Na podstawie wywiadu, badania klinicznego i dodatkowych badań diagnostycznych ustalono indywidulny plan leczenia ortodontycznego dla każdego pacjenta. Średni czas terapii wynosił 2 lata. U pacjentów przed i po leczeniu wykonano cefalogramy boczne w maksymalnej interkuspidacji, ze swobodnie ułożonymi wargami. Pacjentów podzielono na dwie grupy w zależności od wartości nagryzu poziomego przed leczeniem. Do grupy A zakwalifikowano pacjentów z nagryzem poziomym do 5 mm, do grupy B pacjentów z nagryzem poziomym ponad 5 mm. Na cefalogramach bocznych oceniono zmianę położenia siekaczy górnych i dolnych podczas leczenia ortodontycznego oraz towarzyszące zmiany w pozycji tkanek miękkich. Metoda: retrospektywne porównanie zmian zębowych i zmian miękkich tkanek na cefalogramach bocznych. Wyliczono średnią arytmetyczną dwóch pomiarów dla każdego parametru, przeprowadzonych w odstępie dwóch tygodni. Wyniki: W grupie A nie doszło do istotnych statystycznie zmian w pozycji tkanek miękkich. W grupie B doszło do istotnej statystycznie zmiany położenia górnej i dolnej wargi. Wnioski: Cefalogram boczny ma zastosowanie do oceny zmian położenia tkanek twardych i miękkich u pacjentów leczonych ortodontycznie. Większe zmiany w pozycji siekaczy i tkanek miękkich zaobserwowano w grupie B. (Forum Ortod. 2011; 7: 140 - 150). Olomouc. Based on anamnesis, clinical examination and additional diagnostic examination, an individual plan of orthodontic tratment was established for each patient. The mean treatment time was 2 years. The patients were subjected to lateral cephalograms made in maximal intercuspation with lips relaxed before and after the treatment. The group of patients was divided into 2 groups in terms of the the overjet value before the treatment. Group A included patients with the overjet up to 5 mm, whereas group B consisted of patients with the overjet greater than 5 mm. Selected dental and soft tissue parameters before and after an active phase of the orthodontic treatment were evaluated in each patient. Method: Involved a retrospective comparison of the dental and soft tissue parameters analysis on lateral cephalograms. The parameters were measured again after 2 weeks. The mean value for each parameter was used. Results: In group A no statistically relevant changes in the position of soft tissue were observed. In group B statistically relevant changes in the position of upper and lower lips occured. Conclusions: Lateral cephalogram analysis is very useful in evaluating facial esthetics as well as in planning orthodontic treatment. Greater changes in the incisors and the soft tissue profile position were observed in group B. (Orthod. Forum 2011; 7: 140 -150). Jak najlepszy estetyczny wynik leczenia powinien dotyczyć zarówno zgryzu jak i twarzy, której poszczególne elementy powinny pozostawać w harmonii (1,2,3). Leczenie pacjentów z powiększonym nagryzem poziomym wymaga poprawy sagitalnej relacji łuków zębowych, korekty stłoczeń i przechylenia górnych siekaczy (4,5). Terapia często związana jest z ekstrakcjami dwóch górnych (kamuflaż ortodontyczny) lub czterech przedtrzonowców (w przypadku dużych stłoczeń w łuku dolnym). Na przestrzeni lat zmieniały się kanony dotyczące estetyki twarzy. Jej oceny można dokonywać na cefalogramach bocznych, przednio-tylnych oraz na fotografiach (6). W latach 70. i 80. zauważono, że leczenie ekstrakcyjne prowadzi do niekorzystnego estetycznie spłaszczenia profilu i pogorszenia rysów twarzy (7). The aim of orthodontic treatment is to achieve and preserve maximal esthetic harmony and functional occlusion (1). The best esthetical result of the treatment should concerning the occlusion as well as the face whose particular parts should remain in harmony (1,2,3). The treatment of patients with increased overjet requires the correction of a relationship of dental arches to eliminate crowdings, to rise bite and to retrude upper