BADANIA WIELOLETNIE TOBACCO SMOKING AMONG STUDENTS OF THE UNIVERSITY OF PHYSICAL EDUCATION AND SPORT IN 2002-2010 IN THE CONTEXT OF THEIR PARENT’S EDUCATIONAL BACKGROUND AND SOCIOECONOMIC STATUS OF THEIR A LONG-TERM STUDY Słowa kluczowe: studenci, palenie tytoniu, modele logistyczne, czynniki socjoekonomiczne, czynnik czasowy Key words: students, smoking, logistic models, socioeconomic factors, time factor Streszczenie Wstęp. Cel badań to ocena kierunku zmian, jakie zaszły w odsetku osób palących papierosy na przestrzeni dziewięcioletnich badań wśród studentów I roku studiów dziennych Akademii Wychowania Fizycznego i Sportu (AWFiS) w Gdańsku oraz ocena związku między paleniem papierosów, sytuacją społeczno-ekonomiczną rodzin i wykształceniem rodziców. Materiał i metody. Badania ankietowe przeprowadzono w latach 2002-2010 na grupie 1990 studentów pierwszego roku studiów dziennych AWFiS w Gdańsku oraz wśród ich rodziców. Rodziców pytano o wykształcenie, sytuację społeczno-ekonomiczną rodziny (bardzo dobra, dobra, średnia, zła), a studentów o palenie papierosów. Oceniano kierunek zmian w odsetku palących papierosy w latach 2002-2010, siłę i istotność związków między paleniem papierosów i sytuacją społeczno-ekonomiczną rodziny oraz wykształceniem rodziców. Utworzono modele logistyczne oceniające prawdopodobieństwo palenia papierosów przez studentów w zależności od poziomu rozpatrywanych czynników. Materiał opracowano statystycznie. Zastosowano analizę regresji logistycznej oraz test χ2. Wyniki. Na podstawie otrzymanych wyników można stwierdzić, że w badanej grupie studentów odsetek palących papierosy stopniowo malał z 17,95% w 2002 roku do 13,47% w 2010 roku. Wraz ze wzrostem wykształcenia matki (OR=0,66) lub ojca (OR=0,57) malała szansa na niepalenie papierosów przez studenta. Ponadto im lepsza sytuacja społeczno-ekonomiczna rodziny, tym mniejsza była szansa na niepalenie papierosów przez studenta (OR=0,74). Wnioski. Zauważalna tendencja spadkowa w odsetku studentów palących papierosy w kolejnych latach badań jest zjawiskiem pozytywnym. Jednak z uwagi na zagrożenia zdrowotne związane z nikotynizmem, edukacja antynikotynowa powinna być nadal niezbędnym elementem nauczania, zwłaszcza w tych jednostkach oświatowych, które kształcą ludzi odpowiedzialnych za promocję zdrowego stylu życia oraz wśród młodzieży pochodzącej z dobrze wykształconych rodzin deklarujących bardzo dobrą i dobrą sytuację społeczno-ekonomiczną. The goal of the study was to assess the direction of changes in the percentage of tobacco smokers during the 9 year study, among first year full-time students of the Gdansk University of Physical Education and Sport, and to assess the relationship between tobacco smoking, socioeconomic status of the family and parents” educational background. Material and methods. The survey was conducted between 2002 and 2010 in a sample of 1990 first year full-time students of the Gdansk University of Physical Education and Sport and their parents. The parents were asked about their educational level and socioeconomic status of their families while the students were asked about their attitudes to tobacco smoking. The direction of changes in the percentage of smokers between 2002 and 2010 was assessed as well as the significance of the relationship between tobacco smoking, the socioeconomic status of the student’s family and parents” educational level. Logistic models were developed for the assessment of the likelihood of smoking in the sample of students, depending on the level of the analyzed factors. The material was subjected to statistical analysis. Logistic regression analysis and χ2 test were applied. Results. Based on the obtained results we can conclude that a gradual decrease in the percentage of smoker, from 17.95% in 2002 to 13.47% was observed in the studied sample in 2010. The chance of non-smoking decreased with higher educational levels of the student’s mother (OR=0.66) and father (OR=0.57). It was also inversely proportional to socioeconomic status of the family (the higher the socioeconomic status, the lower the chance of