Materiał i metody. Grupę badaną stanowiły 102 osoby, zawodnicy (n=53) i zawodniczki (n=49) uprawiający podnoszenie ciężarów. Zawodnicy reprezentowali wszystkie kategorie wagowe, a czas przeprowadzania badań przypadał w okresach dużej intensywności treningowej. Wśród uczestników badań przeprowadzono autorską ankietę. Do oceny funkcjonalnej użyto testu Functional Movement Screen (FMS). Wyniki. Wnioski. Istnieje silna zależność pomiędzy poziomem sportowym a wysoką oceną funkcjonalną. Korzystne efekty ćwiczeń stosowanych w specjalistycznym treningu ciężarowców uzasadniają wprowadzenie ich do ogólnego treningu sprawności fizycznej jako elementu profilaktyki występowania kontuzji. The purpose of the study was functional evaluation of people training weightlifting. Material and methods. Totally 102 participants (53 men and 49 women) practicing weightlifting took part in the examination. A study group consisted 69 Polish National Team competitors and further 33 persons with lower level of advancement. Weightlifters represented all weight categories and testing time was during high intensity period of training. Participants fill the author’s questionnaire form. For the functional evaluation, Functional Movement Screening Test was used. Results. Average result of the FMS test for the weightlifters was 18,08 points, what suggest that studied group is not highly exposed for injuries due to functional defects. A correlation between years of practice and average number of FMS score has been detected (r=0,48 ; p<0.05). Higher sport level of competitors was also a determining factor of better functional level of weightlifters (p<0.05). Conclusions. There is a strong correlation between sports level and high functional evaluation. Profitable effects of exercises used in weightlifters training motivate to applied them to general fitness training as a part of injury prevention. Coraz częściej spotykamy zawodników, którzy do granic możliwości eksploatują swój organizm. Jeszcze przed rozpoczęciem ukierunkowanego treningu należy te punkty znaleźć i wzmocnić. In present times, sport in the last period of twenty years the very impetuous progression of training loads is getting quite perceptible [1]. More and more often we meet competitors who exploit their own organism to the limits of the possibility. Situations in which one exceeds the level of suitable weights for the given training-unit, or worse still for the whole training-period, became more and more common. The effect of such exceeding of the optimum-level of the work performed on trainings can be the negative state of health of a competitor, what in near future will translate itself into the level of attained results. Formerly the weightlifting was considered as one of sport disciplines which in particular predisposed to damages of the spine. In the light of research weightlifters are a class of sportsmen at whom these damages appear considerably more often than at other competitors training strength-based sports [2]. Usually at women realizing trainings of the strength character, trauma of the back are the most common reason of breaks in trainings [3]. Competitors beginning their own adventure with the discipline of the heavy athletics consisting of barbell lifting, have several neuralgic points in the musculoskeletal chain, what particularly raises the risk of the experience of some contusion. Even before the beginning of the oriented training, one ought to find and strengthen these points. Beginner weightlifters should concentrate on the development to the general flexibility, strengthening of muscles of the abdomen and the segment of rotators of the shoulder, simultaneously perfecting the individual technique of fights and the harmony of muscle functioning [4]. The two lifts, the snatch and the clean and jerk, performed during a contest are simultaneously basic exercises in the weightlifting. During the snatching, the bar of definite mass is situated horizontally before the feet of a competitor. The barbell gripped overhand, is raised from the rack with one, continuous movement till the moment of full extension of both upper limbs overhead, at the simultaneous realization of the subsquat. From the subsquat the competitor gets up at any time, accepting the straight position. The second technique, performed after the snatch – the clean and jerk, consists of two phases: the clean and the jerk of the barbell from the chest. In the clean of the barbell on the chest there appear the same elements as in the snatch, the difference is connected only with the greater lowering and a shorter distance of the barbell lifting [5,6]. According to some authors men doing the weightlifting are from the very beginning sentenced to the cripplehood [7,8]. Adding up micro traumas of intervertebral discs and fractures of terminal plates, in the future can lead to the pathology of the spine at weightlifters [9,10]. Pain complaints of weightlifters are the result of training overloads which are essentially relative to the time of training the given discipline [11]. The most frequent traumata occurring among men doing the weightlifting are biceps and quadriceps muscle tears, and also acute overload changes such as traumatic spondylolisthesis and discopathy [7,12,13]. Within last 20 years, both in sport as well as in rehabilitation, there has been a perceptible trippant 268 115 Adamczyk1:Layout 1 2013-01-29 11:57 Strona 3 Adamczyk J.G. et al., Functional evaluation of weightlifters niania. Test wymaga siły mięśni, elastyczności, zakresu ruchu, koordynacji, równowagi i propriocepcji, aby pomyślnie ukończyć siedem podstawowych wzorów ruchowych. Wychodząc z tych przesłanek, celem pracy uczyniono ocenę możliwości