Wieloœæ i nasilenie tych schorzeñ jest czêsto przeciwwskazaniem do wczesnej rehabilitacji po udarze mózgu. Rzadkim przypadkiem jest chory po przeszczepie serca leczony w ramach wczesnej rehabilitacji neurologicznej po udarze niedokrwiennym mózgu. Celem pracy jest prezentacja wczesnej rehabilitacji poudarowej w przypadku 53-letniego chorego po przeszczepie serca w 2000 roku z powodu kardiomiopatii zastoinowej. Chory by³ badany czterokrotnie: w 3, 6, 9 i 15 tygodniu od udaru mózgu. W badaniu oceniano si³ê miêœniow¹ niedow³adnych koñczyn wg. skali Lovetta, stopieñ nasilenia napiêcia miêœniowego wg. skali Ashwortha. Prowadzono ocenê funkcjonaln¹ w skali Barthel oraz stopieñ koniecznoœci korzystania z opieki osoby drugiej w skali Rankina. Praca przedstawia indywidualny schemat postêpowania rehabilitacyjnego podzielony na dwa etapy: fazê wczesn¹ (0-6 tydzieñ) i przejœciow¹ (69 tydzieñ) dostosowane do aktualnego stanu neurologicznego i kardiologicznego chorego. Zasadnicza poprawa po zastosowanym leczeniu w zakresie odzyskiwanych funkcji ruchowych nast¹pi³a do 9 tygodnia od wyst¹pienia udaru. Praca przedstawia pozytywne wyniki wczesnej rehabilitacji poudarowej, po operacji transplantacji serca. W œwietle uzyskanych wyników cenna staje siê wymiana doœwiadczeñ dotycz¹cych rehabilitacji chorych z nietypowym wywiadem. pe rs SUMMARY is c op y is for Brain stroke is the leading cause of disability, and usually affects people after 60. At this age, the risk of concomitant diseases, affecting not only the cardiovascular system is high. The number and intensity of the diseases are often contraindications for early post stroke rehabilitation. Patients after brain stroke and heart transplantation are rare cases. The aim of the study was to present the effectiveness of early post-stroke rehabilitation in case of the 53-year-old man who underwent heart transplantation in 2000, due to congestive cardiomyopathy. The patient was examined 4 times during the 3rd,6th, 9th and 15th week after the stroke. The test examined muscle strength in Lovett scale and the grade of muscle tone in Ashworth scale. A functional test was performed in Barthel scale and the extent of requiring care and attention – in Rankin scale. The paper presents an individual diagram of physiotherapeutic procedures divided into two stages: the early stage (0-6 weeks) and the transient stage (6-9 weeks), adjusted to the patient's actual neurological and cardiological state. Basic recovery after physiotherapeutic treatment was obtained before the 9th week. Patients after heart transplantation can be successfully treated by rehabilitation. A substantial progress in medicine obligates to exchange information about rehabilitation treatment of patients with untypical concomitant diseases. The number and intensity of such diseases is often contraindication for early rehabilitation after brain stroke [[6]. A history of cardiological or oncological conditions may be problematic, not only for disease qualification, but also for potential rehabilitation treatment. Patients after heart transplantation, undergoing early neurological rehabilitation treatment after ischemic brain stroke are rare cases. In Poland in 2004 105 heart transplantations were performed. Presently, in Poland, the results of heart transplantation are very good [7]. tio np roh ibit ed . WSTÊP Udar mózgu najczêœciej dotyczy osób po 60 roku ¿ycia [1,2,3,4,5], stwarza ryzyko wystêpowania dodatkowych schorzeñ, zwi¹zanych nie tylko z uk³adem sercowo-naczyniowym. Wieloœæ i nasilenie tych schorzeñ jest czêsto przeciwwskazaniem do wczesnej rehabilitacji po udarze mózgu [6]. Wspó³istniej¹cy obci¹¿ony wywiad kardiologiczny lub onkologiczny mo¿e stanowiæ problem nie tylko kwalifikacji, lecz tak¿e mo¿liwoœci leczenia usprawniaj¹cego. Rzadkim przypadkiem jest chory po przeszczepie serca leczony w ramach wczesnej rehabilitacji neurologicznej po udarze niedokrwiennym mózgu. W Polsce w roku 2004 zosta³o wykonanych 105 zabiegów transplantacji serca. Obecnie w Polsce wyniki transplantacji serca s¹ bardzo dobre [7]. CASE DESCRIPTION A 53-year old male patient underwent heart transplantation in 2000, due to congestive cardiomyopathy. The patient's circulatory system efficiency was rated II/III according to NYHA scale, cardiac ejection fraction was 