Celem pracy jest kompleksowa ocena uwarunkowañ z³amañ koœci u dzieci z uwzglêdnieniem wskaŸników badania densytometrycznego i metabolizmu kostnego. Materia³ i metody. Zbadano 112 dzieci w wieku 5-18 lat, w tym 81 pacjentów z wielokrotnymi z³amaniami koœci w wywiadzie oraz 31 dzieci uznanych za zdrowe, które zakwalifikowano do grupy porównawczej. U ka¿dego dziecka przeprowadzono badanie densytometryczne metod¹ DXA (aparatem DPX-L firmy Lunar) w projekcji total body i spine. Oznaczono wapñ, fosfor, magnez (w surowicy i dobowej zbiórce moczu), parathormon, metabolit w¹trobowy witaminy D, osteokalcynê, izoenzym kostny fosfatazy zasadowej oraz usieciowany N-koñcowy telopeptyd kolagenu typu I. Wyniki. Œrednie wartoœci Z-score parametrów badania densytometrycznego, zarówno w programie total body, jak i spine, by³y istotnie statystycznie ni¿sze wœród dzieci z wielokrotnymi z³amaniami, w odniesieniu do tych z grupy porównawczej. U dzieci z wielokrotnymi z³amaniami stwierdzono wiêksz¹ czêstoœæ wystêpowania hiperkalciurii, hipermagnezurii i hiperfosfaturii w odniesieniu do grupy porównawczej. U 20/81 (24,7%) pacjentów z tej grupy wykazano obni¿one stê¿enie metabolitu w¹trobowego witaminy D, zaœ w grupie porównawczej u 6/31. Ponadto stwierdzono podwy¿szone stê¿enie NTx u 38/75 (50,7%) pacjentów, zwiêkszon¹ aktywnoœæ izoenzymu kostnego fosfatazy alkalicznej u 29 oraz osteokalcyny u 12 badanych ze z³amaniami. W tej grupie stwierdzono te¿ istotne, ujemne zale¿noœci pomiêdzy biochemicznymi wskaŸnikami koœciotworzenia i resorpcji koœci a mas¹ kostn¹, a tak¿e ni¿sze wartoœci DXA Z-score u dzieci z wy¿sz¹ kalciuri¹. Wnioski. 1. Obni¿enie gêstoœci mineralnej szkieletu by³o najczêœciej wystêpuj¹cym czynnikiem predysponuj¹cym do z³amañ koœci u dzieci; wykazano je u oko³o 2/3 pacjentów z wielokrotnymi z³amaniami. 2. Przyspieszony obrót kostny, a zw³aszcza nasilona resorpcja tkanki kostnej, wskazuje na zaburzenia metabolizmu kostnego u dzieci ze z³amaniami. 3. The aim of the study was to carry out a comprehensive analysis of determinants of multiple bone fractures in children with regard to densitometric indices and markers of bone metabolism. Material and methods. The study involved 112 children aged 5-18 years, including 81 patients with a history of at least 3 bone fractures and 31 healthy patients in a control group. Total body and spinal DXA densitometry of the skeleton (DPX-L apparatus, Lunar) was carried out in all children. Laboratory assays comprised the determination of calcium, phosphorus, magnesium (in the serum and 24-hour urine collection), parathormone, liver metabolite of vitamin D, osteocalcin, bone alkaline phosphatase, and N-terminal cross-linked telopeptide of collagen type I (NTx). Results. Mean values of DXA Z-score, both in total body and in spinal scans, were significantly lower in children with multiple fractures as compared to controls. In children with multiple fractures, there was a higher prevalence of hypercalciuria, hypermagnesuria and hyperphosphaturia. Decreased levels of the liver metabolite of vitamin D were observed in 20/81 (24.7%) patients in this group and in 6/31 controls. Other findings included a higher level of NTx in 38/75 (50.7%) patients with fractures, an increased activity of bone alkaline phosphatase in 29, and of osteocalcin in 12 patients. In this group, there was a significant negative correlation between biochemical bone turnover markers and low bone mass. Also, lower DXA Z-scores were found in children with higher urinary calcium excretion. Conclusions. 1. Decreased bone mineral density was the most frequent risk factor for bone fractures in children; it was found in about 2/3 of the patients with multiple bone fractures. 2. Accelerated bone turnover, and, particularly, increased bone resorption, indicates a derangement of bone metabolism in children with multiple fractures. 