poradni specjalistycznych ( Dz.U. 2013 nr 0 poz. 199 )

Przyjmuję do wiadomości, iż zgodnie z przepisami ustawy z dnia 29 sierpnia 1997 roku o ochronie danych
osobowych (Dz. U. z 2002 r. Nr 101, poz. 926 ze zm.) administratorem danych jest Poradnia PsychologicznoPedagogiczna nr 1. Mam świadomość przysługującego mi prawa wglądu do treści danych oraz ich poprawiania.
Podstawą prawną przetwarzania danych przez poradnię jest art. 23 ust. 1 pkt 2 ustawy z dnia 29 sierpnia 1997
roku o ochronie danych osobowych (Dz. U. z 2002 r. Nr 101, poz. 926 ze zm.).
Zakres danych określony jest w przepisach ustawy z dnia 7 września 1991 roku o systemie oświaty (Dz. U. z
2004 r. Nr 256, poz. 2572, z późn. zm.), ustawy z dnia 15 kwietnia 2011 roku o systemie informacji
oświatowej (Dz. U. z 2012 r. Nr 139, poz. 814, z późn. zm.) oraz rozporządzenia MEN z 1 lutego 2013 r. w
sprawie szczegółowych zasad działania publicznych poradni psychologiczno-pedagogicznych, w tym
publicznych poradni specjalistycznych (Dz. U. z 2013 r., poz. 199).
I acknowledge that an administrator of the submitted data is Psychological and Pedagogical Counseling
Centre No. 1., in accordance with the Personal Data Protection Act dated 29.08.1997 (uniform text:
Journal of Laws of the Republic of Poland 2002 No 101, item 926 with further amendments). I am aware that I
access and adjust my personal data. Personal data processing by the counseling centre is based upon Personal
Data Protection Act dated 29.08.1997 (uniform text:Journal of Laws of the Republic of Poland 2002 No 101,
item 926 with further amendments).
The data scope is defined by the following legislation acts: The Education System Act of 7 September 1991
(uniform text: Journal of Laws of the Republic of Poland 2004 No 256, item 2572 with further amendments), The
Act of 15 April 2011 on the System of Educational Information (uniform text: Journal of Laws of the Republic of
Poland 2012 No 139, item 814 with further amendments), the Ordinance of the Minister of National Education
dated 1st of February 2013 on detailed principles of public psychological - pedagogical counseling centres,
including public specialist centres (uniform text: Journal of Laws of the Republic of Poland 2013, item 199)
