W badaniach postawy zastosowano technikê fotogrametrii przestrzennej wykorzystuj¹c¹ efekt mory projekcyjnej. Równowagê badano na platformie stabilograficznej. Do opisu równowagi zastosowano parametry SPOX i SPOY. Wyniki. Analiza wariancji z klasyfikacj¹ dwukrotn¹ wykaza³a istotny efekt opcji badania (p<0,001). SPOX w teœcie przy CE zwiêkszy³ siê we wszystkich trzech badanych grupach wad w p³aszczyŸnie czo³owej. Zamkniêcie oczu powoduje wzrost wartoœci SPOX, co oznacza zwiêkszenie nacisku lew¹ stop¹. Wzrost niestabilnoœci w tej p³aszczyŸnie jest zazwyczaj skutecznie kompensowany przez strategiê stawu biodrowego. Wnioski. The aim of the study was to analyse the relationship between lateral spinal curvatures and MLPX and MLPY among children aged 12 to 15 years. Material nad methods.The sample consisted of 503 children chosen from among those attending Primary School No. 13 and Junior Secondary School No. 4 in Starachowice. The spatial photogrammetric method, based on Moire's topography, was used. Balance was examined on a stabilographic platform. The MLPX and the MLPY were used to describe balance findings. Results. Analysis of variance showed a significant effect of the testing options. In the test with eyes closed, MLPX increased in all three defect groups in the frontal plane. Closing eyes increases MLPX, which reflects an increased pressure of the left leg. Increased instability in this plane is usually effectively compensated for by the hip joint strategy. Conclusions. Although the analysis of variance did not show any essential effects, the value of MLPY with eyes closed was highest in children with curvatures >10°. Faulty postures seen in children with infantile cerebral palsy, disturbed sensorimotor integration, or Down syndrome may serve as a good example. The development of good postural habits is connected with integration of the functions of individual groups of postural muscles in adjustment and equilibrium responses rather than with strengthening these muscles [1,2,3]. Equilibrium is a particular state of the postural system ensured by nervous system through reflex tensing of appropriate groups of muscles called postural or antigravitational muscles. The modern technique of computer stabilography enables indirect assessment of the function of the nervous system via measurement of body sway, which reflects the maintenance of equilibrium in the upright position. A basic principle of stabilographic examination with the use of a dynamometric platform assumes continuous measurement of the point of application of the resultant ground reaction force defined as the center of foot pressure (COP). This signal is a random slow-changing process "winding" with a higher frequency around the signal representing the center of mass (COM). The difference between the two signals (COPCOM) constitutes the main controlling signal responsible for the maintenance of a stable posture. In this work, the analysis of COP was based on the MLPX and MLPY parameters [4,5,6,7,8,9,10,11,12,13,14]. The aim of this study was to analyse the relationship between lateral spine curvatures and the stabilographic parameters MLPX and MLPY among children and teenagers aged 12 to 15 years. Dziewcz¹t 12-letnich by³o 60 (24,29%), 13-letnich tak¿e 60 (24,29%), 14-letnich 65 (26,32%) i 15-letnich 62 (25,10%). Ch³opców 12-letnich by³o 65 (25,39), 13-letnich 61 (23,83%), 14-letnich 60 (23,44%) i 15-letnich 70 (27,34%). Rozk³ady liczebnoœci w grupach wieku i p³ci nie ró¿ni¹ siê istotnie. Badania wykonano w listopadzie i grudniu 2005 roku. W badaniach postawy zastosowano technikê fotogrametrii przestrzennej wykorzystuj¹c¹ efekt mory projekcyjnej (Ryc. 1) [15]. W badaniach równowagi zastosowano platformê Cosmogamma by Emildue (Ryc. 2). Wykonywano test standardowej oceny stabilnoœci w staniu swobodnym (test Romberga) sk³adaj¹cy siê z dwóch nastêpuj¹cych po sobie prób trwaj¹cych po 30 sekund: pierwsza z oczami otwartymi (OE – open eys), druga z oczami zamkniêtymi (CE – close eys) [16]. Do opisu równowagi zastosowano: œredni punkt obci¹¿enia X (SPOX) i œredni punkt obci¹¿enia Y (SPOY). SPOX (mean loading point X) podaje boczne wspó³rzêdne X (mm). WSTÊP Doœwiadczenia neurokinezjologiczne z dzieæmi z zaburzeniami OUN wskazuj¹, ¿e dysfunkcje tego uk³adu