Pole elektromagnetyczne niskich czêstotliwoœci LF-EMF (Low-Frequency ElectroMagnetic Field), jest w Polsce czêsto stosowan¹ metod¹ fizykoterapii. Dotychczas nie okreœlono na œwiecie standardów czêstotliwoœci, czasu leczenia ani efektów ubocznych, stosowanych pól elektromagnetycznych. Lepsze poznanie LF-EMF powinno byæ oparte na obiektywnych pomiarach fizycznych i badaniach klinicznych zgodnych z aktualnymi standardami badañ naukowych. Materia³ i metody. W pracy okreœlano parametry fizyko-techniczne aplikatorów pola elektromagnetycznego, w szczególnoœci maty cewkowej do terapii ca³ego cia³a (o wymiarach: d³ugoœæ 170 cm, szerokoœæ 70 cm) oraz aplikatora do terapii miejscowej (o wymiarach: d³ugoœæ 7 cm, szerokoœæ 6 cm). Wyznaczono parametry wytwarzanego pola magnetycznego: indukcjê magnetyczn¹, topografiê pola oraz profil czasowy impulsu magnetycznego. Pomiar wartoœci indukcji pola magnetycznego wykonano sond¹ pomiarow¹ wykalibrowan¹ za pomoc¹ magnetometru. Czasowy profil impulsu elektromagnetycznego wyznaczono przy u¿yciu oscyloskopu cyfrowego, do którego pod³¹czono sondê pomiarow¹. Wyniki. Badane pole elektromagnetyczne by³o silnie niejednorodne. Wartoœci indukcji pola magnetycznego badanego urz¹dzenia dla maty cewkowej nie przekroczy³y 300 µT, dla aplikatora miejscowego 4000 µT i szybko zanika³y ze wzrostem odleg³oœci od Ÿród³a pola. Czêstotliwoœæ wynios³a 30 Hz. Kszta³t impulsu okreœlono jako pi³owaty. Wykazano, ¿e zwiêkszanie programu zasilania powoduje liniowy przyrost indukcji magnetycznej. Wnioski. Sposób u³o¿enia cia³a oraz typ zastosowanego aplikatora pola elektromagnetycznego niskich czêstotliwoœci (mata cewkowa lub aplikator miejscowy) maj¹ istotny wp³yw na wielkoœæ dawki pola elektromagnetycznego oddzia³uj¹cego na organizm. SUMMARY is c op y is for Background. The living organism is affected by electromagnetic influences covering a huge range of frequencies and amplitudes. The low-frequency electromagnetic field (LF-EMF) is a popular modality in physical therapy in Poland. As yet no standard frequency or duration of treatment have been determined and no side effects of EMF have been identified. A better understanding of LF-EMF should be based on objective physical measures and evidence-based clinical studies. Material and methods. The physical and technical parameters of the electromagnetic field applicators, in particular a coil mat for whole-body treatment (length 170 cm, width 70 cm) and an applicator for topical use (length 7 cm, width 6 cm) are determined. Besides, the paper reports on determination of the parameters of the generated electromagnetic fields: induction, topography of the field and the temporal profile of the magnetic pulse. Magnetic field induction was measured by a measuring probe calibrated with the use of a magnetometer. The temporal profile of the electromagnetic pulse was determined with a digital oscilloscope to which the probe was connected. Results. The electromagnetic field was very heterogeneous. The value of induction of magnetic field ranged from near zero to 300 µT for the coil mat and to 4000 µT for the topical applicator and fast disappeared with increased distance from the field source. The frequency of EMF was 30 Hz. The pulse shape was saw-toothed. Increased supply program linearly increased magnetic induction. Conclusions. The position of the body during electromagnetic exposure and choice of applicator (coil mat or topical applicator) have a significant effect on the magnitude of electromagnetic exposure. Studies investigating the effect of an electromagnetic field on living organisms target various aspects, using different research methods and different parameters of the electromagnetic field. Thus, the results of these studies are usually inconsistent [1]. Only a few studies [2,3] contain quantitative characterisations of electromagnetic fields used