Celem podjêtych badañ by³o rozpoznanie i zdefiniowanie niektórych kompensacyjnych wzorców posturalnych u dzieci z m. p. dz. Materia³ i metody. Badaniami objêto 18 dzieci z rozwiniêtym niedow³adem po³owiczym, w wieku 5-14 lat. Przeprowadzono zobiektywizowane pomiary wzorców postawy podczas samodzielnego utrzymywania kolejnych pozycji – stoj¹cej obunó¿ i jednonó¿, klêku prostego i jednonó¿ oraz pozycji siedz¹cej. Fotogrametrycznie oceniano przestrzenny uk³ad cia³a, a jednoczeœnie przy u¿yciu platformy barorezystywnej, oceniano rozk³ad si³ nacisku na p³aszczyŸnie podparcia. Wyniki. W dwuno¿nej pozycji stoj¹cej u badanych zaobserwowano dwojakiego rodzaju wzorce posturalne – wzorzec prograwitacyjny oraz antygrawitacyjny. U wszystkich badanych z wzorcem prograwitacyjnym zaobserwowano boczne skrzywienia krêgos³upa w odcinku piersiowo-lêdŸwiowym. Ich wartoœci k¹towe by³y zdecydowanie wiêksze ni¿ analogiczne w grupie niedow³adów antygrawitacyjnych. W pozycjach jednono¿nych uk³ad powy¿szych parametrów pozostawa³ taki sam, podczas gdy w pozycji siedz¹cej u dzieci z wzorcem prograwitacyjnym zmieni³ siê wzajemny uk³ad linii barków i miednicy na równoleg³y. Wœród badanych z wzorcem antygrawitacyjnym dosz³o do przeci¹¿enie strony przeciwnej ni¿ w pozycji stoj¹cej. Wnioski. 1. Wzorce posturalne u dzieci z niedow³adem po³owiczym s¹ stereotypowe i ró¿ni¹ siê pomiêdzy sob¹ w zale¿noœci od anty- lub prograwitacyjnego charakteru niedow³adu. 2. The spontaneous adjustment of abnormal muscle tone in CP children leads to abnormal postural and motor patterns development. The aim of this work was to recognize and define some of compensatory postural patterns in CP children. Material and methods. The group of 18 children with hemiplegia, aged 5-14, were taken into the examination. The objective measurement of they postural patterns while standing, kneeling (of one and both legs) and sitting was done with a help of photogrammetry. In the same time using baroresistive platform a force distribution on the support base were meas-ured. Results. In case of standing position two kinds of postural patterns were observed – the progravitational pattern and the antigravitational pattern. In every children with progravitional pattern lateral spine curvature were observed. Their values were significantly higher than similar in antigravitational pattern. In standing on one leg and half kneeing the arrangement of postural parameters were the same. In sitting in children with progravitional type mutual ar-rangement of shoulders and pelvis line was changed to parallel and in children with anty-gravitional type overloaded side changed to opposite. Conclusions. 1. The postural patterns through hemiparetic children are habitual and have differentiated character according to character of paresis. 2. Development of postural patterns is predictable, it is possible to steer the compensation during the treatment. Normal development of such phenomena enables the child to assume body positions of increasing verticality, thus forming a basis for the correct development of a body scheme and all functional skills, such as articulation, visuomotor coordination and locomotion [1,2]. Damage to the immature brain disrupts this process and every child with cerebral palsy (CP) becomes more or less unable to oppose gravity. This failure is manifested mainly as so-called proximal postural hypotonus, or flaccidity along the head-trunk axis, and as distal hypertonus, or elevated muscle tone of the distal parts of the extremities. In such cases, the ability to stand and a limited range of movement in increasingly vertical body positions are is achieved by compensatory mechanisms [2,3,4]. Being unable to satisfactory stabilize the more proximal segments of body (head, trunk, pelvic girdle), a cerebral palsied child compensates for such shortcomings by spontaneously activating compensatory stabilization. Two mechanisms play a role here. Passive stabilization is achieved by "manipulation" of the support plane and the centre of gravity projection or by utilizing periarticular structures. Pathological distal stabilization is "fixation" activated via reflective tonic activity. Both mechanisms produce forced body positions [2,3]. Postural consequences of passive compensatory stabilization include above all "extraaxial" positioning of individual body segments, mainly the head, trunk and pelvic girdle, observed in various body positions. The influence of reflective tonic activity, being the basis of "fixation", is manifested by global muscular synergies [2,3,4]. Compensatory postural patterns appear in the developing CP child as soon as the first anti-gravitational problems occur, i.e. from the moment of that a postural tone deficit is first signalled, even when the child is still in the supine position. The initially subtle abnormalities become more severe in increasingly upright positions and achieve their final form in vertical positions, when the need arises to oppose gravity in the setting of a more and more elevated centre of gravity and a gradually decreasing size of the support plane [4,5,6]. Unfortunately, the potential to influence the compensatory mechanisms at this stage of motor development is limited. Effective customised therapy ought to be commenced at earlier stages of motor development. This, however, requires a full understanding not only of the possibilities of influencing the compensation (means, methods), but, most importantly, of the development of postural patterns (diagnosis, prognosis) and of ways to plan the development of the best postural patterns possible in a particular child (therapy goals) [7,8]. The aim of the study was to identify and define some compensatory postural patterns in children with CP in vertical positions. WSTÊP Jednym z czynników determinuj¹cych kszta³towanie umiejêtnoœci przeciwstawienia siê sile grawitacji na ka¿dym etapie rozwoju motorycznego jest prawid³owy rozwój napiêcia miêœniowego. Pozwala on nie tylko na rozwój prawid³owej stabilizacji i równowagi cia³a, ale przede wszystkim na kszta³towanie siê zdolnoœci sta³ego dostosowywania siê aktywnoœci miêœniowej do zadañ posturalnych i motorycznych, co okreœla siê mianem „mechanizmu antygrawitacyjnego”. W wyniku prawid³owego rozwoju tego mechanizmu dochodzi do osi¹gania coraz to wy¿szych pozycji, co daje podstawy dla rozwoju prawid³owego schematu cia³a oraz wszystkich umiejêtnoœci funkcjonalnych, takich jak: rozwój artykulacji, koordynacji wzrokowo – ruchowej oraz lokomocji [1,2]. W przypadku uszkodzenia niedojrza³ego mózgu proces ten zostaje zaburzony, a ka¿de dziecko z mózgowym pora¿eniem dzieciêcym (m. p. dz.) pod wzglêdem mo¿liwoœci przeciwstawienia siê sile grawitacji staje siê mniej lub bardziej niewydolne. Manifestuje siê to przede wszystkim w postaci tzw. proksymalnej hipotonii posturalnej, czyli wiotkoœci w osi g³owa – tu³ów oraz dystalnej hipertonii, czyli wzmo¿onego napiêcia miêœniowego w obrêbie obwodowych czêœci koñczyn. Mo¿liwoœæ przyjmowania coraz to wy¿szych pozycji oraz poruszania siê w nich, w pewnym zakresie zapewniaj¹, uruchamiane w takich przypadkach, mechanizmy kompensacyjne [2,3,4]. Dziecko z m. p. dz. nie mog¹c wystarczaj¹co ustabilizowaæ bli¿szych odcinków cia³a (g³owy, tu³owia, obrêczy biodrowej) wyrównuje te niedostatki uruchamiaj¹c samoistnie stabilizacjê zastêpcz¹. Mo¿na wyró¿niæ tu dwa sposoby. Jednym jest tzw. bierna stabilizacja uzyskiwana poprzez „manipulacjê” p³aszczyzn¹ podparcia i rzutem œrodka ciê¿koœci albo poprzez wykorzystanie elementów oko³ostawowych. Drugi sposób stanowi patologiczna stabilizacja dystalna, tzw. „fiksacja” uruchamiana w oparciu o odruchow¹ aktywnoœæ toniczn¹. Jeden i drugi sposób wymusza przymusowe u³o¿enia cia³a [2,3]. Konsekwencje posturalne biernej stabilizacji zastêpczej to przede wszystkim „pozaosiowe” u³o¿enia poszczególnych segmentów cia³a, g³ównie g³owy, tu³owia oraz obrêczy biodrowej, obserwowane w ró¿nych pozycjach cia³a. Wp³yw odruchowej aktywnoœci tonicznej, bêd¹cej podstaw¹ „fiksacji” manifestuje siê natomiast w postaci globalnych synergii miêœniowych [2,3,4]. W trakcie rozwoju dziecka z m. p. dz. kompensacyjne wzorce posturalne pojawiaj¹ siê natychmiast, gdy wyst¹pi¹ pierwsze problemy antygrawitacyjne, tj. od momentu „sygnalizacji o deficycie napiêcia posturalnego, nawet ju¿ w pozycji supinacyjnej. Pocz¹tkowo subtelne nieprawid³owoœci narastaj¹ w coraz to wy¿szych pozycji i osi¹gaj¹ swoje