Oświadczenie wydane zostaje w związku z wymogami
wynikającymi z art. 26 ust. 1g pkt 2 ustawy z dnia 15
lutego 1992 r. o podatku dochodowym od osób
prawnych (Dz.U. z 2000 r. Nr 54 poz. 654 z późn. zm.) w
celu
zastosowania
przez
płatnika
zwolnienia
wypłacanych należności z podatku u źródła w Polsce.
This statement has been issued in reference to the
requirements arising from article 26.1(g).2 of the Act
of 15 February 1992 on corporate income tax
(Journal of Laws of 2000, No. 54, item 654, as
amended) in order to allow withholding tax
exemption by the payer in Poland.
Jednocześnie zobowiązuję/my/ się do niezwłocznego
poinformowania płatnika w przypadku zmiany danych
objętych niniejszym oświadczeniem.

payer about any change to information contained in