in medicine. The parameters of the fields are not always consistent with the data, often imprecise, declared by the manufacturer of the equipment [3]. Even though electromagnetic field therapy is widely used, knowledge of its mechanism of action, the response of the body, doses used, indications and contraindications still leaves many doubts [4,5,6,7]. It is difficult to quantify the effect of a magnetic field has on humans [8,9]. As a result, in the United States, for instance, Pulsed Electromagnetic Fields (PEMF) are classified as a modality of alternative medicine [10]. Vital processes on Earth are subject to the magnetic field of the Earth, which has the average value of induction of approximately 50 µT. Some diagnostic methods use much stronger magnetic fields, for instance the imaging technique of nuclear magnetic resonance uses magnetic fields ranging from 0.15 to 8 T [11,12]. Furthermore, even though the therapeutic effect of a magnetic field on the human body has not been fully elucidated [4], low-frequency electromagnetic field (LF-EMF) therapy is used in various areas of medicine, in prevention and therapy of many diseases, and is often acclaimed by specialists [13,14,15,16]. The aim of this paper is to determine the physical parameters of the low-frequency electromagnetic field generated by equipment for physical therapy available in Poland. pe rs MATERIA£ I METODY on al us eo nly -d istr ibu tio np roh ibit ed . WSTÊP Terapia polem elektromagnetycznym powinna byæ oparta przede wszystkim na przekonuj¹cych podstawach fizycznych i obiektywnych badaniach klinicznych. Badania naukowe dotycz¹ce oddzia³ywania pola elektromagnetycznego na organizmy ¿ywe prowadzone s¹ w ró¿nych aspektach, wykorzystuj¹c ró¿ne metody badawcze, z zastosowaniem ró¿nych parametrów pola elektromagnetycznego, st¹d wyniki tych badañ s¹ najczêœciej niespójne [1]. Tylko nieliczne publikacje naukowe [2,3] zawieraj¹ iloœciowe charakterystyki stosowanych pól elektromagnetycznych. Parametry generowanych pól nie zawsze s¹ zgodne z danymi producenta aparatury, czêsto nieprecyzyjnymi [3]. Mimo rozpowszechnienia terapii polem elektromagnetycznym, wiedza dotycz¹ca mechanizmu dzia³ania, reakcji organizmu, stosowanych dawek, wskazañ i przeciwwskazañ pozostawia nadal wiele w¹tpliwoœci [4,5,6,7]. Wp³yw pola magnetycznego na cz³owieka trudny jest do iloœciowego okreœlenia [8,9] dlatego na przyk³ad w Stanach Zjednoczonych pulsuj¹ce pola elektromagnetyczne (PEMF) klasyfikowane s¹ jak metoda medycyny alternatywnej [10]. Procesy ¿yciowe na Ziemi podlegaj¹ dzia³aniu ziemskiego pola magnetycznego o œredniej wartoœci indukcji oko³o 50 µT. Znacznie silniejsze pola magnetyczne wykorzystuj¹ niektóre metody diagnostyczne, na przyk³ad magnetyczny rezonans j¹drowy stosowany w obrazowaniu, wykorzystuje pola magnetyczne od 0,15 do 8 T [11,12]. I chocia¿ terapeutyczny wp³yw pola magnetycznego na organizm cz³owieka nie jest dobrze poznany i wyjaœniony [4], to leczenie polem elektromagnetycznym niskiej czêstoœci znajduje zastosowanie w ró¿nych obszarach medycyny, w profilaktyce i terapii wielu schorzeñ, uzyskuj¹c czêsto uznanie specjalistów [13,14,15,16]. Celem niniejszej pracy by³o zmierzenie parametrów fizycznych pola elektromagnetycznego niskiej czêstotliwoœci (wartoœci indukcji i jej rozk³adu), generowanego przez urz¹dzenie do fizykoterapii dostêpne w Polsce. W sk³ad zestawu wchodzi p³aska mata cewkowa, aplikator do terapii miejscowej oraz jednostka steruj¹ca (zasilacz). Producent umieœci³ w macie cewkowej o wymiarach 170 cm x 70 cm – 6 cewek wytwarzaj¹cych pole magnetyczne oraz w aplikatorze miejscowym o wymiarach 6 cm x 7 cm jedn¹ cewkê. W celu zwiêkszenia obiektywnoœci pomiarów, do badañ u¿yto 3 niezale¿ne zestawy. Pomiary wykonywano niezale¿nie dla maty cewkowej oraz dla aplikatora do terapii miejscowej. Sondê do pomiaru indukcji magnetycznej stanowi³a