Według szacunków WHO, w 2000 roku w Europie wystąpiło 3.790.000 złamań osteoporotycznych, z czego 890.000 były to złamania bliższego końca kości udowej. Dane instytucji międzynarodowych zajmujących się tą tematyką mówią o życiowym ryzyku złamania wynoszącym 46,4% dla kobiet 50-letnich i 22,4% dla mężczyzn w tym wieku. Szczególnie związane z osteoporozą złamanie bliższego końca kości udowej obarczone jest 20% (kobiety) i 30% (mężczyźni) ryzykiem śmierci w ciągu pierwszego roku na skutek powikłań. W tej grupie pacjentów odnotowuje się również wysoki odsetek chorych, którzy po złamaniu pozostają niepełnosprawni. Zawansowany wiek oraz wielokrotne złamania pogłębiają negatywny wpływ choroby na jakość życia. Osteoporosis constitutes a significant clinical problem in the whole EU, which is still increasing due to the aging of the society. According to WHO estimates, there were 3,790,000 osteoporotic fractures in Europe in 2000, and 890,000 of them concerned the proximal end of the femur. International institutions dealing with this problem report a lifetime fracture risk of 46.4% for 50-year-old women and 22.4% for men at the same age. Fracture of the proximal end of the femur, especially common in osteoporosis, is connected with a 20% (women) and a 30% (men) risk of death in the first year, due to complications. In this group of patients, there is also a high rate of individuals who become disabled after the fracture. Advanced age and multiple fractures increase the negative influence of the disease on the quality of life. Współcześnie stosowane w terapii osteoporozy leki podzielić można na dwie grupy: związki o działaniu antyresorpcyjnym oraz substancje wywierające wpływ anaboliczny na tkankę kostną. Do pierwszej z wymienionych grup należą bisfosfoniany, hormonalna terapia zastępcza, selektywne modulatory receptora estrogenowego (SERM) oraz kalcytonina. Do leków działających anabolicznie na kość należy parathormon oraz fluor. Medicines currently used in the treatment of osteoporosis can be divided into two groups: antiresorptive compounds and substances having an anabolic effect on bones. The first group includes bisphosphonates, hormone replacement therapy, selective estrogen receptor modulators (SERM), and calcitonin. Medicines with anabolic effects on bones include parathromone and fluorine. - - - - Funkcją leków antyresorpcyjnych jest hamowanie resorpcji kości i zwiększanie jej gęstości mineralnej. Obecnie standardem jest stosowanie bisfosfonianów, które przyśpieszają apoptozę osteoblastów. Jednak przewlekłe podawanie tych leków może prowadzić do niekorzystnej przebudowy kostnej i innych działań niepożądanych. W wyniku poszukiwania alternatywy dla bisfosfonianów do grupy leków antyresorpcyjnych dołączył denosumab, działający poprzez szlak RANK/RANKL/OPG. Inne leki z tej grupy to bloker integryn oraz inhibitory katepsyny K [1]. - Deszczyński J. i wsp. – Współczesna farmakoterapia osteoporozy The function of antiresorptive medicines is to inhibit bone resorption and increase its mineral density. The current standard is to use bisphosphonates which increase the process of osteoblast apoptosis. However, chronic application of these drugs may lead to unfavourable bone remodelling and other adverse effects. As an alternative for bisphosphonates, denosumab (with effects on RANK/RANKL/OPG pathway) was included in the group of antiresorptive medicines. Other medicines from this groups include integrin blockers and inhibitors of cathepsin K [1]. Przeciwciała Monoklonalne Monoclonal Antibodies Niedawno zarejestrowanym do w Polsce lekiem przeciw osteoporozie jest denosumab. Jest to w istocie ludzkie przeciwciało monoklonalne, które wiąże się z rozpuszczalną cytokiną RANK-L. Cytokina ta, wytwarzana przez osteoblasty, w połączeniu z denosumabem zapobiega aktywacji osteoklastów w mechanizmie