Jej składową jest pomiar kąta nosowo-wargowego, zawartego pomiędzy styczną do podstawy nosa oraz styczną do konturu wargi górnej. Cel: zbadanie zależności pomiędzy kątem nosowo-wargowym, a pozycją szczęki (kąt SNA) i stopniem nachylenia osi siekaczy centralnych górnych (kąt 1/SN). Materiał i metody: przeprowadzono badania na 163 zdjęciach cefalometrycznych bocznych losowo wybranych pacjentów. Wszystkie zdjęcia rentgenowskie zostały obrysowane przez jednego badacza za pomocą ołówka o grubości 0,3 mm na przeźroczystej kalce. Zmierzono kąty: nosowo-wargowy, SNA oraz 1/SN. Pacjentów podzielono na trzy grupy ze Assessment of patient`s soft-tissue profile is essential in making diagnosis and establishing treatment plan. Materials and methods: 163 lateral, randomly selected, cephalometric radiographs were investigated. All of them were traced by one investigator with a 0,3 mm pencil on acetate paper. SNA and 1/SN angles were measured. Patients were divided into three groups: with decreased, increased or correct values of NLA, which, following Mc Namara, is 102±8 degrees. Do zbadania zależności między dwoma parametrami mierzalnymi użyto testu istotności współczynnika korelacji r Pearsona. Przyjęto 5% błąd wnioskowania i związany z nim poziom istotności p<0,05. Wyniki: wartości kąta nosowo-wargowego wahały się w przedziale od 74° do 147,2° (średnia 111,8°). Wykryto istotną zależność statystyczną pomiędzy wartością kąta nosowo-wargowego i kąta nachylenia siekaczy górnych do linii SN (1/SN) przy p<0,05. Nie wykazano zależności pomiędzy wartością kąta nosowo-wargowego a kątem SNA. Średnia wartość konta nosowo-wargowego wśród osób płci żeńskiej wynosiła 110,5° a płci męskiej 113,8°, ale różnica ta nie była istotna statystycznie. Wnioski: Wykazano, że kąt nosowo-wargowy zależy od nachylenia osi siekaczy górnych, a nie od położenia szczęki (SNA). (Forum Ortod. 2008; 4: 167 - 173). Statistically significant correlation between NLA and inclination of upper incisors` axis towards SN line (1/SN) at p<0,05 was detected. No significant correlation was found between NLA and SNA angles. Mean NLA value was 110,5 degrees in females and 113,8 degrees in males, which had no statistical significance. Ludzie atrakcyjni są pewni siebie, szybko uzyskują akceptację społeczną, łatwiej zawierają znajomości, dostają lepszą pracę, a ładne dzieci - lepsze stopnie w szkole (1). Niektóre specjalizacje medyczne jak np. chirurgia plastyczna, ortodoncja lub protetyka mogą wpływać na zmianę wyglądu twarzy. W praktyce klinicznej zachodzi potrzeba oceny estetyki twarzy i odchyleń od standardu estetycznego oraz przewidzenia wyniku leczenia. W ortodoncji jedną z najważniejszych składowych planu leczenia jest ocena tkanek miękkich twarzy pacjenta widzianej na wprost oraz z profilu (2). Do oceny rysów twarzy wykorzystuje się wiele metod, tj. antropometria, fotometria, wizerunek komputerowy i cefalometria (3, 4, 5, 6, 7). Jedną ze składowych takiej oceny jest wartość kąta nosowo-wargowego, zawartego pomiędzy styczną do podstawy nosa oraz styczną do konturu wargi górnej (7). Wg McNamary u dorosłych wynosi on 102°± 8° i zależny jest od położenia szczęki lub siekaczy górnych oraz od podstawy nosa (7). Powstało wiele prac dotyczących kąta nosowo-wargowego, ale niewiele z nich dokładnie wyjaśnia istotne dla ortodontów zagadnienie, co może powodować zmianę powyższego kąta: zmiana pozycji szczęki, czy siekaczy górnych a może jedno i drugie. Celem pracy było znalezienie zależności pomiędzy kątem nosowo-wargowym, a pozycją szczęki w stosunku do przedniego dołu czaszkowego (kąt SNA) lub stopniem nachylenia osi siekaczy centralnych górnych względem powyższego dołu czaszkowego (kąt 1/SN). FORUM ORTODONTYCZNE The look of the face is very important in building up