Czy zdają sobie oni sprawę z możliwości zastosowania prostych, skutecznych metod profilaktyki i wczesnego leczenia wad zgryzu, w tym ćwiczeń mięśniowych? Czy w trakcie studiów zdołali wyrobić sobie opinię na ten temat i jakie jest ich nastawienie do tej metody leczenia ortodontycznego? MATERIAŁ: w pracy posłużono się ankietą, składającą się z 10 pytań, którą przeprowadzono wśród 59 studentów V roku Wydziału Stomatologii PAM. Przeprowadzona ankieta była anonimowa. METODA: pytania ankiety zostały tak skonstruowane, aby możliwa była ocena wiedzy i opinii studentów na temat ćwiczeń mięśniowych w leczeniu oraz profilaktyce nieprawidłowości zgryzowych. Respondenci dokonali oceny własnego poziomu posiadanej wiedzy o roli ćwiczeń mięśniowych i możliwości ich zastosowania oraz odpowiedzieli na pyta- AIM: to test the level of knowledge about the functions of muscles exercises, as one of the methods of orthodontic treatment, among the students of the V year of the Dentistry Faculty Department of PAM as future dentists. It was conducted among 59 students of the V year of the dentistry faculty of PAM. METHOD: the questions were specially constructed to evaluate the level of knowledge and to ask students for opinions about the myotherapy in prevention and treatment of malocclusion. Students judged their own level of knowledge (concering muscular exercises) and answered on a question: did you form your own opinion about myotherapy during the studies? RESULTS: most of the students consider the muscles exercises as an effective method of prophylaxis and malocclusion treatment. WNIOSKI: ćwiczenia mięśniowe jako najprostsza metoda leczenia ortodontycznego rozumiana jest przez studentów Wydziału Stomatologii jako metoda skuteczna, której wyniki są zadowalające, co jest zgodne z opiniami specjalistów opisanymi w wielu pracach. Studenci krytycznie i obiektywnie odnoszą się do stanu swojej wiedzy na temat ćwiczeń mięśniowych. Pogląd ten panuje nie tylko wśród rodziców, ale i wśród lekarzy stomatologów. Spośród ortodontycznych metod leczenia ćwiczenia mięśniowe są metodą najprostszą. Około 100 lat temu ukazała się praca Rogersa, który jako pierwszy zwrócił uwagę na znaczenie mięśni w leczeniu ortodontycznym. Wyszedł on z założenia, że zaburzona równowaga czynnościowa poszczególnych grup mięśni ustno-twarzowych, powoduje lub podtrzymuje większość nieprawidłowości twarzowozgryzowo-zębowych. Prawidłowa funkcja, wyrażająca się równowagą czynnościową mięśni, kształtuje prawidłowo szczęki, łuki zębowe i warunki zgryzowe. Rogers zapoczątkował stosowanie przebudowy czynności mięśni układu stomatognatycznego przez odpowiednie ćwiczenia [1,2,3]. Mioterapia ma na celu rehabilitację mięśniową tzn. wzrost siły i napięcia mięśni hipotonicznych oraz rozluźnienie mięśni hipertonicznych za pomocą odpowiednich ćwiczeń. Terapia mięśniowa odgrywa dużą rolę jako metoda profilaktyki w zwalczaniu szkodliwych nawyków [4,5,6], a także jako metoda (niekiedy samodzielna) leczenia nieprawidłowości zgryzowych [1,7,8,9]. Ćwiczenia mięśniowe powinno się dostosowywać do wieku dziecka, rodzaju nieprawidłowości zgryzu z uwzględnieniem fizjopatologii mięśni oraz ogólnego rozwoju dziecka. Wymaga to regularności, systematyczności i właściwego sposobu wykonywania ćwiczeń [1]. Celem pracy jest sprawdzenie poziomu wiedzy na temat roli ćwiczeń mięśniowych jako jednej z metod leczenia ortodontycznego i profilaktyki wśród studentów V roku Wydziału Stomatologii PAM jako przyszłych dentystów pierwszego kontaktu. Czy zdają sobie oni sprawę z możliwości zastosowania prostych, skutecznych metod profilaktyki i wczesnego leczenia wad zgryzu, w tym ćwiczeń mięśniowych? Czy w trakcie studiów zdołali wyrobić sobie opinię na ten temat i jakie jest ich nastawienie do tej metody leczenia ortodontycznego? The orthodontic