ich kostnienie rozpoczynało się. W wieku 14 lat obserwowano stabilizację krzywej u dziewcząt, a u chłopców następowało to w wieku 15 lat, to znaczy, że w tym wieku prawie wszystkie (ok.100%) wtórne centra kostnienia można było dostrzec, a ich kostnienie postępowało. Korelacja między obecnością dolno-tylnych wtórnych ośrodków kostnienia była wysoce istotna statystycznie (p≤0,0001) w populacji żeńskiej i męskiej – Ryc.7. Wartości wyrażono w procentach. Ponownie zauważono wcześniejsze występowanie zmian wzrostowych u dziewcząt niż u chłopców. U dziewcząt nasilenie pojawiania się wtórnych ośrodków kostnienia przypadało na 10, a u chłopców na 12 rok życia, tzn. ośrodki te były dobrze widoczne w tym wieku i rozpoczynało się ich kostnienie. Stabilizacja krzywej następowała u dziewcząt w wieku 14 a u chłopców w wieku 15 lat, to znaczy że w takim wieku wtórne ośrodki kostnienia były widoczne u 100% badanych. Zauważono bardzo wysoką korelację (p ≤ 0,0001) między wiekiem chronologicznym a zwiększaniem się wklęsłości na dolnych krawędziach trzonów kręgów C2, C3 i C4 – Ryc.8. Wklęsłość ta pojawiała się wcześniej u dziewcząt niż u chłopców, jednak końcowe wartości jej pomiaru były takie same, co można nazwać dogonieniem wzrostu. Przednia i tylna wysokość trzonów kręgów C3 i C4 wykazywała statystycznie istotną zależność z wiekiem (p≤0,0001) w badanej populacji. U dziewcząt tylko dolny-tylny kąt na C3 wykazywał statystyczną zależność od wieku (p≤ 0.021). Żaden z kątów na C4 u dziewcząt nie był zależny statystycznie od wieku -Tab.1,2. W populacji męskiej kąt dolny- przedni i dolny-tylny na C3 nie wykazywał zależności statystycznej od wieku i tylko dolny-tylny kąt na C4 Był istotnie statystycznie zależny od wieku (p≤ 0.039 ) – Tab. 3,4. Można sądzić, że wartości kątów nie miały wielkiego znaczenia w analizie statystycznej. An earlier appearance of the ossification centers in girls could be observed. In the female sample these points showed a great increase in their appearance at an age of 10 years, in boys at an age of 12. This means that at these ages the ossification centers were visible on the X-Rays and their ossification started. At the age of 14 years a stabilization of the curve in the girls group could be observed, in the boys this happened at an age of 15, which means that at this age almost all (ca. 100%) of the secondary ossification centers, were capable of being observed and continued their ossification. The correlation between the presence of the lower-posterior secondary ossification center in C2, C3, C4 and C5 and the chronological age was highly significant (p≤0.0001) in the male and female population - Fig 7. The values were expressed in percentages. An earlier appearance in girls than in boys could again be observed. In the female sample the increment in their appearance was at the age of 10, in boys at 12. This means that at this stage the secondary ossification centers were well observable on the X-Ray and had started their ossification.At the age of 14 years a stabilisation of this curve in girls could be observed, in boys only at 15 years, this means that by this age 100 % of the ossifications centers were present. A very highly significant correlation (p≤0.0001) between age and the increase of the lower concavity was observed in C2, C3, and C4 - Fig 8. The lower concavity showed an earlier increase in girls than in the boys group, although the final values came out the same, this can be called growth catch up. The anterior and posterior height on C3 and C4 showed a significant statistical correlation to age (p≤0.0001) in the studied population. In the female population only the lower-posterior angle of C3 showed a significant correlation to age (p≤0.021) On C4 neither of the two angles showed a significant correlation to age in the female sample – Tab. 1, 2. The correlation between the lower-anterior and lower-posterior angle of C3 and age in the male population was not statistically significant. In the males only the lower-posterior angle of C4 showed a statistically significant relationship to age (p≤0.039) – Tab. 3, 4. It can be concluded that the angles do not have a statistically important meaning. Dyskusja Discussion Na wykresach dotyczących dolnych przednich i tylnych wtórnych ośrodków kostnienia u dziewcząt widoczne jest obniżenie krzywej w wieku lat 9. Można to przypisać mniejszej liczbie badanych w tej próbie. Pomiar ten może uchwycić wyraźne zmiany, gdy ośrodki kostnienia stają się wystarczająco duże i dobrze widoczne. Wyniki przeprowadzonych badań wykazują, że wartości kątów przednio- i tylno dolnych u osób płci żeńskiej i męskiej były nieistotne statystycznie. Można to przypisać morfologii poszczególnych kręgów, które wykazywały różnice indywidualne. Spośród czynników wpływających na budowę kręgów można wymienić mięśnie pacjenta, stan odżywienia i ćwiczenia fizyczne. Zauważono również trudności z odnalezieniem i zaznaczeniem