incisors (4,5). The treatment is often connected with the upper (camouflage treatment) or the upper and lower (severe crowding in lower arch) premolars extraction. Over a distance of years the standards of facial esthetics have changed. The evaluation of the face may be done by means of lateral or anterior-posterior cephalograms and photographs (6). Ocena estetyki jest trudna i subiektywna. Twarz zmienia się z wiekiem. Sarver uważa, (9) że estetyczny profil, zwłaszcza u młodych ludzi powinno cechować nieznacznie większe wysunięcie górnej wargi w stosunku do dolnej oraz położenie dolnej wargi i bródki mniej więcej na jednej linii w płaszczyźnie strzałkowej, przy czym dolna warga powinna leżeć nieznacznie doprzednio względem tkanek miękkich bródki. Przekonanie to jest związane z badaniami Mammandrasa z 1988 roku (10) i Nandy z 1990 roku (11), według których nos i broda rosną nieprzerwanie przez całe życie, zaś wargi już po skoku wzrostowym stają się coraz bardziej płaskie i cienkie. Właściwe planowanie leczenia pozwala uniknąć „postarzenia” pacjenta. Według Czarneckiego podstawową zasadą jest, aby estetyki poszczególnych części twarzy nie oceniać w oderwaniu od całości, a leczenie młodzieży powinno uwzględniać przyszłe zmiany tkanek miękkich nosa, warg i bródki (3). Najczęściej w literaturze autorzy badają wpływ leczenia ortodontycznego na profil twarzy pacjenta porównując pacjentów leczonych bez ekstrakcji zębów z pacjentami, u których leczenie ortodontyczne było skojarzone z usunięciem czterech przedtrzonowców. Porównując wyniki po leczeniu ortodontycznym z usunięciem czterech przedtrzonowców stwierdza się w tej grupie bardziej cofnięte wargi: zarówno górną jak i dolną, w porównaniu z grupą nieekstrakcyjną. W grupie pacjentów, u których wykonano ekstrakcje obserwuje się tendencję do bardziej prostego profilu i nieznacznie bardziej wyprostowanych siekaczy szczęki i żuchwy, podczas gdy w grupie pacjentów leczonych bez usuwania zębów tendencje są odwrotne. Jednak decyzja o ekstrakcjach nie musi oznaczać automatycznie pogorszenia profilu twarzy (12, 13). Długofalowe badania pokazują, że progresywne spłaszczenie profilu twarzy obserwuje się w obu grupach. Może to być związane z dojrzewaniem, ciągłym wzrostem żuchwy i nosa, ale ekstrakcja zębów wydaje się nie mieć na ten proces wpływu (7). Według Bishary (14) leczenie ortodontyczne przynosi korzystny efekt w obu badanych grupach, zarówno krótko- jak i długoterminowy. Ostatnio pojawia się coraz więcej prac badających wpływ ekstrakcji dwóch górnych przedtrzonowców na profil twarzy pacjenta (8,15,16,17,18). Conley (8) uważa, że usunięcie górnych przedtrzonowców w leczeniu za pomocą kamuflażu ortodontycznego może być dobrą metodą leczenia, szczególnie u pacjentów, którzy mają pełne usta. Bokas (15) zauważył jedynie niewielkie zmiany w pozycji górnej wargi po leczeniu z ekstrakcjami górnych przedtrzonowców. of the profile and the deterioration of the face look (7). Currently, some authors such as Nanda, Conley (8) have noticed a trend towards the nonextraction treatment. Nowadays, according to Sarver (9) an esthetical profile is characterized by the imperceptible anterior position of the upper lip in comparison with the lower lip, and the position of the lower lip and the chin more or less in one line on the sagittal plane, but the lower lip should lie imperceptibly more forward than the soft tissue of the chin. This statement is supported by Mamandras` (1984) and Nanda`s (1990) researches (11) according to them the nose and the chin grow consecutively throughout the all life, but the lips are more and more flat and thin just after the pubertal acceleration. According to Czarnecki the basic rule is not to evaluate the esthetic of particular parts of the face separately and to foresee in the treatment of adolescents the upcoming changes of the soft tissue of the nose, lips and the chin (3). In the