Przedstawiono wyniki badañ wykonanych w 60-osobowej grupie dzieci z mózgowym pora¿eniem dzieciêcym. Badanie wykonano w Stacjonarnym Oœrodku Rehabilitacji SP ZOZ w Zgorzelcu. Celem badania by³o udowodnienie, ¿e czterotygodniowy program indywidualnie realizowanej kinezyterapii z wykorzystaniem kriostymulacji miejscowej koñczyn dolnych, wykonanej parami ciek³ego azotu u dzieci z mózgowym pora¿eniem dzieciêcym ma korzystny wp³yw na ich stan motoryczny, napiêcie miêœniowe oraz temperaturê powierzchniow¹ skóry. Materia³ i metody. Badaniem objêto grupê dzieci z mózgowym pora¿eniem dzieciêcym obojga p³ci, w przedziale wiekowym 3-14 lat, które poddano czterotygodniowemu turnusowi zabiegów indywidualnie dobranej kinezyterapii oraz kriostymulacji obu koñczyn dolnych. Ocenê napiêcia miêœniowego wykonano miotonometrem Szirmai wed³ug specjalnej metodyki. Poziom spastycznoœci oceniono testem Ashwortha, a ocenê motoryki dziecka wed³ug w³asnej tabeli punktowej, przed rozpoczêciem zabiegów i po ich zakoñczeniu. Wykonano rejestracjê termowizyjn¹ temperatur powierzchniowych skóry koñczyn dolnych na pocz¹tku i koñcu turnusu, przed kriostymulacj¹, bezpoœrednio po oraz w 5 i 15 minut po niej. Wyniki i Wnioski. Porównane wyniki oceny motoryki dziecka, testu Ashworth'a i miotonometrii wykonane przed turnusem i po jego zakoñczeniu wskazuj¹, ¿e kriostymulacja w programie usprawniania dziecka z mózgowym pora¿eniem dzieciêcym przyczyni³a siê istotnie do zmniejszenia napiêcia spastycznego miêœni koñczyn dolnych oraz poprawy motoryki. Analiza temperatur powierzchniowych skóry zarejestrowanych termowizyjnie dowodzi pe³nej adaptacji uk³adu naczyniowego dzieci z mózgowym pora¿eniem dzieciêcym do temperatur kriogenicznych, czyli pe³nego bezpieczeñstwa kriostymulacji miejscowej. SUMMARY is c op y is for Background. The author presents the results of a study of 60 children with infantile cerebral palsy carried out at a rehabilitation centre in the town of Zgorzelec. The study aimed to demonstrate that a 4-week programme of customised kinesiotherapy with cryostimulation of the lower limbs with liquid nitrogen vapour could have a beneficial effect on motor status as well as muscle tone and skin surface temperature in children with infantile cerebral palsy. Material and methods. A group of children of both sexes aged 3-14 with infantile cerebral palsy underwent 4 weeks of customised kinesiotherapy combined with cryostimulation of both lower limbs. Muscle tone was determined with a Szirmai myotonometer using special methodology. The level of spasticity was estimated with the Ashworth test, and motor activity assessment was made before and after the treatment according to a score table designed by the authors. Thermovisual recordings of skin surface temperature in the lower limbs were made immediately before and 5 and 15 minutes after cryostimulation procedures at the beginning and end of the 4-week programme. Results and Conclusions. A comparison of the results of the motor activity assessment, Ashworth test and myotonometry before and after the rehabilitation programme shows that the use of cryostimulation in the rehabilitation of children with infantile cerebral palsy results in a reduction of spasticity in the lower limbs and improvement in motor activity. Thermovisual analysis of skin surface temperature demonstrates full adaptation of the children's vascular system, i.e. excellent safety of topical cryostimulation. Therefore it ought to be expected that cryostimulating procedures will produce a thermoregulatory response [1,2]. If it is assumed that good tolerance of cryotherapy in children depends on their nervous, hormonal and vascular reactivity, then infantile cerebral palsy, which represents a particular dysfunction of the central nervous system, may cause completely different reactions to cryotherapy procedures of the same length. Children patients pose a therapeutic and physiotherapeutic challenge as their patterns of nervous, hormonal and vascular reactivity remains unknown. Extreme cold is commonly known to decrease muscle tension, muscular spasms and the intensity of pain sensations. While the influence a thermal stimulus of this kind has on the tissues exposed to the procedure appears to have been satisfactorily explained, the amount of available knowledge concerning the peripheral reflex and segmental effects of local procedures should be regarded as