treatment often involves various types of appliances. This opinion appears not only among the parents but also in the dentist community. Among all the methods of orthodontic treatment, muscles exercises are the simplest. Rogers in his work about 100 years ago pointed the importance of muscles in the orthodontic therapy. He assumed, that the disorder of functional equilibrium of oral- and face-muscles causes (or maintains) most of the facial-malocclusion. The proper function, shown by the functional balance of the muscles, influence on proper shape of jaws, teeth and occlusion. Rogers has initiated the usage of appropriate exercises for rebuilding the functions of stomatognathic system muscles [1,2,3]. The purpose of myotherapy is myo-rehabilitation, that is: to increase the strength and tension of the hypotonicmuscles and to relax the hipertonic-muscles, by the use of adequate exercises. Myotherapy plays a great role in prevention of habits [4,5,6], and also is a method (often by itself) of malocclusion treatment [1,7,8,9]. The muscles exercises ought to be differentiated by 3 points of view: age of a child, general postnatal development (in relation to regularity and the proper way of doing exercises) and the kind of malocclusions subjected to muscles physiopathology [1]. The aim of this study is to test the level of knowledge about the functions of muscles exercises, as one of the methods of orthodontic treatment, among the dentistry students of PAM as future dentists. The main question is: are they aware of the possibility of using simple and effective methods, such as myotherapy in the prevention and early treatment of malocclusion? Did they form their own opinion on muscles exercises during the studies? 155 2006 Prace oryginalne Original papers tom 2 nr 6 2006 volume 2 no 6 Materiały i metody Materials and methods W pracy posłużono się ankietą, składającą się z 10 pytań, którą przeprowadzono wśród 59 studentów V roku Wydziału Stomatologii PAM. Przeprowadzona ankieta była anonimowa. Pytania zostały tak skonstruowane, aby możliwa była ocena wiedzy i opinii studentów na temat ćwiczeń mięśniowych w leczeniu oraz profilaktyce nieprawidłowości zgryzowych. Pytania dotyczyły oceny skuteczności ćwiczeń mięśniowych, a tym samym opinii na temat chęci ich zastosowania przez studentów w swojej przyszłej praktyce stomatologicznej z uwzględnieniem przyczyn negatywnego stosunku niektórych studentów do tej metody leczenia (pytania nr 1,2,3). Zapytano studentów o ich opinię na temat czasochłonności i wyników stosowania ćwiczeń mięśniowych (pytanie nr 4). Dokonano oceny wyższości aparatów ortodontycznych nad ćwiczeniami w leczeniu i profilaktyce wad zgryzu oraz oceny skuteczności ćwiczeń mięśniowych jako samodzielnej metody leczenia i w połączeniu z innymi metodami (pytania nr 5,6,7). Zapytano także o opinię na temat, od kiedy należałoby stosować ćwiczenia mięśniowe (pytanie nr 8). Respondenci dokonali oceny własnego poziomu posiadanej wiedzy o roli ćwiczeń mięśniowych i możliwości ich zastosowania oraz odpowiedzieli na pytanie czy w trakcie studiów zdołali oni wyrobić sobie zdanie na temat ćwiczeń mięśniowych (pytania nr 9,10). For the needs of this study a 10-question inquire was created. It was conducted among 59 students of the V year of the Dentistry Faculty Department of PAM. The survey was anonymous. The questions were specially constructed to evaluate the level of knowledge and to ask students for opinions about the myotherapy in treatment and prevention of malocclusion. They were also concerned about assessing the effectiveness of muscles exercises, causes of negative opinions about the muscles exercises and by that they polled the opinions on the possibility of using this kind of therapy in their future dental practice (questions 1,2,3). The timeconsuming