Pierwszy test był wykonany na bieżni (bieg), a drugi na ergometrze narciarskim (podwójne pociągnięcie). Cel badań był podwójny: 1. ocenić wydolność fizyczną, używając testowych wysiłków na bieżni i ergometrze narciarskim, 2. określić chwilowe zmiany wybranych wskaźników krwi uważanych za markery stresu wysiłkowego i biologicznego kosztu. W czasie obu testów monitorowano częstość skurczów serca. Krew kapilarna była pobrana rano, 10 minut przed każdym testem, i w czwartej minucie powysiłkowej restytucji, w celu określenia stężeń mleczanu i hormonów. Badana zawodniczka demonstrowała wysoką wydolność fizyczną. Stężenie mleczanów we krwi wyniosło 10,2 i 13,4 mmol/L, a częstość skurczów serca 193 i 190 uderzeń/min, odpowiednio po teście biegowym i po teście podwójnych pociągnięć. Każdy test spowodował znaczący wzrost stężenia kortyzolu i hormonu wzrostu w surowicy krwi, podczas gdy zmiany stężeń SHBG, testosteronu i DHEA-S były znacznie mniejsze. Ponadto, powysiłkowe przyrosty stężeń kortyzolu i hormonu wzrostu były zaskakująco różne dla każdego testu. Ogromną kortyzolemię i bardzo słabe odpowiedzi hormonu wzrostu odnotowano po biegu, ale bardzo niskie stężenia kortyzolu i wysokie odpowiedzi hormonu wzrostu pojawiły się po teście podwójnych pociągnięć. Przyczyną różnych odpowiedzi na każdy test była prawdopodobnie interakcja dwóch kolejnych wyczerpujących wysiłków. The first test was undergone on a motorized treadmill (running), and the second one on a ski ergometer (double poling test). The purpose of this study was two-fold: 1. to assess physical fitness using two incremental exercise tests undergone on a same day, 2. to determined acute changes in blood indices considered as markers of exercise stress and biological cost. Heart rate (HR) was monitored during the whole exercise tests. Capillary blood was sampled in the morning, ten minutes prior to the each test and at 4th minute of recovery to determine lactate and hormones levels. Examined athlete demonstrated excellent physical capacity. Her blood lactate levels were of 10.2 and 13.4 mmol/L and HR were of 193 and 190 beats/min after the running test and the double poling respectively. Each test brought about meaningful rise of plasma cortisol and growth hormone levels, while changes in SHBG, testosterone and DHEA-S concentrations were much more less. Moreover, the post-exertions increments of cortisol and growth hormone levels were surprisingly vary for each the test. Huge cortisolism and very low responses of growth hormone were recorded during running test, but very low cortisol levels and high growth hormone response appeared during double pooling test. W czasie treningów lub zawodów zaangażowane w wysiłek są jednocześnie kończyny dolne i górne. Specyficznym rodzajem techniki w stylu klasycznym jest tzw. podwójne pociągnięcie (DP), które obejmuje powtarzane akcje skurczów i rozkurczów łokci, kolan, barków i bioder, wykonywanych z określoną częstością i w określonej sekwencji ruchów. Wielu badaczy prezentuje wyniki swoich badań biomechanicznych i analiz kinematycznych ruchu w czasie DP [1-3]. Mniej wiadomo o fizjologicznych odpowiedziach na ten specyficzny wysiłek, natomiast żadnych badań nie prowadzono, by określić wpływ dwóch powtarzanych i wyczerpujących wysiłków na równowagę we krwi pomiędzy hormonami o własnościach anabolicznych i katabolicznych. Warto zauważyć, że anaboliczny i kataboliczny status ocenia się zwykle na podstawie stężeń dwóch steroidowych hormonów, kortyzolu (C) i testosteronu (T). Ich molowy stosunek (T/C) [4] lub wolny wskaźnik androgenu/C [5-7] używane są do oceny bieżącego stanu metabolizmu endogennych białek w spoczynku, w trakcie okresu treningowego. Ponadto, badanie chwilowych zmian wskaźnika anaboliczno-katabolicznego, obliczonego ze statusu androgenicznego i kortykoidowego we krwi po wysiłku, dostarcza informacji o koszcie biologicznym tegoż wysiłku [8]. Cel niniejszych badań był podwójny: 1. ocenić fizyczną wydolność, stosując stopniowane testy, niespecyficzny na bieżni i specyficzny na ergometrze narciarskim, 2. określić chwilowe, powysiłkowe zmiany we krwi wybranych wskaźników stresu i kosztu biologicznego wysiłku u biegaczki narciarskiej o wysokim poziomie wyszkolenia. Cross country skier are endurance-trained athletes of high aerobic capacity. During training or competition upper and lower extremities are involved simultaneously. The