przyłączania RANK-L do receptora RANK [2,3]. Wskutek inhibicji aktywności osteoklastów poprzez działanie denosumabu spowolniony zostaje proces resorpcji tkanki kostnej i wzrasta gęstość mineralna kości [4,5]. A recently registered in Poland medicine against osteoporosis is denosumab. This is actually a human monoclonal antibody, which binds with soluble cytokine RANK-L. This cytokine, produced by osteoblasts, in combination with denosumab prevents from activation of osteoclasts by binding RANK-L to RANK receptor [2,3]. As a result of inhibition of osteoclasts activity (by denosumab action), the process of bone resorption becomes slower and the mineral density of the bones increases [4,5]. Przeprowadzone dotychczas badania potwierdziły skuteczność denosumabu jako czynnika antyresorpcyjnego w odniesieniu do tkanki kostnej w leczeniu osteoporozy postmenopauzalnej, zmian kostnych w przebiegu RZS oraz nowotworach przerzutowych. Kliniczne efekty działania leku to spadek częstości złamań w obrębie kręgosłupa i biodra w grupie kobiet po menopauzie w porównaniu z grupą kontrolną [6]. The studies conducted so far have proved the efficacy of denosumab as bone antiresorptive agent, in the treatment of postmenopausal osteoporosis, bone lesions in the course of rheumatoid arthritis, and metastases. Clinical effects of this medicine include the decrease of incidence of fractures within spine and hips of postmenopausal women, as compared to the control group [6]. W badaniu Lewiecki i wsp. wzięło udział 412 kobiet z osteoporozą pomenopauzalną. Grupa badana leczona różnymi dawkami podskórnymi denosumabu (14,60, 100 i 210 mg) porównywana była do grupy pacjentek leczonych w sposób klasyczny – doustnym alendronianem. Po 24 miesiącach 38 The study by Lewiecki et al. included 412 women with postmenopausal osteoporosis. The study group, receiving different doses of subcutaneous denosumab (14, 60, 100, and 210 mg) was compared to the group of patients treated in a standard way – with oral alendronate. After 24 months, there was a significant increase in bone density in all studied Arthroscopy and Joint Surgery, 2011; 7(1-2): 37-43 stwierdzono istotnie zwiększoną gęstość kości we wszystkich badanych rejonach ciała. Efekty leczenia, w grupie z dawkami >60 mg (ocena BMD po 24 miesiącach) były lepsze niż w grupie kontrolnej – z alendronianem [7]. We wszystkich grupach odnotowano podobną częstość występowania działań niepożądanych. W drugiej fazie wyżej wymienionych badań oceniano ciągłość efektu leczniczego stosowania denosumabu w przypadku zaprzestania terapii. Lek podawano podskórnie w schematach terapeutycznych: 6, 14 lub 30 mg co trzy miesiące; 14, 60, 100 lub 210 mg co sześć miesięcy. Po 24 miesiącach u pacjentów odstawiano leczenie. Efekty kliniczne porównywano z równoległymi grupami placebo i doustnym alendronianem [8]. Po dwuletnim okresie leczenia denosumabem BMD mierzone w rejonie kręgosłupa lędźwiowego i kości udowej wzrosło kolejno o 9,4–11,8% i 4,0–6,1%. 12 miesięcy po odstawieniu leku BMD spadło o wartości 6,6% (kręgosłup lędźwiowy) i 5,3% (kość udowa). W wyniku ponownego włączenia leczenia gęstość mineralna kości zaczęła rosnąć. Brown i wsp. W 2009 r. przeprowadzili duże (n=1189), wieloośrodkowe badanie z podwójną ślepą próba porównujące skuteczność i bezpieczeństwo zastosowania denosumabu (60 mg co sześć miesięcy sc) i alendronianu (70 mg co tydzień po) u kobiet z osteoporozą pomenopauzalną. Po 12 miesiącach odnotowano wzrost BMD w grupie leczonej denosumabem na poziomie 3,5% oraz w grupie leczonej alendronianem na poziomie 2,6%. Profil bezpieczeństwa i częstość występowania działań niepożądanych w przypadku obu leków były podobne [9]. - - - - - Działanie denosumabu znajduje potwierdzenie w badaniach biopsyjnych, gdzie stwierdzono silną inhibicję przebudowy kostnej oraz w badaniach