funkcjonalnych osób uprawiających podnoszenie ciężarów. trend of abandoning the traditional and isolated evaluating. The changes consist of strengthening of the integrated functional approach and the inclusion of rules of proprioceptive neuromuscular facilitation. It is important to control and understand common basic aspects of the human movement. One ought to understand that for the purpose of the preparation of a person to the wide range of activities, fundamental movements should be evaluated in a complex way [14,15]. A new manner of research of a motor system is being promoted. It requires the use of complex movement patterns for the purpose of traumata foreseeing. Research evaluating many functions (equilibrium, strength, the range of movement, co-ordination and elasticity) can simultaneously enlarge the exactitude of the diagnostics of the risk of damages at persons undergoing the testing. Functional Movement Screening (FMS) is a complex testing which evaluates the quality of basic movement patterns to identify the person having motor limitations or asymmetry. Basic movement patterns are used to test the range of the movement, stability and equilibrium. The test demands muscle strength, elasticity, the range of the movement, co-ordination, equilibrium and proprioception, to finish favourably seven basic movement patterns. The points from seven motor tests are added up and evaluated. The system of points was designed so as to determine essential motor limitations and the right- or the left-sided asymmetry connected with the exercise of the functional movement. Seven tests using the set of basic positions and movements, which are thought to be the base for more complex sports-movements, should be performed efficiently and correctly [16,17]. A modern and functional motor training has to diminish the risk of the contusion, and consequently an extension of the duration of a sports-career, making it better „qualitatively” by the improvement of attained sports-results. Therefore it seems that though mechanisms of the formation of traumata in the weightlifting are well enough recognized there is a lack of elaborations concerning the early detection of functional shortages and the preventive conduct. Going out from these premises one made the estimation of functional possibility of persons doing the weightlifting the aim of the work. Materiał i metody Material and methods Badaniu poddano 102 osoby, zawodników i zawodniczki uprawiające podnoszenie ciężarów. W badaniu wzięło udział 49 kobiet (48.03% badanych) i 53 mężczyzn (51.97% badanych). Sztangiści i sztangistki reprezentowali wszystkie kategorie wagowe, a średnia waga badanych zawodników w przypadku mężczyzn wyniosła 82.56 (±20.63), a u kobiet 64.73 (±17.75) kilograma. Średni staż treningowy badanych sztangistów to 4.67 lat (±2.62), podczas gdy u sztangistek 5.51 lat (±3.17). FMS został opracowany w celu wykrycia ograniczeń funkcjonalnych lub asymetrii pomiędzy lewą a prawą stroną wśród zdrowej populacji, w odniesieniu do wykonywanych zadań ruchowych. Owe zadania wymagają od ćwiczącego fundamentalnego poziomu równowagi, stabilizacji, siły i balansu ciała. Ekstremalne pozycje w trakcie wykonywanych wzorców ruchowych zapewniają szybkie oraz bezproblemowe wychwycenie słabych punktów i nieprawidłowości ruchowych. Test FMS składa się z następujących prób: głębokiego przysiadu, przeniesienia nogi nad płotkiem, przysiadu w wykroku, oceny ruchomości obręczy barkowej, aktywnego uniesienia wyprostowanej nogi (ASLR – active straight leg raise), ugięcia ramion w podporze i stabilizacji rotacyjnej tułowia. Sposób wykonania oceniany jest od 3 do 0 punktów, gdzie: • 3 – prawidłowe wykonanie wzorca • 2 – wykonanie podstawowego wzorca ruchowego z elementami kompensacji • 1 – osoba jest niezdolna do wykonania wzorca ruchowego • 0 – ból uniemożliwiający wykonanie ruchu [16]. Wyniki badań przedstawiono w postaci uśrednionych wartości z uwzględnieniem odchyleń standardowych. Do oceny istotności różnic pomiędzy grupami wykorzystano test t-Studenta. Do oceny zależności pomiędzy zmiennymi zależnymi wykorzystano współczynnik korelacji Pearsona. Do badania siły związku pomiędzy dwiema zmiennymi jakościowymi wykorzystano współczynnik Yule’a. class in the weightlifting. The time of running the research was set within a period of preparatory of the large training-intensity. In the case of the U-20 group, it was a week before the Polish Junior Championships. The representatives of seniors were in the progress of preparations to the approaching European Championships in Turkey. The main objective of the group was the start in the approaching Olympics in London. The U-17 were preparing themselves to the Polish Championships of Folk Sport. The average age of the examined group was 19 years (±3.32). The weightlifters represented all categories of the weight, and the average weight of the examined competitors in case of men was 82.56kg (±20.63), and women 64.73kg (±17.75). The average training experience of the examined male weightlifters was 4.67 years (±2.62), while female ones 5.51 years (±3.17). The average number of trainings per week-days for the whole group was 5.33 (±1.29), the competitors realized from maximally nine to minimally three trainings every week. For the functional estimation one applied the Functional Movement Screen (FMS) test, which was created by Gray Cook [16]. FMS was developed for the purpose of the detection of functional limitations or asymmetries among the left and right side among the healthy population, with reference to the performed motor exercises. Those exercises demand from those exercising a fundamental level of equilibrium, stabilization, strength and body