non-smoking) (OR=0,74). Conclusions. The apparent decremental tendency in the percentage of smoking students subsequent years of the study is a positive phenomenon. However, due to health-related consequences of tobacco smoking, anti-smoking education still should be the necessary element of education, particularly in the units where people responsible for healthy lifestyle promotion are educated and among youth from well educated families whose reported socioeconomic status is very good or good. Palenie tytoniu przez studentów uczelni sportowej w latach 2002-2010 Wstęp Background Mimo licznych akcji antynikotynowych, palenie papierosów przez młodzież jest wciąż aktualnym problemem niosącym za sobą poważne skutki zdrowotne. Tytoń jest używką, palenie papierosów uzależnia, a związki chemiczne zawarte w dymie tytoniowym są przyczyną całej gamy chorób i dolegliwości m.in. ze strony układu krążenia, oddechowego, pokarmowego, moczowo-płciowego. Są one również przyczyną zaburzeń wewnątrzmacicznego rozwoju płodu, a także osłabienia odporności organizmu. Między innymi w związku z powyższym, Światowa Organizacja Zdrowia uznała uzależnienie od tytoniu za odrębną jednostkę chorobową. W Międzynarodowej Statystycznej Klasyfikacji Chorób i Problemów Zdrowotnych (International Classification of Diseases, ICD) zapisana jest ona pod numerem F17 jako zaburzenia psychiczne i zaburzenia spowodowane paleniem tytoniu [1]. Palenie papierosów często traktowane jest jak „lek” na różnego rodzaju sytuacje wywołujące niepokój, stres. Jednym z trudniejszych okresów w życiu młodego człowieka jest moment przejścia ze szkoły średniej do uczelni wyższej. Wiąże się z dużym stresem związanym m.in. ze zmianą otoczenia, brakiem pewności siebie w nowej sytuacji, zmianą znajomych i stylu życia. Ma miejsce częstsze przebywanie poza domem rodzinnym, zmniejszenie wpływu rodziców na dokonywane wybory, chęć „pokazania się” wśród rówieśników, chęć bycia podziwianym przez nowe środowisko oraz chęć stania się bardziej dorosłym. Wciąż zbyt wielu młodych ludzi szuka rozwiązania tego problemu sięgając po papierosy. Należy stale monitorować zachowania antyzdrowotne młodzieży, obserwować tendencje i analizować przyczyny takich zachowań. Pozwoli to na modyfikację i ulepszanie programów oraz działań prozdrowotnych, którymi należy obejmować młodych ludzi jak najwcześniej, ponieważ styl życia młodego pokolenia w bezpośredni sposób przełoży się na stan zdrowia dorosłej populacji w przyszłości. Na szczególną uwagę zasługuje młodzież tych uczelni (m. in. uczelni sportowych), których absolwenci w przyszłej pracy zawodowej będą kształtować postawy i zachowania korzystne dla zdrowia. Despite numerous anti-smoking campaigns, tobacco smoking in youth population is still a vital problem, entailing serious health-related consequences. Tobacco is a stimulant and tobacco smoking is addictive; the chemical compounds in the smoke are responsible for a plethora of illnesses and ailments of the circulatory, respiratory, gastrointestinal and genitourinary systems. It is also the reason of the disorders of intra-uterine foetal development and impairment of body resistance. Due to, inter alia, the above mentioned factors, the World Health Organization considered addiction to tobacco smoking a separate disease unit. According to International Classification of Diseases (ICD) it is described as “mental and behavioural disorders due to use of tobacco – F17 [1]. Tobacco smoking is often treated as a “remedy” for various circumstances which evoke stress and anxiety. One of the most difficult periods in the life of young people is the moment of starting university education. It is connected with substantial stress due to numerous factors, including the new environment, new friends and the new lifestyle. Young people more often stay out of their families, therefore their choices are less influenced by their parents. They would like to be admired by their peers and become more adult. Many of them try to solve this problem by tobacco smoking. The behaviours which can adversely affect health in youth should be continuously monitored, the trends should