Mgr Monika Dragunajtys - Sudoł Streszczenie pracy doktorskiej „Czynniki osobowości jako predyktory stylów radzenia sobie ze stresem oraz psychologicznych efektów rehabilitacji w chorobach układu krążenia i narządu ruchu” Problem badawczy Podstawowym celem rozprawy jest poszukiwanie i ocena związków między wybranymi zmiennymi psychologicznymi a efektami rehabilitacji osób chorych somatycznie. Ważnym obszarem eksploracji jest poszukiwanie odpowiedzi na pytania o wpływ osobowości na prezentowane przez chorych style radzenia sobie ze stresem. Zasadnicza teza rozprawy zakłada występowanie zmiennych osobowościowych mających istotny wpływ na radzenie sobie ze stresem choroby oraz z efektami rehabilitacji. Zakłada się również, że czynniki osobowościowe determinujące zachowanie człowieka w sytuacji stresującej mogą też wpływać na zaangażowanie w działania prozdrowotne. Jednym z takich działań jest uczestniczenie w programach rehabilitacyjnych, a udział w nich pośrednio decyduje o psychologicznych efektach leczenia. Jednocześnie literatura przedmiotu oraz badania empiryczne są przesłanką do poszukiwania specyfiki funkcjonowania psychicznego dotkniętych różnymi chorobami somatycznymi także w zakresie psychologicznych konsekwencji choroby i odpowiedzi na leczenie. Przeprowadzone na potrzeby rozprawy badania ukierunkowane są na porównanie dwóch grup chorych, a mianowicie osób ze schorzeniami układu sercowo-naczyniowego i narządu ruchu. Zasadnicza różnica między tymi rodzajami chorób wynika z ich specyfiki (m.in. charakter towarzyszących dolegliwości). Różnią się one także rodzajem i nasileniem wpływu czynników psychologicznych w ich etiologii. W niektórych schorzeniach narządu ruchu wpływ czynników psychologicznych w etiologii nie jest uwzględniany lub uznawany za mało istotny, natomiast w schorzeniach kardiologicznych podkreśla się wieloczynnikową etiologię, ze szczególnym uwzględnieniem czynników psychicznych. Założenia psychosomatyki wskazują również na konieczność uwzględnienia zależności somatopsychicznych, a więc na psychologiczne następstwa choroby somatycznej, co stanowi istotę niniejszej rozprawy. Innowacyjność dysertacji Podjęta w rozprawie tematyka była w pewnym zakresie eksplorowana, jednak niewiele jest aktualnych badań z tego zakresu. Wydaje się ważną sprawą uwzględnienie zmian, jakie wraz ze zmianami społeczno – gospodarczymi występują w sferze postaw wobec problemów zdrowotnych, nowych możliwości, stopnia dostępności i zróżnicowanych form terapii osób z chorobami somatycznymi. Poznanie zmiennych osobowościowych, m.in. struktury potrzeb osób badanych, stylu radzenia sobie ze stresem, może okazać się przydatne w dostosowaniu programów terapeutycznych do poszczególnych grup chorych. Kolejnym aspektem tak sformułowanego problemu badawczego jest eksploracja dynamiki zmian wskaźników psychologicznych efektów krótkotrwałej rehabilitacji. Ze względów finansowych i społecznych ograniczenie czasowe rehabilitacji, jeśli niesie za sobą pozytywne efekty, może stanowić dużą wartość. Materiał i metody Zmiennymi zależnymi w proponowanym modelu badawczym są style radzenia sobie ze stresem oraz psychologiczne efekty rehabilitacji. Zmienną niezależną w przyjętym modelu badań są cechy osobowości. Zastosowano następujące narzędzia badawcze: - Kwestionariusz Radzenia Sobie w Sytuacjach Stresowych CISS (Coping Inventory for Stressful Situations) autorstwa N. S. Endlera i J. D. A. Parkera, - skalę SOPER (Skala Oceny Psychologicznej Efektywności Rehabilitacji) autorstwa J. Tylki i M. Makowskiej - Test Przymiotnikowy ACL (The Adjective Check List) skonstruowany przez G. H. Gougha i A. B. Heilburna w opracowaniu Z. Płużek. Dwukrotny pomiar wskaźników psychologicznych efektywności rehabilitacji – na początku i na zakończenie turnusu rehabilitacyjnego - posłużył określeniu zmian zachodzących podczas oddziaływań rehabilitacyjnych. Dodatkowo osoby badane wypełniały ankietę osobową, która dostarczyła danych demograficznych (wiek, płeć) oraz informacji na temat czasu leczenia z powodu danego schorzenia i intensywności podejmowanej rehabilitacji (mierzonej liczbą programów rehabilitacji stacjonarnej). Badanymi (łącznie 385) były osoby ze schorzeniami układu sercowo – naczyniowego (118) i narządu ruchu (267) uczestniczące w 24 dniowych