ma³a cewka pomiarowa, któr¹ wyskalowano za pomoc¹ fabrycznego magnetometru TESLAMETR RX-21. Sonda taka pozwala³a okreœliæ uœrednion¹ w danym po³o¿eniu wartoœæ indukcji magnetycznej. The set consists of a flat coil mat, an applicator for topical use and a control unit (power pack). The manufacturer placed six coils generating magnetic field in the coil mat (length 170 cm, width 70 cm) and one coil in the applicator for topical use (length 7 cm, width 6 cm). Three independent sets were used in the study in order to enhance the objectivity of measurements. The measurements were carried out independently for the coil mat and the applicator for topical use. A small measuring coil, calibrated with the use of the manufacturer's TESLAMETR RX-21 magnetometer, was used as a probe to measure magnetic induction. With this probe, it was possible to determine the averaged value of magnetic induction at a given point. It was determined that for induction not exceeding 4 mT the measuring system functioned linearly. tio np roh ibit ibu RESULTS -d istr WYNIKI Magnetic induction for the coil mats ranged from 0 to 300 µT (for the strongest, 10th mode of power supply), with uneven distribution of magnetic field induction on the surface of the mat. The temporal profile of the electromagnetic pulse was determined with a Tektronix TDS 220digital oscilloscope to which the measuring probe was connected. The frequency of the pulse electromagnetic field was also determined in the same way. The pulse shape for the coil mat was determined by defining a topographic grid of 10 x 10 cm squares. For each point in the grid, the value of induction was determined for different power supply modes. a topographic grid consisting of 1 cm x 1 cm squares was designed for the topical applicator. Measurements were carried out by placing the measuring coil on the surface of the mat or the applicator. Before actual measurements were performed, the background electromagnetic field around the coil mat or the applicator was determined. No electromagnetic noises were recorded which could significantly affect the measurements. ed . Dla ka¿dego punktu siatki wyznaczono wartoœæ indukcji pola przy ró¿nych programach zasilania maty. Dla aplikatora miejscowego stworzono siatkê topograficzn¹, sk³adaj¹c¹ siê z kwadratów o bokach 1 cm x 1 cm. Pomiary wykonywano umieszczaj¹c cewkê pomiarow¹ na powierzchni maty lub aplikatora. Przed wykonaniem pomiarów w³aœciwych okreœlono tak¿e t³o pola elektromagnetycznego w otoczeniu maty cewkowej i aplikatora. Zmierzone wartoœci indukcji magnetycznej dla maty cewkowej wynosz¹ od 0 do 300 µT, natomiast dla aplikatora miejscowego od 0 do 4000 µT. Graficzny sposób przedstawienia topografii pola magnetycznego maty cewkowej u³atwia œwiadomy wybór pozycji cia³a, tym bardziej, i¿ wiêkszoœæ pacjentów ma wzrost ni¿szy lub wy¿szy od d³ugoœci maty (170 cm). W macie urz¹dzenia Bemer obszar 30 cm od górnego brzegu jest przeznaczony na po³o¿enie g³owy Biological effects of exposure to electromagnetic fields in humans depend on the parameters of the field, in particular frequency and induction [17,18]. In our study, the values of magnetic induction ranged from 0 to 300 µT for the coil mat and from 0 to 4000 µT for the applicator for topical use. A graphic representation of the magnetic pulse shape for the coil mat helps users to choose a body position, especially since most patients are either taller or shorter than the mat's length (170 cm). The magnetic induction of the field generated by the applicator for topical use did not exceed 4 mT (for the strongest power supply) and its topography is presented on Fig. 2. Measurements of temporal stability of field induction did not show any variation over time. It was also demonstrated that changes in power supply (voltage) programs increased magnetic induction linearly. Fig. 3 presents the