be regularly observed and the reasons for such behaviours should be analyzed. This will allow modifications and improvements of the programs and implementation of pro-health activities for youth population as early as possible, as the lifestyles of the young generation will directly translate into health of the future adult population. Special attention should be focused on the students of the universities (including universities of physical education whose graduates in their future career will for the pro-health attitude and behaviours. Cel Dyrektywa 2001/37/WE Parlamentu Europejskiego i Rady z dnia 05.06.2001 roku w sprawie zbliżenia przepisów ustawowych, wykonawczych i administracyjnych Państw członkowskich, dotyczących produkcji, prezentowania i sprzedaży wyrobów tytoniowych nakazuje stale przeprowadzać ocenę bezpośrednich i pośrednich kosztów społeczno-ekonomicznych aktywnego i biernego używania tytoniu, a jej wynik podawać do publicznej wiadomości [2]. W związku z tym prowadzenie badań dotyczących powszechności palenia w różnych grupach wiekowych i społecznych jest uzasadnione. Ponadto znajomość powszechności nałogu i czynników korelujących z paleniem tytoniu jest jednym z pierwszych kroków do skutecznej prewencji oraz skutecznej walki z paleniem tytoniu. W związku z powyższym celem badań jest wykorzystanie regresji logistycznej do: – oceny kierunku zmian, jakie zaszły w odsetku osób palących papierosy na przestrzeni dziewięciolet- Objective According to the Directive 2001/37/WE of the European Parliament and the Council from June 5, 2001 about the unification of statutory, executive and administrative provisions of the member states of the EU, concerning the manufacture, presentation and sale of tobacco products, the direct and indirect socioeconomic costs of active and passive use of tobacco should be assessed and the result of this assessment should be publicly announced [2]. Therefore, studies concerning the frequency of tobacco smoking in different age and social groups are justifiable. Moreover, the knowledge of the prevalence of tobacco use and the factors correlating with tobacco smoking is one of the first steps of successful prevention and fighting tobacco smoking. Materiał i metody Material and methods Badania ankietowe przeprowadzono w latach 20022010 na grupie 1990 studentów pierwszego roku studiów dziennych Akademii Wychowania Fizycznego i Sportu w Gdańsku oraz wśród ich rodziców. Studentów pytano w ankiecie o palenie papierosów (tak/ nie), datę urodzenia, a rodziców o ich wykształcenie (podstawowe, zasadnicze-zawodowe, średnie, wyższe) i sytuację społeczno-ekonomiczną rodziny (bardzo dobra, dobra, średnia, zła). Średni wiek badanych studentów wyniósł 20,48 lat (mediana =20, min.=19, max.=26). Różnice w liczbie obserwacji w poszczególnych etapach analizy wynikają z braku zgody ankietowanego na wykorzystanie danej informacji do celów naukowych. Oceniono kierunek zmian w odsetku palących studentów na przestrzeni lat 2002-2010 oraz siłę i istotność związków między paleniem papierosów przez badanych studentów i sytuacją społeczno-ekonomiczną ich rodzin oraz wykształceniem rodziców. Statystyczne opracowanie materiału przeprowadzono z wykorzystaniem programu Statystyka 6.0. W pracy zastosowano analizę regresji logistycznej oraz nieparametryczny test χ2 [3]. Tworząc model regresji logistycznej dla zmiennej Y oceniono wartość statystyki χ2 dla dobroci dopasowania analizowanego modelu do modelu tylko z wyrazem wolnym. Przyjęto, że model różnił się istotnie od modelu teoretycznego dla p≤0,05. Do oceny istotności wpływu poszczególnych zmiennych niezależnych włączonych do modelu, zastosowano test Walda wykorzystując estymator parametru i jego asymptotyczną wariancję. Iloraz szans (OR=odd ratio), czyli stosunek wystąpienia danego zjawiska na poziomie A do wystąpienia danego zjawiska na poziomie B zmiennej niezależnej wyliczono stosując wzór: A survey was conducted between 2002 and 2010 in a sample of 1990 first year full-time students of the Gdansk University of Physical Education and Sport and their parents. The students were asked whether they smoked (yes/no), about their date of birth while their parents were asked about their education level (primary, vocational, high, university) and socioeconomic status (very good, good, average and bad). The mean age of the sample was 20.48 (median =20, min.