insufficient, particularly as regards children with infantile cerebral palsy acquired as a result of a perinatal injury. Therefore, the present study could contribute to the development of therapeutic applications of local cryotherapy in children suffering from infantile cerebral palsy [3,4]. Long-term observations of these patients reveal diverse motor activity disorders [5,6], which hinder or completely disable independent locomotion or activities of daily living such as eating, washing oneself or getting dressed. The main cause of disturbances of normal motor development is increased muscle tone associated with upper motor neuron injury, frequently presenting as tetraplegia. Local cryostimulation was expected to reduce spastic muscle tone and improve tolerance of exertion in children with infantile cerebral palsy included in the study. Thermal stimulation mostly influences skin perfusion. The associated mechanism of thermoregulation is called Aschoff's reflex, which represents a vasomotor reaction of the body exposed to low and high temperatures. The experiment was designed to determine whether a 4-week program of customized kinesiotherapy combined with cryostimulation with liquid nitrogen vapor would have a beneficial effect on motor status, spontaneous motor activity, muscle tone in the lower limbs and skin surface temperature of the lower limbs in children with cerebral palsy. Consequently, the following hypotheses had to be verified: 1. Local cryostimulation of the lower limbs for a total of 12 minutes effects a change of spastic muscle tone in the lower limbs in the Ashworth test and myotonometer studies. 2. Cryogenic temperatures produced by liquid nitrogen vapor change the skin surface temperature of the lower limbs in children with infantile cerebral palsy as demonstrated by thermovisual recording. 3. The customized program of rehabilitation of children suffering from infantile cerebral palsy with the use of local cryostimulation contributes to the improvement of spontaneous motor reactions in the study group. 4. WSTÊP Kriostymulacja jest stymulacj¹ czuciow¹ i krótkotrwa³¹, nale¿y zatem spodziewaæ siê, ¿e jej zabiegi bêd¹ wyzwala³y reakcjê termoregulacyjn¹ [1,2]. Je¿eli przyjmiemy, ¿e dobra tolerancja zabiegów krioterapii przez dzieci zale¿na jest od ich reaktywnoœci nerwowej, hormonalnej i naczyniowej, to okreœlona dysfunkcja OUN, jak¹ jest mpd, mo¿e uwalniaæ zupe³nie ró¿ne reakcje na identyczny czas trwania zabiegu krioterapii stosowanego u nich. Dzieci stanowi¹ bardzo trudny problem leczniczy i fizjoterapeutyczny, bowiem nie jest znana ich reaktywnoœæ nerwowa, wegetatywna i naczyniowa. Wiadomo powszechnie, ¿e krañcowe zimno zmniejsza napiêcie miêœniowe, spazm miêœniowy i odczuwanie bólu. Pomimo i¿ wp³yw tego rodzaju bodŸca termicznego na tkanki w miejscu wykonywania zabiegu wydaje siê byæ poznany, to stan wiedzy na temat obwodowych konsekwencji reflektoryczno-segmentarnych zabiegów lokalnych nale¿y uznaæ za niewystarczaj¹cy. Sytuacja ta dotyczy g³ównie dzieci z mpd powsta³ym wskutek urazu oko³oporodowego. Dlatego przeprowadzone badania mog³yby przyczyniæ siê do rozwoju zastosowañ leczniczych krioterapii miejscowej u dzieci z mpd [3,4]. D³ugoletnia obserwacja tych dzieci pozwala zauwa¿yæ wyst¹pienie ró¿norodnych nieprawid³owoœci w rozwoju motorycznym [5,6]. Utrudniaj¹ one, a czêsto uniemo¿liwiaj¹ samodzieln¹ lokomocjê czy wykonywanie czynnoœci samoobs³ugi takich, jak: jedzenie, mycie siê, ubieranie siê itd. G³ówn¹ przyczyn¹ zaburzeñ prawid³owego rozwoju motorycznego jest wzmo¿one napiêcie miêœni zwi¹zane z uszkodzeniem GNR, objawiaj¹ce siê czêsto pora¿eniem czterokoñczynowym. Kriostymulacja miejscowa ma za zadanie zmniejszyæ spastyczne napiêcie miêœni i poprawiæ tolerancjê wysi³kow¹ dzieci z mpd uczestnicz¹cych w badaniu. Zmiany zachodz¹ce pod wp³ywem czynnika termicznego to przede wszystkim zmiany ukrwienia skóry. Mechanizm termoregulacji z tym zwi¹zany to odruch Aschoffa, który przedstawia reakcjê naczynioruchow¹ organizmu poddawanego dzia³aniu niskich i wysokich temperatur. Celem przeprowadzonego eksperymentu by³o sprawdzenie, czy czterotygodniowy program indywidualnie dobranej fizjoterapii z wykorzystaniem kriostymulacji parami ciek³ego azotu u dzieci z mpd ma korzystny wp³yw na ich stan motoryczny, motorykê spontaniczn¹, napiêcie miêœniowe koñczyn dolnych oraz temperaturê mierzon¹ na powierzchni skóry koñczyn dolnych. Z tych powodów