3. Repeated fractures during the body growth period are an indication for a quantitative evaluation of bone mass, calcium-phosphate metabolism and bone turnover markers. Zwykle przyjmuje siê, i¿ s¹ one wynikiem zwiêkszonej urazowoœci w populacji wieku rozwojowego i st¹d nie s¹ analizowane jako objaw zaburzeñ mineralizacji i struktury koœæca. Tymczasem wyniki dotychczas prowadzonych badañ w³asnych, jak te¿ obserwacje innych autorów œwiadcz¹ o tym, i¿ niektóre typy z³amañ u dzieci, a szczególnie z³amania wielokrotne oraz z³amania o charakterze patologicznym, mog¹ byæ nastêpstwem deficytu masy kostnej, czêsto o trudnej do ustalenia etiologii. Szacuje siê, ¿e oko³o 42% ch³opców i 27% dziewczynek do 16 roku ¿ycia doznaje z³amañ, a szczyt czêstoœci przypada u dziewcz¹t pomiêdzy 11 a 12 rokiem ¿ycia oraz u ch³opców pomiêdzy 13 a 14 rokiem ¿ycia, a wiêc w okresie pokwitania [3,4,5]. Z³amania stanowi¹ zatem istotny problem wieku rozwojowego, zarówno z powodu czêstoœci ich wystêpowania, a tak¿e towarzysz¹cych im objawów, takich jak ból czy czasowe upoœledzenie sprawnoœci ruchowej, a tak¿e pogorszenie jakoœci ¿ycia [4]. W piœmiennictwie rzadko spotyka siê doniesienia na temat stanu masy kostnej u dzieci i m³odzie¿y ze z³amaniami; równie¿ nieliczne s¹ u nich opracowania dotycz¹ce wskaŸników gospodarki wapniowo-fosforanowej i markerów obrotu kostnego. Dlatego podjêliœmy badania, których celem jest analiza uwarunkowañ wielokrotnych z³amañ koœci u dzieci, ze szczególnym uwzglêdnieniem oceny densytometrycznej i biochemicznej metabolizmu kostnego tych pacjentów. The incidence of fractures in children and adolescents has been increasing in recent years (1, 2, 3). It is usually assumed that fractures are the result of an increased incidence of injuries in the population of children and adolescents and therefore they are not analysed as a sign of bone mineralization or structural disorder. However, the results of our previous investigations and studies by other authors prove that some fracture types in children, especially multiple and pathological fractures, may occur due to a bone mass deficit, often of an obscure aetiology. It is estimated that approximately 42% of boys and 27% of girls under 16 years of age experience fractures with peak incidence between 11 and 12 years in girls and between 13 and 14 in boys, i.e. in the puberty period [3,4,5]. Therefore, fractures constitute a significant problem in childhood and adolescence, both because of their high incidence and the accompanying signs and symptoms such as pain or temporary impairment of mobility and deterioration of the quality of life [4]. Professional literature contains few reports concerned with bone mass status in children and adolescents with fractures, neither are there many studies concerning the indices of calcium-phosphate metabolism and bone turnover markers in this patient population. Therefore, we have conducted a study whose aim was to review the determinants of multiple fractures in children, with special regard to densitometric and biochemical assessment of bone metabolism in those patients. on MATERIA£ I METODY al us eo nly -d istr ibu tio np roh ibit ed . WSTÊP Th is c op y is for pe rs Badaniami objêto ³¹cznie 112 dzieci w wieku 5,217,8 lat, wœród których by³o 37 dziewczynek i 75 ch³opców. Grupê badan¹ stanowi³o 81 pacjentów z co najmniej 3 z³amaniami koœci d³ugich w wywiadzie (u 3 tak¿e krêgów) leczonych z tego powodu w przedziale czasowym objêtym badaniem. Natomiast 31 dzieci, uznanych za zdrowe, bez objawów klinicznych ze strony uk³adu miêœniowo-szkieletowego, u których nie wystêpowa³y z³amania lub wyst¹pi³o jedno z³amanie koœci zwi¹zane z silnym urazem (w wywiadzie) zakwalifikowano do grupy porównawczej. U ka¿dego dziecka przeprowadzono: • badanie ankietowe uwzglêdniaj¹ce miêdzy innymi okolicznoœci, w jakich dosz³o do wyst¹pienia z³amania, choroby przebyte, stosowanie leków wp³ywaj¹cych na metabolizm kostny; • badanie lekarskie ze szczególnym uwzglêdnieniem nieprawid³owoœci w zakresie narz¹dów ruchu i podstawowych pomiarów somatycznych; MATERIAL AND METHODS The study enrolled a total of 112 children aged 5.2-17.8 (37 girls and 75 boys). The study group consisted of 81 patients with a history of at least 3 fractures of long bones (also vertebrae in 3 patients) treated within the time range studied. The control