=19, max.=26).The differences in the number of observations at each stage of the analysis result from the lack of the subject’s consent to use the information for research purposes. The direction of changes in the percentage of smoking students was assessed between 2002 and 2010 as well as the significance of the relationship between tobacco smoking in the sample of students and the socioeconomic status and education level of their parents. Statistical analysis was conducted using Statistica 6.0. software. Logistic regression analysis and χ2 non-parametric test were used for the study [3]. Creating the model of linear regression for Y variable, the authors assessed the value of χ2statistic for matching goodness of fit of the analysed model and the model with a constant term only. It was accepted that the model significantly differed from the theoretical model for p≤0.05. For assessment of the significance level of the effect of each independent variable included in the model, the Wald test was applied using the parameter estimator and its asymptotic variance. Palenie tytoniu przez studentów uczelni sportowej w latach 2002-2010 Wyniki Results Odnotowano tendencję spadkową w odsetku studentów palących papierosy. W roku 2002 stanowili oni 17,95%, a w roku 2010 już 13,47% (Tab. 1, Ryc. 1). Największy odsetek studentów palących odnotowano wśród matek z wykształceniem wyższym (16,95%), wśród ojców z wykształceniem wyższym (20,30%), w pierwszym roku badań (17,95%) oraz wśród rodzin o bardzo dobrej sytuacji społeczno-ekonomicznej (16,78%) (Tab. 1). Po wykonaniu estymacji metodą Rosenbrocka i quasi-Newtona otrzymano model regresji logistycznej, dla którego wartość różnicy między tak utworzonym modelem złożonym z czterech zmiennych niezależnych (x1, x2, x3, x4) a modelem tylko z wyrazem wolnym jest istotna. Jednak tylko rok badań (x1) i wykształcenie ojca (x3) okazały się istotne statystycznie (Tab. 2). Zatem rozważono prostsze modele: trzy modele złożone z dwóch zmiennych (x1 i x2, x1 i x3, x1 A decremental trend was noted in the percentage of smoking students. The highest percentage of smoking students was noted among those whose mothers’ (16.95%) and fathers’ (20.30%), educational background was higher and among the families with a very good socioeconomic status (16.78%) (Table1). After the estimation using Rosenbrock and quasiNewton methods, a model of logistic regression was obtained, for which the value of the difference between the model composed of 4 independent variables (x1, x2,x3,x4) and the model with a constant term only is significant. However, only the year of survey (x1) and father’s educational background (x3) turned out statistically significant (Table 2). Therefore, simple models, namely three models composed of two variables (x1 i x2, x1 i x3, x1 and x4) were considered. Given the aim Tab. 1. W układach dwóch zmiennych niezależnych, ze względu na istotę badań (ocena kierunku zmian, jakie zaszły w odsetku osób palących papierosy na przestrzeni 9-letnich badań) zawsze jedną ze zmiennych jest rok badań. Powstałe modele złożone z dwóch zmiennych, także różnią się istotnie od modeli jedynie z wyrazem wolnym. Modele logitowe dla par zmiennych zależnych przyjmują postać (Tab. 2): • Logit P =2,33+0,08x1-0,42x2 • Logit P =2,61+0,08x1-0,55x3 • Logit P =2,13+0,11x1-0,299x4. Wyliczone szacunki parametrów dla modeli dwóch zmiennych (Tab. 2) wskazują, że prawdopodobieństwo niepalenia papierosów przez studentów rośnie z każdym kolejnym rokiem badań oraz maleje wraz z wyższym poziomem wykształcenia matki, wyższym poziomem wykształcenia ojca oraz lepszą sytuacją społeczno-ekonomiczną rodziny (Tab. 2, Ryc. 1, 2). Korzystając z jednostkowego ilorazu szans dla zmiennych x1, x2, x3, x4 (Tab. 2) oraz odpowiedniego wzoru (1) zobrazowano zwiększanie się (Ryc. 1) lub zmniejszanie (Ryc. 2) szansy na