punktów rejestracyjnych, widoczne na radiogramach przy nakładaniu obrazów. Sposób obrysowywania radiogramów i wykonywania pomiarów był zawsze taki sam. Mimo to, być może z powodu ręcznego wykonywania narysów, mógł wystąpić błąd metody. Aby to wyjaśnić, dokonano ponownego obrysowania trzech radiogramów losowo wybranych z każdej grupy. Błąd uznano za nieistotny, ponieważ 95% wszystkich pomiarów z pierwszego i drugiego badania leżały poniżej przyjętej granicy tolerancji. Potrzebne są dalsze badania z zastosowaniem tej samej metody, ale na większej liczbie pacjentów, w celu opracowania tabeli norm i krzywej wzrostu dla osób rosnących. Należy podkreślić, że każdy pacjent może przejawiać osobniczą zmienność w rytmie wzrostu, na który wpływają czynniki wewnętrzne i zewnętrzne. Trzeba też wziąć pod uwagę zróżnicowanie budowy kręgów, na które może wpływać: rotacja kręgów, odchylenia od normy krzywizny kręgosłupa szyjnego spowodowane uszkodzeniem więzadeł oraz zaburzenia w ośrodkach kostnienia wywołane złamaniem (4). Z wymienionych powodów konieczne jest zbieranie dokładnego wywiadu. Wszystkie metody określania dojrzałości (kostnej, seksualnej, zębowej itd.) postępują w podobnym rytmie, zgodnie z chronologiczną kolejnością. Zmiany morfologiczne kości są stałe i powtarzalne. The measurement of the lower concavity showed a later increase than in the appearance of the ossification centers. This could be due to the construction and method of measuring. This measurement is only capable of significant changes when the ossification centers are bigger and more visible. The results of this study show that the obtained values for the anterior- and posterior- lower angles in females and males were not statistically significant. This could be attributed to the morphology of each vertebra, which have individual variations. The patient’s musculatures, posture, nutrition, exercise, among other factors are determinant for morphology of the vertebrae. It could also be due to the difficulties that were found in the observation and marking of the reference points on the vertebras, shown on the X-Rays with super-imposed positions. The method used for tracing and measuring the X-Rays was the same in each case. Even though and perhaps due to the manual tracing, a methodic error could happen. To discard this, 3 X-Rays randomly selected from each group were retraced. The error was qualified as insignificant because 95% of all our measurements from the first and second tracing were below the acceptable limit of tolerance. Future investigations should be carried out using this type of measurements but on a bigger sample, in order to develop tables and norms for a larger population, as well as growth curves for growing individuals. This would be a very important tool for the orthodontic treatments. It is very important to notice that each patient may present individual variations in his or her growth rhythm that are influenced by external and internal factors. Morphological variations on the vertebras should be taken into account also. These may be caused by: vertebrae rotation; deviations in the cervical column curvature caused by ligamental lesions and disorders in the ossifications centers caused by fractures (4). This is why a careful anamnesis has to be done. All the methods used for determining the maturity (skeletal, sexual, dental, etc) progress at the same rhythm, following a chronological sequence. Mito i Sato (15,16) sądzą, że każdy ortodonta powinien poznać i opanować metody określania wieku kostnego. Używanie kilku diagnostycznych metod oceny dojrzałości jest niezwykle ważne w prognozowaniu i diagnozowaniu rosnących pacjentów (17). W piśmiennictwie Lamparski (11) stwierdził wysoką korelację zmian morfologicznych występujących w każdym wieku w obrębie kręgów szyjnych, a wzrostem żuchwy. Hassel i Farman (2) oraz Garcia Fernandez (18) opisali ścisłą zależność między kostnieniem kręgów szyjnych i kości nadgarstka. Wyróżnili oni sześć stadiów rozwojów kręgów szyjnych i nazwali je Wskaźnikiem Dojrzałości Kręgów Szyjnych. Za pomocą tego wskaźnika można porównać zmiany morfologiczne w kręgach szyjnych oraz ich wtórnych ośrodków kostnienia ze zmianami wzrostowymi w nadgarstku opisanymi w metodzie Fishmana i Björka. Można wnioskować, że istnieje bardzo wysoka korelacja wieku chronologicznego i metody diagnostycznej opartej na ocenie nadgarstka. Mimo to, patrząc krytycznie na badania Hassela i Farmana, wskaźnik ten ma charakter subiektywny jeżeli pacjent znajduje się w okresie przejściowym między dwiema fazami rozwojowymi. Problem ten może również wystąpić przy zastosowaniu oceny nadgarstka. Pewne ryzyko w dokładności klasyfikacji istnieje w każdej metodzie diagnostycznej i dlatego ortodonta musi posługiwać się więcej niż jedną metodą przy określaniu wieku kostnego. Mito i Sato (15,16) wyrazili przekonanie, że określenie wieku kostnego na podstawie kręgów szyjnych ma taką