group consisted of 31 children diagnosed as healthy with no signs of musculoskeletal disorders who had no history of fractures or a history of one fracture associated with a forceful trauma. The following work-up was conducted for each participants: • a questionnaire concerning, among others, the circumstances of the fractures, history of past illnesses, and use of drugs which may influence bone mineral metabolism; • a physical examination with special regard to abnormalities within the musculoskeletal system and basic somatic measurements; - This copy is for personal use only - distribution prohibited. - This copy is for personal use only - distribution prohibited. - This copy is for personal use only - distribution prohibited. - This copy is for personal use only - distribution prohibited. - Micha³us I., Ocena gêstoœci mineralnej koœci u dzieci z wielokrotnymi z³amaniami koœci 603 • total body and spinal densitometric scans using dual-energy X-ray absorptiometry (DXA); densitometric indices included a Z-score (the number of standard deviations from the mean for age and gender); in case of somatic deficiencies the result was standardized to the current biological age; • an ultrasound examination of the calcaneus (Achilles Plus Solo, Lunar) during which we analysed absolute and relative (Z-score) values of parameters such as: the speed of sound (SOS), broadband ultrasound attentuation (BUA) and a Stiffness index, calculated automatically; • determination of biochemical indices of calciumphosphate metabolism and bone turnover markers: routine determinations were carried out of the concentration of calcium, phosphorus, magnesium (in the serum and 24-hour urine collection), parathormone (PTH) in the serum by chemiluminescent method, liver metabolite of vitamin D (25OHD) in the serum by a radio immuno assay, osteocalcin (OC) and bone alkaline phosphatase (markers of bone formation) in the serum by an ELISA assay, and the N-terminal cross-linked telopeptide of collagen type I (NTx, a bone resorption marker) by immunochemiluminescent method in the urine. Depending on indications, various additional tests were performed in order to find the cause of low bone mass. Statistical analysis was carried out using Statistica 6.0 PL software. Spearman's correlation coefficient was used to assess relations between variables. The level of statistical significance was set at p<0.05. DXA) w projekcji total body i spine; w ocenie wyników tego badania uwzglêdniono wskaŸnik Z-score (liczbê odchyleñ standardowych od œredniej dla wieku i p³ci); w przypadku stwierdzenia niedoborów somatycznych wynik normalizowano do aktualnego wieku biologicznego. • badanie ultradŸwiêkowe koœci piêtowej z wykorzystaniem aparatu Achilless solo Plus firmy Lunar, w którym analizowano bezwzglêdne i wzglêdne (Z-score) wartoœci parametrów: prêdkoœæ przechodzenia ultradŸwiêków (Speed of Sound – SOS), szerokopasmowe t³umienie ultradŸwiêków (Broadband Ultrasound Attenuation – BUA) i wyliczany automatycznie wskaŸnik Stiffness; • badania biochemiczne wskaŸników gospodarki wapniowo-fosforanowej oraz markerów obrotu kostnego: oznaczono (w sposób ogólnie przyjêty) stê¿enie wapnia, fosforu, magnezu (w surowicy i w dobowej zbiórce moczu – DZM), parathormonu – PTH (w surowicy) metod¹ chemiluminescencyjn¹, metabolitu w¹trobowego witaminy D – 25OHD (w surowicy) metod¹ radioimmunokompetycyjn¹, oraz metod¹ Elisa osteokalcyny (OC) i izoenzymu kostnego fosfatazy zasadowej (IzkFA) – markery koœciotworzenia (w surowicy) oraz usieciowanego N-koñcowego telopeptydu kolagenu typu I – NTx (w moczu) metod¹ immunochemiluminescencji – marker resorpcji koœci. W zale¿noœci od wskazañ, celem wyjaœnienia przyczyny niskiej masy koœci, wykonywano te¿ inne badania dodatkowe. Analizê statystyczn¹ przeprowadzono za pomoc¹ programu Statistica 6.0. W ocenie zale¿noœci miêdzy cechami zastosowano wspó³czynnik korelacji Spearmana. Zarówno w pierwotnych, jak i wtórnych („objawowych”) z³amaniach, czynnikiem wspó³istniej¹cym i prawdopodobnie bezpoœredni¹ przyczyn¹ by³a osteoporoza, któr¹ stwierdzono u 23 dzieci lub osteopenia – u 29. In the remaining 26 patients the occurrence of fractures was secondary to a chronic medical condition. In both primary and secondary ("symptomatic") fractures, a coexisting factor and probably the direct cause was osteoporosis, diagnosed in 23 children, or osteopaenia, seen in 29 children. In 29 out of the 81 patients, however, BMD, and thus quantitative assessment of bone mass, was within normal limits. Number of children with spontaneous fractures and fractures associated with chronic illnesses (n=112) - This copy is for personal use only - distribution prohibited. ing 11 it accompanied chronic diseases such as osteogenesis imperfecta (2 children), and single cases of infantile cerebral palsy, epilepsy, hyperparathyroidism, type 1 diabetes mellitus, coeliac disease, Crohn's disease, hepatitis C, lipoblastoma, and bronchial asthma. Among the 29 children with osteopaenia, 23 had primary causes of low BMD, and 6 had secondary osteopaenia, with causes including bronchial asthma (3 children) and single cases of allergy to cow's milk proteins, generalized bone dysplasia, and chronic renal disease. 10 out of 26 patients had been treated with glycocorticosteroid drugs (inhaled and/or systemic). Figure 1 presents the number of children with primary and chronic disease-related fractures vs. BMD. There were 384 fractures in the study group, most of which occurred in the upper limbs, and especially in the forearm and finger bones. Fracture sites and numbers are presented in Table 1. Only 13 fractures were suffered in the control group, nine of which were in the forearm and were associated with forceful traumas. The majority of children in the study group had been treated conservatively with a plaster cast. Fracture healing was uneventful, with prolonged healing in 4 patients. Complications included reduced muscle strength in 9 children and gait disorders in 5 (in one case it was associated with a shorter lower limb). In the control group, 23/31 (74.2%) children had normal skeletal BMD. The others, however, had decreased BMD, the cause of which could not be found. The values of Z-score, both in total body and in spinal scans, were significantly lower in children with multiple fractures as compared to controls (see Table 2). A major decrease in the value of at least one parameter of the calcaneus ultrasound examination us przewlek³ych, takich jak: wrodzona ³amliwoœæ koœci – 2 dzieci, mózgowe pora¿enie dzieciêce, padaczka, nadczynnoœæ przytarczyc, cukrzyca typu I, celiakia, choroba Leœniowskiego-Crohn'a, wirusowe zapalenie w¹troby typu C, lipoblastoma, astma oskrzelowa, które dotyczy³y pojedynczych pacjentów. Natomiast wœród 29 pozosta³ych dzieci, u których ustalono rozpoznanie osteopenii, u 23/29 zaburzenia gêstoœci mineralnej mia³y charakter pierwotny, a u 6 – wtórny w przebiegu: astmy oskrzelowej (3 dzieci) oraz alergii na bia³ka mleka krowiego, dysplazji uogólnionej koœci, przewlek³ej choroby nerek (po jednym dziecku). U 10/26 pacjentów stosowano leczenie glikokortykosteroidami (wziewnymi i/lub systemowymi). Liczbê dzieci ze z³amaniami pierwotnymi i w przebiegu chorób przewlek³ych, w odniesieniu do stanu gêstoœci mineralnej (BMD), przedstawiono na Rycinie 1. W grupie badanej odnotowano 384 z³amania. Najczêœciej dotyczy³y one koñczyn górnych, a szczególnie koœci przedramienia i koœci palców rêki. Lokalizacjê i liczbê z³amañ przedstawia Tabela 1. W grupie kontrolnej pacjenci doznali tylko 13 z³amañ, w tym 9 dotyczy³a koœci przedramienia i zwi¹zana by³a z silnymi urazami. U wiêkszoœci dzieci z grupy badanej zastosowano leczenie zachowawcze – opatrunek gipsowy. Gojenie z³amañ przebiega³o prawid³owo, jedynie u 4 pacjentów by³o przed³u¿one. Wœród powik³añ u 9 dzieci stwierdzono os³abienie si³y miêœniowej, u 5 zaburzenia chodu – w jednym przypadku zwi¹zane ze skróceniem koñczyny dolnej. W grupie porównawczej 23/31 (74,2%) dzieci charakteryzowa³o siê prawid³ow¹ gêstoœci¹ mineraln¹ szkieletu, u pozosta³ych jednak by³a ona obni¿ona (o niewyjaœnionej przyczynie). Znacznie obni¿one wartoœci przynajmniej jednego z parametrów badania ultradŸwiêkowego