self-esteem .Attractive people are self-confident, they gain social acceptance immediately, strike up new acquaintances easier, get better jobs and nice looking children get better grades at school (1). Certain medical specialties, e.g. plastic surgery, orthodontics or prosthodontics, are capable of making changes in the facial look. In the clinical practice assessment of facial esthetics and degree of deviations from esthetical standards as well as prediction of treatment results has become mandatory. Assessment of the soft-tissue appearance, both anterior and lateral, is one of the fundamental components of the treatment plan in orthodontics (2). In order to assess facial features, many methods are used, i.e. anthropometry, photometry, computer imaging and cephalometry (3, 4, 5, 6, 7). The value of the nasolabial angle (NLA), formed between a line tangent to the nasal base and a line tangent to the contour of the upper lip, is one of the components of this assessment (7). According to McNamara, NLA is equal to 102±8 degrees in adults and its value is dependent on the position of the maxilla or upper incisors as well as the base of the nose (7). In the literature there are many reports concerned with the nasolabial angle but few of them point out the possible causes of change in the value of this angle: is it due to a change in the position of the maxilla or upper incisors or maybe both? The purpose of the study was to detect correlation between the nasolabial angle and the position of the maxilla in relation to the anterior cranial base (SNA angle) or inclination of the upper central incisors` axis in relation to the anterior cranial base (1/SN angle). 167 ORTHODONTIC FORUM Prace oryginalne Original papers Tom 4 nr 6 2008 Volume 4 no 6 2008 Materiał i metody Materials and methods Badania zostały przeprowadzone na 163 zdjęciach cefalometrycznych bocznych losowo wybranych pacjentów Zakładu i Katedry Ortopedii Szczękowej Uniwersytetu Medycznego w Lublinie w wieku od 6 do 30 lat. Byli to pacjenci z wadami klasy I, II i III. Odrzucone zostały zdjęcia pacjentów z wadami wrodzonymi: z zespołem Bindera, połowiczym niedorozwojem twarzy, rozszczepem wargi i podniebienia, itp. Przeanalizowano 163 rentgenogramy w tym 100 osób płci żeńskiej (61,3 %) oraz 63 płci męskiej (38,7 %). Wszystkie zdjęcia rentgenowskie obrysował jeden badacz za pomocą ołówka o grubości 0,3 mm na przeźroczystej kalce. Wyznaczono punkty: sella turcica (S), nasion (N), punkt A, upper lip (UL), subnasale (Sn) oraz linie: SN, NA, oś siekaczy centralnych górnych, styczną do podstawy nosa i Sn-UL. Zmierzono kąty: nosowo-wargowy, SNA, 1/SN (Ryc. 1.). Prawidłowe wartości kąta nosowo-wargowego mieściły się w przedziale od 94° do 110° podanym przez McNamarę. Uzyskane wyniki poddano analizie statystycznej w celu określenia zależności pomiędzy wartościami kąta nosowo-wargowego a kątem SNA i 1/SN. Zbadano również zależność między wartością kąta nosowo-wargowego a płcią. Wartości analizowanych parametrów mierzonych w skali nominalnej scharakteryzowano za pomocą liczności i odsetka, natomiast w skali ilorazowej za pomocą wartości średniej i odchylenia standardowego z podaniem zakresu zmienności. Do porównania dla dwóch grup niezależnych użyto testu t-Studenta, natomiast do zbadania istnienia zależności między dwoma parametrami mierzalnymi użyto testu istotności współczynnika korelacji r Pearsona. Przyjęto poziom istotności p<0,05 wskazujący na obecność istotnych statystycznie różnic bądź zależności. The study was conducted on 163 lateral cephalometric radiographs selected at random from patients within the age range from 6 to 30 years at the Chair and Department of Jaw Orthopedics, Medical University of Lublin. The patients presented Class I, II and