aspects and the results of muscular exercises were also taken into consideration (question 4). Next, there was evaluated the superiority of orthodontic appliances over muscles exercises, and theirs effectiveness as a way of treatment both: itself and in cooperation with other methods (questions 5,6,7). There were also questions about accurate age when the myotherapy should be taken (question 8). At the end, students judged their own level of knowledge (concering muscular exercises) and answered on a question: did you form your own opinion about myotherapy during the studies (questions 9,10)? Jak wynika z tabeli 1 w zakresie skuteczności ćwiczeń (pytanie nr 1 i 2) większość ankietowanych (48 studentów - 81,3% badanych) uważa je za skuteczną metodę profilaktyki oraz leczenia wad zgryzu oraz zastosowałaby bądź stosuje taką metodę u swoich pacjentów. Wśród osób, które uważają, że ćwiczenia są nieskuteczne lub nie mają zdania na ten temat (11 osób- 18,7% studentów) na pytanie 3. odpowiadają, że to dlatego, iż nie wierzą w ich skuteczność oraz brakuje im wiedzy na temat mioterapii (tabela 2). Z oceną skuteczności ćwiczeń wiąże się również problem czasochłonności i wyników ich stosowania. Na pytanie 4. czy ćwiczenia mięśniowe są czasochłonne, a wyniki niezadowalające 61% badanych (36 osób) odpowiada, że ćwiczenia nie są czasochłonne, a wyniki są zadowalające. Z tabeli 3 wynika, że 76,3% studentów (45 osób) uważa, iż aparaty ortodontyczne są lepszą metodą leczenia. 35 studentów (59,3% ankietowanych) opowiada się za tym, że mimo skuteczności i zadowalających wyników ćwiczeń mięśniowych nie należy stosować jako samodzielnej metody leczenia wad zgryzu, z czym wiążą również możliwość ich stosowania w połączeniu z innymi metodami leczenia ortodontycznego (61% - 36 studentów). Tabela 4 dotyczy opinii studentów na temat dolnej granicy wieku, od której należałoby stosować ćwiczenia mięśniowe. Przeważająca liczba studentów uważa za odpowiedni wiek 6-7 lat (34 badane osoby - 57,6%). Z tabeli 5 wynika, że 74,6% badanych (44 osoby) krytycznie ocenia swoją wiedzę na temat mioterapii i jej roli w zapobieganiu oraz leczeniu wad zgryzu. Tylko 3 osoby są zdania, że posiadają na ten temat wystarczającą wiedzę. Większość ankietowanych (41 osób - 69,5% badanych) uważa, że w trakcie studiów nie zdobyło wystarczającej wiedzy i nie zdołało w pełni wyrobić sobie opinii o ćwiczeniach mięśniowych. Jak wynika z powyższych danych ćwiczenia mięśniowe jako najprostsza metoda leczenia ortodontycznego rozu- All 59 questionnaires have been analyzed. Table 1, which refers to efficacy of exercises (question 1,2), tells us that most of the students (48 - 81.3%) consider them to be an effective method of prophylaxis and malocclusion treatment and most of them would recommend it to their patients. Among the people who find the exercises non-efficient or do not have their own opinion (11 students-18.7%), most common answer was that they do not believe in their effectiveness of that method or that their knowledge in that matter is insufficient (table 2). 61% of examined people (36 students), responded to question nr 4, that muscles exercises are not time-consuming and the results are satisfactory. Table 3 shown, that 76.3% of students (45 people) consider orthodontic appliances to be a better method of therapy than muscles exercises. 59.3% of examined people (35 students) claim that despite the effectiveness and sufficient results, muscles exercises should not be used as an only treatment of malocclusion, and 61% (36 students) accept the possibility of using them combined with other orthodontic ways of treatment. The students’ opinions on the lowest level of age, from which the muscles exercises should be taken is presented in table 4. The majority of students has considered the age from 6 to 7 year-old to be the most suitable (34 students57.6%). 