niepalenie papierosów przez studentów wraz z kolejnymi wyższymi poziomami zmiennych niezależnych. Using a unit odds ratio for the variables: x1, x2, x3, x4 (Table 2) and a proper formula (1), the increase (Figure 1) or decrease (Figure 2) of the odds of non-smoking among students with subsequent higher levels of independent variables was illustrated. Przy nie zmieniających się wartościach zmiennej x1 wraz z kolejnymi wyższymi poziomami wykształcenia matki, wykształcenia ojca i sytuacji społeczno-ekonomicznej rodziny, maleje szansa na niepalenie papierosów przez studenta (Ryc. 2). Iloraz szans na niepalenie papierosów wyniósł (Ryc. 1): – przy niezmieniających się wartościach zmiennej x2 – 1,90115, – przy niezmieniających się wartościach zmiennej x3 – 1,85627, – przy niezmieniających się wartościach zmiennej x4 – 2,40231. 24 changing values of x1 variable at increasingly higher levels of educational background of the student’s mother and father and the socioeconomic status of the family, the likelihood of non-smoking decreases (Figure 2). Tobacco smoking among students of the University of Physical Education and Sport in 2002-2010 Dyskusja Discussion W Polsce w latach 1946-1980 wystąpił wzrost spożycia papierosów z tytoniu w przeliczeniu na jednego Polaka z 1000 sztuk w 1946 roku do 2679 sztuk w 1980 roku. Natomiast od roku 1980 obserwowana jest tendencja spadkowa, tj. z 2679 sztuk papierosów w 1980 roku do 1885 sztuk papierosów w 2007 roku [4]. Także wśród studentów Akademii Wychowania Fizycznego i Sportu w Gdańsku w latach 2002-2010 zaobserwowano tendencję spadkową. Odsetek osób palących papierosy zmalał w tym okresie o 4,5%. Wśród studentów zbadanych w roku 2010 odnotowano blisko dwukrotnie mniejszą szansę na niepalenie niż w 2002 roku (Tab. 2, Ryc. 1). Tendencja spadkowa w odsetku osób palących papierosy i ilości sztuk papierosów wypalanych w przeliczeniu na jednego Polaka, związana jest między innymi z docieraniem do Polski mody na niepalenie oraz wzrostem świadomości społecznej związanej z negatywnymi skutkami zdrowotnymi, ekonomicznymi i społecznymi, jakie niesie ze sobą palenie papierosów. Monitorowanie powszechności palenia papierosów wśród młodzieży jest niezwykle istotne z uwagi na modyfikacje i tworzenie nowych programów prozdrowotnych. Zaobserwowana wśród studentów AWFiS w Gdańsku tendencja spadkowa w odsetku osób palących papierosy w kolejnych latach badań (Tab. 1) obserwowana była także m.in. wśród studentów Wydziału Lekarskiego Akademii Medycznej w Gdańsku [5] oraz wśród studentów Wydziału Lekarskiego Akademii Medycznej w Warszawie [6]. W roku 2002 studenci AWFiS palący papierosy stanowili 17,95% (Tab. 1). Jest to odsetek zbliżony do uzyskanego w 2002 roku przez Hublet i wsp. [7] wśród polskich chłopców w wieku 14-15 lat i niższy niż wśród studentów AMG w latach 2002-2003 (28%) [5]. Rok akademicki 2006/2007 przyniósł jeszcze większe dysproporcje w odsetku palących studentów w AMG i AWFiS. Odsetek studentów palących papierosy w AMG to 32,6% [8], a w AWFiS to 10,41% (Tab. 1). Także wśród studentów Uniwersytetu Medycznego w Lublinie odnotowano wyższy odsetek studentów palących papierosy niż w AWFiS. Wśród studentów Uniwersytetu Medycznego w Lublinie w roku akademickim 2007/2008 papierosy paliło 51% studentów, a w tym samym roku w AWFiS paliło tylko 13,23% studentów pierwszego roku (Tab. 1) [9]. Znacznie wyższy odsetek studentów palących papierosy (w stosunku do studentów AWFiS) odnotowano także w czterech największych uczelniach publicznych Wrocławia (Uniwersytet Wrocławski, Politechnika Wrocławska, Uniwersytet Przyrodniczy, Akademia Medyczna) oraz jednej z niepublicznych pomaturalnych szkół wyższych (Wyższa Szkoła Fizjoterapii)-39,6% [10]. Badania przeprowadzane we Francji wskazują, że aż 43,23% studentów II roku socjologii i medycyny pali papierosy [11]. Także w badaniach pakistańskich studentów (Karachi) odsetek palących jest wysoki i wynosi, w zależności od wieku badanych, od 26,5% (18-20-letni) do 65% (w wieku powyżej 20 lat) [12]. Przeprowadzone 9-letnie badania wśród studentów AWFiS wskazują, że na tle innych polskich studentów oraz