koœci piêtowej (Z-score poni¿ej – 2,0 liczby odchyleñ standardowych) wykazano u 26/81 (32,1%) pacjentów z grupy badanej i jedynie u 5/31 (16%) z grupy porównawczej. Wartoœci œrednie i ró¿nice miêdzy grupami zamieszczono w Tabeli 3. Spoœród nieprawid³owoœci dotycz¹cych wskaŸników gospodarki wapniowo-fosforanowo-magnezowej w grupie dzieci ze z³amaniami stwierdzono m. in.: hipomagnezemiê u 13; hiperfosfatemiê u 8; hipermagnezuriê u 35; hiperkalciuriê u 10, a zmniejszone wydalanie jonów wapnia z moczem u 26. Mean values and differences between the groups can be found in Table 3. The following abnormalities of calcium-phosphatemagnesium metabolism indices were seen in the group of children with fractures: hypomagnesaemia (13 children), hyperphosphataemia (8), hypermagnesuria (35), hypercalciuria (10), and reduced urinary calcium ion excretion (26). Low PTH levels were found only in 4 patients in the study group, and slightly increased PTH in one patient in the study group and one in the control group. Bone turnover markers were determined in 75 children in the study group and 31 of the controls. Abnormal findings included increased activity of bone alkaline phosphatase in 29/75 (38.7%) patients and of osteocalcin in 12 patients (16%) and elevated NTx in 38 (50.7%) subjects. We did not demonstrate any statistically significant differences in mean levels of calcium-phosphate-magnesium metabolism indices or bone turnover markers between the groups. An analysis of correlations between biochemical indices and BMD assessed with the DXA method in both groups demonstrated statistically significant negative correlations between BMD and 24-hour urinary phosphorus excretion (the higher the excretion, the lower the BMD). In children with fractures, we also found statistically significant negative relationships between bone formation and resorption indices on the one hand and bone mass on the other – the more rapid the bone turnover (and especially increased bone resorption), the lower the BMD. This trend was not observed in the control group. Moreover, children with fractures had lower BMD Z-scores if increased urinary calcium excretion was present (hypercalciuria). These relations are presented in Table 4. The correlations between biochemical marker levels and calcaneus ultrasound indices were less numerous and considerably less robust. Nie wykazano natomiast istotnych statystycznie ró¿nic w zakresie œrednich wartoœci stê¿eñ wskaŸników gospodarki wapniowo-fosforanowo-magnezowej oraz markerów obrotu kostnego pomiêdzy obydwiema grupami. Several factors, both genetic and environmental, are taken into consideration as regards the aetiopathogenesis of fractures [3,4,6,7,8]. It is also important to determine the effect of bone mass and bone turnover indices on fracture incidence [1,5,9]. According to professional literature, fractures occur most commonly in girls between 11 and 12 years old and boys between 13 and 14 years old [5, 10]. In our study the situation was similar, with girls aged 11-12 years and boys aged 10-14 years constituting the largest group. In our study, as in studies conducted by other authors [2,11], fractures were experienced more frequently by boys (53/81, i.e. 65%) as compared with girls (28/81, 34%). Similar findings were obtained by Ferrari [5] and Landin [12], who demonstrated that approx. 42% of boys and 27% of girls had experienced fractures, and in Kraabe's study 15-19% of fractures were experienced by boys and 11% by girls [13]. Clark et al. also reported a higher risk of fracture in boys as compared with girls, which was put down to boys taking up more risky behaviour and suffering more injuries [1]. Bone mineralization disorders are frequently diagnosed on the basis of an X-ray only after the patient suffers a fracture. The DXA densitometric examination is a basic diagnostic criterion in bone mass assessment. In the study group, a DXA examination found reduced BMD in as many as 52/81 patients (64%). Osteoporosis was diagnosed according to this densitometric criterion in 23 patients and osteopaenia in 29. The effect of bone mass