temporal profile of magnetic field pulses. The pulse shape was saw-toothed. Registration of the temporal profile made it possible to determine the frequency of the pulsed field, which was about 30 Hz. The distribution of the lines of the magnetic field generated by the applicator for topical use was photographed using iron filings (Fig. 4). Zarejestrowany przebieg czasowy pozwoli³ okreœliæ czêstotliwoœæ impulsowego pola – w naszym przypadku jest to oko³o 30 Hz. Wykorzystuj¹c ¿elazne opi³ki sfotografowano rozk³ad linii pola magnetycznego wytwarzanego przez aplikator miejscowy (Rycina 4). Niestety, wartoœæ indukcji pola wytwarzanego przez matê cewkow¹ jest zbyt ma³a i podobna rejestracja wizualna rozk³adu linii pola magnetycznego dla maty cewkowej nie by³a mo¿liwa. nly Ryc. 4. An increasing number of studies on the therapeutic effects of stmulating the head with electromagnetic field are being published [19,20] and transcranial magnetic stimulation is acknowledged world-wide as a diagnostic method and considered not to be associated with any adverse effects [11,18,21]. There are authors, however, who enumerate possible risks associated with head exposure [20, 22]. For the coil mat the standard body position during the exposure is the supine position as the areas where magnetic field was strongest are in the regions of the humeral, coxofemoral and knee joints. This is probably due to the primary use of the coil mat being therapy of various conditions of the osteoarticular system [1,10,13,23]. It should be pointed out, however, that the mat may be used by patients who, for different reasons (e.g. pain, respiratory disorders), need to assume other positions for the treatment. The measurements also demonstrated that the electromagnetic field fast disappeared above the surfaces of the mat and the applicator. Thus, it may be concluded that tissues located close to the source of the field are exposed to LF-EMF. This may be of crucial importance e.g. for the distribution of flowing erythrocytes [24] and their metabolism [25]. The measurements also demonstrated a markedly heterogenous distribution of the magnetic field on the surface of the applicator for topical use. -d istr ibu tio np roh ibit ed . – pole elektromagnetyczne jest w tym obszarze niemierzalne. Jest coraz wiêcej publikacji na temat leczniczych efektów stymulacji polem elektromagnetycznym g³owy [19,20] natomiast uznana na œwiecie metoda diagnostyczna – przezczaszkowa stymulacja magnetyczna (ang. transcranial magnetic stimulation) jest uwa¿ana za pozbawion¹ dzia³añ ubocznych [11,18,21]. S¹ jednak autorzy, którzy wymieniaj¹ mo¿liwe zagro¿enia zwi¹zane z ekspozycj¹ g³owy [20, 22]. Dla maty cewkowej standardowym u³o¿eniem cia³a w trakcie zabiegu jest le¿enie ty³em – miejsca, w których zmierzone pole magnetyczne by³o najsilniejsze, znajduj¹ siê w okolicy stawów barkowych, biodrowych i kolanowych. Wynika to zapewne z podstawowego przeznaczenia maty cewkowej do terapii ró¿nego rodzaju patologii uk³adu kostno-stawowego [1,10,13,23]. Nale¿y jednak zwróciæ uwagê na mo¿liwoœæ stosowania maty przez chorych, którzy z ró¿nych przyczyn (np. ból, zaburzenia oddychania) korzystaj¹ z zabiegu w innych pozycjach. Pomiary wykaza³y tak¿e du¿y zanik pola elektromagnetycznego nad powierzchni¹ maty i aplikatora, mo¿na wiêc wnioskowaæ, i¿ oddzia³ywaniu LF-EMF poddane s¹ tkanki po³o¿one blisko Ÿród³a pola. Mo¿e to mieæ decyduj¹cy wp³yw np. na dystrybucjê przep³ywaj¹cych erytrocytów [24] oraz ich metabolizm [25]. Pomiary wykaza³y tak¿e silnie niejednorodny rozk³ad pola magnetycznego na powierzchni aplikatora do terapii miejscowej. nly WNIOSKI CONCLUSIONS eo 1. The coil mat of the Bemer system produces a magnetic field of a markedly heterogenous distribution, so the patient's position during exposure to electromagnetic field significantly affects the dose of LF-EMF received. 2.