studentów francuskich czy pakistańskich (cytowanych prac), rozpowszechnienie palenia tytoniu w gdańskiej uczelni sportowej jest niższe, co jest niezwykle pozytywnym zjawiskiem. Skąd więc tak In Poland, between 1946 and 1980 an increase in consumption of tobacco cigarettes was noted from 1000 in 1946 to 2679 cigarettes in 1980 per capita. Conversely, since 1980, a decremental tendency was observed; it means that tobacco consumption decreased from 2679 cigarettes in 1980 to 1885 cigarettes per capita in 2007 [4]. Also among the students of the University of Physical Education and Sport, a decremental tendency was observed between 2002-and 2010. That time, the percentage of tobacco smokers decreased by 4.5%. Among the students surveyed in 2010, the likelihood of non-smoking was nearly twice lower than in 2002 (Table 2, Figure 1). The decremental tendency in the percentage of smokers and the number of cigarettes per capita is connected, inter alia, with the trend of quitting smoking and raised social awareness of adverse health-related, economic and social consequences of tobacco smoking. Monitoring of the frequency of tobacco smoking among youth is essential given the modifications and the development of new pro-health programs. The decremental trend in the percentage of smokers among the students of the Gdansk University of Physical Education and Sport during subsequent years of the study (Table 1) was also observed, inter alia, among the students of the Faculty of Medicine of the Medical University in Gdansk [5] and among the students of the Faculty of Medicine of Warsaw Medical University [6]. In 2002 there were 17.95% of tobacco smokers among the students of the Gdansk University of Physical Education and Sport (Table 1). This percentage value is similar to that obtained in 2002 by Hublet et al. [7] among Polish boys aged 14-15 years, and lower that that noted among the population of students of the Medical University of Gdansk between 2002 and 2003 (28%) [5]. During the academic year 2006/ 2007 still bigger disproportions were noted in the percentage of smoking students of the Medical University of Gdansk and Gdansk University of Physical Education and Sport. The percentage values corresponding to smoking students at the Medical University of Gdansk and at the Gdansk University of Physical Education and Sport are 32.6% [8] and 10.41% respectively (Table 1). Also among the students of the Medical University of Lublin a higher percentage of smoking students was noted compared with that at the Gdansk University of Physical Education and Sport. Among the students of the Medical University of Lublin, in the academic year 2007/2008, 51% were smokers and, that year, only 13.23% of the first year students of the Gdansk University of Physical Education and Sport were smokers (Table 1) [9]. A significantly higher percentage of smoking students (as compared with the students of the Gdansk University of Physical Education and Sport) was noted in four biggest public universities in Wroclaw (Wroclaw University, Wroclaw University of Technology, Wroclaw University of Environmental and Life Sciences and Wroclaw Medical University) and one of the private postgraduate colleges (College of Physiotherapy) where the corresponding percentage value was 39.6% [10]. The study conducted in France indicates that as much as 43.23% of second year students of sociology and medicine smoke cigarettes [11]. Jedną z przyczyn mogą być lepsze zdrowotne nawyki studentów AWFiS i aktywny (bogaty w zajęcia sportowe) tryb życia. Pankiewicz i wsp.