on the risk of fractures in children has been studied, among others, by Clark et al. [1], Ferrari et al. [5], and Manias et al. [4]. They all emphasised the influence of BMD on increased risk of fractures. In the study by Manias et al., children with multiple fractures had considerably lower values of BMD and BMC on densitometry [4] than the control group. Key et al. analysed bone mineral density in 16 children with fractures following a slight trauma and found osteoporosis and osteopaenia in 15/16 children, the diagnosis being based on DXA-based measurements of different segments of the skeleton [14]. In the present study, the most important factor predisposing to the occurrence of fractures (in 2/3 subjects) was the reduction both skeletal BMD, and thus in bone mass, which is consistent with the conclusions drawn by the above-mentioned authors. The present study also found considerably decreased values of at least one of the parameters determined during the ultrasound examination of the calcaneus (Z-score<-2.0). W etiopatogenezie bierze siê pod uwagê szereg czynników zarówno genetycznych, jak i œrodowiskowych [3,4,6,7,8], wa¿ne jest tak¿e ustalenie wp³ywu masy kostnej i wskaŸników obrotu kostnego na wystepowanie z³amañ [1,5,9]. Jak wynika z piœmiennictwa, z³amania dotycz¹ najczêœciej dziewczynek pomiêdzy 11 a 12 rokiem ¿ycia oraz ch³opców pomiêdzy 13 a 14 rokiem ¿ycia [5,10], podobnie w naszej pracy najliczniejsz¹ grupê stanowi³y dziewczêta w wieku 11-12 lat i ch³opcy w wieku 10-14 lat. Analogicznie jak w badaniach innych autorów [2,11] równie¿ w naszych – czêœciej z³amañ doznawali ch³opcy (53/81, tj. 65%) w porównaniu do dziewcz¹t (28/81, 34%). Podobne spostrze¿enia wysuwaj¹ Ferrari [5] i Landin [12], którzy wykazali, i¿ z³amaniom uleg³o oko³o 42% ch³opców i 27% dziewczynek, w badaniach Krabbe natomiast 15-19% z³amañ dotyczy³o p³ci mêskiej, a 11% p³ci ¿eñskiej [13]. Clark i wsp. stwierdzili tak¿e wy¿sze ryzyko z³amania u ch³opców w porównaniu z dziewczêtami, co jest t³umaczone wiêkszymi ryzykownymi zachowaniami i zwiêkszon¹ urazowoœci¹ u ch³opców [1]. Zaburzenia mineralizacji koœæca s¹ czêsto wykrywane dopiero po z³amaniu na podstawie badania rentgenowskiego. Podstawowym kryterium diagnostycznym w ocenie masy kostnej jest badanie densytometryczne wykonywane metod¹ DXA. W ocenianej przez nas grupie dzieci ze z³amaniami, a¿ u 52/81 (64%), stwierdzilismy obni¿enie gêstoœci mineralnej koœæca w badaniu DXA, w tym u 23 stanowi³o ono podstawê, jako kryterium densytometryczne, rozpoznania osteoporozy, a u 29 – osteopenii. Wœród doniesieñ w piœmiennictwie o wp³ywie masy kostnej na ryzyko z³amañ u dzieci nale¿y wymieniæ badania Clark'a i wsp. [1], Ferrari i wsp. [5] oraz Manias'a z zespo³em [4], którzy podkreœlaj¹ zwi¹zek BMD ze zwiêkszonym ryzykiem z³amañ. I tak, w badaniach Manias i wsp. dzieci z wielokrotnymi z³amaniami mia³y znacz¹co ni¿sze, ani¿eli grupa kontrolna, wartoœci BMD i BMC w badaniu densytometrycznym [4]. Key i wsp. analizuj¹c gêstoœæ mineraln¹ koœci u 16 dzieci ze z³amaniami w wyniku niewielkiego urazu stwierdzili u 15/16 osteoporozê lub osteopeniê rozpoznan¹ z uwzglednieniem pomiarów ró¿nych odcinków koœæca metod¹ DXA [14]. W prezentowanej pracy najwa¿niejszym czynnikiem predysponuj¹cym do z³amañ (u 2/3 badanych) by³o obni¿enie gêstoœci mineralnej szkieletu, a tym samym masy kostnej, co jest zgodne z wnioskami wymienionych autorów. The results of ultrasound assessment may point to disrupted bone structure as a causal factor in fractures, because professional literature suggests the possibility of assessing bone structure with an ultrasound examination [15,16,17, 18,19]. As was shown earlier in this paper, the reduced BMD in 17 children with fractures could be regarded as secondary to a coexisting chronic disease. What is more, glycocorticosteroids had been administered to 10 of those patients, and the negative effect of these drugs on bone mineral metabolism is widely known [20,21,22]. We observed, however, spontaneous reduction in bone mineralization in the majority of our patients, the cause of which could not be elucidated