Celem pracy było zbadanie zależności między występowaniem agenezji trzecich zębów trzonowych a płcią oraz między lokalizacją braków zawiązków a płcią. Materiał i metody. Badany materiał w zakresie dotyczącym zagadnienia agenezji trzecich zębów trzonowych stanowiło 381 losowo wybranych zdjęć pantomograficznych (215 dziewcząt i 166 chłopców) w wieku 10–18 lat. Natomiast Abstract Aim. To examine the dependence among agenesis, degree of development of third molar buds with their localization and sex and examine the correlation between the phases of their formation in relation to age. Material and methods. Ocenę fazy rozwoju zawiązków trzecich zębów trzonowych dokonywano na podstawie klasyfikacji wg Moorreesa (5). Wyniki poddano analizie statystycznej. Wyniki. W badanym materiale stwierdzono agenezję co najmniej jednego zawiązka trzeciego zęba trzonowego u 30% dziewcząt i 23% chłopców. U dziewcząt stwierdzono brak 15,1% zawiązków w szczęce i 16,5% zawiązków trzecich zębów trzonowych w żuchwie. U chłopców wystąpił brak 14,2% zawiązków w szczęce i 11,7% zawiązków trzecich zębów trzonowych w żuchwie. U obu płci szybkość rozwoju zawiązków była porównywalna. Podsumowanie. W badanym materiale nie stwierdzono statystycznie istotnych różnic między płcią a agenezją trzecich zębów trzonowych oraz między lokalizacją braków zawiązków a płcią. W badanej grupie agenezja trzecich zębów trzonowych wystąpiła u 24% dzieci i młodzieży. W tej grupie stwierdzono istotną statystycznie korelację między wiekiem i fazą rozwoju zawiązka. Wyniki skłoniły autorów do dalszych badań przedstawionego wcześniej materiału. Większość autorów podaje, że w obrazie radiologicznym zawiązki trzecich zębów trzonowych są widoczne między 7 a 10 rokiem życia (1, 2, 3, 4), w związku z tym dalszą analizę dotyczącą agenezji trzecich zębów trzonowych przeprowadzono tylko na zdjęciach pantomograficznych pacjentów w wieku 10–18 lat. Natomiast badaniami dotyczącymi rozwoju zawiązków objęto całą 500-osobową grupę. Cel pracy Celem pracy było zbadanie zależności między występowaniem agenezji trzecich zębów trzonowych a płcią, lokalizacją braków zawiązków oraz zbadanie korelacji fazy rozwoju zawiązka z wiekiem oraz z jego lokalizacją. and Material related to the buds’ development evaluation covered all group – 500 patients aged 8-18 years. Identified buds were assessed according to the Moorrees classification. Results were statistically analyzed. Results. In investigated material agenesis at least of one third molar was found in 30% of girls and 23% of boys. For girls, there was lack of 15,1% of buds in maxilla and 16,5% of third molars’ buds in mandible. For boys, there was lack of 14,2% of buds in maxilla and 11,5% of third molars’ buds in mandible. For both sexes, the rate of growth of buds was comparable. Conclusion. In the investigated material there were not statistically significant differences between sex and agenesis of third molars, and between the localization of buds’ gaps and sex. In the study group the agenesis of third molars was present in 24% children and adolescents. In this group a statistically significant correlation between the age and the development phase of tooth buds was observed. The results made the authors conduct further studies on the material presented earlier. The majority of authors state that third molar tooth buds are visible between the 7th and 10th year of age (1, 2, 3, 4) on radiological images, therefore a further analysis of the agenesis of third molars was performed only on pantomographic scans of patients aged 10-18 years. On the other hand, studies regarding the development of tooth buds involved the whole group of 500 patients. Materiał i metody Material and methods Badany materiał stanowiło 381 losowo wybranych zdjęć pantomograficznych (215 dziewcząt i 166 chłopców) w wieku 10–18 lat, którzy byli leczeni w Zakładzie Ortodoncji Warszawskiego Uniwersytetu Medycznego (analizowano tylko zdjęcia pacjentów z pełnym uzębieniem). Natomiast badany materiał dotyczący rozwoju zawiązków obejmował całą 500-osobową grupę w wieku 8–18 lat. Ocenę fazy rozwoju zawiązków trzecich zębów trzonowych dokonywano na podstawie klasyfikacji wg Moorreesa (5). Uzyskane dane poddano analizie statystycznej, którą przeprowadzono osobno dla szczęki i żuchwy w grupach dziewcząt i chłopców. Dla każdej z ocenianych faz rozwojowych trzecich zębów trzonowych obliczono średnią wieku pacjentów wraz z odchyleniem standardowym; znajdowano wartości minimalne i maksymalne obserwowanego przedziału wieku pacjentów oraz obliczano wariancję, medianę, kwartyle – pierwszy i trzeci. Wyniki wykorzystano do wykreślenia wykresu pudełkowego (box-and-whisker plot), dokonano także porównania uzyskanych prostych regresji. Na wykres naniesiono wartości średnich dla poszczególnych faz rozwoju i obliczono prostą regresji wraz ze