III malocclusions. Radiographs obtained from patients with congenital defects, such as Binder`s syndrome, hemifacial microsomia, cleft lip and palate etc., were excluded from the study. From 163 analysed radiographs, 100 came from females (61.3%) and 63 from males (38.7%). All the radiographs were traced by one investigator with a 0,3mm pencil on tracing paper. The following points were marked: sella turcica (S), nasion (N), point A, upper lip (UL), subnasale (Sn). The following lines were also outlined: SN, NA, the axis of upper central incisors, a line tangent to the base of the nose as well as Sn-UL. The following angles were measured: nasolabial, SNA, 1/SN (Fig. 1). After McNamara, correct values of the nasolabial angle were considered within limits from 94 to 110 degrees. In order to determine correlation between the values of nasolanbial angle and SNA, 1/SN angles, obtained results were the subject of statistical analysis. Correlation between the value of the nasolabial angle and sex was also studied. The values of the analysed parameters measured in a nominal scale were characterized according to the size and percentage, whereas those measured in a ratio scale were characterized by means of the mean value and standard deviation with specification of the change range. For comparison, t-Student test was used for two independent groups, whereas in order to investigate correlation between two measurable parameters, r Pearson correlation coefficient significance test was made use of. A significance level amounted to p<0,05 indicating statistically significant difference was accepted. Wyniki Results Wartości kąta nosowo-wargowego wahały się w przedziale od 74° do 147,2°, średnia wynosiła 111,8°. Pacjentów zaszeregowano do trzech grup: A, B i C. W grupie A o wartościach zmniejszonego kąta nosowo-wargowego (poniżej 94°) znajdowało się 20 pacjentów, w grupie B o wartościach prawidłowych (94°- 110°) było 54 pacjentów, a w grupie C o wartościach zwiększonych (powyżej 110°) znajdowało się 89 pacjentów - rycina 2. Średnia wartość kąta nosowo-wargowego w grupach A, B i C wynosiła odpowiednio, 87,8°, 105,2° i 121,2°. Wartości średnie kątów: nosowo-wargowego, SNA, 1/SN w poszczególnych grupach obrazuje tabela.1. U 72 pacjentów występowała wada klasy I, u 67 pacjentów wada klasy II oraz u 24 badanych wada klasy III. Klasy szkieletowe diagnozowano na podstawie wartości kąta ANB. Średnia wartość kąta nosowo-wargowego w poszczególnych klasach wynosiła: klasa I – 111,4º, klasa II 115,1º, a klasa III 104,1º (Tab. 2). Zbadano również wartość kąta nosowo-wargowego w zależności od płci. Średnia dla płci żeńskiej wynosiła 110,5º a dla męskiej 113,8º. FORUM ORTODONTYCZNE The NLA values were within the range of 74 to 147,2 degrees, and the mean value was 111,8 degrees. The patients were classified into three groups: A, B and C. Group A, with decreased values of the nasolabial angle (less than 94 degrees), included 20 patients; Group B, with correct NLA values (94 – 110 degrees), included 54 patients; and Group C comprised 89 patients with increased NLA values (more than 110 degrees) - Fig.2. The mean values of the nasolabial angle in Groups A, B i C were: 87,8 degrees, 105,2 degrees and 121,2degrees, respectively. The mean values of the NLA, SNA and 1/SN in each group are presented in Table 1. Class I malocclusion occurred in 72 patients, Class II - in 67 patients and Class III - in 24 patients. Skeletal classes were diagnosed on the basis of the ANB angle values. The mean value of NLA in individual classes was: Class I – 114,4 degrees, Class II – 115,1 degrees and Class III – 104,1 degrees (Table 2). Values of the nasolabial angle in relation to the patients` sex were also studied. W wyniku przeprowadzonej analizy