uzasadnione sta³o siê zweryfikowanie poni¿szych tez: 1. The medical experiment involved topical cryostimulation in the cryogenic temperature range (below -100°C), which produces a different vascular reaction. Despite the application of an extreme thermal stimulus, the arteriovenous anastomoses remain open throughout the entire duration of the procedure as the patient can sense warmth at the skin surface within a few seconds of the beginning of exposure to liquid nitrogen vapor. Maintaining the body's homeothermy and fear of damage to the surface skin layers activates a heating mechanism in these layers based on a temperature gradient. It is this gradient that equalizes the temperature inside the body (37°C) with that in tissues situated above by heating up consecutive layers. Failure of this vascular mechanism could lead to skin damage, which is, however, not observed in routine cryostimulation procedures in children and adults alike. Thus this reflex is responsible for the safety of the procedure even in small children. -d istr ibu tio np roh ibit ed . 4. Przeprowadzone doœwiadczenie medyczne z wykorzystaniem krótkotrwa³ego bodŸca termicznego mog³oby byæ wykorzystywane w leczeniu dzieci z mpd, u których wzmo¿one napiêcie miêœni utrudnia proces usprawniania, zw³aszcza lokomocji. W doœwiadczeniu medycznym zastosowano zabiegi kriostymulacji miejscowej, wykorzystuj¹c zakres temperatur kriogenicznych, to jest temperatur poni¿ej – 100°C, które powoduj¹ inn¹ reakcjê naczyniow¹. Pomimo tak ekstremalnego bodŸca nie dochodzi do zamkniêcia anastomoz têtniczo – ¿ylnych przez ca³y okres trwania zabiegu, albowiem po kilku sekundach dzia³ania par ciek³ego azotu, pacjent odczuwa ciep³o na powierzchni skóry. Zachowanie homeotermii organizmu i obawa przed uszkodzeniem warstw powierzchniowych skóry uruchamia mechanizm ocieplania tych warstw oparty o gradient temperatury. To ten gradient wyrównuje wartoœæ temperatury wewn¹trz cia³a (37°C), z warstwami po³o¿onymi powy¿ej na zasadzie ogrzewania kolejnych warstw tkanek. Niezadzia³anie tego odruchu naczyniowego mog³oby doprowadziæ do uszkodzenia skóry, czego nie obserwuje siê w rutynowo wykonanych zabiegach kriostymulacji zarówno u dzieci, jak i u doros³ych. Jest to jednoczeœnie odruch, który stanowi o bezpieczeñstwie tego zabiegu, nawet u ma³ych dzieci. MATERIAL AND METHODS The study involved a group of 60 children of both sexes aged 3-14, where girls accounted for 38.3% and boys for 61.7%. The average age of the girls was 9.7 years, in the case of boys it was 7.6 years, with the average age of the entire group amounting to 8.4 years. All children were diagnosed with tetraplegia with increased muscle tone characteristic of infantile cerebral palsy. The patients had no additional disorders, such as sight impediment, hearing problems or significant mental retardation. The average age at which the children were diagnosed was 6.3 years in boys, 6.9 years in girls and 6.5 years for the entire group. The children included in the study came from various social backgrounds, with 20% born in rural areas and 80% coming from urban districts. The duration of their disease and the degree of disability also varied. Additionally, the study involved a group of 36 children aged 3-13 suffering from tetraplegia with spasticity. The average age of the children was 8.3 years, with boys representing 58.33% and girls accounting for 41.66% of the group. The patients underwent a four-week customized program of kinesiotherapy carried out in the same manner and conditions as in the case of the study group. They were a clinically homogeneous group which could not be exposed to cryostimulation for various reasons, predominantly because of intolerance of low temperatures. These children constituted the control group. The children were qualified for the program at their admission to the rehabilitation centre in the town of Zgorzelec, according to the waiting list of patients. The diagnosis was verified by a neurologist and a pediatrician. The physiotherapist performing the experiment completed a questionnaire and carried out a motor function assessment. Dziewczêta stanowi³y 38,3%, a ch³opcy 61,7%. Œredni wiek badanych dziewcz¹t wynosi³ 9,7, a ch³opców 7,6 roku, œredni wiek badanej grupy wynosi³ 8,4 roku. U wszystkich dzieci rozpoznano niedow³ad czterokoñczynowy ze wzmo¿onym napiêciem miêœniowym, wystêpuj¹cym w przebiegu mpd. U dzieci nie