na modelu zwierzęcym, które potwierdziły pozytywne działanie w leczeniu złamań. Zarejestrowany lek dostępny jest w formie iniekcji podskórnych, które należy powtarzać co sześć miesięcy. Deszczyński J. et al. – Current pharmacotherapy of osteoporosis body regions. Treatment results of the group receiving doses of over 60 mg (BMD evaluation after 24 months) were better than in the control group – with alendronate [7]. In all the groups, there was a similar incidence of adverse effects. In the second phase of the above mentioned studies, the continuity of the therapeutical effect of denosumab was evaluated in case of treatment discontinuation. The medicine was administered subcutaneously in the following treatment regimes: 6, 14, or 30 mg every three months; 14, 60, 100 or 210 mg every six months. After 24 months, the treatment was discontinued. The clinical effects were compared with parallel groups receiving placebo or oral alendronate [8]. After a two-year treatment with denosumab, BMD measured in the lumbar spine and femur increased by 9.4–11.8% and 4.0–6.1%, respectively. After 12 months following treatment discontinuation, BMD decreased by 6.6% (lumbar spine) and 5.3% (femur). As a result of reintroduction of the therapy, the mineral density of the bones started rising again. Brown et al. conducted in 2009 a large (n=1189), multicentre, double-blind study comparing the efficacy and safety of denosumab (60 mg every six months, s.c.) and alendronate (70 mg every week, p.o.) in women with postmenopausal osteoporosis. After 12 months, there was an increase of BMD in the group treated with denosumab (3.5%) and in the group receiving alendronate (2.6%). Safety profiles and the incidence of adverse effects were similar for both medicines [9]. Effectiveness of denosumab was confirmed by biopsy studies, showing a strong inhibition of bone remodelling, and by studies on animal model, which confirmed therapeutic effects in the treatment of fractures. The registered medicine is available in the form of subcutaneous injections that should be repeated every six months. Selektywne Modulatory Receptora Estrogenowego Selective Estrogen Receptor Modulators Selektywne modulatory receptora estrogenowego (SERM) łączą się z receptorem estrogenowym i wywierają zależny od tkanki, na którą oddziałują, efekt agonistyczny lub antagonistyczny do działania estrogenów. W badaniach klinicznych stwierdzono zmniejszenie częstości występowania złamań kręgosłupa, jednak nie udowodniono redukcji częstości złamań w obrębie biodra i innych okolic [10,11]. Selective estrogen receptor modulators (SERM) bind with estrogen receptor and induce a tissue-dependent effect (within tissue that they have influence on) that may be agonistic or antagonistic to estrogen activity. Clinical studies have shown a decreased incidence of spine fractures but not a reduction of fractures within hips or other body regions [10,11]. Dokładny mechanizm poprzez który SERM wywierają swój, specyficzny dla tkanek, wpływ pozostaje do końca niewyjaśniony. Wytłumaczenia takiego działania leków upatruje się we wpływie równowagi koregulatorów receptorów estrogenowych i środowiska komórkowego na konkretny efekt biologiczny wywierany prze określony SERM na dany typ komórki [12,13]. An exact mechanism of tissue-specific SERM action remains unclear. The explanation is searched for in the influence of balance of estrogen receptor coregulators and cellular environment on a given biological effect exerted by a given SERM on a given type of cell [12,13]. Postępy w rozwoju farmakoterapii osteoporozy lekami z grupy SERM polegają na tworzeniu coraz doskonalszych modulatorów receptora estrogenowego w zakresie profilu selektywności tkankowej, czyli utrzymaniu korzystnego działania na tkankę kostną z jednoczesnym wyeliminowaniem działań niepożądanych leku