74.6% of students (44 people) evaluate their own knowledge on myotherapy very critically (table 5). Only 3 pupils claim that they have sufficient knowledge on muscles exercises. Most of examined students (41 people-69.5%) maintain that during the time of their studies they did not possess adequate knowledge and did not form their own opinions in that matter. As results from the data presented above, the muscles exercises are the simplest method of the orthodontic treatment, it is also considered as an effective method which results are satisfactory at a minimal time-consumption. 158 2006 Prace oryginalne Original papers tom 2 nr 6 2006 volume 2 no 65 miana jest przez studentów Wydziału Stomatologii jako metoda skuteczna, której wyniki są zadowalające przy małej czasochłonności stosowania. Piśmiennictwo podaje wiele przykładów korzystnego oddziaływania ćwiczeń mięśniowych, które dowodzą ich skuteczności m.in. w leczeniu zaburzeń z grupy przodozgryzów, tyłozgryzów całkowitych i częściowych, przodożuchwiu czynnościowym, zgryzach otwartych częściowych spowodowanych nawykiem ssania smoczka lub palca, zgryzach krzyżowych częściowych, niektórych bocznych przemieszczeniach żuchwy [1,7,8,9], a także jako metoda profilaktyki w zwalczaniu szkodliwych nawyków (dysfunkcje, parafunkcje) [4,5,6]. Na temat wyższości aparatów ortodontycznych nad ćwiczeniami mięśniowymi jako metody leczenia i profilaktyki oraz możliwości zastosowania mioterapii jako terapii samodzielnej spotykamy w piśmiennictwie wiele rozbieżności. Studenci w większości uważają leczenie aparatami za lepszy sposób leczenia i pomimo przekonania o skuteczności ćwiczeń mięśniowych jako metody leczniczej wyrażają swój negatywny stosunek do ćwiczeń mięśniowych stosowanych samodzielnie. Rogers, który ok. 100 lat temu jako pierwszy zwrócił uwagę na znaczenie mięśni w leczeniu ortodontycznym wyodrębnia leczenie mioterapią jako samodzielną metodę oraz uważa, że każde leczenie aparatem powinno być poprzedzone ćwiczeniami mięśniowymi [1]. Szlachetko na podstawie własnych doświadczeń klinicznych stwierdziła możliwość stosowania ćwiczeń jako wyłącznej metody leczenia nieprawidłowości z grupy przodozgryzów w różnych okresach rozwojowych [1]. Jednocześnie wyraża opinię, że nie w każdym przypadku przez stosowanie wyłącznie ćwiczeń mięśniowych uzyska się normę czynnościową i morfologiczną. Natomiast Przylipiak opisuje przypadek wyleczenia przodozgryzu całkowitego lewostronnego u 22-letniej pacjentki za pomocą wyłącznie ćwiczeń mięśniowych [9]. Perzyna zwraca uwagę na fakt, że ćwiczenia mogą być zarówno leczeniem jedynym jak i wspomagającym leczenie aparatami, wymagają jednak indywidualnego ustalenia czasu ich wykonywania, odpowiedniej kontroli, właściwej dokumentacji [8]. Jackowska podkreśla, że leczenie za pomocą ćwiczeń mięśniowych daje dobre wyniki w przypadkach wcześnie uchwyconych nieprawidłowości, powstałych wskutek czynników egzogennych [7], co wydaje się poglądem słusznym i obowiązującym nie tylko w piśmiennictwie polskim, ale i zagranicznym [4]. Za przykład może posłużyć praca badawcza Trankmanna, dowodząca skuteczności terapii miofunkcjonalnej w zwalczaniu szkodliwych dyskinez języka i warg. Według niego ten rodzaj ortopedii szczękowej bez użycia aparatów skraca i redukuje nakłady ponoszone przez ortopedię szczękową korzystającą z aparatów a ponadto zapobiega nawrotom [4]. Rozbieżne są również opinie studentów na temat dolnej granicy wieku dla stosowania ćwiczeń mięśniowych. Studenci uważają, że powinno się je stosować od 6-7rż, zaś w piśmiennictwie obserwuje się tendencje do wcześniejszego ich stosowania [1]. There are a lot of examples in the literature, that prove the usefulness of the muscles exercises in the treatment of: mesio-occlusion, partial- and