specific type of technique for the classical style of this sport is the double poling (DP), which comprises repeated flexion and extension actions of elbows, knees, ankles, shoulders and hips performed with determined frequency and sequence of motions. Results of biomechanical and kinematical analysis of motions during DP has been reported by several investigators [1-3]. Less is known about physiological responses to that specific effort, whereas no studies were undertaken to examine an effects of two subsequent exhaustive exertions on equilibrium between blood hormones of anabolic and katabolic properties. It is worth to notice, that anabolic and catabolic status in blood, is usually estimated using concentrations of two steroidal hormones cortisol (C) and testosterone (T). Their molar ratio (T/C) [4] or Free Androgen Index/C [5-7] are utilized for estimation of current state metabolism of endogenous protein throughout training periods in athletes at rest condition. Moreover, examination of acute changes in anabolic-catabolic index calculated from androgenic and glucocorticoid status in blood following an exercise provides information about biological cost of the effort [8]. Background Materiał i metody is c op y is for pe rso na Zawodniczkę narciarstwa biegowego, prezentującą wysoki stopień wyszkolenia, poddano badaniom wysiłkowym, stosując dwa rutynowe testy o stopniowanej intensywności do wyczerpania. Testy były wykonane w tym samym dniu, a 3-godzinna przerwa rozdzielała oba wysiłki. Pierwszy test (biegowy) wykonany był przed południem na bieżni mechanicznej. Drugi, angażujący górne i dolne części ciała, wykonano po 3 godzinach na ergometrze narciarskim. Test ten jest użyteczny dla określenia mocy wykonywania podwójnych pociągnięć, będących główną techniką stosowaną w klasycznym narciarstwie biegowym. Częstość skurczów serca (HR) monitorowano przez cały czas. Krew kapilarną pobrano rano, 10 minut przed każdym testem i w czwartej minucie powysiłkowej odnowy. Poziom mleczanu (LA) oznaczono we krwi, a hormonalnych parametrów, czyli kortyzolu (C), testosteronu (T), hormonu wzrostu (GH), siarczanu dwuhydroepiandrosteronu (DHEA-S) i globuliny specyficznie wiążącej hormony płciowe (SHBG), w surowicy. Wskaźnik wolnego androgenu (FAI), marker wolnego, biologicznie aktywnego testosteronu, obliczono wg wzoru: FAI=T/SHBG*10. W oparciu o te zmienne obliczono wskaźnik anaboliczno-kataboliczny wg wzoru: A-CI=FAI/C*103. Hormony oznaczono stosując zestawy ELISA (DRG-GERMANY), LA stosując zestaw Dr-LANGE (GERMANY). Obmiński Z. i wsp., Reakcje biologiczne na test wysiłkowy – narciarka biegowa 346 Material and methods Highly skilled female cross country skier was subjected to exercise study with using two routine, laboratory exercise tests of graded intensity until exhaustion. These tests were undergone on the same day, and 3 h intermission separated both exertions. First exertion which was the running test on a motorized treadmill took place before noon. The second one, which involves both upper and lower body, was undergone 3 hours later one on the ski ergometer. The test on ski ergometer is useful for determination of power output during double poling exertion, being the main technique utilized in the classic cross country skiing. Heart rates (HR) was monitored during the whole exercise tests. Capillary blood was sampled in the morning, ten minutes prior to each test and at 4th minute of recovery. Lactate level (LA) was assessed in blood and hormonal parameters, cortisol (C), testosterone (T), growth hormone (GH), dihydroepiandrosterone sulfate (DHEA-S), and sex hormone binding globulin (SHBG) were measured in plasma. The Free Androgen Index (FAI), a marker of free, biologically active testosterone was calculated from the formula: FAI= T/SHBG*10. Based on these variables Anabolic-Catabolic Index (A-CI) was calculated from the formula: A-CI=FAI/C*103. Hormones were determined using ELISA kits (DRG-GERMANY), LA using DrLANGE kit (GERMANY). The study was approved by - Wyniki Results Wyniki rejestrowanych zmiennych zamieszczono w Tabelach 1, 2 i 3. Każdy test wysiłkowy spowodował wzrost stężeń hormonów w surowicy, ale ich przyrosty były zróżnicowane. Mniejsze powysiłkowe względne zmiany odnotowano w androgenach T i DHEA-S, większe zmiany odnotowano w stężeniach C i GH. Można spekulować, że powysiłkowe zmiany objętości surowicy