statystycznej wykryto istotną zależność pomiędzy wartościami kąta nosowo-wargowego a kątem nachylenia osi siekaczy górnych do przedniego dołu czaszki (p=0,005). Stwierdzono również, że wraz ze wzrostem wartości kąta nosowo-wargowego zmniejsza się wartość kąta 1/SN (R= - 0,22). Natomiast nie było istotności statystycznej pomiędzy wartością kąta nosowo-wargowego a kątem SNA (Tab. 3.) oraz wartością tego kąta i płcią badanych pacjentów. FORUM ORTODONTYCZNE Statistical analysis revealed a significant correlation between the values of the nasolabial angle and inclination of the axis of the upper incisors towards the anterior cranial base (p= 0,005). It was also observed that while the value of nasolabial angle increased, the value of the 1/SN angle decreased (R= - 0,22). No statistical significance was found between the value of the nasolabial angle and the SNA angle (Tab. 3) nor was there any significance between the value of this angle and the patients` sex. 169 ORTHODONTIC FORUM Prace oryginalne Original papers Tom 4 nr 6 2008 Volume 4 no 6 2008 Dyskusja Discussion Podczas leczenia ortodontycznego dochodzi do zmiany profilu tkanek miękkich twarzy pacjenta, które ortodonta przed leczeniem musi przewidzieć. Oceniając kąt nosowo-wargowy należy wziąć pod uwagę położenie siekaczy, które wpływają na powyższy kąt. W omawianym badaniu stwierdzono istotną zależność między kątem nosowo-wargowym a nachyleniem osi siekaczy górnych (1/SN). Te same spostrzeżenia przedstawili w swojej pracy Talass i Baker (8). Leczyli oni ekstrakcyjnie pacjentów z wadą klasy II podgrupy 1 i zauważyli, że cofając siekacze szczęki o 6,7 mm, warga górna ulega cofnięciu średnio o 4,3 mm, a kąt nosowo-wargowy ulega zwiększeniu średnio o 10,5°. W grupie kontrolnej natomiast, stwierdzili oni tylko nieznaczny wzrost wartości powyższego kąta. Nasze wyniki potwierdzają powyższe badania, które obrazują, że wpływ na wartość kąta nosowo-wargowego mają siekacze górne, a nie położenie punktu A względem linii SN. Podobne wnioski, do przedstawionych w omawianej pracy, wyciągnęli Ramos i wsp. (9), którzy pacjentów dorosłych z wadą klasy II podgrupy 1 leczyli za pomocą ekstrakcji pierwszych przedtrzonowców górnych oraz aparatu stałego. Umieścili implanty w szczęce dla lepszej demonstracji zmian, jakie zachodzą w twarzoczaszce i profilu podczas cofania siekaczy. W trakcie leczenia siekacze górne zostały cofnięte od 3,7 mm do 5,6 mm, a kąt nosowo-wargowy uległ rozwarciu średnio o 4°. Praca Lo i Hunter (10) o podobnej tematyce wykazuje, że kąt nosowo-wargowy ulega zwiększeniu średnio o 1,6° podczas cofania siekaczy górnych o 1 mm. Zauważono również istotną zależność pomiędzy wzrostem powyższego kąta, a wzrostem dolnej wysokości twarzy, wydłużanie dolnego odcinka twarzy o 1 mm powoduje rozwieranie kąta nosowo-wargowego o 2,2°. Stwierdzili także, że wzrost o 1° kąta zawartego między płaszczyzną żuchwy a płaszczyzną frankfurcką powoduje powiększanie kąta nosowo-wargowego średnio o 3°. W piśmiennictwie są zdania odmienne do wyników przedstawionych w omawianej pracy, gdzie nie wykazano istotnej korelacji pomiędzy kątem nosowo-wargowym a położeniem siekaczy górnych (11). W literaturze podawane są różne średnie wartości kąta nosowo-wargowego, które mieszczą się w przedziale od 88,14° do 114,08° (7, 12, 13, 14). Właściwą interpretację utrudnia również fakt nieujednoliconego sposobu jego wyznaczania. Potrzebne są do tego dwie linie, które można wyznaczyć na różne sposoby: metoda linii stycznych, metoda punktów anatomicznych oraz połączenie dwóch w/w metod (2). W naszej pracy przyjęliśmy normy i sposób wyznaczania podany przez McNamarę jako najbardziej rozpowszechniony i uwiarygodniony w licznych doniesieniach. Rozbieżności spowodowane są pochodzeniem etnicznym