[8] tak wysoki odsetek studentów palących papierosy na pierwszym roku studiów w AMG tłumaczą m.in. kryzysem tożsamości, zmianą środowiska, próbą zamanifestowania własnej niezależności, chęcią zaakceptowania przez nową, nieznaną grupę rówieśniczą, dużym obciążeniem nauką i chęcią oddalenia się od obarczeń emocjonalnych. Argumenty te można niewątpliwie przypisać także studentom I roku AWFiS. Jednak studenci AWFiS znacznie więcej czasu poświęcają na aktywność fizyczną, która, jak wiadomo, jest jedną z metod pozwalających zrelaksować się, nabrać dystansu do problemów, walczyć ze stresem. AWFiS jest uczelnią sportową, a więc już sam program nauczania bogaty jest w zajęcia sportowe, ponadto w badanej grupie młodzieży AWFiS 54,11% studentów, pod kierunkiem trenera, regularnie uczestniczy w dodatkowych zajęciach sportowych, nieobjętych programem nauczania, trwających minimum rok. Natomiast 34,58% kiedyś uczestniczyło w takich zajęciach, a obecnie już nie, głównie z uwagi na obciążenia wynikające z podjęcia nauki w uczelni wyższej oraz zmianę środowiska (oddalenie od macierzystych klubów sportowych). Zaobserwowany w badanym okresie wzrost odsetka studentów AWFiS palących papierosy wraz ze wzrostem poziomu wykształcenia matki (OR=0,57452) i ojca (OR=0,65929) (Tab. 2) być może związany jest z lepszą sytuacja materialną rodziców lepiej wykształconych. Związek między poziomem wykształcenia ojca i sytuacją społeczno-ekonomiczną rodziny jest istotny statystycznie (χ2=111,74; p=0,0000). Również związek między poziomem wykształcenia matki i sytuacją społeczno-ekonomiczną rodziny jest istotny statystycznie (χ2=102,1816; p=0,0000). Studenci pochodzący z rodzin o lepszej sytuacji społeczno-ekonomicznej prawdopodobnie otrzymują większe środki finansowe na utrzymanie, co pozwala na zakup papierosów. Jak wskazują badania Friedel i Bloomberg [13,14] podwyższanie cen wyrobów tytoniowych jest jedną z najskuteczniejszych metod zmniejszania konsumpcji tytoniu, szczególnie wśród ludzi młodych. Natomiast w Polsce ceny papierosów są jeszcze jednymi z najniższych w Unii Europejskiej. Podobnie, jak wśród studentów AWFiS, także w badaniach Siemińskiej i wsp. [5] zaobserwowano wśród studentów I roku AMG wzrost odsetka studentów palących papierosy wraz z wyższym poziomem wykształcenia matki lub ojca. Największy odsetek studentów palących papierosy, autorzy odnotowali wśród ojców z wykształceniem wyższym (26,59%) i matek z wykształceniem wyższym (25,9%) [5]. Analogicznie wśród studentów AWFiS, największy odsetek studentów palących papierosy odnotowano wśród ojców (20,3%) i matek z wykształceniem wyższym (16,95%) (Tab. 1, Tab. 2). 26 students (Karachi) indicates that the percentage of smoking students is high and depends of age. These values range from 26.5% (18-20 year olds) to 65% (above 20 years of age) [12]. The 9-year study presented in this paper, conducted among the students of the Gdansk University of Physical Education and Sport indicates that, compared with other Polish as well as French and Pakistani students (cited papers), the prevalence of tobacco smoking at the sport university of Gdansk is lower, which is a very positive phenomenon. We wonder why the differences in the percentage of smoking students at Gdansk University of Physical Education and Sport and other selected universities in Poland are so big. Pankiewicz et al. [8] attribute the high percentage of tobacco smokers among first year students of Gdansk Medical University to, inter alia, identity crisis, new environment, an attempt to manifest being independent, the desire to be accepted by the new, an unknown group of peers, overburden with duties and the desire to ease emotional burden. Such argumentation also concerns the first year students of Gdansk University of Physical Education and Sport. However, these students devote a lot of time to physical activity which obviously is one of the methods allowing them to relax, distance themselves from their problems and fight stress. The Gdansk University of Physical Education and Sport is a sport university, so the sole curriculum includes involvement in various sports classes. Moreover, in the reported group of students of the Gdansk University of Physical Education and Sport, 54.11% regularly participated in extracurricular activities with a trainer within minimum one year. 34.58% used to participate in such activities, but they quit, mainly due to being overburdened at the university or due to the new environment (their sports clubs were too far away from the place they lived). The increase in the percentage of smoking students of the Gdansk University of Physical Education and Sport, directly proportional