55%, and he had a malignant tumour of the urinary bladder removed in 2004 . He was admitted to the Neurology Department of K. Jonscher Hospital in £ódŸ because of left limb paresis, where he stayed from 23.04 to 15.05.2006. Computer tomography revealed large ischemic area in the right temporal- parietal lobe. Rehabilitation treatment started on the third day following the stroke. Then the patient was referred to Rehabilitation Department for early neurological rehabilitation. He was treated from 15.05 to 30.06.2006. The patient was examined four times during the third, sixth, ninth and fifteenth week after brain stroke. The study evaluated muscle force in the lower limbs according to Lovett scale (norm from 0 to 5) and the degree of muscle tone intensity according to Ashworth scale. Functional evaluation was conducted in Barthel scale from 1 to 10 points. The score comprised the ability to perform 10 different everyday life activities – getting dressed, having meals, walking etc. Rankin's scale evaluated the necessity of assistance – 0 – no need, the return to normal lifestyle to 5 – the necessity of constant care and other people's assistance. The patient underwent internal examination and the results were as follows: cardiac action - 100 bpm (normal value for heart transplant), after physical effort - 130 bpm. Post-exercise ECG record, carried out several times, revealed no pathological changes. Besides, basic laboratory tests were conducted, and their results were normal. The aim of rehabilitation was reaching the highest possible levels of self-sufficiency of this patient, enabling his independent functioning at home, with the family. Przyjêty do Oddzia³u Neurologii Szpitala im. K. Jonschera w £odzi z powodu bezw³adu koñczyn lewych, gdzie przebywa³ w dniach 23.04-15.05.2006. W badaniu tomografii komputerowej – rozleg³y obszar niedokrwienny w prawym p³acie skroniowo-ciemieniowym. Rehabilitacjê rozpoczêto w 3 dobie od wyst¹pienia udaru. Nastêpnie pacjent zosta³ przeniesiony do Oddzia³u Rehabilitacji w ramach wczesnej rehabilitacji neurologicznej. Leczony by³ w dniach 15.05-30.06.2006. Chory by³ badany 4-krotnie w 3,6,9 i 15 tygodniu po wyst¹pieniu udaru mózgu. W badaniu oceniano si³ê miêœniow¹ koñczyn wg skali Lovetta (0 – 5-norma), stopieñ nasilenia napiêcia miêœniowego w skali Ashwortha. Prowadzono ocenê funkcjonaln¹ w skali Barthel, od 0 do 10 punktowano mo¿liwoœæ wykonywania 10 ró¿nych czynnoœci ¿ycia codziennego, np. ubierania siê, spo¿ywania posi³ków, chodzenia itp. Stopieñ koniecznoœci korzystania z opieki osoby drugiej ocenia³a skala Rankina od 0 – brak potrzeby opieki, pe³en powrót do ¿ycia w spo³eczeñstwie do 5 – koniecznoœæ sta³ej opieki i pomocy osoby drugiej. Chory by³ badany internistycznie: akcja serca miarowa 100/min (jest to norma dla serca transplantowanego), po wysi³ku fizycznym 130/min. Kilkakrotne, równie¿ powysi³kowe badanie EKG nie wykazywa³o zmian patologicznych. Wykonano równie¿ podstawowe badania laboratoryjne, które nie odbiega³y od normy. Dla chorego opracowano indywidualny program leczenia usprawniaj¹cego. Celem prowadzonego leczenia usprawniaj¹cego by³o uzyskanie mo¿liwie najwiêkszej samodzielnoœci ¿yciowej daj¹cej mo¿liwoœæ powrotu do ¿ycia w domu i rodzinie. Postêpowanie usprawniaj¹ce przebiega³o w nastêpuj¹cych fazach: Th Faza wczesna (0-6 tydzieñ) 1. Terapia u³o¿eniowa: oparta na metodzie Bobath. 2. Positioning therapy based on Bobath method. 2. Compensation of negligence syndrome: procedures should be performed at the site of paresis, it also con- sists in talks and caring procedures. Head slightly turned towards the side of paresis. 3. Optimal muscle activity – using exteroreceptive and proprioceptive stimuli. 4. The highest possible trunk activity – exercises: turn from a supine position to a lateral recumbent position, moving from a supine position to sitting, trunk stretching in order to maintain an unburdening position. Improvement of balanced reactions in different positions. Faza przejœciowa (6-9 tydzieñ) 1. Poprawa kontroli g³owy i tu³owia przy odruchach wstawania i podpierania siê w ró¿nych pozycjach wyjœciowych i momentach ruchu. 2. Zabiegi aktywizuj¹ce miêœnie – krótka 1-2 min. krioterapia. 