general distocclusion, functional inferior prognathism, partial open bite caused by habitual dummy or finger sucking, partial cross bite, some lateral mandibular dislocation [1,7,8,9] and also as the method of prophylaxis in elimination of habits (disfunction, parafunction) [4,5,6]. We find many discrepancies in the literature about the superiority of orthodontic appliances over the usage of muscles exercises as a method of treatment and prevention in orthodontics and the possibility of using myotherapy, as an only therapy. Students clearly declare that orthodontic appliances are a better method of treatment and express their own negative opinion to the applying of muscles exercises as a standalone method. Rogers, who pointed about 100 years ago the importance of muscles therapy in orthodontics, distinguishes the myotherapy as an only method, and claims that every treatment with orthodontic appliances should be preceded by the muscles exercises [1]. Szlachetko on the basis of her own experience, stated a possibility of using exercises as an only method of curing malocclusion, like mesio-occlusions in various periods of postnatal development. At the same time he express an opinion that the only usage of muscles exercises may not result in obtaining the functional and physiological standard in all cases [1]. Przylipiak described the case of mesioocclusion on at 22-year-old patient successfully treated by using only muscular exercises [9]. Perzyna noticed that exercises can be both: way of treatment as itself and adjunct of orthodontic appliances, but they need however an indywidual time-treatment, suitable control, and appropriate documentation [8]. Jackowska emphasized that muscle treatment achieves positive results in cases when anomalies as an effect of egzogenic factors were early diagnosed [7]. This view is presented not only in Polish, but also in foreign literature [4]. As an example we can take Trankmann’s work, who proves the effectiveness of myo-functional therapy in controling of harmful tongue and lips dyskinesia. He claims that this kind of maxillofacial orthopedics (without usage of appliances) takes less of time, is cheaper and prevents recurrence [4]. There’s also disagreement about the lowest age-level for myotherapy. Students claim that it should be 6-7-yearold, while the literature is forcing 3-4-year-old time [1]. Conclusions As we can see, by looking at the answers presented by students, they have rather critical and objective opinion about their own level of knowledge (considered myotherapy). Maybe if they have had more informations about it, results of questionnaires would be different. Their statement of efficacy and positiveness of muscle treatment is parallel with the view presented by many specialists. Być może wyniki ankiet byłyby nieco inne gdyby posiadali na ten temat więcej wiadomości. Choć ich opinie odnośnie skuteczności i wyników leczenia za pomocą ćwiczeń mięśniowych są zgodne z opiniami specjalistów opisanymi w wielu pracach, to jednak przekonanie o wyższości aparatów oraz sceptycyzm w odniesieniu do ćwiczeń jako metody samodzielnej pozwalają wnioskować, że prawdopodobnie istnieją również inne przyczyny takich opinii respondentów. Studenci uważają, że w trakcie studiów nie zdołali zdobyć wystarczającej wiedzy i w pełni wyrobić sobie opinii na temat ćwiczeń mięśniowych. Wydaje się, że aktualne piśmiennictwo nie poświęca zbyt wiele uwagi ćwiczeniom mięśniowym jako najprostszej metodzie leczenia wad zgryzu. Students claim, that they hadn’t got sufficient knowledge during the studies, to form their own opinion about it. It seems that nowadays literature don’t sacrifice too much attention on muscles exercises. Maybe it’s caused that myotherapy as the simplest method of orthodontic treatment isn’t profitable?