to educational background of the student’s mother (OR=0.57452) and father (OR=0.65929) (Table 2), may be connected with a better economic status of parents with higher educational background. The relationship between the educational background of the student’s father and socioeconomic status of his/her family is statistically significant (χ2=111.74; p=0.0000). Also the relationship between the educational background of the student’s mother and the socioeconomic status of his/ her family is statistically significant (χ2=102.1816; p=0.0000). The students from families of higher socioeconomic status have more money for living, thus they can afford buying cigarettes. The results obtained by Friedel and Bloomberg [13,14] indicate that rising the prices of tobacco products is one of the most effective methods of decreasing tobacco consumption, particularly in young people. Conversely, in Poland, prices of cigarettes are among the lowest ones in the EU. A trend, similar to that observed in the students of the Gdansk University of Physical Education and Sport, was noted by Siemińska et al. [5] among first year students of the Medical University of Gdansk. In this sample, the percentage of tobacco smokers increased proportionally to the educational background of the student’s mother or father. Analogically, among the students of Gdansk University of Physical Education and Sport, the highest percentage of smoking students was noted among fathers (20.3%) and mothers (16.95%) with higher educational background (Tables 1 and 2). Wnioski Conclusions 1. Zauważalna tendencja spadkowa w odsetku studentów AWFiS palących papierosy w kolejnych latach badań (2002-2010) jest zjawiskiem pozytywnym. Tendencja ta zauważalna jest w Polsce, a także wśród innych państw Unii Europejskiej. Mimo to, z uwagi na poważne konsekwencje zdrowotne związane z nikotynizmem edukacja antynikotynowa powinna być nadal niezbędnym elementem nauczania, zwłaszcza w tych jednostkach oświatowych, które kształcą ludzi odpowiedzialnych za promocję zdrowego stylu życia. Należy pamiętać, jak niezwykle ważne jest zachowanie czujności w kwestii palenia papierosów przez młodzież, nawet w tych regionach i grupach, w których odnotowano stabilizację lub trend spadkowy w odsetku osób palących. 2. Dokonana w pracy ocena związku między paleniem papierosów przez studentów AWFiS i sytuacją społeczno-ekonomiczną ich rodzin oraz wykształceniem rodziców wskazuje, że palenie papierosów w dużym stopniu związane jest z bardzo-dobrą i dobrą sytuacją społeczno-ekonomiczną ich rodzin oraz wyższym poziomem wykształcenia rodziców. W związku z tym działaniami propagującymi zdrowy styl życia, uświadamiającymi jakie zagrożenia zdrowotne niesie za sobą palenie papierosów, należy objąć zwłaszcza młodzież wywodzącą się z rodzin dobrze wykształconych. 1. The apparent decremental tendency in the percentage of tobacco smoking students of the Gdansk University of Physical Education and Sport in consecutive years of the study (2002-2010) is a positive phenomenon. This tendency has been noticed both in Poland and in other EU countries. Despite this fact, due to serious health-related consequences of tobacco smoking, anti-smoking education should be still a necessary component of education, particularly in the units where people responsible for healthy lifestyle promotion are educated. We should remember how important it is to control tobacco smoking among youth, even in the regions and groups where stability or a decremental trend in the percentage of smokers were noted. 2. The assessment of the relationship between tobacco smoking by the students of the Gdansk University of Physical Education and Sport and the socioeconomic status and educational background of their parents indicates that tobacco smoking is connected to a high extent with a good and very good socioeconomic status and high level of education of their parents. The actions aimed at healthy lifestyle promotion and raising people’s awareness of health-related consequences of tobacco smoking should be mainly focused on youth from families of high educational background.