w obrębie endometrium i sutka. Advancements in the development of treatment of osteoporosis with SERM medicines include the elaboration of increasingly supreme modulators of estrogen receptors with respect to tissue selectivity, i.e. provision of an advantageous effect on bone tissue, with elimination of adverse effects of the medicine on endometrium and breasts. One of the latest medicines from the described group is bazedoxifene which reveals a high affinity to ERa and ERb 39 Artroskopia i Chirurgia Stawów, 2011; 7(1-2): 37-43 Jednym z nowszych leków należących do opisywanej grupy jest bazedoxifen, który cechuje się wysokim powinowactwem do receptorów ERa i ERb [14]. Połączenie z receptorami estrogenowymi prowadzi to do dimeryzacji receptora a następnie powiązana ze składowymi odpowiedzi na estrogeny [15]. W badaniach z bazedoxifenem na modelu zwierzęcym stwierdzono zależny od dawki wpływ na zachowanie masy kostnej [16]. Dodatkowo wykazano brak wpływu stosowania bazedoxifenu na przerost nabłonka śluzówki macicy i myometrium. W badaniach porównujących wpływ bazedoxifenu, raloxyfenu i lasoxifenu na gruczoł piersiowy myszy, ten pierwszy wykazał najsłabszą stymulację i okazał się najbardziej efektywnym antagonistą wpływu skoniugowanych estrogenów na gruczoł piersiowy, co potwierdziły badania histopatologiczne [17]. Kliniczne badania I fazy, z udziałem kobiet po menopauzie wykazały bezpieczeństwo i dobrą tolerancję bazedoxifenu, który podawany był w codziennych doustnych dawkach 20 mg [18]. Dodatkowo stwierdzono u objętych badaniem kobiet brak wpływu leku na grubość endometrium oraz objętość jajników [19]. Skuteczność działania bazedoxifenu została przeanalizowana w randomizowanym, kontrolowanym badaniu z placebo z udziałem 494 zdrowych kobiet po menopauzie [20]. Ronkin i wsp. opisali istotne statystycznie obniżenie markerów obrotu kostnego w grupie stosującej nawet minimalne dawki bazedoxifenu w porównaniu z grupą placebo. - - - - - Wiązane dotychczas z grupą leków SERM działania niepożądane w zakresie narządów wrażliwych na działanie estrogenów w przypadku bazedoxifenu okazały się znacznie rzadsze. W badaniu z grupą 497 kobiet po menopauzie podczas 6 miesięcznej terapii dawkami 2,5–20 mg leku, kontrola grubości endometrium poprzez usg transwaginalne nie wykazała zmian jego grubości w stosunku do grupy placebo. Przy dawkach 30 i 40 mg opisywano natomiast zmniejszenie grubości endometrium. W innych badaniach oceniano częstość występowania bólu w obrębie gruczołu piersiowego u kobiet przyjmujących lek. W przypadku dawek 2,5–20 mg opisywano częstość takich epizodów na poziomie grupy placebo, natomiast kobiety przyjmujące dawki 30 i 40 mg bazedoxifenu zgłaszały bóle gruczołu piersiowego rzadziej niż kobiety przyjmujące placebo [21]. Korzystnie dla bazedoxifenu zakończyły się również wieloośrodnkowe badania porównawcze z raloxifenem skuteczności w zapobieganiu i leczeniu osteoporozy. W badaniach z udziałem w sumie ok. 9000 kobiet, w dwuletniej obserwacji, stwierdzono korzystne zmiany w BMD mierzonego zarówno w obrębie biodra jak i kręgów lędźwiowych, na poziomie zbliżonym wśród kobiet przyjmujących 20, 40 mg bazedoxifen lub 60 mg raloxifenu [22]. Badany lek stosowany przez 2 lata wywoływał istotny statystycznie wzrost gęstości mineralnej kości w obu wymienionych okolicach w stosunku do placebo, a jego skuteczność potwierdziło obniżenie stężenia w osoczu markerów obrotu kostnego: osteokalcyny i telopeptydu C łańcucha alfa kolagenu typu I [23]. Także częstość złamań kręgów ulega istotnej redukcji w przypadku 36 miesięcznego stosowania bazedoxifenu (2,3–2,5%) w stosunku do grupy placebo (4,1%). Jak wynika z badań klinicznych ryzyko złamań kręgów w przypadku przewlekłego stosowania bazedoxifenu jest zmniejszone podobnie jak 40 Deszczyński J. i wsp. – Współczesna farmakoterapia osteoporozy