literature, authors generally evaluate the influence of the orthodontic treatment on the profile of a patient by comparing patients treated without extraction with those whose orthodontic treatment was associated with the four premolars extraction. Comparing the results after the orthodontic treatment connected with the extraction of four premolars, it was stated that in this group lips were more retruded: the upper- and the lower-one and, contrary to more protruded lips in the nonextraction group. The extraction group shows a tendency for a more straight profile and imperceptibly more retruded upper and lower incisors, whereas in the nonextraction group the tendencies are the opposite. However, the decision on extractions does not have to mean automatically the deterioration of the face profile (11,12). The long term evaluation stated that the progressive flattening of the face profile is observed in both groups. It may be associated with maturational changes associated with the continuing mandibular growth and nasal development and it is not influenced by the fact whether or not the teeth were extracted (7). According to Bishara (14) the orthodontic treatment has a beneficial effect in both groups in a short term as well as in a long one. Lately there have been more and more articles whose authors investigate the influence of the two upper premolars extraction on the patients face profile (8,15,117,18). Conley (8) states that the extraction of two upper premolars during the treatment by camouflage may be a good method for the orthodontic treatment, especially in patients who have full lips. Bokas (15) notices only minor changes in the position of the upper lip after the treatment with the extractions of upper premolars. After the orthodontic treatment patients expect: a nice smile, an improvement of the face, regular occlusion, therefore the aim of the study was the evaluation of the influence of the orthodontic treatment of the increased overjet on the patients` face profile, especially on the lips` position. Materiał i metoda Material and method Zbadano 82 pacjentów leczonych w Przychodni Ortodontycznej Lek-Med w Warszawie oraz w Zakładzie Ortodoncji Uniwersytetu Palackého w Ołomuńcu - 53 kobiety i 29 mężczyzn. Średnio czas leczenia wynosił 2 lata. Zakwalifikowano cefalogramy boczne wykonane w maksymalnej interkuspidacji, ze swobodnie ułożonymi wargami. U każdego pacjenta dokonano analizy wybranych parametrów zębowych (5) i tkanek miękkich przed rozpoczęciem leczenia i po zakończeniu aktywnej fazy leczenia ortodontycznego. Pacjentów podzielono na 2 grupy zależnie od wielkości nagryzu poziomego przed leczeniem ortodontycznym. Grupę A – stanowiło 41 pacjentów z nagryzem poziomym – OJ do 5 mm (OJ ≤ 5 mm; średnio 3,71 mm przed leczeniem). Czas leczenia wynosił średnio 23 miesiące (od 12 do 41 miesięcy). Pacjenci byli leczeni za pomocą stałych aparatów według indywidualnego planu leczenia. W grupie A nie planowano przechylenia siekaczy ani zmian w profilu twarzy. U 16 pacjentów wykonano ekstrakcje przedtrzonowców (39%), zależnie od indywidualnych wskazań tak, aby uzyskać pierwszą klasę kłową i skorygować stłoczenia. Grupę B - stanowiło 41 pacjentów z powiększonym nagryzem poziomym - OJ ponad 5 mm ( OJ > 5 mm, średnio 9,43 mm), a czas leczenia wynosił średnio 2 lata, 3 miesiące (od 16 do 50 miesięcy). Pacjenci byli leczeni według indywidualnego planu. W grupie B leczenie często było skojarzone z ekstrakcją dwóch górnych przedtrzonowców (31 pacientów, 75,6%) w celu osiągnięcia pierwszej klasy kłowej. W grupie B planowano zmianę inklinacji siekaczy – przechylenie górnych i wychylenie dolnych, mimo jednoczesnego wykonywania strippingu. U każdego pacjenta wykonano analizę cefalogramów bocznych przed leczeniem i po leczeniu ortodontycznym, aby dokonać oceny zmian w położeniu tkanek miękkich w odpowiedzi na zmiany w położeniu siekaczy. Metodą było