3. Reedukacja miêœni metod¹ proprioceptywnego torowania nerwowo-miêœniowgo (PNF) – pobudzenie proprioceptywne przez zastosowanie oporu. PNF zak³ada dzia³anie bezpoœrednie na usprawnian¹ czêœæ cia³a lub poœrednie przy wykorzystaniu synergii [8-10]. 4. Nauka chodzenia: æwiczenia równowa¿ne w pozycji stoj¹cej, przemieszczanie ciê¿aru, kontrolowanie kolana (unikanie nadmiernego wyprostu). 5. Bezbolesny bark: krioterapia, æwiczenia izometryczne obrêczy barkowej. 6. Usprawnianie czynnoœci ¿yciowych. 7. Poprawa funkcji kognitywnych – poprawa zdolnoœci zapamiêtywania i uczenia siê, orientacja przestrzenna, koncentracja, logiczne myœlenie, rozumienie mowy i czytania, planowanie czynnoœci, rozwi¹zywanie problemów. Transitional phase (6-9 week) 1. Improvement of head and trunk control with standing up and leaning reflexes in different baseline positions and moments of movements. 2. Muscle activating procedures , brief – 2 min. cryotherapy. 3. Muscle reeducation – the method of proprioceptive neuromuscular facilitation (PNF) – proprioceptive stimulation using resistance. PNF implies a direct effect on the improved part of the body or an indirect effect using synergy [8,9,10]. 4. Learning to walk: balancing exercises in a standing position, weight shift, knee control (avoiding excessive stretching). 5. Painless shoulder: cryotherapy, isometric exercises of the shoulder girdle. 6. Improvement of everyday life activities. 7. Improvement of cognitive functions – improvement of the ability to memorize and learn, spatial orientation, concentration, logical thinking, understanding speech and reading, activity planning, problem solving. us eo nly -d istr ibu tio np roh ibit ed . zabiegi pielêgnacyjne. Wystêpowa³y cechy zespo³u depresyjnego. W 6 tyg. po udarze rehabilitacja uwzglêdnia³a naukê chodzenia za pomoc¹ czwórnoga, z pomoc¹ fizjoterapeuty. Nadal wystêpowa³y cechy zespo³u zaniedbania po³owiczego. Nast¹pi³a poprawa stanu psychicznego, wiêksza motywacja do dzia³añ. W 9 tygodniu od udaru chory chodzi³ samodzielnie za pomoc¹ czwórnoga, w ³usce DAFO (zabezpieczaj¹cej opadaj¹c¹ stopê) w obrêbie sali, korytarza. Bez cech zespo³u zaniedbania po³owiczego. Kontrolne badanie w Poradni Rehabilitacji zosta³o wykonane 15 tygodni po udarze mózgu. Chory przebywa³ w domu pod opiek¹ rodziny, æwiczy³ samodzielnie wyuczony w Oddziale schemat, uzyskuj¹c chód samodzielny równie¿ w terenie za pomoc¹ kuli ³okciowej, z zachowan¹ symetri¹ RESULTS During the examination conducted three weeks following the stroke, the patient's rehabilitation was at the stage of verticalisation next to the bed, assisted by a physiotherapist; the patient with a little help assumes a sitting position and maintains it for a relatively long time. The extreme movement range is limited in the left shoulder joint and cervical spine, lateral bows and unilateral rotations are also limited, the patient experiences difficulties with moving eyeballs to the left, associated most probably with the syndrome of semi-negligence. The trait of depressive syndrome occurred. directed gazing, proprioception disorders in left lower extremities, probably connected with semi-negligence syndrome; Traits of depression syndrome occurred. 6 weeks following the stroke, rehabilitation treatment included learning to walk, aided by a walking frame, with physiotherapist's assistance. Still, some symptoms of semi-negligence syndrome occurred. Improvement in mental condition occurred and motivation to undertake actions was higher. During the 9th week following the stroke, the patient walked by himself, aided by a walking frame in DAFO (Dynamic Ankle Foot Orthotics) braces (protecting the dropping foot) in the room and hall. No traits of semi-negligence syndrome. The control examination was performed at the Rehabilitation Clinic 15 weeks after brain stroke. The results of early post-stroke rehabilitation were good in the patient after heart transplantation 2. Basic improvement of function regaining occurred before the 9th week after stroke. 3. The suggested treatment is a verified way of rehabilitation in patients after brain stroke with additional disorders. 4. At the same time significant painfulness and motion range limitation was observed in the shoulder joint and the increase in spasticity, particularly in the arm and foot. The patient's mental condition improved and there were no traits of depression syndrome.