pacjentów. Szczególnie widoczne różnice stwierdza się wśród rasy czarnej (88,14°±12,52°), u której częściej występuje protruzja obuszczękowa (12). In the process of orthodontic treatment changes of the facial soft-tissue profile will occur, therefore they must be predicted by the orthodontist prior to launching appropriate therapy. On evaluation of the nasolabial angle, position of the incisors, which affect the value of this angle, must be taken into consideration. In the presented study a significant correlation was found between the nasolabial angle and the inclination of the axis of the upper incisors (1/SN). The same observations were reported by Talass and Baker (8). They carried out extraction treatment in the patients with Class II malocclusion` division 1 and noticed that moving back of the maxillary incisors by 6,7 mm resulted in retraction of the upper lip by approx. 4,3 mm, and also the nasolabial angle increased by 10,5 degrees on average. In the control group, however, they observed only a slight increase in the value of the NLA. Our results confirm Talass and Baker`s findings which represent the view that it is the upper incisors that affect the values of the nasolabial angle and not the position of point A in relation to the SN line. Similar conclusions were drawn by Ramos and his coworkers (9) who applied extraction treatment in the adult patients with Class II maloclussion` division 1. They performed extraction of the first upper premolars and fixed a permanent orthodontic appliance. For better demonstration of the changes that occur in the facial skeleton and the facial profile while the incisors are moved distally, they placed implants in the maxilla. In the course of treatment, the upper incisors were retracted by 3,7 mm-5,6 mm, and the nasolabial angle increased by 4 degrees on average. The study of Lo and Hunter (10), concerning the same issue, points out that the nasolabial angle increases by 1,6 degrees on average while the upper incisors are being retracted by 1 mm. A significant correlation was also found between the increase in the nasolabial angle and the increase in the lower face height, i.e. elongation of the lower segment of the face by 1 mm causes the increase in the nasalabial angle by 2,2 degrees. The investigators also claimed that the increase in the angle between the maxillary plane and the Frankfort plane causes the increase in the nasolabial angle by approx. 3 degrees. The results presented in this study are not in agreement with the opinions found in the literature, where no significant correlation has been reported between the nasolabial angle and the position of the upper incisors (11). The literature provides different mean values of the nasolabial angle, and they are within the range of 88,14 – 114,08 degrees (7, 12, 13, 14). Proper data interpretation is also hindered by an ununified way of the NLA determination. In order to do that, two lines are needed and they may be outlined in different ways: following the method of tangent lines, or the method of anatomical points or a combination of both (2). Mean value of nasolabial angle in particular skeletal Class Średnia wartość kąta nosowo-wargowego Mean nasolabial angle Analiza statystyczna wyników omawianych badań nie wykazała zależności pomiędzy wartością kąta nosowo-wargowego a płcią badanych pacjentów. Podobne wnioski przedstawili w swoich pracach Bishara (15), Lo i Hunter (10), którzy nie zauważyli dużych zmian kąta nosowo-wargowego zachodzących wraz z wiekiem oraz nie stwierdzili zależności od płci. Jednak większość autorów, na czele z Prahl-Andersen i Nandą (16), twierdzi, że występuje zależność od płci i wieku. Powyższy kąt jest większy u dziewcząt niż u chłopców i zmniejsza się wraz z wiekiem bardziej u