retrospektywne porównanie zmian twardych i miękkich tkanek na cefalogramach bocznych. Pomiary wykonano na folii acetatowej ułożonej na cefalogramie, za pomocą kątomierza. W badaniu użyto średnią arytmetyczną wyniku dwóch pomiarów dla każdego parametru, przeprowadzonych z odstępem dwóch A group of 82 patients treated in the LEK-MED Clinic in Warsaw and in the Department of Orthodontics in Olomouc was selected: 53 women and 29 men. The mean treatment time was 2 years. The patients were subjected to lateral cephalograms made in maximal intercuspation with lips relaxed. Selected dental and soft tissue parameters were evaluated in each patient before and after an active phase of the orthodontic treatment. The group of patients was divided into 2 subgroups in terms of the the overjet value. Group A included 41 patients with normal or slightly increased overjet: the OJ up to 5 mm (OJ≤5 mm – mean 3.71 mm before the treatment). The mean time of the treatment was 23 months (from 12 to 41 months). The patients were treated by a fixed appliance according to the an individual plan of the treatment. In group A there was not planned the retraction of incisors and changes of a facial profile. The extractions of premolars were performed in 16 patients (39%) according to the individual indications to achieve Class I canine relationship and to correct crowdings. Group B included 41 patients with increased overjet: the OJ bigger than 5 mm (OJ> 5 mm, the mean figure 9.43 mm before the treatment). The mean time of treatment was 2 years 3 months (from 16 to 50 months). The patients were treated by a fixed appliance. In the group B extractions were performed in 31 patients (75.6%) In this group the treatment was often associated with the two upper premolars extraction in order to achieve Class I canine relationship. The change in incisors inclination – the retrusion of the upper one and the protrusion of the lower one was planned in subgroup B despite stripping in the lower arch. The analysis of the cephalograms was made before and after the orthodontic treatment in each patient for the evaluation of the change in the position of the soft tissue as a result of the change in the position of the upper and lower incisors. The method involved a retrospective comparison of the dental and soft tissue parameters analysis on the lateral cephalograms. The measurements were performed on the acetate foil lying on the cephalograms by using an angular gauge. These parameters were measured again after 2 weeks. The mean value for each parameter was used. Dla wszystkich parametrów w obu grupach wyliczono: minimalne i maksymalne wielkości, średnie arytmetyczne, medianę, odchylenie standardowe przed i po leczeniu ortodontycznym. Do oceny statystycznej istotności zmian po leczeniu ortodontycznym dla obu grup zastosowano test parametryczny t-test, a wyniki porównano nieparametrycznym testem Wilcoxa. Wyniki były analizowane za pomocą programu softwar STATISTIKA w Katedrze Biofizyki Wydziału Lekarskiego Uniwersytetu w Ołomuńcu. a b c Forum ortodontyczne statistics methods. The minimum, maximum, arithmetic mean values, the median and the standard deviation before and after the treatment for each parameter were calculated. To assess the reliability of the measurements for each subgroup a pair t-test was used. The validity of the results was proved by using the pair Wilcoxon test. The results of the pair t-test were marked in tables: *** - p<0.001; ** - p<0.01; * - p<0.05; ns - p>0.05. The results were processed with the use of the STATISTIKA software. Z parametrów zębowych statystycznie istotnie zmieniły się: nagryz poziomy i pionowy (dla p<0,001). W tkankach miękkich w grupie A doszło do jedynej dla p<0,001 statystycznie istotnej zmiany w położeniu górnej wargi: doszło do zwiększenia odległości punktu wargi górnej Ls w stosunku do linii Estetycznej E Rickettsa. Nie zmieniło się statystycznie