INFORMACJE OGÓLNE
Przed wypełnieniem niniejszego formularza należy zapoznać się z odpowiednimi rozdziałami
Przewodnika Programu „Uczenie się przez całe życie” (Lifelong Learning Programme Guide) oraz z
Zaproszeniem do składania wniosków na rok 2013 (2013 Call for proposals), opublikowanymi przez
Komisję Europejską oraz Narodową Agencję, które zawierają dodatkowe informacje, np. o terminach
składania wniosków, adresach Narodowych Agencji, do których należy wysyłać wnioski oraz o
określonych priorytetach obowiązujących w danym roku.
W 2013 r. obowiązuje w Polsce jeden termin składania wniosków: 28 marca (decyduje data
stempla pocztowego).
Asystentura musi być zrealizowana w nieprzekraczalnym terminie podanym w punkcie 3.3 poniżej.
Prosimy zwrócić uwagę, że każda Narodowa Agencja może wymagać od wnioskodawców przesłania
dodatkowych informacji nt. asystentury. Wnioskodawca, przed przesłaniem wniosku o
dofinansowanie, powinien to sprawdzić na stronie internetowej Narodowej Agencji w
swoim kraju.

Polska Narodowa Agencja wymaga złożenia wersji elektronicznej wniosku w Systemie
dokumentów OnLine FRSE pod adresem http://online.frse.org.pl

Wniosek został złożony do Narodowej Agencji w kraju zamieszkania wnioskodawcy.
W przypadku, gdy wnioskodawca pracuje w kraju innym niż ten, w którym jest rezydentem,

Wnioskodawca jest obywatelem kraju uczestniczącego w programie „Uczenie się przez całe
życie” albo obywatelem innego kraju, pracującym albo mieszkającym w kraju uczestniczącym,
zgodnie z krajowymi wymogami prawnymi (należy sprawdzić na stronie internetowej właściwej
Narodowej Agencji). Lista krajów uczestniczących znajduje się w części I Przewodnika
Programu „Uczenie się przez całe życie” 2013, punkt 1.C: „Które kraje uczestniczą w
programie?".
Wnioskodawca należy do jednej z kategorii osób uprawnionych do udziału w niniejszej akcji
(proszę zapoznać się ze „Szczegółowymi kryteriami formalnymi” dla akcji Asystentury
Grundtviga w Części II Przewodnika Programu „Uczenie się przez całe życie” 2013).


Działanie, jakie ma być podjęte w ramach wnioskowanego dofinansowania, jest uprawnione w

Działanie odbędzie się w jednym z krajów uczestniczących w programie „Uczenie się przez całe

Kraj wysyłający i/lub goszczący musi być krajem członkowskim UE.

Asystentura odbędzie się w kraju innym niż kraj, w którym wnioskodawca mieszka lub pracuje.

Asystentura odbywa się w uprawnionym terminie.

Asystentura spełnia wymóg formalny dotyczący długości trwania: od 13 pełnych tygodni (91
dni) do 45 tygodni (315 dni).

Do polskiej Narodowej Agencji należy złożyć:
- 1 wniosek wypełniony w języku polskim lub angielskim z oryginalnymi podpisami i pieczęciami,
- 2 kopie ww. wniosku,
- list z organizacji goszczącej potwierdzający przyjęcie wnioskodawcy na asystenturę, której
dotyczy wniosek (Letter of Acceptance) - musi zawierać imię i nazwisko wnioskodawcy, nazwę i
adres oraz informacje o profilu działalności organizacji goszczącej Asystenta, termin i miejsce
Asystentury oraz jej program,
- dokumenty informujące o prawnym statusie organizacji macierzystej wnioskodawcy i
potwierdzające profil działalności edukacyjnej dla dorosłych w rozumieniu programu Grundtvig,
- w przypadku pracowników uczelni: właściwa dokumentacja potwierdzająca pracę wnioskodawcy
w specjalnie wydzielonych centrach edukacji niezawodowej dla osób dorosłych znajdujących się w
strukturze organizacyjnej uczelni (np. Uniwersytet Trzeciego Wieku),
- w przypadku osób, które nie mają organizacji macierzystej, tzn. które chcą powrócić do pracy w

obszarze edukacji dorosłych lub przekwalifikowują się do pracy w tym sektorze albo osób
posiadających kwalifikacje do pracy w edukacji dorosłych, które zamierzają dopiero rozpocząć
karierę w tym obszarze: właściwe dokumenty jednoznacznie potwierdzające te plany i zamiary
oraz (jeśli dotyczy) kwalifikacje,
- w przypadku studentów: szczegółowa informacja nt. odbywanych studiów i kwalifikacji, do jakich
uzyskania prowadzą te studia, potwierdzona przez właściwą uczelnię – w Polsce uprawnieni są
studenci ostatniego roku studiów magisterskich z zakresu edukacji dorosłych,
- dokument potwierdzający znajomość języka obcego, w jakim odbywa się Asystentura.
W przypadku niespełnienia któregokolwiek z wyżej wymienionych warunków, w tym niezłożenia
wymaganych dokumentów w obowiązującym terminie składania wniosków, wniosek zostanie
odrzucony na etapie oceny formalnej.