miały udział w zmianach stężeń T, DHEA-S i SHBG. Stopień fluktuacji obserwowanych stężeń SHBG może być obrazem zmian objętości surowicy, ponieważ poziom tej globuliny nie zależy od pory dnia, a pojedynczy wysiłek nie wpływa na jej całkowitą ilość w krążeniu. Nieoczekiwane różnice odnotowano w odpowiedziach C i GH na każdy wysiłek. Ogromną kortyzolemię i słabą reakcję GH zauważono po pierwszym wysiłku (bieg), natomiast odwrotne zachowanie tych hormonów stwierdzono po drugim (podwójne pociągnięcia). The results of recorded variables are displayed in Table 1, 2 and 3. Each exercise test brought about rise of the plasma hormones but their increments were vary. Lesser end-exertion relative changes were in androgens, T and DHEA-S, higher ones were observed in C and GH. One may speculate, that post-exercises plasma changes of plasma volume contributed to the changes in T, DHEA-S and SHBG levels. The extent of SHBG fluctuations may by a reflex of changes of plasma volume, for the globulin is not depended on daytime, and its total amount in circulation does not change following single exertion. Unexpected differences were recorded in C and GH responses to each of exercise test. Huge cortisolism and weak response of GH were noted after first exertion (running), whereas the opposite behavior of these hormones were found after the second one (double poling). tio np roh ibit ibu dis tr ly - Tab. 1. Obmiński Z. et al., Biological responses to exercise test – female cross country skier 347 Badanie pokazało podobne częstości skurczów serca po obu testach, ale stężenie LA było wyższe (o 31%) po teście podwójnych pociągnięć. Różnica ta jest wynikiem większej masy mięśni szkieletowych włączonych w czasie wysiłku na ergometrze narciarskim. Bojsen-Møller i wsp. donieśli [9], że wysiłek podwójnych pociągnięć angażuje kończyny górne, tj. mięśnie, które obejmują stawy barkowe i łokciowe, ale także mięśnie dolnych kończyn: bioder i kolan. Tak czy inaczej, poziomy LA we krwi wskazywały na maksymalną prędkość i moc wyjściową pod koniec stosowanych testów. Maksymalna szybkość i moc wyjściowa, zarejestrowane w czasie testu biegowego i symulowanego wysiłku narciarskiego, dostarczyły dowodu na doskonałą wydolność, typową dla wysoko wyszkolonych biegaczy narciarskich. Jak wspomniano, kortyzol i hormon wzrostu objawiły się jako zmienne bardziej wrażliwe na wysiłek, w porównaniu do androgenów i SHBG. To znaczy, że kora nadnerczy – źródło kortyzolu i przysadka – źródło hormonu wzrostu w krążeniu, demonstrowały dużą hormonalną aktywność w czasie wysiłku. Ponadto, zachowanie kortyzolu w surowicy (wzrost z 283 do 552 nmol/L), w przedziale czasu od rana (8:00) do rozpoczęcia biegu (10:00), sugeruje pobudzenie przed pierwszym maksymalnym testem, co jest typowym symptomem aktywacji psychicznej osi przysadkowo-nadnerczowej. Pojawia się ona przed zawodami [10] lub ciężkim wysiłkiem laboratoryjnym [8]. Zmiana ta u badanej zawodniczki (przed próbą laboratoryjną) ujawniła silną motywację do wykonania pierwszego zadania tak dobrze, jak to możliwe. Odpowiedź kory nadnerczy na drugi, również wyczerpujący wysiłek, była dużo mniejsza, a spadek hormonu po nim trudny do zrozumienia. Podobny brak powysiłkowej sekrecji lub bardzo stępiona odpowiedź były obserwowane u sportowców przetrenowanych, ale taka hormonalna dysfunkcja zwykle związana jest z obniżoną zdolnością do pracy. Ponadto, nie możemy wyjaśnić dlaczego stężenie hormonu wzrostu po teście na ergometrze narciarskim (16,2 ng/ml) było tak różne od tego po teście biegowym (4,3 ng/ml). W oparciu o badania innych [11-14], możemy podejrzewać, że zróżnicowana gra hormonalna w naszym eksperymencie była prawdopodobnie wywołana fluktuacją glukozy (G) i wolnych kwasów tłuszczowych (FFA) przed testami. Wiadomo, że oba substraty energetyczne wywierają hamujący wpływ na odpowiedź hormonu wzrostu, a wysokie stężenie kortyzolu i adrenaliny (przed biegiem) prawdopodobnie aktywowało lipazę, enzym uwalniający FFA z komórek tkanki tłuszczowej. Jednakże kierunek naszej spekulacji nie jest w pełni zgodny z doniesieniami innych autorów [14], którzy wykazali zmniejszone odpowiedzi we krwi kortyzolu i hormonu wzrostu na drugi wysiłek wykonany po południu, gdy pierwszy wykonano przed południem w tym samym dniu. Wskaźnik anaboliczno-kataboliczny (A-CI) w naszych badaniach był zaskakująco wyższy w czasie drugiego wysiłku. Było to efektem bardzo