wystêpowa³y dodatkowe zaburzenia np. wzroku, s³uchu, czy znaczny stopieñ upoœledzenia umys³owego. Œrednia wieku, w którym postawiono diagnozê, wœród ch³opców wynosi³a 6,3 roku, u dziewcz¹t – 6,9, a dla ca³ej grupy – 6,5 roku. Dzieci wywodzi³y siê z ró¿norakich œrodowisk, 20% pochodzi³o ze wsi, 80% z miasta, ró¿ni³y siê okresem trwania choroby i prezentowa³y zró¿nicowany stopieñ niepe³nosprawnoœci. Badaniu poddano równie¿ 36 dzieci z mpd, w przedziale wiekowym 3-13 lat, z czterokoñczynow¹ postaci¹ mpd ze wzmo¿onym napiêciem spastycznym. Œrednia wieku dzieci wynosi³a 8,3 roku. W grupie by³o 58,33% ch³opców i 41,66% dziewcz¹t. Dzieci te by³y objête czterotygodniowym indywidualnie dobranym programem kinezyterapii realizowanym w identycznej formie i warunkach jak dzieci z grupy badawczej. Tworzy³y one jednorodn¹ klinicznie grupê, która z ró¿nych przyczyn nie mog³a byæ poddawana kriostymulacji, g³ównie z powodu nietolerancji na zimno. Stanowi³y one grupê kontroln¹. Badanie kwalifikacyjne do programu przeprowadzono przy przyjêciu dzieci do Stacjonarnego Oœrodka Rehabilitacji SP ZOZ w Zgorzelcu, zgodnie z kolejk¹ chorych oczekuj¹cych. Rozpoznanie by³o weryfikowane przez neurologa i pediatrê. Ankietê i ocenê czynnoœciow¹ wykona³ przeprowadzaj¹cy eksperyment – fizjoterapeuta. Each child was admitted together with their guardian (a parent or legal guardian). A neurologist, a pediatrician and a physiotherapist were present throughout the entire duration of the rehabilitation program. Physiotherapeutic procedures were customized to meet the individual needs of each patient and accounted for patients' age, level of motor activity, mental status and the type and results of previous rehabilitation. The nonstandard element of the rehabilitation program was physiotherapy including local cryotherapy of the lower limbs in the region of the quadriceps muscle of thigh, the biceps muscle of thigh and the triceps muscle of calf. All parents (guardians) signed a form to express their consent to their child's participation in the study. They were also given "instructions for the parents of the children participating in the experiment". The following forms were used to record the data required for the experiment: – Child questionnaire – filled in by the person performing the experiment on the day of the child's admission to the rehabilitation center and after the treatment. – Child motor activity assessment – the assessment was conducted twice: on the first and the last day of the experiment. It involved the evaluation of motor skills with respect to crawling, sitting down and walking. – Assessment of spasticity level according to Ashworth scale – the Ashworth test was performed before and after the treatment, always in the same conditions. – Assessment of muscle tone performed with a Szirmai myotonometer. – the Szirmai myotonometer is a device manufactured in Hungary and designed for determination of muscle tone at rest expressed in myotones, where 1 myotone stands for the force (in millinewtons) required to produce an indentation in the skin above the muscle at a constant surface of the bolt of 0.18 cm2 Local cryostimulation of the lower limbs was performed according to a specially designed method. The children's lower limbs were subjected to cooling with liquid nitrogen vapor for 12 minutes once a day, 6 minutes per limb The first three procedures were shorter to enhance adaptation. Gradual cooling of the front and back parts of the limbs during the first three procedures was aimed at eliminating possible adverse reactions such as malaise, elevated body temperature, or increased spasticity. Cryotherapy was performed using a Krioter KS-1 device manufactured by Hans-Sped, G³ogów, Poland, in the following manner: the outlet of the device was placed 15-20 cm away from the limb; the posterior kinematic chain of one lower limb (the thigh and shank at the same time) was cooled for four minutes; this was followed by the cooling of the anterior kinematic chain (thigh) of the same limb for two minutes, the total duration of the procedure on both limbs amounting to 12 minutes [7,8,9,10,11,12,16]. The areas subject to the procedure were cooled according to a dynamic technique where the nozzle was moved over the biceps muscle of thigh, the triceps muscle of calf and finally the quadriceps muscle of thigh. Low temperature was produced by liquid nitrogen vapor released through