położenie dolnej wargi w stosunku do linii Estetycznej E Rickettsa ani pozostałe poddane analizie parametry. Wyniki dla grupy B przedstawiono w Tabeli 2. W grupie B z parametrów zębowych statystycznie istotnie zmieniły się podobnie jak w grupie A: nagryz poziomy i pionowy dla p<0,001. Wysoce statystycznie istotnie dla p<0,001 zmieniły się następujące parametry tkanek miękkich: SNLs, SnGLs, SnPog`Ls, LsE, LiE, CtgSnLs, Ls+1 opisujących położenie górnej wargi w profilu twarzy. Zmiany istotnie znamienne, dla p<0,01, zaobserwowano dla: SNPog` i LiB`Pog`. Dla p<0,05 zmieniły się statystycznie dwie wartości: Li-1 i PogPog`. Nie zmieniły się statystycznie istotnie dla grupy B następujące parametry tkanek miękkich: SNLi, SnGLi, SnGPog` oraz SnPog`Li. The results of measurements for group A are given in Table 1. As for the dental parameters, the overjet and overbite were statistically changed (p<0.001). As far as the soft tissue parameters are concerned, in group A only one parameter in the position of the upper lip changed statistically relevant (p<0.001). The distance between the upper lip point Ls and the Rickett`s Esthetic line (LsE) increased. There was no significant difference in the position of the lower lip to the Rickett`s Esthetic E line (LiE) and other analysed parameters. The results of the measurements for group B are given in Table 2. As for the dental parameters, the overjet and overbite were statistically changed (p<0.001) similar to group A. As far as the soft tissue parameters are concerned: SNLs, SnGLs, SnPog`Ls, LsE, LiE, CtgSnLs, Ls+1 describing the position of the upper lip in facial profile changed statistically relevant (p<0.001). SNPog`, LiB`Pog` changed statistically relevant for p<0.01. Li-1, PogPog` changed statistically relevant for p<0.05. The parameters of soft tissue as: SNLi, SnGLi, SnGPog` and SnPog`Li were not statistically changed in a significant way in group B. Dyskusja Discussion W grupie A dokonano wychylenia górnych i dolnych siekaczy, dlatego ich wzajemne nachylenie zmniejszyło się, co było statystycznie istotne. W grupie A odległość Ls do linii Rickettsa zwiększyła się średnio o 0,96 mm, a pozycja Li się niezmieniła. Porównując zmiany zębowe ze zmianami w tkankach miękkich w grupie A przed i po leczeniu można stwierdzić, że średnia zmiana w położeniu górnego siekacza wyniosła 2,26o, ale nie doszło do zmiany w położeniu górnej wargi (nie zmieniły się statystycznie: SNLs, SnGLs, SnPog`Ls). Podczas leczenia ortodontycznego w grupie A doszło do wychylenia dolnych siekaczy o 4,26o, ale nie zaobserwowano zmian w położeniu dolnej wargi, nie doszło także do zmiany grubości górnej i dolnej wargi. W grupie B doszło do przechylenia górnych siekaczy i wychylenia dolnych siekaczy. Wzajemne nachylenie górnych i dolnych siekaczy zwiększyło się. Odległość Ls do linii Rickettsa zwiększyła się średnio o 2,63 mm, a odległość Li zwiększyła się średnio o 1,36 mm. W grupie B dzięki przechyleniu górnych siekaczy o 11,02o i zmianie nagryzu poziomego o średnio o 5,95 mm doszło do zmiany w położeniu górnej wargi (SNLs, SnGLs, SnPog`Ls, LsE). W konsekwencji kąt nosowo-wargowy CtgSnLs zwiększył się średnio o 4,26⁰. Wszystkie zmiany były istotne statystycznie. Ponadto inklinacja dolnego siekacza zwiększyła się o 6,04⁰, a w konsekwencji kąt The protrusion of the upper and lower incisors were achieved in group A and the interincizal angle increased in a significant way. The distance between the Ls point and the Rickett`s line increased on average by 0.96 mm, but the position of the Li point did not change. Comparing the dental changes with the changes in the soft tissues in group A before and after the treatment it is stated that the mean change in the position of the upper incisor had 2.26 degrees, but there was not any change in the position of the upper lip (SNLs, SnGLs, SnPog`Ls did not changed statistically relevant. In group