receptors [14]. Binding with estrogen receptors leads to receptor dimerisation and then to binding with components of the response to estrogens [15]. In animal-model studies on bazedoxifene, there was a dosedependent influence on the retention of bone mass [16]. Moreover, there was no influence of bazedoxifene on the overgrowth of endometrium or myometrium. In the studies comparing the effect of bazedoxifene, raloxifene, and lasofoxifene on the breasts of the mice, the first medicine showed the lowest stimulation and turned out to be the most effective antagonist of influence of conjugated estrogens on the breast, which was confirmed by histopathological tests [17]. Clinical studies of the first phase, including women after menopause, showed a good safety and tolerability profile of bazedoxifene, which was administered in daily p.o. doses of 20 mg [18]. Moreover, there was no influence of the drug on the thickness of endometrium and on the volume of the ovaries [19]. Efficacy of bazedoxifene was analysed in a randomized, placebo-controlled study including 494 healthy women after menopause [20]. Ronkin et al. described a statistically significant reduction in the level of markers of bone turnover, in the group of patients using even the minimal doses of bazedoxifene, as compared to the placebo group. Adverse effects on estrogen-sensitive organs, ascribed to SERM drugs, turned out to be much less frequent in case of bazedoxifene. In a study including 497 postmenopausal women administered 2.5–20 mg of medicine during a 6-month therapy, the analysis of endometrium thickness with transvaginal usg did not reveal any changes, as compared to the placebo-controlled group. With 30 and 40 mg, the thickness of endometrium was reduced. There were studies that evaluated the incidence of breast pain in women taking medicines. In case of doses ranging from 2.5 to 20 mg, the incidence of such episodes was similar to the one in the placebo group; women taking 30 and 40 mg of the bazedoxifene reported breast pain less frequently than women taking placebo [21]. Multicentre studies comparing the efficacy of prevention and treatment of osteoporosis with the study drug and with raloxifene also spoke in favour of bazedoxifene. In the studies that included approx. 9000 women in total, a two-year observation showed positive changes of BMD in the hips and lumbar spine, within a range similar to the one in women taking 20 and 40 mg of bazedoxifene or 60 mg of raloxifene [22]. The study medicine, applied for 2 years, caused a statistically significant increase in the mineral density of bones in both mentioned areas, as compared to placebo. Its efficacy was confirmed by a decrease in the level of serum markers of bone turnover: osteocalcin and C-telopeptide of alpha 1 chain of type I collagen [23]. Also the incidence of vertebral fractures got significantly reduced in case of a 36-month administration of bazedoxifene (2.3–2.5%) as compared to placebo (4.1%). According to clinical study results, the risk of vertebral fractures in case of chronic administration of bazedoxifene is lower, as in case of raloxifene, by approx. 42%, in comparison to placebo (which is statistically significant, P<0.05). Data collected by Arthroscopy and Joint Surgery, 2011; 7(1-2): 37-43 przy stosowaniu raloxifenu o ok 42% w porównaniu do grupy placebo, co jest wynikiem istotnym statystycznie (p<0,05). Z danych zgromadzonych przez Kanis i wsp. można również wyciągnąć dalej idące wnioski. Mianowicie w badaniu klinicznym obejmującym kobiety z podwyższonym ryzykiem złamań wg FRAX odnotowano istotnie statystycznie zredukowane ryzyko złamań również poza kręgosłupem i bliższą nasadą kości udowej [24]. Główne, szeroko opisywane w literaturze, działania niepożądane SERM to uderzenia gorąca, poty, skurcze łydek a także epizody