during the study. The assessment of calcium-phosphate-magesium metabolism demonstrated numerous biochemical abnormalities in the study group. Most frequently they were hypomagnesaemia (13 patients), hypercalciuria [10] and hypermagnesuria [35]. Similar results had been obtained in our earlier pilot study including a smaller group of children with multiple fractures [23]. Vitamin D is known to have an important role in skeletal growth and mineralization, so its deficiency leads to excessive loss of bone mass, which may in turn contribute to the development of osteoporosis [24,25]. It needs to be noted that, in our study, the levels of the liver metabolite of vitamin D were reduced in 20 patients (24.7%) in the group with a history of multiple fractures (including 9 patients with major reductions). The deficiency occurred 3 times more often in the study group than in the control group, which emphasises a considerable significance of vitamin D deficiency in the aetiology of fractures in childhood and adolescence. The analysis of correlations between the levels of biochemical indices and bone turnover markers versus BMD (DXA method) demonstrated significantly accelerated bone turnover with particular intensification of bone resorption in the children with fractures. We also observed increased excretion of ions in urine in this group. These observations clearly demonstrate the presence of a marked derangement of bone mineral metabolism. It is associated mainly with osteoporosis and osteopaenia, in the course of which patients experience fractures. The results of this study point to the utility of a detailed assessment of calcium-phosphate metabolism and BMD in children with multiple fractures. Jak przedstawiono wczeœniej w naszych badaniach, u 17 dzieci ze z³amaniami mo¿na obni¿on¹ gêstoœæ mineraln¹ koœci uznaæ za wtórn¹ do przewlek³ej choroby rozpoznanej u tych pacjentów, przy czym u 10 z nich stosowano w terapii glikokortykosteroidy; negatywny wp³yw tych leków na metabolizm kostny jest powszechnie znany [20,21,22]. Jednak¿e wiêkszoœæ naszych badanych charakteryzowa³a siê spontanicznym obni¿eniem mineralizacji koœæca, której przyczyny podczas niniejszych badañ nie uda³o siê wyjaœniæ. Ocena gospodarki wapniowo-fosforanowo-magnezowej wykaza³a wystêpowanie licznych nieprawid³owoœci biochemicznych w grupie dzieci ze z³amaniami, a najczêœciej obserwowano hipomagnezemiê – u 13 pacjentów, hiperkalciuriê u 10 oraz hipermagnezuriê u 35. Podobne wyniki uzyskaliœmy w naszych wczeœniejszych pilota¿owych badaniach w mniejszej grupie dzieci z wielokrotnymi z³amaniami koœæca [23]. Jak wiadomo witamina D pe³ni wa¿n¹ rolê w procesach wzrostu i mineralizacji koœæca, a zatem jej niedobór prowadzi do nadmiernej utraty masy kostnej, co z kolei przyczyniæ siê mo¿e do rozwoju osteoporozy [24,25]. Zwrócenia uwagi w naszych badaniach wymaga obni¿one stê¿enie metabolitu w¹trobowego witaminy D u 20 pacjentów (24,7%) z grupy z wielokrotnymi z³amaniami koœci w wywiadzie (w tym u 9 znacznego stopnia). Niedobór ten wystêpowa³ trzykrotnie czêœciej w grupie dzieci ze z³amaniami w stosunku do porównawczej, co pozwala na podkreœlenie du¿ego znaczenia niedoboru witaminy D w etiologii z³amañ koœci w wieku rozwojowym. CONCLUSIONS 1. In our study the factor which most frequently predisposed to fractures in children and adolescents was reduced BMD, which also indicated a reduction in bone mass. This abnormality was diagnosed in approx. 2/3 of the patients with a history of multiple fractures. 2. Accelerated bone turnover, and, particularly, increased bone resorption, indicates a marked derangement of bone metabolism in children and adolescents with a history of fractures. 3. Vitamin D deficiency was confirmed in more than three times as many children in the study group in comparison with the control group, which indicates the importance of hypovitaminosis D in the aetiopathogenesis of fractures, also in childhood and adolescence. 4. In approx. a third of the patients the multiple fractures were due to chronic diseases, including those requiring glycocorticosteroid treatment. 5.