balance. The extreme positions during performed movement patterns assured quick and trouble-free determining of vulnerabilities and movement faults. The FMS test consists of following tests: deep squat, hurdle step, in-line squat, shoulder mobility, active straight leg raise (ASLR), core stability and trunk stability push-up. The way of exacting tests is rated from 3 to 0 points, where: • 3 – the correct realization of the pattern, • 2 – the realization of the basic movement pattern with elements of the compensation, • 1 – the person is unable to exact the movement pattern, • 0 – the pain is making the realization of movement impossible [16]. The results of the research were introduced in the form of average values with the regard of standard deviations. To evaluate the importance of differences among groups one used the t-Student test. As statistically essential one accepted values of coefficients on level p≤0.05. To estimate dependence among dependent variables one used Pearson’s coefficient of correlation. For the examination of the relationship strength between two qualitative variables, one used Yule′s coefficient. Wyniki Results W badanej grupie 102 sztangistów i sztangistek średnia liczba pkt. w teście FMS wyniosła 18.08. In the examined group of 102 male and female weightlifters the average number of points in the FMS test was 18.08. Best results were obtained for the test of deep squat (Fig. 1) – the basic movement pattern, systematically copied during specialistic exercises included in the training of weightlifters from the very beginning of the process of instruction. During this exercise results were indeed higher (p<0.05) than those of remaining tests with the exception „push-up” and ASLR. 270 115 Adamczyk1:Layout 1 2013-01-29 11:57 Strona 5 Adamczyk J.G. et al., Functional evaluation of weightlifters W rezultacie przeprowadzonych badań stwierdzono zależność pomiędzy stażem treningowym a średnią liczbą pkt. uzyskiwaną w teście FMS (r=0.48; p<0.05). The FMS test as a diagnostic tool in the estimation of functional possibilities is used in many sport disciplines. Fritz′ and Wainner’s results permit to ascertain, that FMS is an extremely specific test (0.91) in the case of the qualification of the risk of experiencing the serious contusion [18] both in the case of men as well as women [19]. Elaborations evidence a very high (the internal coefficient of the reliability – ICC) [14, 15] or high [20] reliability of Functional Movement Screening. Basic 272 115 Adamczyk1:Layout 1 2013-01-29 11:57 Strona 7 Adamczyk J.G. et al., Functional evaluation of weightlifters Screening. Dane te wskazują, że FMS można wykorzystywać do oceny schematów ruchowych sportowców i podejmować decyzje związane z interwencjami w celu poprawy wydajności. Stopień urazowości był jednak niewielki w porównaniu do innych dyscyplin sportowych [24]. movement patterns, such as these evaluated by FMS, can be easily clinically tested. As the most reliable test from the FMS set one regards the test of shoulder mobility, and the least the hurdle step [21]. Kiesel’s research which was run on professional footballers before the beginning of the season, showed that the average result of the test for all the examined was 16.9 (±3.0) while for persons who experienced the trauma it was 14.3 (±2.3). The test showed the essential difference between the average of tens injured footballers and those who did not become injured. After the analysis of the results one ascertained, that the test identifies correctly the greatest number of persons prone to experience trauma. For one showed that professional footballers with the result ≤14 had greater risk of the break in professional sport training, due to serious contusion within one season [17]. In the research run by Schneiders and co. on 209 physically active persons, aged from 18 to 40 years, the average result was 15.7 pts. One did not ascertain statistically essential difference in the performance between women and men or those who notified earlier problems with the motor system. For the test one reached very high reliability which amounted 0.971. This sectional FMS research assures values of reference for young, active persons which are helpful in the interpretation of each scoring during screening research of sportsmen [22]. The idea of the estimation of basic movements shows, that competitors training American football with the sum of points equal and lower than 14 were over eleven fold more prone to traumata, while persons with the asymmetric result had an eight times greater risk of the experience of the contusion. The results of the research did not show the essential relationship among the training advancement and the strength level, and the susceptibility to contusions [16]. As previously stated, in FMS the key for the analysis is the result, especially such amounting 14 and less points, aside from of the appearing asymmetry. For instance, the result of 18 points with the asymmetric estimation evidences the three times greater risk of the exposure to trauma, in the case ≤14 points the risk increased to eleven fold greater values [14, 15]. The data show that FMS can be used for the estimation of motor schemata of sportsmen and undertake connected decisions with interventions to the improvement of the efficiency. FMS can help with the identification of competitors subjected to the high risk of the contusion [23]. Elaborations concerning traumatism in the weightlifting are not univocal. The result of examining run by Siewe and co. of 245 weightlifters of the elite in the given discipline was the percentage of 43.3% weightlifters complaining about problems during the routine training. The coefficient of the traumatism one counted as 0.3, while the number of traumata per one weightlifter per 1000 training-hours carried out 1. There were no evidence that external or internal factors influence this coefficient. Most often injured body parts were shoulders, back and knees. The usage of belts and the greater mass of the body of competitors was connected with the height of the number of traumata of the lumbar section of the spine. The number of injuries of upper limbs increased considerably for the age of >40 years. The degree of traumatism was however not large in comparison to other sport disciplines [24]. 