Can understand and use familiar everyday expressions and very basic phrases
aimed at the satisfaction of needs of a concrete type. Can introduce him/herself and
others and can ask and answer questions about personal details such as where
he/she lives, people he/she knows and things he/she has. Can interact in a simple

Can understand sentences and frequently used expressions related to areas of most
immediate relevance (e.g. very basic personal and family information, shopping,

ZASADY UDZIELANIA DOFINANSOWANIA
Niniejsze informacje stanowią uzupełnienie do Przewodnika Programu „Uczenie się przez całe życie”
(Lifelong Learning Programme Guide) na rok 2013. Dlatego też należy dokładnie zapoznać się z
rozdziałem 4 (Przepisy Finansowe) tego dokumentu.
Jeśli Pani/Pana wniosek zostanie zaakceptowany, dofinansowanie będzie przeznaczone na pokrycie
kosztów podróży, utrzymania oraz, jeżeli mają zastosowanie, kosztów przygotowania
językowego, kulturowego i pedagogicznego. Narodowa Agencja zadecyduje o wysokości kwoty
dofinansowania.
Osoby ze specjalnymi potrzebami mogą ubiegać się o zwiększenie dofinansowania, jak wskazano
w części „Specjalne potrzeby” poniżej.
Koszty podroży
Asystentury są dłuższą formą mobilności (minimalny czas trwania musi przekroczyć 13 tygodni –
minimum 91 dni) i w związku z tym dofinansowanie kosztów podróży zawiera się w dofinansowaniu
na koszty utrzymania.
Jeżeli osoby indywidualne mieszkają w jednym z krajów lub terytoriów zamorskich podanych
w Decyzji Rady 2001/822/EC lub jeden z takich krajów lub terytoriów jest miejscem przeznaczenia,
rzeczywiste poniesione koszty podróży będą podlegać zwrotowi w całości, bez względu na czas
trwania mobilności.
Dofinansowanie nie obejmuje kosztów ubezpieczenia na czas podróży. Mogą one zostać pokryte
w ramach standardowych kwot przeznaczonych na koszty utrzymania.
Koszty utrzymania
Koszty utrzymania będą obliczane i dofinansowywane na podstawie odpowiednich stawek,
w zależności od długości trwania mobilności. Osoby, które otrzymały dofinansowanie nie muszą
przechowywać rachunków w celu udokumentowania kosztów utrzymania.
Prosimy zapoznać się ze stroną Narodowej Agencji w celu zasięgnięcia informacji nt. obowiązujących
stawek dla wnioskodawców z Polski i zasad obliczania kosztów utrzymania.
Przygotowanie językowe, kulturowe i pedagogiczne
Możliwe jest ubieganie się o dofinansowanie kosztów przygotowania językowego, kulturowego
i pedagogicznego, jeżeli uważa Pani/Pan, że jest to niezbędne dla pomyślnej realizacji Asystentury.
W przypadku wnioskodawców z Polski maksymalne dofinansowanie kosztów przygotowania
językowego, kulturowego i pedagogicznego w roku 2013 nie może przekroczyć kwoty 300 EUR
i będzie przyznawane w formie ryczałtu. Przygotowanie językowe może obejmować np. samodzielną
naukę lub udział w kursach językowych. Szczegóły należy określić w punkcie 3.4. niniejszego
formularza (np. wyszczególnić usługi, np. kurs językowy, ewentualnie materiały dydaktyczne, jakie
wnioskodawca planuje zakupić z podaniem ich kosztu).
Na etapie raportowania beneficjent musi przedstawić dowód potwierdzający podjęcie działań
przygotowawczych, np. kopię certyfikatu uczestnictwa w kursie językowym.

Specjalne potrzeby wnioskodawcy
Jeśli wnioskodawca ma specjalne potrzeby, na przykład wynikające z niepełnosprawności, może
zwrócić się z prośbą o dofinansowanie dodatkowych wydatków, jakie są niezbędne do poniesienia,
aby umożliwić wnioskodawcy udział w Asystenturze (np. udział osoby towarzyszącej). W takim
przypadku wysokość dofinansowania kosztów utrzymania i podróży będzie ustalana indywidualnie,
w oparciu o ponoszone koszty rzeczywiste.
Narodowa Agencja rozpatrzy wniosek o zwiększenie dofinansowania z tytułu specjalnych potrzeb
w stosunku do informacji podanych w punkcie 2.4 i 7 formularza wniosku. Beneficjent zobowiązany
będzie do zachowania dowodów poniesionych kosztów z tytułu specjalnych potrzeb.