the nozzle of the device, where its temperature was -196°C. Opiekê nad dzieæmi podczas ca³ego turnusu rehabilitacyjnego sprawowali: neurolog, pediatra i fizjoterapeuta. Program postêpowania fizjoterapeutycznego by³ dobierany indywidualnie dla ka¿dego dziecka, a jego sk³adowe by³y zale¿ne od: wieku, poziomu motorycznego, psychicznego oraz od dotychczas prowadzonej rehabilitacji i jej efektów. Postêpowaniem wykraczaj¹cym ponad standardowy program turnusu rehabilitacyjnego by³a fizykoterapia, a w niej krioterapia miejscowa koñczyn dolnych w obrêbie miêœni: czworog³owego uda, dwug³owego uda, trójg³owego ³ydki. Wszyscy rodzice (opiekunowie prawni) wyrazili zgodê na udzia³ dzieci w badaniu naukowym, podpisuj¹c stosowne oœwiadczenie. Zostali tak¿e zapoznani z „informacj¹ dla rodziców dzieci uczestnicz¹cych w eksperymencie”. Badania dla potrzeb eksperymentu zaewidencjonowano za pomoc¹ formularzy: – Ankieta dziecka. – Ankietê wype³nia³ badaj¹cy w dniu przyjêcia dziecka na turnus rehabilitacyjny w Oœrodku i po jego zakoñczeniu – Ocena rozwoju motorycznego dziecka. – Ocenê rozwoju motorycznego dziecka wykonano dwukrotnie: w dniu przyjêcia do programu badawczego oraz po jego zakoñczeniu. Cryotherapy procedures were performed in specially designed rooms where only one child could be present at a time (small children were accompanied by their parents). Following the procedure the patient was relocated to a neighboring room with the same thermal conditions where thermovisual scans and motor tests were performed. This guaranteed adherence to all safety standards. Thermovisual recordings with the Swedish Agema 570 camera were conducted twice in the course of the program – on the first and on the last day. Local cryotherapy and thermovisual assessment of its efficacy, which was a key element of the study, were performed in the following manner: 1. In order to facilitate uniform assessment, a number of points, lines and circles were marked on the scans as locations of temperature measurements. The temperatures (°C) were entered into appropriate tables. For the purpose of statistical analysis of the collected data, measurable characteristics were represented by ranges (min-max) and average values (means and medians) as well as measures of internal variation (standard deviation). Non-measurable characteristics were represented by the frequency of occurrence of their particular categories, which was determined by calculating the percentage share of these categories in particular groups. The Shapiro-Wilk test was used to determine the type of distribution of quantitative variables. When the distribution approximated the normal distribution, a test for two independent samples was used to compare mean values in the two groups following verification of the equality of variance of the variable in both groups with the Fisher-Snedecor test. When the variances were not equal, the Cochran-Cox test was used to carry out the comparison. For characteristics whose distribution was only insignificantly different from the normal distribution, analysis of variance was used to compare mean values at several different time points, while the difference test was used for comparison of data for two time points in the same children. When the distribution differed significantly from normal, the Mann-Whitney test was used to compare the means in two groups [13,14,15] Other tests used in the statistical analyses included the t-Student test and the Levene test. The results obtained in the control group were also subjected to a statistical assessment and then compared with the results from the study group. The initial temperature of the right lower limb estimated thermovisually prior to the rehabilitation program was 30.4°C, and fell to 23.3 °C after the first cryostimulation procedure. The decrease in skin surface temperature amounted to 7.1°C. An identical examination conducted at the end of the program produced the results of 31.6°C and 21.5°C, while the temperature amplitude amounted to 10.1°C. Temperature at other stages in the course of the study continued to increase gradually, however, the initial temperature values before the treatment and at its end are statistically significantly higher than the ones recorded at the end of the study. The initial temperature of the left lower limb estimated in thermovisual study prior to the rehabilitation program amounted to 30.7°C, while