programach rehabilitacji stacjonarnej w sanatorium uzdrowiskowym i szpitalu sanatoryjnym. Osoby uczestniczące w kompleksowych programach rehabilitacji kardiologicznej i ortopedycznej były dla siebie wzajemnie grupami odniesienia. Udział w badaniu był dobrowolny. Badania odbywały się w dwóch etapach. W drugim dniu pobytu osoby badane wypełniały ankietę osobową oraz kwestionariusze psychologiczne: ACL (w wersji ”Jaki jestem?”), CISS i SOPER. W 22 lub 23 dniu pobytu badani ponownie wypełniali skalę SOPER. Wyniki Na podstawie przeprowadzonych badań otrzymano m.in. następujące wyniki dotyczące funkcjonowania psychicznego chorych z badanych grup: 1. Badane grupy osób ze schorzeniami kardiologicznymi i ortopedycznymi znacząco nie różnią się pod względem cech osobowości. Istotne różnice zaznaczyły się w 2 spośród 37 badanych cech. Odrębności te były większe po uwzględnieniu czynników podmiotowych i związanych z leczeniem. 2. Chorzy kardiologicznie i ortopedycznie prezentują odmienne style radzenia sobie ze stresem. Osoby ze schorzeniami układu sercowo-naczyniowego częściej unikają konfrontacji z sytuacją trudną, podczas gdy osoby ze schorzeniami narządu ruchu częściej koncentrują się na działaniach zmierzających do zmiany sytuacji lub swojego w niej położenia. 3. Uczestnictwo w kompleksowym programie rehabilitacji kardiologicznej wiązało się ze zmniejszeniem poziomu lęku oraz zmianą na bardziej negatywne postaw wobec terapii i wobec celów życiowych. Osoby uczestniczące w kompleksowej stacjonarnej rehabilitacji ortopedycznej na jej zakończenie odczuwały mniejsze napięcie psychiczne i poprawę nastroju. Ponadto wyniki wskazują na związek czynników osobowości i stylów radzenia sobie ze stresem z psychologicznymi efektami rehabilitacji: 4. Obserwuje się negatywny wpływ skłonności do uległości w relacjach i poszukiwania wsparcia oraz nadmiernego samokrytycyzmu i braku wiary w siebie na psychologiczne efekty rehabilitacji. 5. Efekty rehabilitacji zależą od stylu radzenia sobie skoncentrowanego na zadaniu. 6. Wiek osób oraz czas leczenia wpływają istotnie na efekty psychologiczne rehabilitacji – im osoby są młodsze oraz im wcześniej podejmują działania rehabilitacyjne, tym większe są ich efekty psychologiczne. Konkluzje Na podstawie uzyskanych wyników badań można sformułować wnioski dotyczące funkcjonowania psychicznego osób z dwoma rodzajami chorób somatycznych. Istotne różnice między chorymi kardiologicznie i ortopedycznie uwidocznione zostały zwłaszcza w stylach radzenia sobie ze stresem oraz w psychologicznych efektach rehabilitacji. Określono również związek czynników osobowości i stylów radzenia sobie ze stresem, który w grupach badanych chorych prezentował się odmiennie wskazując na odmienne mechanizmy regulujące. W czasie krótkotrwałej kompleksowej rehabilitacji kardiologicznej i ortopedycznej zaobserwowano zmiany czynników wyznaczających jej efektywność. Ustalono ponadto, że im silniejsza tendencja do skupiania się na emocjach osób chorych, tym gorsze są efekty psychologiczne rehabilitacji. Koncentrowanie się na podejmowaniu działań zmierzających do rozwiązania problemu lub poznawcze przekształcenia mają natomiast pozytywny wpływ na efekty rehabilitacji. Istotnie, choć w mniejszym stopniu, na efektywność rehabilitacji wpływają cechy osobowości związane z Uległością i Unikaniem, a także czas leczenia i wiek osób chorych. Im większa tendencja do wchodzenia w podporządkowane role i unikania nowych sytuacji oraz im większy samokrytycyzm i brak wiary w siebie, tym bardziej można spodziewać się mniejszych psychologicznych efektów rehabilitacji. Podobne zależności dotyczą wieku i czasu leczenia – efekty rehabilitacji są mniejsze, im osoby uczestniczące w programie rehabilitacji są starsze i im dłużej się leczą. Wyniki przeprowadzonych badań mogą znaleźć zastosowanie w praktyce rehabilitacji chorób somatycznych. Na krótkoterminową rehabilitację stacjonarną, aby przynosiła jak najwięcej korzyści, powinny być kierowane zwłaszcza osoby młodsze i jak najwcześniej po powstaniu lub rozpoznaniu choroby. Programy kompleksowej rehabilitacji chorób somatycznych powinny uwzględniać również oddziaływania psychologiczne m.in. wzmacniające wiarę w siebie, asertywność oraz budujące poczucie własnej skuteczności w podejmowaniu