zakrzepowo-zatorowe. W badaniach z bazedoxifenem najczęściej opisywanymi działaniami niepożądanymi były: bóle głowy, infekcje, bóle stawowe, uderzenia gorąca, jednak częstość ich występowania nie była istotnie statystycznie wyższa niż w grupie placebo. Co istotne w żadnej z grup badanych nie stwierdzono zwiększonej częstości przypadków raka sutka, szyjki macicy ani zwiększonego przerostu endometrium [25,26]. Częstym działaniem niepożądanym w przypadku stosowania leków z grupy SERM pozostaje występowanie uderzeń gorąca i zlewnych potów, dlatego też nie zaleca się kontynuacji tego typu terapii u pacjentek po menopauzie, u których w trakcie leczenia pojawiły się objawy wazomotoryczne [27]. Współcześnie w opisie schorzenia jakim jest osteoporoza coraz większą uwagę zwraca się na jej najgroźniejsze skutki – złamania. Gęstość kości wyrażona w BMD jakkolwiek nadal istotna w diagnostyce osteoporozy i podejmowaniu decyzji o wdrożeniu leczenia uzupełniana jest poprzez uwzględnienie innych czynników ryzyka w algorytmie szacującym ryzyko złamań FRAX, który został stworzony i jest propagowany przez WHO od wielu lat. Istotne więc wydaje się spostrzeżenie, że bis fosfoniany będąc lekami skuteczniej niż SERM zwiększającymi BMD, w badaniach porównawczych nie zmniejszają w tym samym stopniu ryzyka wystąpienia złamań [28]. - - - - - Dostępne publikacje wskazują, iż wzrost BMD jedynie w niewielkim stopniu wpływa na zmniejszenie ryzyka złamań. W przypadku raloxifenu jest to ok. 4% a wśród leków z grupy bisfosfonianów ok. 16–28% [29,30]. Tak więc, wydaje się iż na ryzyko wystąpienia złamań wpływają w dużym stopniu również inne wywierane przez leki antyresorpcyjne na tkankę kostną efekty, takie jak zmniejszenie obrotu kostnego czy poprawa mikroarchitektury kości [31]. In his clinical study including women with an increased risk of fractures according to FRAX, there was a statistically significantly reduced risk of fractures, also beyond the spine and proximal epiphysis of the femur [24]. Main adverse effects of SERM, widely described in the literature, include hot flushes, sweating, calf cramps, and thromboembolic episodes. In the studies on bazedoxifene, the most common adverse effects included: headaches, infections, arterial pains, and hot flushes. However, their incidence was not statistically significantly higher than in the placebo group. What is important, no study group showed an increased incidence of breast cancer, cervix cancer or endometrium overgrowth [25,26]. A frequent adverse effect in case of SERM medicines remains the presence of hot flushes and profuse sweating. That is why, it is not recommended to continue this type of therapy in postmenopausal patients if there were vasomotor symptoms in the course of treatment [27]. Currently, increasingly more attention is devoted to the most dangerous results of the disease – the fractures. Bone density expressed in BMD, although still important in the diagnostics of osteoporosis and in decisions on treatment introduction, it is now supported by other risk factors in the risk-assessment algorithm, FRAX, which was developed and has been propagated by WHO for many years now. It seems important then to note that bisphosphonates, being more effective in increasing BMD than SERM medicines are, do not decrease the risk of fractures to the same degree (according to comparative studies) [28]. Available publications report that even the slightest increase of BMD influences the reduction of the fracture risk. Thus, it seems that the risk of fractures is strongly influenced by other effects exerted by antiresorptive drugs on bones, such as reduction of bone turnover or improvement of bone microarchitecture [31]. From among all described proteases of this family, cathepsin K has the biggest meaning in the processes of bone remodeling owing to its high