after the first cryostimulation procedure it fell to 21.8°C. The decrease in skin surface temperature recorded after the first procedure amounted to 8.9°C. In the examination performed at the end of the rehabilitation program the temperature of the left lower limb amounted to 31.6°C prior to the cryotherapy procedure and 20.2°C following the procedure for an amplitude of 11.4°C. At other time points of the study the temperature of the left lower limb tended to increase. Fifteen minutes following a cryotherapy procedure it was 28.4°C both at the beginning and at the end of the rehabilitation program. The difference between temperatures recorded prior to cryostimulation of both lower limbs at the beginning and at the end of the program amounted to approximately 1°C, while directly following the procedure at the end of the program each limb was colder by approx. 2°C. Temperatures recorded fifteen minutes after the first and last cryotherapy procedure were almost identical. The temperature recorded at the end of the program was significantly lower than the temperature prior to the treatment, differing from the latter by about 3°C. The assessment of the children's motor activity included the skills of crawling, sitting and walking and was performed prior to the rehabilitation program and after its completion. If the values are positive, the growth of one of them is accompanied by the growth of the other. This tendency intensifies as the values approach "1". No relationship is observed if the value of the coefficient is 0. In the present study, Spearman's rank correlation coefficient was used to assess the relationship of muscle tone and skin surface temperature of the right lower limb and the left lower limb and was positive in the initial and the final stage of the experiment. This finding means that increased muscle tone was strongly correlated with elevated skin surface temperature at these stages of the experiment. The results demonstrate that a decrease in temperature was associated with a decrease in muscle tone, and, consequently, an increase in temperature was accompanied by increasing muscle tone. A myotonometer test performed 20 minutes after the first cryotherapy procedure compared with the thermovisual test conducted 20 minutes after the first cryotherapy procedure and the tests performed prior to the cryotherapy procedure at the end of the program demonstrated a negative correlation close to zero. This relationship was most conspicuous for the biceps muscle of thigh and the triceps muscle of calf. The comparison of muscle tone in the quadriceps muscles of thigh in the study group and the control group has revealed that following a four-week rehabilitation program the tone in the quadriceps muscles of thigh of both lower limbs in the control group was higher than that in the study group. The respective patterns of change during the rehabilitation period were different, with measurements at the end of the program only slightly different from the initial ones. After the rehabilitation period the tone in the biceps muscle of thigh in both lower limbs was slightly higher in the control group than in the study group. The pattern of change throughout the four weeks of rehabilitation was similar, with the final values lower than the initial ones. The results of the myotonometer test for the triceps muscle of calf show that muscle tone in both lower limbs after a four-week rehabilitation program was higher in the control group than in the study group, even though the final results demonstrated an overall decrease. The pattern of change throughout the rehabilitation program was different here than in the case of other muscles. Z porównania wartoœci napiêcia miêœniowego – m. czworog³owych uda kkd w grupach badanej i kontrolnej wynika, ¿e po czterotygodniowym usprawnianiu napiêcie m. czworog³owego uda w obu kkd w grupie kontrolnej jest wy¿sze ni¿ w grupie badawczej. Inny jest rozk³ad jego przebiegu w okresie ca³ego cyklu usprawniania, a koñcowe wyniki s¹ tylko nieznacznie ni¿sze od pocz¹tkowych. Po okresie usprawniania napiêcie m. dwug³owego uda w obu kkd w grupie kontrolnej jest nieznacznie wy¿sze ni¿ w grupie badawczej. Podobny jest rozk³ad jego przebiegu w trakcie czterech tygodni usprawniania, a koñcowe wyniki s¹ ni¿sze od pocz¹tkowych. Wyniki miotonometrii dla m. trójg³owego ³ydki pokazuj¹ ¿e po czterotygodniowym usprawnianiu napiêcie