273 115 Adamczyk1:Layout 1 2013-01-29 11:57 Strona 8 Adamczyk J.G. i wsp., Ocena funkcjonalna zawodników uprawiających podnoszenie ciężarów Badania Raske i Norlin przeprowadzone wśród elity ciężarowców miały na celu określenie częstości występowania urazów, ze szczególnym uwzględnieniem ewentualnych kontuzji prowokowanych ćwiczeniami. 274 The research of Raske and Norlin run among the elite of weightlifters targeted the qualification of the frequency of the occurrence of traumata, with particular reference to possible contusions provoked with exercises. As most frequent appeared low situated traumata of back and shoulders [2]. Poor results of the part of tests (the shoulder mobility, the rotary stability test and the trunk stability push-up) confirm the most frequent location of pain complaints which in the case of weightlifters are the lumbar section of the spine and the shoulder-hoop. Insufficiently strong, in the case of persons training the weightlifting muscles of the abdomen, mostly the transverse muscle, and oblique external and internal muscles of the abdomen which together are responsible in 80% for lower part of the thorax as well as disorders of their harmonious synergy with the group of muscles responsible for the extension of the spine (the unsettled neuromuscular co-ordination between these two muscular groups), predispose these sections to traumata. Interesting is the fact, that the exercise of muscles of the abdomen is included in the set of five basic points (exercises) realized during every training. The question arises, whether the manner of their realization, and also the genus of engaged muscles is suitable. Probably one does not apply the properly large attention to exercises engaging oblique and transverse muscles which together play essential part in the stabilization of the lumbar section. This attention is directed on the strengthening of the straight abdomen muscle which does not play so significant part in this case. It is similarly with the participation of exercises engaging muscles of external rotators of the shoulder. In the case of weightlifters, the specificity of performed exercises during the training causes the considerably greater engagement of muscles of internal rotators of the shoulder. At male and female weightlifters undergoing the examination one ascertained the dependence between the class of sport of the given competitor and the frequency and the length of the trainings duration, what in the effect translates into the perfect mastery of movement patterns evaluated in the FMS test. The research confirmed also the essential correlation between the time spent on training the discipline and the number of places in which there are notified pain complaints resulting surely from protracted changes of workload, nascent during long-lasting training of the weightlifting. The analysis of the test results permitted to show the place of the trauma very well, in the case of the occurrence of asymmetric dysfunctions. Complaints of the various genus and appearing limitations in the training direct the attention toward functional possibilities. Functional research was created so that basing on its results one could create the optimum-program of exercises, to control its effectiveness. The defining of vulnerabilities in bio kinematic chain is very important, because their existence can be a factor limiting the further sports-development [25]. From among potential representatives of Poland at Olympics in London at four persons during the research one showed the potential threat with the contusion as the result of low result in the test. This fact can prove the reliability of FMS. On the eve of IO in 2012 three from among these weightlifters had to undergo the operation of the places indicated in the research by us. 115 Adamczyk1:Layout 1 2013-01-29 11:57 Strona 9 Adamczyk J.G. et al., Functional evaluation of weightlifters Wnioski Conclusion 1. 1. The very high estimation 4 from 7 motor exercises is a results of usage of these movement patterns as elements of specialistic exercises, practiced in the weightlifting already from the beginning of the training. 2. Persons who obtain low results in tests: the deep squat, the hurdle step, the in-line lunge and the ASLR, have no possibilities to effectively train the weightlifting in the future. This way the FMS test can become the element of selection and choosing to this discipline of sport. 3. Poor results of the part of tests confirm the most frequent location of pain complaints which in the case of weightlifters is the lumbar section of the spine, the shoulder-hoop and knee joints. 4. The functional training should be an object of the compact interest first of all for lightest (under 56 kg) male competitors and female competitors (under 48 kg) and women starting in the under 75 kg weight – category, because they showed evidence of the highest susceptibility to contusions. 5. The FMS test can be a useful tool for situating of vulnerabilities in the bio kinematic system of the competitor. Piśmiennictwo / References 1. Sozański H, Siewierski M, Adamczyk J.