A, during the orthodontic treatment the lower incisors were protruded by 4.26 degrees, but no change in the position of the lower lip was noticed, nor did any change in the thickness of the upper and lower lips occur. The retrusion of the upper incisors and the protrusion of the lower incisors were achieved in group B. The distance between the Ls point and the Rickett`s line increased on average by 2.63 mm, and the distance between the Li point and the Rickett`s line increased on average by 1.36 mm. In group B, thanks to the retrusion of the upper incisors by 11.02 degrees and the change of overbite on average 5.95 mm, the change in the position of the upper lip was achieved (SNLs, SnGLs, SnPog`Ls, LiE). W grupie B grubość górnej wargi zwiększyła się średnio o 2,26 mm, a grubość dolnej wargi zmniejszyła się o 0,82 mm. Zmiany były statystycznie istotne. Najbardziej istotną klinicznie zmianą było uzyskanie u pacjentów z grupy B zmiany położenia dolnej wargi, która przed leczeniem „uwięziona” pomiędzy górnymi i dolnymi siekaczami („lip trap”), po leczeniu pokrywała wargową powierzchnię górnych i dolnych siekaczy (Ryc 1.) Zmiany w rysach twarzy mogły być spowodowane mechaniką leczenia, która oprócz przechylenia górnych siekaczy zakładała także podniesienie zgryzu. Zwiększenie kąta nosowo-wargowego może być potencjalnie korzystne u pacjentów z prognacją szczęki (8). Podobnie zwiększenie kąta wargowo-bródkowego u pacjentów z pogłębioną bruzdą wargowo-bródkową oraz wywiniętą wargą dolną może przynieść pozytywny efekt estetyczny, szczególnie u pacjentów z powiększonym nagryzem poziomym i „pułapką wargową”. Uważa się, że leczenie nieekstrakcyjne (19) ma niewielki wpływ na profil, podczas gdy leczenie ekstrakcyjne zależy od protruzji tkanek miękkich przed leczeniem. Im większa początkowa protruzja tym korzystniejszy efekt przynosi leczenie. Leczenie za pomocą ekstrakcji może poprawić estetykę u pacjentów, u których występuje kombinacja stłoczenia i protruzji, jest ono potencjalnie korzystne, gdy wargi są wysunięte 2-3 mm przed linię Estetyczną Rickettsa. Weyrich (18) stwierdził, że ekstrakcja górnych przedtrzonowców oznacza większą retruzję siekaczy niż w przypadku ekstrakcji w obu łukach zębowych. Nie zaobserwował pogorszenia profilu u pacjentów po terapii ekstrakcyjnej. Basciftci i Usumez (20) nie znaleźli żadnych istotnych różnic w profilu i pozycji warg u pacjentów leczonych ekstrakcyjnie i nieekstrakcyjnie. Uważają, że na wynik leczenia ma wpływ szereg czynników, takich jak stłoczenie przed leczeniem, możliwości zakotwiczenia, grubość i napięcie tkanek miękkich. Dlatego stwierdzenie, że ekstrakcje oznaczają automatycznie bardziej retruzyjny lub spłaszczony profil jest nie przez wszystkich akceptowane. Tadic and Woods (21) analizując piśmiennictwo na temat leczenia klasy II stwierdzili, że istnieje wiele możliwości skutecznego leczenia klasy II, ekstrakcje przedtrzonowców niekoniecznie mają bezpośredni wpływ na tkanki miękkie, a jeśli zachodzą zmiany położenia warg w profilu to ich efekt jest pozytywny estetycznie. angle increased on average by 4.26 degrees. All these changes were statistically relevantnt. The inclination of the lower incisors increased by 6.04 degrees and as a consquence, the labiomental angle increased on average by 6.72 degrees. In group B the thickness of the upper lip increased on average by 2.26 mm and the thickness of the lower lip decreased by 0.82 mm. The changes were statistically relevant. Clinically, the essential change was the change in the position of the lower lip, which before the treatment was trapped between the upper and lower incisors („lip trap”) and after the treatment it covered the labial surface of upper and lower incisors (Fig. 1). These changes in the face could be a result of the mechanics of the treatment, which, apart from the retraction of the upper incisors, included also the rising of the bite. The