samą wartość diagnostyczną jak ocena wieku kostnego na zdjęciu radiologicznym nadgarstka przy zastosowaniu metody Tannera i Whitehouse’a (TW2), użytej w tym badaniu. Badacze ci donosili, że za wskaźniki dojrzałości kostnej można przyjąć nasady trzonów kręgów szyjnych (19). Niektórzy autorzy (20,21,22) stwierdzili ścisłą zależność między kostnieniem kręgów szyjnych i wzrostem żuchwy. Hellsing porównywał dojrzewanie kręgów szyjnych ze zmianą postawy ciała i jego wysokości. Obie cechy mogą znaleźć zastosowanie jako wskaźniki wzrostu kostnego, ponieważ istnieje ścisły związek między nimi. Catalbiano i wsp. (24) zaobserwowali ścisłą korelację przy określaniu wieku kostnego na radiogramach nadgarstka z oceną kręgów szyjnych na bocznych radiogramach głowy. Większość autorów stwierdza, że analiza kręgów szyjnych jest metodą wiarygodną przy określaniu wieku kostnego i bardzo użyteczną w diagnostyce ortodontycznej, ponieważ jest wysoko skorelowana ze wzrostem żuchwy (25). The use of several diagnostic methods of maturity may lead to a prognosis and diagnosis of the utmost importance for a growing patient (17). In the literature Lamparski (11) has found a high correlation between the morphological changes that occur in the cervical vertebras at each age and the lower jaw growth. Hassel and Farman (2) and Garcia Fernandez (18) describe a high correlation between the cervical vertebras and the hand wrist ossification. They classified the cervical vertebras growth in six stages and called it Cervical Vertebrae Maturation Index. In their index morphological changes of the cervical vertebras and their secondary ossification centres are shown and compared to the hand wrist growth by Fishman and Björk’s hand wrist diagnostic method. It could be concluded that there is a very high correlation between chronological age and the hand wrist diagnostic method. Even though and with a critical eye on Hassel and Farman’s study, the index has a subjective character, when the patient was in the transition between two phases. This problem related to the subjectivity can also be seen when using the hand wrist diagnostic methods. A certain risk on the classification exists in every diagnostic method, that’s why the orthodontist should use more than one method of diagnosis when determining the skeletal age. Mito and Sato (15, 16) think that the skeletal age determination on the cervical vertebras has the same diagnostic value as the skeletal age determination on the hand wrist X-Ray utilizing Tanner and Whitehouse’s 2 method (TW2), used in their study. They reported that the epiphysis of the bodies of the cervical vertebras can be used as skeletal maturity indicators (19). Several authors (20, 21,22) found a high correlation between the ossification of the cervical vertebras and the mandible growth. Hellsing (23) compared the cervical vertebrae maturation with the stature, or growth in height. Both can be used as skeletal growth indicators, since there is a close relation between them. Caltabiano, Leonardi and Zaborra (24) observed a high correlation between the determination of the skeletal age on a hand-wrist X-Ray and on the cervical vertebras on a lateral X-Ray. Most of the authors conclude that the cervical vertebrae analysis is a reliable method for determining skeletal age, very useful in the orthodontic diagnosis, since it is very highly correlated to the mandible growth (25). 42 ORTHODONTIC FORUM Prace oryginalne Original papers Tom 4 nr 2 2008 Volume 4 no 2 2008 Wnioski Conclusions – Określenie wieku kostnego na podstawie identyfikacji wtórnych ośrodków kostnienia oraz cech anatomicznych trzonów kręgów szyjnych jest możliwe, niezależnie od zmian w morfologii tych kręgów. – W badaniu zauważono istnienie wysokiej korelacji między wiekiem chronologicznym i szkieletowym, co powoduje że opisana metoda stanowi ważne narzędzie przy określaniu wieku szkieletowego. – Wiek szkieletowy jest ważniejszy niż chronologiczny w ortodontycznej diagnostyce i planowaniu leczenia. – Powoduje to uniknięcie dodatkowego napromieniowania związanego z wykonaniem radiogramu nadgarstka. – Powoduje to również redukcje kosztu i czasu badania. – Dziewczęta osiągają dojrzałość szkieletową wcześniej niż chłopcy. – It is possible to determine the skeletal age identifying the secondary ossification centers as maturity indicators and anatomical features on the cervical vertebral bodies, independently from the cervical vertebrae morphology changes. – As observed in this study there was a high correlation between chronological age and skeletal age, resulting in a valid tool for determining skeletal age. – For orthodontic diagnosis and planning the skeletal age is more important than the chronological age. – This results in the avoidance of additional radiation that comes from a hand-wrist X-Ray. – It also results in a cost and time reduction. – In skeletal maturity girls are more advanced than boys.