działań, koncentrujące się na poszukiwaniu rozwiązań. Słowa kluczowe Rehabilitacja kardiologiczna, rehabilitacja ortopedyczna, psychologiczne efekty rehabilitacji, radzenie sobie ze stresem choroby Abstract of the dissertation Factors of personality as predictors styles of coping with stress and psychological effects of rehabilitation in somatic diseases Research problem The primary objective of the trial is to seek and assess the relationships between selected psychological variables and the effects of rehabilitation in patients with somatic diseases. An important area of exploration is to seek answers to questions about the impact of personality on presented by patients styles of coping. The main thesis of the trial implies the existence of personality variables that have a significant impact on coping with the effects of disease and rehabilitation. It is also assumed that personality factors determining human behavior in stressful situations can also affect involvement in health-related activities. One of these actions is to participate in rehabilitation programs, and participation in them indirectly determines the psychological effects of treatment. At the same time literature and empirical studies are a reason to seek the specifics of mental functioning affected by a variety of somatic diseases also in terms of the psychological consequences of the disease and response to treatment. Carried out for the purpose of dissertation, tests are aimed at comparing the two groups of patients, namely those with diseases of the cardiovascular system and the musculoskeletal system. The main difference between these types of diseases is due to their specifics (including the nature of accompanying symptoms). They also vary in type and severity of the impact of psychological factors in their etiology. In some diseases of the musculoskeletal system impact of psychological factors in the etiology is not included or considered to be negligible, whereas in cardiac diseases emphasizes the multifactorial etiology, with particular emphasis on psychological factors. Assumptions of psychosomatic also point to the need to consider somatopsychic relationships, and so on the psychological consequences of somatic disease, which is the essence of this dissertation. Innovation of dissertation Theme taken in the dissertation was explored to some extent, but there is little current research in this field. It seems important to take into account changes that along with changes of socio economic occur in the domain of attitudes towards health problems, new opportunities, the degree of accessibility and diverse forms of therapy of somatic diseases. Knowing the personality variables, including the structure of the needs of subjects, styles of coping with stress, it may be useful in adjusting treatment programs for specific groups of patients. Another aspect of the so-formulated research problem is to explore the dynamics of changes in indicators of the psychological effects of short-term rehabilitation. Limiting the time of rehabilitation due to financial and social aspects, if it also brings positive effects, can be a great value. Research tools The dependent variables in the proposed research model are coping with stress and the psychological effects of rehabilitation. The independent variable in the accepted research model are personality traits. The following research tools were used: - Coping Inventory for Stressful Situations by N. S. Endler and J. D. A. Parker, - Rating Scale for Effectiveness of Psychological Rehabilitation by J. Tylka and M. Makowska - ACL (The Adjective Check List) created by G. H. Gough and A. B. Heilburn in the adaptation of Z. Płużek. Double measurement of performance indicators of psychological rehabilitation - at the beginning and at the end of the rehabilitation period - helped define the changes during the rehabilitation process. In addition, the subjects filled personal questionnaire, which provided demographic data (age, gender), and information on the time of disease treatment and the intensity of the rehabilitation undertaken. Subject to the test (total 385) were people with cardio - vascular diseases (118) and musculoskeletal diseases (267) participating in the 24 day rehabilitation programs held in