A subtotal excision of the tu- - Th is c op Keywords: cavernous hemangioma, forearm, operative treatment 75 BACKGROUND Cavernous hemangioma of skeletal muscles is a rare soft-tissue tumour [1,2,3,4], most often located in lower extremities [1,5,6,7,8]. Preoperative diagnosis is difficult to establish and is mainly based on Magnetic Resonance Imaging studies [1,2]. Currently, other diagnostic imaging modalities, such as CT or plain radiographs, are of limited use [9,10]. When clinical and MRI findings are unclear, the definite diagnosis is made by microscopic study of intraoperative tissue specimens [1,3,8]. - OPIS PRZYPADKU CASE DESCRIPTION 18-letnia pacjentka zg³osi³a siê do Poradni Ortopedycznej z powodu obrzêku i okresowego bólu prawego przedramienia, który pojawi³ siê przed rokiem. Ból nasila³ siê podczas pisania. Wyst¹pienie pierwszych objawów nie wi¹za³o siê z przebytym urazem b¹dŸ infekcj¹ miejscow¹. Klinicznie stwierdzono obrzêk 1/3 bli¿szej przedramienia i niewielk¹ bolesnoœæ w badaniu palpacyjnym, brak innych odchyleñ w badaniu fizykalnym i badaniach laboratoryjnych. W wykonanym TK (Ryc. 1) stwierdzono jedynie obecnoœæ odczynu ze œródkostnej (zgrubienie korowe) tylnej czêœci koœci ³okciowej. W obrazie MR (Ryc. 2) An 18-year-old woman presented at our orthopaedic outpatient clinic with a one-year history of swelling and intermittent pain in her right forearm. The pain usually aggravated during writing. The onset of symptoms was not related to trauma or infection. Physical examination revealed swelling of the proximal third of the right forearm as well as mild tenderness to palpation. No other abnormalities were found. Results of laboratory tests were normal. A CT scan of the forearm was made (Fig. 1) but it showed only endosteal reaction (cortical sclerosis and thickening) within posterior ulna. This copy is for personal use only - distribution prohibited. WSTÊP Naczyniak jamisty miêœni szkieletowych jest rzadko wystêpuj¹cym nowotworem ³agodnym tkanek miêkkich [1,2,3,4]. Najczêstsz¹ lokalizacjê stanowi¹ miêœnie koñczyn dolnych [1,5,6,7,8]. Przedoperacyjne ustalenie rozpoznania jest trudne i opiera siê przede wszystkim na wykonaniu badania MR [1,2]. Inne badania obrazowe – TK, RTG maj¹ obecnie mniejsze zastosowanie [9,10]. W przypadku niejednoznacznej oceny ostateczne rozpoznanie ustala siê na podstawie badania histopatologicznego materia³u pobranego œródoperacyjnie [1,3,8]. In order to establish a definite diagnosis, the patient was referred for a surgical biopsy. During the biopsy, a 1x1x1 cm tissue sample was taken from the ulnar aspect of the FDP muscle. The microscopic study of the specimen resulted in a diagnosis of cavernous hemangioma with chronic inflammation. After 5 months of follow-up, the patient was referred for surgical excision of the tumour due to persistent pain. The surgery was performed under general anaesthesia and local ischaemia with a tourniquet. Intraoperatively, a 7x3x3 cm pathological mass was found within the ulnar part of the FDP muscle. The entire mass, which grossly appeared abnormal, was excised. However, due to the large dimensions of the tumour it was not feasible to perform an excision with a clear margin of normal tissues. The postoperative period was uneventful and the wound healed by first intention. The final diagnosis made by a pathologist after examination of an intraoperative specimen was "mixed hemangioma with inflammation and thrombosis". Subsequently, the patient was scheduled for the next stage of treatment, i.e. reconstruction of the FDP muscle. During the surgery, which was performed 2 months