CONTRACT – SPONSORSHIP
REVOLUTION IN THE MUNICIPAL SOLID WASTE MANAGEMENT SYSTEM
AN OPPORTUNITY FOR SECTOR BUSINESSES
I wish to participate in the conference Revolution in the municipal solid waste management system
– an opportunity for sector businesses to be held on 6 October 2011 at the InterContinental Hotel,
ul. Emilii Plater 49, Warsaw.

Platinum Sponsor Package – PLN 20,000 + 23% VAT
Gold Sponsor Package – PLN 15,000 + 23% VAT
Silver Sponsor Package – PLN 10,000 + 23% VAT
Publicity in the conference booklet – PLN 1,000 + 23% VAT
VAT does not apply to companies registered outside Poland
I declare that:
1. I have received, read and accepted the general terms and conditions of the service provided.
2. the company is a VAT payer, its TIN is:................................................... and it authorises the Polish-Spanish
Chamber of Commerce (PHIG) to draw up a VAT invoice without the recipient’s signature.
3. I am not a VAT payer and I authorise the PHIG to draw up a VAT invoice without the recipient’s TIN or
signature.
4. I agree to my personal data being stored and processed by the PHIG for application and internal marketing
purposes (in accordance with the Personal Data Protection Act of 29 August 1997).
On receipt of your application we will send you a sponsorship agreement.
The conference will be held in Polish with simultaneous interpretation into English.

Platinum Sponsor Package
Title of Platinum Sponsor; Information about the Sponsor on the websites of all five bilateral
Chambers (logo, redirection to the Sponsor’s website); Logo of the Sponsor in all the materials
promoting the conference, including the conference booklet; One-page colour advertisement in the
conference booklet (on condition that the advertisement is sent to the Organiser by 19 September
2011); Logo of the Sponsor on the screen in the conference venue; Presentation on USB containing
extended conference materials; Possibility of placing a rollup in the conference venue (reception);
Possibility of publicity by funding conference bags with the logos of the Sponsor and organisers and
advertising gadgets; Sponsor’s table in the conference venue on which brochures can be laid; Free
participation in the conference for three Sponsor’s representatives.

General terms and conditions of the service provided:
1. The condition for participating in the conference is that payment is made for the chosen option in
accordance with the agreement: PLN ……………… on the basis of an invoice received.
2. Resignation from participating should be sent by registered letter to the PHIG.
3. If you resign from participating before 22 August 2011, the whole amount paid to the Organiser’s account
will be refunded to you.
4. If you resign from participating after 22 August 2011, the company that made the reservation will be charged
100% of the costs.
5. If the conference is cancelled through a fault on the Organiser’s part, the applicant will receive a refund of
the entire amount paid within ten days of the event being cancelled.
6. The Organiser reserves the right to make changes to the conference programme.
