Declaration regarding the list of product main deliveries over the last 3 years
1. I, the undersigned, authorized representative of Apator Metrix S.A., hereby declare on own
liability, under the sanctions applied for the crime of forgery in official documents, that the
data in the enclosed table are real.
2. I, the undersigned, declare that the information provided is complete and truthful to reality in
detail and I fully comprehend that the contracting authority has the legal right to require,
with a view to verifying and confirming the declarations, statements and documents attached
to the offer, any additional information n view of verifying the data from the present
declaration.
3. I, the undersigned, hereby authorize any institution, a trade company, bank, and other
corporate bodies to provide any information to the authorized representatives of the
contracting authority E.ON GAZ DISTRIBUTIE SA with regard to any technical and
financial matter which has to do with our activity.
4. The present declaration is valid until 12.01.2008.