after excision of the tumour, previously ed . uwidoczniono w obrêbie miêœnia zginacza g³êbokiego palców guz o podwy¿szonym sygnale we wszystkich sekwencjach i celem weryfikacji zakwalifikowano pacjentkê do biopsji. W trakcie zabiegu biopsji pobrano wycinek 1x1x1 cm z obszaru masy miêœnia zginacza g³êbokiego palców po stronie ³okciowej. Wynik badania histopatologicznego: Haemangioma cavernosum cum inflammatione chronica. Ze wzglêdu na dalsze utrzymywanie siê dolegliwoœci bólowych po 5 miesi¹cach zakwalifikowano pacjentkê do resekcji ca³ego guza. Zabieg wykonano w znieczuleniu ogólnym i niedokrwieniu. Œródoperacyjnie uwidoczniono nieprawid³ow¹ masê guza wielkoœci 7x3x3 cm obejmuj¹c¹ miêsieñ zginacz g³êboki palców po stronie ³okciowej. Usuniêto ca³y widoczny zmieniony obszar tkanki miêœniowej. Ze wzglêdu na jego rozleg³oœæ nie dokonano resekcji w granicach zdrowych tkanek. Rana wygoi³a siê przez rych³ozrost. Po uzyskaniu ostatecznego wyniku badania histopatologicznego: Haemangioma racemosum cum inflammatio et thrombosis pacjentkê zakwalifikowano do trzeciego etapu leczenia – wszycia czêœci bli¿szych œciêgien zginaczy g³êbokich palców IV i V do pozosta³ych brzuœców miêœniowych. Operacjê przeprowadzono po 2 miesi¹cach od poprzedzaj¹cej. After the operative wound had healed, the patient underwent an intensive rehabilitation programme which, after two months, resulted in full restoration of hand function and complete pain relief. An MRI study performed 3 months later (Fig. 3) did not reveal any pathological lesions, even with administration of the contrast agent. The patient received no adjuvant radiotherapy but was closely monitored. The patient has remained symptom-free and another MRI (Fig. 4.) taken at the last follow-up (5 years postoperatively) did not reveal recurrence of the tumour. DYSKUSJA DISCUSSION Naczyniaki jamiste miêœni szkieletowych s¹ rzadkimi nowotworami tkanek miêkkich [1,5,10]. Ze wzglêdu na wolno postêpuj¹cy wzrost i stopniowe nasilanie niecharakterystycznych objawów, diagnostyka tych zmian jest trudna i opiera siê na wykonaniu dodatkowych badañ obrazowych. Najwiêksz¹ czu³oœæ i swoistoœæ wykazuje badanie MR [1,2], podczas gdy klasyczne RTG i TK [1,4,9] maj¹ mniejsze zastosowanie. Klasyczna angiografia lub wenografia s¹ rzadziej stosowane, zw³aszcza wobec nieinwazyjnego, a dodatkowo wykazuj¹cego du¿¹ czu³oœæ wzglê- Cavernous hemangiomas of skeletal muscles are rare soft-tissue tumours [1,5,10]. Due to their slow growth and slow, gradual worsening of unspecific symptoms, the diagnosis is often difficult to establish and is based on imaging studies. Magnetic Resonance Imaging has the best sensitivity and specificity in detecting these tumours [1,2]. Other diagnostic modalities, such as plain radiographs or Computed Tomography are of limited use [1,4,9]. Po 3 miesi¹cach wykonano kontrolne badanie MR (Ryc. 3) – nie uwidoczniono patologicznych struktur, tak¿e po wzmocnieniu kontrastem. Nie w³¹czono radioterapii, jednak pacjentka pozosta³a pod sta³¹ kontrol¹ ambulatoryjn¹. W badaniu kontrolnym wykonanym po 5 latach pacjentka nie zg³asza³a dolegliwoœci, a badanie MR (Ryc. 4) nie wykaza³o cech odnowy guza. eo Ryc. 4. The extent of resection of a cavernous hemangioma located within skeletal muscles should be determined individually for each patient, taking into consideration the rather low potential for recurrence, as well as the importance of preserving as much of the limb function as possible. MRI angiography seems to be the best imaging modality for evaluation of vascular malformations. It distinguishes between low-flow and highflow