niskiego stężenia kortyzolu, który nieoczekiwanie obniżył się po tym wysiłku. Jak wspomniano, spadek ten mógł być przyczyną niższego poziomu G i FFA, a to z kolei – konsekwencją większej odpowiedzi hormonu wzrostu. Warto rozważyć, czy wyższe stężenie we This study showed similar heart rates, but LA was higher (by 31%) after double poling. That difference resulted in larger mass of skeletal muscles involved into exertion on ski ergometer. Bojsen-Møller and co-worker reported [9], that double poling engages not only upper body limbs, i.e. the muscles that span shoulders and elbows joints, but also the muscles spanning lower body limbs: hips and knees, that contribute to generation of power output. Anyway, blood levels of LA indicated maximal intensity of exertions at the end of each employed test. The maximal speed and power output recorded respectively during running test and simulated ski exercise provided evidence for excellent physical capacity typical for highly skilled ski runners. As mentioned, cortisol and growth hormone appeared to be more sensitive variable to exertions compared to androgens and SHBG. That means, that adrenal cortexsource of cortisol, and anterior pituitary-source of growth hormone in blood demonstrated high hormonal activities after physical stimuli. Moreover, the behavior of plasma cortisol (rise from 283 to 552 nmol/L) within the period from morning (8:00) to prerunning state (10:00) suggests arousal prior to the first maximal test, that is typical symptom of psychical excitation of pituitary-adrenal axis. That change appears itself prior to an athletic competition [10] or to a heavy laboratory exercise [8]. That change before laboratory task highlighted strong motivation in examined subject to perform the first task as well as she was able. Adrenal responses to the second task, being also exhaustive exertion was much more smaller, and the fall of cortisol after it is hard to understand. Similar lack of cortisol secretion or its blunted response after exercise was observed among over trained athletes, but that hormonal dysfunction was usually associated with a physical underperformance. Moreover, we cannot explain why growth hormone level after exercise undergone on ski ergometer (16.2 ng/ml) was so differ than that after running test (4.3 ng/ml). Based on studies by the others [11-14] we may suspect, that such various hormonal the game in our experiment was probably induced by fluctuation of blood glucose (G) and free fatty acids (FFA) prior to the tests. Both energy substrates, G and FFA are known to have inhibitory effect on growth hormone response, and high level of cortisol together with adrenaline (prior to running) probably activated lipase, the enzyme which release FFA from adipose tissue. That direction of our speculation, however, is not fully agree with the results reported by others [14], who found blunted responses of both plasma cortisol and growth hormone to the second, afternoon exertion, when the first one was performed in the morning on a same day. Interestingly, that in our study, anabolic-catabolic index (A-CI) was higher during the second test. It was, however, due to very low cortisol level, which unexpectedly, in addition, dropped after that test. As mentioned, that fall might be a reason for lower G and FFA, and in a consequence a reason for higher response of growth hormone. It is worth to deliberate, whether that higher plasma growth hormone having anabolic property, may enhance the metabolic effect of higher A-CI. is c op y is for pe rso na lu se on ly - dis tr ibu tio np roh ibit ed . Obmiński Z. i wsp., Reakcje biologiczne na test wysiłkowy – narciarka biegowa 348 - Conclusions Badana zawodniczka narciarstwa biegowego prezentowała w warunkach laboratoryjnych doskonałą wydolność fizyczną. Dwa kolejne wyczerpujące wysiłki o podobnej intensywności, miały różny wpływ na odpowiedzi kortyzolu i hormonu wzrostu. Hipoteza, że różnice te były spowodowane przez różny status metaboliczny, zasługuje na dodatkowe badania. Examined female practicing cross country skiers presented excellent fitness in lab condition. Two subsequent exhaustive effort of a similar intensity had various effect on cortisol and growth hormone responses. The hypothesis, that these differences were caused by effects of various blood metabolic status during the tests merits further investigations.