the sanatorium and spa sanatorium hospital. Those involved in comprehensive programs of cardio - vascular and orthopedic rehabilitation were reference groups for each other. Participation in the survey was voluntary. Studies conducted in two stages. On the second day of stay, examined persons filled in a personal questionnaire and psychological questionnaires: ACL (in the version of "What am I?"), CISS and SOPER. In the 22 or 23 day stay subjects again filled SOPER scale. Results Based on the survey, among others, the following results concerning the mental functioning of the patients of the treatment groups were received: 1. The studied group of patients suffering from cardiac and orthopedic did not significantly differ in terms of personality traits. Significant differences were marked in 2 out of 37 studied traits. Differences were higher after taking into account subjective factors and treatment-related factors. 2. Cardiac and orthopedic patients present different styles of coping with stress. People with diseases of the cardiovascular system frequently avoid confrontation with a difficult situation, while people with diseases of the musculoskeletal system often focus on efforts to change the situation or his position in it. 3. Participation in the comprehensive cardiac rehabilitation program was associated with a reduction in anxiety levels and a change to a more negative attitudes towards treatment and life goals. Those involved in orthopedic rehabilitation at its end had lower mental stress and improvement of the mood. Furthermore, the results indicate a relationship between personality factors and styles of coping with the stress of the psychological effects of rehabilitation: 1. There has been a negative impact in the tendency to submission in relationships and seeking support and excessive self-criticism and lack of confidence on the psychological effects of rehabilitation. 2. The effects of rehabilitation depend on the style of coping focused on the task. 3. Age of patients and the duration of treatment significantly affect the psychological effects of rehabilitation - the people are younger, and the sooner they take rehabilitation, the greater their psychological effects. Conclusion The results of research can draw conclusions concerning the mental functioning of people with two types of somatic diseases. Significant differences between cardiac and orthopedic patients have been shown especially in the styles of coping with stress and psychological effects of rehabilitation. Also determined the relationship between personality factors and styles of coping with stress, which groups of patients presented differently pointing to the different regulatory mechanisms. During the short-term comprehensive cardio and orthopedic rehabilitation observed changes in factors determining its effectiveness. It was further found that the stronger the tendency to focus on the emotions of patients, the worse the effects of psychological rehabilitation became. Focusing on making efforts to solve the problem or cognitive transformations have a positive impact on the effects of rehabilitation. Significantly, but to a lesser extent, personality traits associated with Submission and Avoidance impact the effectiveness of rehabilitation, as well as the duration of treatment and the age of patients. The greater the tendency to enter in subordinate roles and avoidance of the new situation and the greater self-criticism and lack of self - confidence, the more you can expect to lower the psychological effects of rehabilitation. Similar relationships apply to age and time of treatment - the effects of rehabilitation are weaker, the persons participating in the rehabilitation program are older and take longer to cure. Results of this study can be used in the practice of rehabilitation of somatic diseases. For shortterm stationary rehabilitation to bring the most benefits there should be directed especially younger people, and as soon as possible after the establishment or diagnosis of disease. Comprehensive rehabilitation programs of somatic diseases should also take into account the psychological impact among enhancing self-confidence, assertiveness and building selfefficacy in taking action, focusing on finding solutions.