lesions, with the former being angiomas, cavernomas, cavernous hemangiomas, or venous hemangiomas and the latter being consistent with arteriovenous hemangiomas [10]. Ultrasound studies, including color Doppler, seem to be of limited use in the preoperative evaluation due to the subjective nature of the study, its low specificity, low accuracy (as compared to the MRI studies) and inability to assess lesions within bone. In the patient described here, MRI findings did not allow for a definite diagnosis of the tumour. Therefore, a surgical (open) biopsy was required. A wide, total resection of the tumour remains a commonly accepted method of treatment for such lesions. It allows for a definite diagnosis to be made and eliminates the source of patient's complaints [1,2,4,5,6]. However, the extent of resection and subsequent treatment (including radiotherapy) is controversial [3,5,7]. Most authors recommend radical resection of the tumour with a clear margin of normal tissue in order to prevent recurrence [1,4,6]. For the same reason, resection might be preceded by embolisation of vessels providing blood supply to the tumour. However, preserving as much limb function as possible is also very important [1,6]. In our patient, the excision was limited to the area of muscle which appeared grossly abnormal. Thus, approximately 60% of the muscle bellies were spared, especially on the radial side of forearm, which enabled the reinsertion of flexor tendons of the 4th and 5th finger to be made at the last stage of operative treatment. Also, it allowed for full restoration of hand function [4]. Despite the fact that the patient received no postoperative radiotherapy, there is no evidence of recurrence after five years of follow up. us dem tkanki t³uszczowej i tkanek zawieraj¹cych kana³y naczyniowe (sensivity to fat and vascular channels) badania rezonansu magnetycznego [9]. Najbardziej optymalnym narzêdziem w przypadku diagnostyki malformacji naczyniowych wydaje siê byæ MR angiografia, która dzieli guzy na nisko przep³ywowe (low-flow) i wysoko przep³ywowe (high flow). Te pierwsze odpowiadaj¹ rozpoznaniom angioma, cavernoma, cavernous haemangioma, venous heamangioma, z kolei wysoko przep³ywowe odpowiadaj¹ arteriovenous hemangioma [10]. Badanie ultrasonograficzne, w tym USG Doppler, jako badanie przedoperacyjne, z przyczyn takich jak: subiektywnoœæ oceny, niska swoistoœæ, niedok³adnoœæ w stosunku do MR oraz brak nawet przybli¿onej oceny koœæca wydaje siê mieæ mniejsz¹ wartoœæ w stosunku do wczeœniej wymienionych. W prezentowanym przypadku wynik uzyskany w czasie pierwszego badania MR nie pozwoli³ na jednoznaczn¹ ocenê typu guza, z czym wi¹za³a siê koniecznoœæ wykonania diagnostyki operacyjnej. Powszechnie akceptowanym sposobem leczenia pozostaje ca³kowite usuniêcie guza, pozwalaj¹ce ustaliæ rozpoznanie i wyeliminowaæ Ÿród³o dolegliwoœci zg³aszanych przez pacjenta [1,2, 4,5,6]. Dyskusyjny pozostaje zakres wyciêcia guza oraz postêpowanie pooperacyjne, w tym radioterapia [3,5,7]. Wiêkszoœæ autorów zaleca radykaln¹ resekcjê w granicach tkanek zdrowych, celem przeciwdzia³ania ewentualnej wznowie [1,4,6], poprzedzone ewentualn¹ embolizacj¹ naczyniow¹. Jednak równie istotne jest maksymalne utrzymanie funkcji koñczyny [1,6]. W omawianym przypadku wyciêcie ograniczono do obszaru makroskopowo zmienionego, co pozwoli³o zachowaæ oko³o 60% masy brzuœców miêœniowych, szczególnie po stronie promieniowej przedramienia. Dziêki temu podczas trzeciego zabiegu operacyjnego mo¿liwe by³o wszycie pozostawionych wolno œciêgien palca IV i V oraz przywrócenie pe³nej sprawnoœci koñczyny [4].