collagenase activity, with regard to collagen type I and II in acidic environment (because in these conditions there follows a breakdown of calcific hydroxyapatite – the main mineral component of bones). Jednym z obiecujących wydaje się być odanacatib, inhibitor katepsyny K – proteazy ulegającej ekspresji na osteoklastach rozkładających kolagen typu I. Jest to inhibitor selektywny, co sprawia iż kolagen nie kumuluje się w fibroblastach skóry [32]. Brak selektywności w przypadku wcześniejszych inhibitorów katepsyny powodował zmiany skórne [33]. Dotychczas nie donoszono o występowaniu istotnych działań niepożądanych leku [34]. Ciekawe wnioski płyną również z badań histologicznych. Odanacatib mianowicie, zwiększając gęstość mineralną kości wpływa odmiennie na kość korową i gąbczastą. W kości gąbczastej obserwowano efekt inhibicji przebudowy kostnej, natomiast w kości korowej występowało zjawisko zwiększonej formacji kostnej [35]. Innym, potencjalnie skutecznym, lekiem przyszłości w terapii osteoporozy może stać się przeciwciało monoklonalne sklerostyny. Sklerostyna jest białkowym produktem osteocytu, który kompetycyjnie wiąże się z LRP5 (białoko związane z receptorem dla lipoprotein o niskiej gęstości) i blokuje szlak aktywacji osteoblastów Wnt. Tak więc przeciwciało dla LRP5 może prowadzić do klinicznie korzystnego efektu anabolicznego w tkance kostnej u pacjentów cierpiących na osteoporozę. - In clinical studies of the second phase, on odanacatib, 399 women with postmenopausal osteoporosis showed a dosedependent increase of bone mineral density. The drug, when administered in the dose of 50 mg a week, caused an increase in BMD of 4.1–5.7%, depending on the analysed area, within 24 months. So far, there have been no reports on significant adverse effects of the drug [34]. Interesting conclusions followed also from histological studies: odanacatib, by increasing mineral density of bones, exerts different effects on cortical and on cancellous bone. In the cancellous bone, there was inhibition of bone remodeling, while in the cortical bone, there was an increased bone formation [35]. Another, potentially effective drug of the future, used in the treatment of osteoporosis, may become the sclerostin monoclonal antibody. Sclerostin is a protein product of osteocytes which binds competitively to LRP5 (protein connected with a receptor for low-density lipoproteins) and blocks the Wnt pathway of osteoblast activation. Thus, the antibody for LRP5 may lead to a clinically advantageous anabolic effect in bones of patients suffering from osteoporosis. Szczególnie w przypadku osteoporozy pomenopauzalnej istotne są postępy w zakresie skuteczności i bezpieczeństwa stosowania leków z grupy SERM. The multitude of currently published reports on new or improved forms of pharmacotherapy in osteoporosis gives the physicians dealing with osteoporosis a wide selection of treatment choices and allows them to select the most appropriate therapy for a given patient. Especially in postmenopausal osteoporosis, the advances in effectiveness and safety of medicines from the SERM group are important. The basic advantages of monoclonal antibodies acting on RANKL include a faster and stronger inhibition of bone resorption in comparison to bisphosphonates, and a full reversibility of the effect exerted on metabolism of bone tissue, as well as a good safety profile [36,37]. The development of new, more successful and safer forms of pharmacotherapy of osteoporosis presents the field of medicine with new questions concerning their place in the whole treatment process and shows the need of modification of the existing therapeutic schemes or development of some new ones, which will be the most effective in preventing the decrease of the mineral density of bones, and, first of all, in reducing the number of fractures.