współczynnikiem R2. Wartość R2 to liczba od 0 do 1, która pokazuje, jak bardzo szacunkowe wartości dla trendu odpowiadają rzeczywistym danym. Linia trendu jest najbardziej wiarygodna, gdy wartość R2 jest równa lub bliska 1. Study material with regard to the analysis of the agenesis of third molars included 381 randomly selected pantomographic scans of patients aged 10 - 18 years (215 girls and 166 boys) treated at the Department of Orthodontics, Warsaw Medical University (only scans of patients with complete dentures were analysed). On the other hand, the study group with regard to the analysis of tooth bud development included the whole group of 500 patients aged 8-18 years. The evaluation of a development phase of third molar tooth buds was performed based on the Moorrees classification system (5). The obtained results were subject to a statistical analysis. The statistical analysis was performed individually for the maxilla and mandible in groups of boys and girls. For each development phase of third molars being assessed the mean age of patients with a standard deviation was calculated, minimum and maximum values of the observed age compartment of patients were found, and the variance, median, first and third quartile were calculated. The results were used to plot a box-and-whisker graph and moreover, regression lines were compared. The plot also presented the values of means for individual development phases and a regression line with the R2 coefficient was calculated. The R2 value – is the number from between 0 and 1 demonstrating the degree to which estimated trend values reflect real data. A trend line is the most reliable when the R2-square value is equal or close to 1. Results Wyniki In the studied material the agenesis of at least one third molar tooth bud was observed in 30% of girls and 23% of boys (tab.1). Pietrzak-Bilińska, M. Bilińska et al. U pacjentów w analizowanym przedziale wiekowym zbadano także różnice występowania agenezji w szczęce i w żuchwie. U dziewcząt stwierdzono brak 15,1% zawiązków w szczęce i 16,5% zawiązków trzecich zębów trzonowych w żuchwie. Natomiast u chłopców wystąpił brak 14,2% zawiązków w szczęce i 11,7% zawiązków trzecich zębów trzonowych w żuchwie. U chłopców różnica procentowa między występowaniem braków zawiązków szczęce i żuchwie (z przewagą w żuchwie) była nieznacznie większa . Wyniki ilustruje tabela 2. W całej grupie 10–18 lat stwierdzono brak 14,7% zawiązków w górnym łuku zębowym i 14,4% zawiązków w dolnym łuku zębowym. W badanej grupie nie stwierdzono istotnych statystycznie różnic między lokalizacją braków zawiązków w szczęce i w żuchwie (tab. 3). W kolejnym etapie badań oceniono fazy rozwoju obecnych zawiązków trzecich zębów trzonowych u dziewcząt i chłopców w wieku 10–18 lat. Wyniki ilustruje tabela 4. Ocenę statystyczną faz rozwoju zawiązków trzecich zębów trzonowych, stwierdzonych w całej 500-osobowej badanej grupie dzieci w wieku 8–18 lat, umieszczono w tabelach 5, 6, 7, 8. Uzyskane dane pokazano w formie wykresów pudełkowych na rycinach 1–4. In patients in the analysed age group of 10-18 years the differences with regard to the agenesis in the mandible and the maxilla were also analysed. In girls the lack of 15.1% of tooth buds in the maxilla was observed and 16.5% of third molar tooth buds in the mandible. On the other hand, in boys the lack of 14.2% of tooth buds in the maxilla was observed and 11.7% of third molar tooth buds in the mandible. In boys a percentage difference between missing tooth buds in the mandible and maxilla was slightly larger (more in the mandible). Table 2 presents the results. In the whole age group of 10-18 years the lack of 14.7% of tooth buds in the upper dental arch, and 14.4% of tooth buds in the lower dental arch was observed. In the study group there were no statistically significant differences between the location of missing tooth buds in the maxilla and mandible (tab. 3). In the subsequent part of studies development phases of present third molar tooth buds were assessed in boys and girls aged 10-18 years. Table 4 presents the results. Tables 5, 6, 7, 8 present a statistical analysis of development phases of third molar tooth buds in the whole group of 500 patients including girls and boys aged 8-18 years. Dyskusja Discussion W badanym materiale w grupie młodzieży w wieku 10–18 lat agenezja trzecich zębów trzonowych występowała u 30% pacjentów (nieznacznie częściej u dziewcząt niż u chłopców). Natomiast Rozkovcowa i wsp. agenezję ósemek stwierdzili u 22,5% pacjentów, częściej u chłopców (6). Według badań Fekonja i wsp. braki trzecich zębów trzonowych występowały u 6–10% badanych (7), a według badań Bhawendeep i wsp. agenezja tych zębów dotyczyła aż u 44,8% pacjentów (8). Badania Kajii i wsp. wykazały, że brak zawiązków w szczęce występuje częściej niż w żuchwie i jest