14. Wypełniając niniejszy formularz aplikacyjny na TRANSATLANTYK FESTIVAL oświadczam, iż
wyrażam zgodę na umieszczenie moich danych osobowych w bazie danych organizatora Festiwalu,
tj. Fundacji TRANSATLANTYK, ul. Św. Marcin 80/82 pok. 208, 61-809 Poznao, NIP 7831663735,
REGON 301597976 (dalej Fundacja) oraz ich przetwarzanie – zgodnie z postanowieniami ustawy z
dnia 29 sierpnia 1997 r. o ochronie danych osobowych (t. j. Dz. U. 2002, Nr 101, poz. 926 ze zm.) –
co jest niezbędne dla celów związanych z organizacją TRANSATLANTYK FESTIVAL. Zgoda na
przetwarzanie danych osobowych, o której mowa powyżej, obejmuje swoim zakresem również
zgodę na udostępnienie moich danych osobowych podmiotom trzecim, tj. partnerom oraz
sponsorom współpracującym z Fundacją przy organizacji Festiwalu. Oświadczam, iż powyższe dane
udostępniam dobrowolnie, zostałam/-em poinformowana/-y o celu zbierania danych i zakresie ich
przetwarzania oraz o tym, że mam prawo dostępu do treści swoich danych, jak również ich
poprawiania czy żądania zaprzestania przetwarzania moich danych osobowych (By filling in this

information in the database of the organizer of the Festival, that is the TRANSATLANTYK Foundation,

National Business Registry Number REGON 301597976 (hereinafter referred to as the Foundation)
and to its processing in accordance with the resolutions of the personal data protection act as of
29th August 1997 (Journal of Laws, 2002, No 101, pos. 926 with consecutive changes), which is
necessary for the purposes connected with the organisation of the TRANSATLANTYK FESTIVAL. The
consent for personal information processing mentioned above also includes the permission to share
my personal information with third parties, that is partners and sponsors who work with the
Foundation on the organisation of the Festival. I declare that I provide the abovementioned
information voluntarily, I have been informed about the purpose of information collection and its
processing scope, as well as about my right to access, correct and claim to stop processing my
personal information)