miêœniowe w obu kkd w grupie kontrolnej jest wy¿sze ni¿ w grupie badawczej, choæ i tak koñcowe wyniki pokazuj¹ jego spadek. Rozk³ad jego przebiegu w okresie ca³ego cyklu usprawniania jest inny ni¿ w przypadku pozosta³ych miêœni. Stan napiêcia miêœniowego we wszystkich badanych miêœniach w grupie kontrolnej jest statystycznie wy¿szy od tego w grupie dzieci, u których stosowano dodatkowo kriostymulacjê miejscow¹. In this study, the myotonometric records of muscle tone in the lower limbs (quadriceps and biceps muscles of thigh and triceps muscle of calf) demonstrated the greatest increase after the first week of physiotherapeutic treatment combined with local cryostimulation. This outcome was not observed in the control group. During further stages of the experiment, it oscillated at a level lower than the initial one. No statistically significant differences of muscle tone between the right and the left lower limbs were observed throughout the experiment. Similar results following cryotherapy of the lower limbs in healthy people were presented by Ksiê¿opolska [16]. Analysis of the distribution of muscle tension revealed the greatest decrease in the area of the quadriceps muscle of thigh and the smallest decrease in the area of the biceps muscle of thigh. This difference appears to be related to the fact that spastic flexion is much more frequent. An examination of changes in mean values of temperature recorded thermovisually following local cryostimulation in this study provided some interesting insights. The 28-day program of customized physiotherapy combined with oncedaily local cryostimulation turned out to be a stimulus powerful enough to induce adaptation of the vascular system in the area of the lower limbs of the children. This might be a result of increased compliance of blood vessels of the lower limbs as well as decreased spasticity. Improved thermoregulatory efficiency might also play a role. According to Œliwiñski [3,5] cryotherapy is a safe procedure also in children and can be used in the treatment of spasticity, an indication confirmed in the present study of children above three years old. The study has also revealed that children with infantile cerebral palsy subjected to local cryotherapy of the lower limbs demonstrate improved motor activity facilitating locomotion. W badaniach w³asnych, w rozk³adzie napiêcia miêœniowego najwiêksze jego obni¿enie uzyskano w obrêbie miêœnia czworog³owego uda, a najmniejsze w obrêbie miêœnia dwug³owego uda. Wydaje siê, ¿e to zró¿nicowanie zwi¹zane jest ze znacznie czêœciej wystêpuj¹cym wzmo¿onym napiêciem spastycznym o charakterze zgiêciowym. Interesuj¹ce obserwacje w niniejszej pracy dotycz¹ zmian œredniej wartoœci temperatur mierzonych termowizyjnie po zabiegu kriostymulacji miejscowej. Okaza³o siê, ¿e 28dniowy program indywidualnej fizjoterapii z wykorzystaniem zabiegów kriostymulacji miejscowej wykonywanej raz dziennie jest wystarczaj¹co du¿ym bodŸcem, aby uwidoczni³ siê efekt adaptacji uk³adu kr¹¿eniowo – naczyniowego w obrêbie kkd badanych dzieci. Mo¿e to œwiadczyæ o uelastycznieniu naczyñ krwionoœnych w koñczynach dolnych, a tak¿e o zmniejszeniu napiêcia spastycznego. Nie mo¿na tak¿e wykluczyæ poprawy sprawnoœci uk³adu termoregulacji. Œliwiñski [3,5] opisuje, ¿e krioterapia jest postêpowaniem bezpiecznym tak¿e u dzieci i nadaje siê do leczenia spastycznoœci, co w badaniach w³asnych zosta³o potwierdzone u dzieci od trzeciego roku ¿ycia. Ponadto w ramach tych badañ ustalono, ¿e u dzieci z mpd poddanych krioterapii miejscowej kkd, poprawia siê ich stan motoryczny, u³atwiaj¹cy poruszania siê. Local cryostimulation of the lower limbs significantly reduces spasticity in children suffering from infantile cerebral palsy. 2. Cryogenic-range temperature applied in the form of liquid nitrogen vapor reduces surface skin temperature of the lower limbs in children suffering from infantile cerebral palsy. 3. The customized rehabilitation program including local cryostimulation of the lower limbs brought about a significant improvement of motor activity in children with infantile cerebral palsy as a result of reducing muscle spasticity. 4. Cryogenic-range temperatures are safe and well tolerated by children with infantile cerebral palsy above three years of age. These results indicate that cryotherapy could become a routine treatment procedure in the therapy of children with infantile cerebral palsy.