związany z przednio-tylnym wymiarem szczęki. Wykluczyli oni istnienie korelacji między długością żuchwy a agenezją trzecich zębów trzonowych (1, 9). Tucker i wsp. wykazali istnienie różnic w kodzie genetycznym dla zębów górnych i dolnych. Mniejszy odsetek braku trzecich zębów trzonowych w żuchwie, wcześniejszy termin formowania się zawiązków zębów dolnych niż górnych, brak korelacji ich rozwoju z długością żuchwy oraz różnice w podłożu genetycznym mogą wskazywać pewną niezależność zębów trzecich trzonowych w żuchwie od ewolucyjnej eliminacji – w przeciwieństwie do ich odpowiedników w szczęce (10). Podobne wyniki uzyskał Orhan i wsp. W badanej przez nich grupie 26% pacjentów nie miało zawiązków w żuchwie i aż 45% – w szczęce (11). Odwrotne tendencje przedstawiła w swojej pracy Rozkovcova i wsp. W ich materiale wystąpiła nieznaczna przewaga braku zawiązków trzecich zębów trzonowych w żuchwie (6). W przedstawionym przez nas materiale nie stwierdzono statystycznie istotnych różnic między lokalizacją braków zawiązków (14,7% – w szczęce i 14,4% – w żuchwie). W badanym materiale podczas oceny faz rozwoju zawiązków nie stwierdzono także statystycznie istotnej korelacji między daną fazą rozwoju a płcią i lokalizacją zawiązka. Podobne wyniki przedstawili w swoich pracach Orhan i wsp. (11) oraz Olze i wsp. (12). Inne wyniki uzyskali Bolanos i wsp. (13), Bang-Seob i wsp. (14), Meinl i wsp. (15) Lee i wsp. (16) Arany i wsp. (17), Bai i wsp. (18) oraz Zeng i wsp. (19). W swoich materiałach stwierdzili oni wcześniejszą dojrzałość zawiązków w szczęce oraz obserwowali fazę zakończonego rozwoju korzenia wcześniej u pacjentów płci męskiej (13, 14, 15, 20). Wyniki uzyskiwane przez cytowanych autorów znacznie różnią się między sobą. Najprawdopodobniej jest to uzależnione między innymi od wielkości badanej populacji, rasy i strefy geograficznej, którą zamieszkuje poddana badaniu grupa. Podsumowanie 1. W badanym materiale nie stwierdzono statystycznie istotnych różnic między płcią a agenezją trzecich zębów trzonowych. In the study material in the group of adolescents aged 10-18 years the agenesis of third molars was present in 30% of patients (slightly more often in girls than in boys). On the other hand, Rozkovcowa et al. observed the agenesis of third molars in 22.5% of patients, and it was more often observed in boys (6). Based on the studies by Fekonja et al. missing third molars were present in 6-10% of subjects (7). In the studies by Bhawendeep et al. the agenesis of these teeth was present in as many as 44.8% of patients (8). Based on the studies by Kajii et al. it can be concluded that missing tooth buds in the maxilla appear more often than in the mandible and this phenomenon is associated with the anteroposterior dimension of the maxilla. They excluded a correlation between the mandible length and the agenesis of third molars (1, 9). Tucker et al. demonstrated differences with regard to a genetic code for the upper and lower teeth. A lower rate of missing third molars in the mandible, earlier formation of tooth buds of lower teeth compared to upper teeth, lack of a correlation between their development with the mandible length and differences with regard to the genetic background may indicate that third molars in the mandible are partially independent from evolutionary elimination, contrary to their counterparts in the maxilla (10). Similar results were obtained by Orhan et al. In the group they studied 26% of patients had missing tooth buds in the mandible and as many as 45% in the maxilla (11). Contrary tendencies were presented by Rozkovcova et al. in their paper. In their material missing third molar tooth buds in the mandible were slightly more predominant (6). In our material there were no statistically significant differences between the location of missing tooth buds (14.7% in the maxilla and 14.4% in the mandible). With regard to the study material during the assessment of development phases for tooth buds there were no statistically significant correlations observed between a given development phase and sex or location of a tooth bud. Similar results were presented by Orhan et al. (11) and Olze et al. (12) in their papers. Other results were observed by Bolanos et al. (13), Bang-Seob et al. (14), Meinl et al. (15) Lee et al. (16) Arany et al. (17), Bai et al. (18) and Zeng et al. (19). In their material they described early maturation of tooth buds in the maxilla, and observed a phase of complete root development earlier in male patients (13,14,15,20). The results obtained by authors quoted are extremely varied. It is probably associated with the size of a studied population, race and geographical zone where the studied group lives. Summary 1. In the study material there were no statistically significant differences between the sex and the agenesis of third molars.
