ANNEX NO 1 TO THE AUTHORIZATION/ ZAŁĄCZNIK NR 1 DO UPOWAŻNIENIA

§1. Being aware of the penal responsibility resulting from Art. 233 of the Penal code, I hereby state that the
following goods entrusted for logistic service within the good trade with abroad to the company ARKO LOGISTICS
S.C./ Świadomy odpowiedzialności karnej wynikającej z Art. 233 Kodeksu karnego niniejszym oświadczam, że
powierzane do obsługi logistycznej w ramach obrotu towarowego z zagranicą firmie ARKO LOGISTICS S.C:
-exported goods are not the goods with strategic meaning for the state security within the meaning of the act of
29th of November 2000 (unified text of 2004, J. of L. 229, it. 2315) on trade with abroad in goods, technologies
and services with strategic meaning for the state security and also for maintaining international peace and
safety/ -eksportowane towary nie są towarami o znaczeniu strategicznym dla bezpieczeństwa państwa w
rozumieniu ustawy z dnia 29 listopada 2000 (tekst jednolity z 2004 Dz.U. 229 poz 2315( o obrocie z zagranicą
towarami, technologiami i usługami o znaczeniu strategicznym dla bezpieczeństwa państwa, a także dla utrzymania
międzynarodowego pokoju i bezpieczeństwa.

utrzymania międzynarodowego pokoju i bezpieczeństwa.
§ 2. If it is necessary to deliver the goods in order to conduct customs revision, collect samples, conduct analysis
and examinations I undertake to reimburse the costs incurred (art. 69 point 1 and 3 of the Community Customs
Code)/ W przypadku konieczności dostarczenia towaru w celu przeprowadzenia rewizji celnej, pobierania próbek,
przeprowadzenia badania i analiz zobowiązuję się do zwrotu poniesionych kosztów (art. 69 pkt 1 i 3
Wspólnotowego Kodeksu Celnego).
§ 3. In case of obtaining the Binding Tariff Information I undertake to deliver a copy thereof to the Office of
ARKO LOGISTICS s.c. immediately/ W przypadku uzyskania Wiążącej Informacji Taryfowej zobowiązuję się do
niezwłocznego dostarczenia jej kopi do biura ARKO LOGISTICS s.c.
§4. I undertake to keep due diligence in preparing and delivering proper documents each time required to
prepare a customs declaration (art. 201 of the Community Customs Code)/ Zobowiązuję sie do zachowania
należytej staranności w przygotowaniu i dostarczaniu odpowiednich dokumentów każdorazowo wymaganych do
sporządzenia zgłoszenia celnego (art. 201 Wspólnotowego Kodeksu Celnego)
§ 5. Being aware of penal liability resulting from art. 270 and 272 of the Penal code I hereby state that the
documents delivered being the basis to make a customs declaration are true and complete and I certify the
authenticity and correctness of the data contained therein/ Świadomy odpowiedzialności karnej wynikającej z art.
270 i 272 Kodeksu karnego oświadczam, iż dostarczone dokumenty będące podstawą do dokonania zgłoszenia
celnego są prawdziwe i kompletne oraz poświadczam autentyczność i prawdziwość zawartych w nich danych.
§ 6. I undertake to deliver – if it is required by the customs bodies – additional information and documents
intended to make a customs declarations/ Zobowiązuję się do dostarczenia – jeśli to będzie wymagane przez
organy celne – dodatkowych informacji i dokumentów służących do dokonania zgłoszenia celnego.
§ 7. In case of failing to deliver within the defined period the documents required to make a customs declaration,
I undertake to cover the costs of warehouse, storage fees and costs of returnable shipment of goods and the
penalties resulting from art. 94 § 1 and § K.K.s./ W przypadku nie dostarczenia w określenia terminie dokumentów
wymaganych do dokonania zgłoszenia celnego, zobowiązuję się do pokrycia kosztów opłat magazynowych,
składowych oraz kosztów powrotnej wysyłki towaru oraz kar wynikających z art. 94 § 1 i § 2 K.K.s.

The Ordering party should immediately inform the proper customs bodies about any withdrawal of the
authorization/O cofnięciu niniejszego upoważnienia Zlecający winien niezwłocznie poinformować właściwe organy celne.