powoduj¹, oprócz objawów typowych dla poszczególnych zespo³ów klinicznych, ró¿norodne wady postawy cia³a. Jako przyk³ad mog¹ pos³u¿yæ wady postawy obserwowane u dzieci z mózgowym pora¿eniem dzieciêcym, zaburzon¹ integracj¹ sensomotoryczn¹, czy zespo³em Downa. Kszta³towanie nawyku postawy nie jest zwi¹zane ze wzmacnianiem poszczególnych grup miêœni posturalnych, lecz z integracj¹ ich funkcji w reakcjach nastawczych i równowagi [1,2,3]. Równowaga to pewien okreœlony stan uk³adu posturalnego, któr¹ zapewnia uk³ad nerwowy przez odruchowe napiêcie odpowiednich grup miêœni nazywanych miêœniami posturalnymi lub antygrawitacyjnymi. Nowoczesna metoda w postaci stabilografii komputerowej stwarza mo¿liwoœæ poœredniej oceny funkcji uk³adu nerwowego poprzez pomiar wychwiañ cia³a charakteryzuj¹cych utrzymanie równowagi w pozycji stoj¹cej. Podstawowa zasada badañ stabilograficznych z zastosowaniem platformy dynamometrycznej polega na ci¹g³ym pomiarze punktu przy³o¿enia wypadkowej si³y reakcji pod³o¿a okreœlanym œrodkiem nacisku stóp (COP). Sygna³ ten jest losowym wolnozmiennym procesem „owijaj¹cym” siê z wy¿sz¹ czêstotliwoœci¹ wokó³ sygna³u reprezentuj¹cego œrodek masy cia³a (COM). Ró¿nica tych dwóch sygna³ów (COP-COM) stanowi g³ówny sygna³ steruj¹cy odpowiedzialny za utrzymanie stabilnej postawy. W pracy analizowano COP na przyk³adzie SPOX i SPOY [4,5,6,7,8,9,10,11,12,13,14]. There were 60 girls aged 12 (24.29%), 60 girls at the age of 13 (24.29%), 65 girls at the age of 14 (26.32%) and 62 aged 15 (25.10%). The figures for boys were as follows: 65 boys aged 12 (25.39), 61 at the age of 13 (23.83%), 60 aged 14 (23.44%) and 70 at the age of 15 (27.34%). Distribution by age group and gender did not differ significantly. The investigations were performed in November and December 2005. The spatial photogrammetric method, which uses the projection Moiré phenomenon (Fig. 1) [15], was employed for the assessment of posture. Balance was examined with a Cosmogamma by Emildue platform (Fig. 2). Standard measurements of postural stability in a relaxed upright position (Romberg's test) were carried out. The procedure consisted of two successive tests, each lasting 30 seconds: one with eyes open (OE), and one with eyes closed (CE) [16]. The MLPX and MLPY parameters were used to describe balance findings. The MLPX (mean loading point X) shows the lateral coordinate X (in mm). Do analizy statystycznej zastosowano œredni¹ arytmetyczn¹ (x), odchylenie standardowe (s), analizê wariancji Kruskal'a-Wallis'a, analizê post hoc – test Bonferoniego [17]. Lower values of both parameters indicate more accurate postural control. Arithmetic means (x), standard deviations (s), Kruskal-Wallis analysis of variance, and post hoc analysis – Bonferroni's test [17] were used for the statistical analysis. is c op y is for pe rs on Œrednia wysokoœæ cia³a dziewcz¹t wynosi³a 161,45 cm, œrednia masa cia³a 50,84 kg, œrednie BMI 19,43. Analiza wariancji wykaza³a, ¿e w badanej grupie wyst¹pi³o istotne zró¿nicowanie wzrostu wzglêdem p³ci (p<0,001), istotne zró¿nicowanie wzglêdem wieku (p<0,001) i istotna interakcja wieku i p³ci na wzrost badanych (p<0,001). W badanej grupie wyst¹pi³o istotne zró¿nicowanie masy cia³a wzglêdem p³ci (p<0,03), istotne zró¿nicowanie wzglêdem wieku (p<0,001) i istotna interakcja p³ci i wieku na masê cia³a badanych (p<0,001). Wyst¹pi³o istotne zró¿nicowanie BMI wzglêdem wieku (p<0,004). Nie zaobserwowano istotnego zró¿nicowania wzglêdem p³ci. Nie wyst¹pi³a istotna interakcja p³ci i wieku na wskaŸnik BMI. Postawy badanych w p³aszczyŸnie czo³owej podzielono na postawê skoliotyczn¹ 1-5°, skrzywienie 6-10°, skrzywienie >10°. W ca³ej grupie dzieci zaobserwowano tylko 3 (0,60%) postawy prawid³owe, postaw skoliotycznych by³o 238 (47,32%), skrzywieñ w przedziale 6-10° by³o 216 (42,94%), a skrzywieñ >10° by³o 46 (9,15%). Image of body with contour lines and body sec-tion in sagittal and transverse plane [15] RESULTS AN DISCUSSION The mean height of the girls was 161.45 cm, mean body mass was 50.84 kg, mean BMI (Body Mass Index) was 19.43. For the boys, mean height was 165.41 cm, body mass was 52.74 kg, and mean BMI was 19.08. Group sizes did not differ significantly between the age groups. Analysis of variance in the study group showed significant diversification