dziewcząt niż u chłopców. Koniuszek nosa rośnie do przodu i w dół i staje się bardziej dominujący w profilu. Warto zauważyć, że już w 1959 roku Subtelny (17) zaobserwował, że zmiany pomiędzy miękkimi i twardymi tkankami szkieletu nie zachodzą liniowo. Badał on twarz w wymiarze horyzontalnym i wertykalnym i zauważył, że rozwój tkanek miękkich jest niezależny od struktury kostnej twarzoczaszki, a więc nie każda część tkanek miękkich dokładnie podąża za strukturą szkieletu. Any encountered discrepances result from the ethnic origin of the patients. Most striking differences are found in the black patients (88,14±12.52 degrees) who present protrusion of both jaws more frequently (12) Statistical evaluation of the results obtained in the presented study revealed no correlation between the value of the nasolabial angle and the sex of the study subjects. Similar opinions were presented in the reports of Bishara (15), Lo and Hunter (10) who did not find substantial changes in the nasolabial angle in connection with age, there was no correlation with the patients` sex, either. Yet, the majority of authors, with Prahl-Andersen and Nanda (16) in the lead, are of the opinion that the value of the nasaolabial angle correlates with the age and sex. The NLA is larger in girls than in boys and it decreases with age in girls more than in boys. The tip of the nose grows forward and downward, and thus it becomes more dominant in the facial profile. Również średnie wartości kąta nosowo-wargowego podawane w literaturze są bardzo różne i wahają się od 88° do 114°. Dlatego też należy zachować dużą ostrożność podczas interpretacji powyższego kąta (18, 19). Wyniki leczenia ortodontycznego muszą być akceptowane przez społeczeństwo, które podąża za modą. Współcześnie uważa się, że ładniejsze są wydatne, wypukłe wargi i zmniejszony kąt nosowo-wargowy. W przeszłości poglądy były inne, a mianowicie podobały się twarze „subtelne” o zwiększonym kącie nosowo-wargowym, co prawdopodobnie było spowodowane przedwczesną utratą pierwszych zębów przedtrzonowych. Należy więc ostrożnie planować ekstrakcje ortodontyczne w łuku górnym, gdyż powodują one rozwieranie kąta nosowo-wargowego z powodu cofania się wargi górnej. Taki wynik leczniczy może być źle akceptowany przez pacjentów. Wnioski 1. Zmiana położenia siekaczy górnych wpływa na wartość kąta nosowo-wargowego. 2. Nie zaobserwowano związku pomiędzy kątem nosowo-wargowym a położeniem szczęki. occur linearly. He studied faces in the horizontal and vertical dimension and noticed that the development of the soft tissue is independent of the bone structure of the facial skeleton. Hence, not every part of the soft tissue follows precisely the bone structure (model). The thickness of the soft tissue is different in each patient (2). The mean values of the nasolabial angle found in the literature are also very different and remain within the range of 88–114 degrees. Therefore caution should be exercised while interpreting the NLA angle (18, 19). Results of orthodontic treatment must be acceptable in the society who follows the trends in fashion. These days prominent and bulging lips and increased nasolabial angle are fashionable. In the past the views on fashion were different, i.e. “subtle” faces, with increased nasolabial angle, were favoured. This was probably caused by early loss of the first premolars with subsequent upper incisors lingual tipping. Therefore orthodontic extractions in the upper arch should be planned with caution since they result in the increased nasolabial angle due to the upper lip retraction. Such orthodontic treatment may not be acceptable by the patient. Conclusions 1. Changes in the position of the upper incisors has an effect on the value of the nasolabial angle. 2.