increase in the nasolabial angle could be potentially beneficial in patients with the prognation of maxilla (8), similarly the decrease in the labiomental angle in patients with a deep labiomental fold and everted the lower lip could have a positive esthetical effect. It is thought that nonextraction therapy (19) has a small influence on the profile, whereas the extraction therapy depends on the protrusion of the soft tissue before the treatment. They stated that the result of the treatment is influenced by many parameters, for example, the crowding before the treatment, the possibility of anchorage, the thickness and strain of the soft tissue. Therefore the statement that, the extraction automatically means a more retrusive or flatten profile is not acceptable by all authors. Analysing the literature about the treament of Class II. Tadic and Woods (21) emphasized the conclusions of many articles, according to which there are many possibilities of efficient treatment of Class II and the premolars extractions do not necessarily have a direct influence on the soft tissues, and in addition, if there are any changes in the positon of lips in a profil, their effects are rather positive esthetically. Cefalogram boczny ma zastosowanie do oceny estetyki profilu twarzy. 2. Leczenie ortodontyczne może zmienić profil tkanek miękkich pacjenta. 3. Większe zmiany zębowe i w położeniu tkanek miękkich zaobserwowano w grupie B z nagryzem poziomym ponad 5 mm. 4. Zmiana wartości nagryzu poziomego i pionowego w grupie B spowodowała dystalne przemieszczenie górnej wargi w profilu twarzy oraz zmianę ułożenia wargi dolnej, która przed leczeniem u pacjentów z powiększonym nagryzem poziomym pozostawała pomiędzy górnymi i dolnymi siekaczami, a po leczeniu przemieściła się przed powierzchnię wargową górnych siekaczy. 5. Zmiana w położeniu tkanek miękkich nie odpowiadała zmianie w położeniu siekaczy. W grupie B dolne siekacze wychyliły się i doszło do zwiększenia odległości punktu Li od linii E. 1. Lateral cephalogram analysis is very useful in evaluating the facial esthetics. 2. Orthodontic treatment has an influence on patients` soft tissue profile. 3. Greater changes in the incisors position and in the soft tissue profile were observed in group B with OJ > 5 mm. 4. In group B a change in overjet and overbite caused the distal movement of the upper lip in facial profile and the change of the condition for position and shape of lower lip, which before the treatment were trapped between the upper and lower incisors, and after the treatment covered the labial surface of the upper incisors. 5. A change of the soft tissue profile was not correlated with the change in upper and lower incisors. Z wielu tytułów wieje nudą, inne są przegadane. Tytuł powinien składać się z najwyżej 12 wyrazów, warto poćwiczyć skracanie tytułu. Przykład: zamiast „Analiza wyników badań nad skutecznością leczenia ortopedycznego osób z tyłozgryzem za pomocą aparatów czynnościowych” (14 wyrazów) wystarczy napisać: „ Skuteczność leczenia ortopedycznego tyłozgryzu aparatami czynnościowymi” (6 wyrazów). Tytuł jest wizytówką pracy i nie może zawierać wszystkich informacji. Od tego jest streszczenie, a przede wszystkim treść pracy. Przygotowując artykuł do czasopisma dwujęzycznego, warto najpierw sprawdzić jak będzie wyglądał tytuł pracy w języku angielskim. Jest to bowiem język bardzo precyzyjny, źle znosi przestawienie kolejności wyrazów (szyk zdania), uwypukla zbędne powtórzenia i doskonale nadaje się do ćwiczenia stylu. Służy także do unaocznienia błędów, których autor nie dostrzega, pisząc w języku ojczystym. Przykład: płytkie, czy powierzchowne zatrzymanie zęba w kości? Autor wybrał wyraz „płytkie”, co anglista przetłumaczył jako „shallow”, zamiast „superficial” i dopiero wtedy została ujawniła się niezręczność językowa, bo płytki może być zbiornik wodny (jezioro, kałuża), ale nie ząb w kości.