of height in relation to gender (p<0.,001) and age (p<0.001), and a significant interaction between age and sex on the one hand and height on the other (p<0.001). There was also a significant diversification of body mass in relation to gender (p<0.03), a significant diversification in relation to age (p<0.001) and a significant interaction between gender and age on the one hand and body mass on the other (p<0.001). There was a significant diversification of BMI in relation to age (p<0,004), but not in relation to gender. There was no significant interaction between gender sex and age on the one hand and the BMI index on the other. The postures of the patients in the frontal plane were divided into scoliotic postures (1-5°), curvatures in the 610° range and curvatures >10°. Only three children with normal postures were found in the entire sample (0.60%). The value of MLPX in the test with open eyes (OE) was highest in curvatures >10° whereas in the test with closed eyes (CE) it was highest in patients with scoliotic postures (1-5°) (Table 1, Fig. 3). Negative MLPX values imply higher loading of the left foot. Analysis of variance showed a significant effect of the testing options (p< 0.001). This parameter increased in all three defect types in the CE test. Closing eyes increases MLPX, which reflects an increased pressure of the left leg (Table 2, Fig. 3). Post hoc analysis showed a significant difference in MLPX in both OE and CE tests in the scoliotic posture group and in the 6-10o curvature group. A similar significant difference was not demonstrated in the >10° curvature group (Table 3). Analiza post hoc wykaza³a istotn¹ ró¿nicê SPOX w teœcie przy OE i CE w grupie postaw skoliotycznych i grupie skolioz 5-10°. W grupie skolioz >10° takiej istotnej ró¿nicy nie stwierdzono (Tabela 3). Przemieszczenia COP w p³aszczyŸnie czo³owej s¹ znacznie mniejsze, a wzrost niestabilnoœci w tej p³aszczyŸnie jest zazwyczaj skutecznie kompensowany przez strategiê stawu biodrowego. Jest to strategia przywracania równowagi pojawiaj¹ca siê u osób stoj¹cych na w¹skim pod³o¿u. In this setting, the action of the same stimulus causing balance loss is compensated for by a sequence of muscular activity, beginning at the muscles of the thigh and trunk and spreading down to involve other muscles of the lower extremity. Although analysis of variance did not show any significant effects, it is worth noting that in the OE test this parameter was highest in children with scoliotic postures (1-5°) whereas in the CE test, it was highest in children with curvatures >10°. This latter group also demonstrated the biggest difference in Romberg's test (Table 4, 5, Fig. 4). Impaired balance is also compensated for by a forward inclination of the body and this increases the probability of balance loss in this direction. Thus the established, readily available strategy can be used to restore balance. This strategy is defined as specific interactions between the sensory and motor components of the postural system. In this case the regaining of balance involves a characteristic sequence beginning with a contraction of the tarsal joint muscles. The soleus and the anterior tibial muscle are the principal muscles stabilizing the tarsal joint. This sequence has been called the tarsal joint strategy. The dearth of papers connected with the assessment of the ability to maintain balance on stabilographic platforms among healthy children precludes clinical use of the results of such studies for quantitative comparison with pathology. 5. The lack of standards for stabilographic parameters makes it difficult to interpret the results of the study. It is not known whether the results are within or beyond the reference ranges for response. 6. Nevetheless, the results throw light on the relationship between scoliosis and the function of the organ of balance and CNS. There is a need to use exercises improving balance via biofeedback on stabilographic platforms for postural reeducation. us 1. Brak norm parametrów stabilograficznych utrudnia interpretacjê uzyskanych wyników. Nie wiadomo czy uzyskane wyniki mieszcz¹ siê w granicach reakcji prawid³owych, czy je przekraczaj¹. 6. Uzyskane wyniki rzucaj¹ jednak pewne œwiat³o na zwi¹zek skolioz z funkcj¹ narz¹du równowagi i OUN.