ostateczne kszta³ty w pozycjach wertykalnych, kiedy to zachodzi potrzeba przeciwstawienia siê grawitacji w warunkach coraz to wy¿ej uniesionego œrodka ciê¿koœci oraz przy zmniejszaj¹cej siê stopniowo wielkoœci p³aszczyzny podparcia [4,5,6]. Niestety mo¿liwoœci sterowania rozwojem mechanizmów kompensacyjnych na tym etapie rozwoju motorycznego s¹ ju¿ jednak ograniczone. Wymaga to jednak pe³nego rozeznanie nie tylko w zakresie mo¿liwoœci sterowania kompensacj¹ (œrodki, metody), ale przede wszystkim odnoœnie ewolucji kompensacyjnych wzorców posturalnych (diagnoza, prognoza) oraz okreœlenia kierunku tworzenia siê u danego dziecka mo¿liwie najkorzystniejszych wzorców posturalnych (cele terapii) [7,8]. Celem podjêtych badañ by³o rozpoznanie i zdefiniowanie niektórych kompensacyjnych wzorców posturalnych u dzieci z m. p. dz. w pozycjach wertykalnych. MATERIAL AND METHODS Eighteen children with CP of various stages and established hemiparesis, ranging in age from 5 to 14 years, took part in the study. Children unable to maintain a standing position when unassisted were excluded. All participants had already undergone NDT rehabilitation for at least one year but had not undergone surgery, TbxA or so-called restraining plaster treatment. The investigation relied on objective measurements of postural patterns assumed by the children in a series of unassisted positions – standing with both feet, and one (right and left) foot, two-knee kneeling, one-knee (right and left) kneeling and sitting. Photogrammetric measurements of basic posturometric parameters were conducted in each position. Spatial positions of body segments in all three dimensions were evaluated. The distribution of pressure forces on the support plane and the centre of gravity projection movements in statics were simultaneously measured with a baroresistive platform. A photogrammetric device with dedicated software manufactured by INFOMED was used in the posturometric measurements [9]. The angle of mechanical spinal axis deviation from the anatomical axis, the relation of the plumb line to the gluteal slit (direction and distance) the angles of inclination of the shoulder and pelvic lines against the horizontal line and, additionally, in the transverse plane, the angle of potential pelvis rotation, were determined in each position (Fig. 1). Baropedometric measurements were made with a PDM platform (ZEBRIS), and the FootPrint software [10]. Distributions of body mass projection on the support plane were measured in the left-right and front-back arrangement. The measurements were supplemented by digital pictures of each child's silhouette (Fig. 2). y is for pe rs on al us eo nly -d istr ibu MATERIA³ I METODY Badaniami objêto 18 dzieci z rozwiniêtym niedow³adem po³owiczym, w wieku 5-14 lat, z ró¿nie nasilonymi objawami m. p. dz. Warunkiem kwalifikacji do badañ by³a jednak umiejêtnoœæ samodzielnego utrzymania pozycji stoj¹cej. Wszystkie badane dzieci, co najmniej od roku by³y usprawniane zgodnie z koncepcj¹ NDT. U dzieci tych nigdy nie by³y przeprowadzane ¿adne zabiegi chirurgiczne, nie stosowano te¿ u nich TbxA b¹dŸ tzw. gipsów hamuj¹cych. Zasadniczym elementem przeprowadzonych badañ by³y zobiektywizowane pomiary wzorców postawy tych dzieci podczas samodzielnego utrzymywania kolejnych pozycji – stoj¹cej obunó¿ i jednonó¿ (na prawej i lewej koñczynie dolnej), klêku prostego i jednonó¿ (na prawym i lewym kolanie) oraz pozycji siedz¹cej. W ka¿dej z tych pozycji zosta³y przeprowadzone pomiary fotogrametryczne podstawowych parametrów posturometrycznych. Oceniano przestrzenny uk³ad poszczególnych segmentów cia³a we wszystkich trzech p³aszczyznach. Jednoczeœnie, przy u¿yciu platformy barorezystywnej, oceniano rozk³ad si³ nacisku na p³aszczyŸnie podparcia i zachowanie siê rzutu œrodka ciê¿koœci cia³a w statyce. Do badañ posturometrycznych wykorzystano zestaw fotogrametryczny firmy INFOMED z oprogramowaniem [9]. W kolejnych pozycjach oceniano: k¹t odchylenia mechanicznej osi krêgos³upa od jego osi anatomicznej, przebieg pionu wzglêdem szpary poœladkowej (kierunek i odleg³oœæ), k¹ty nachylenia linii barków i miednicy w stosunku do poziomu oraz dodatkowo – w p³aszczyŸnie poprzecznej – k¹t ewentualnego skrêcenia miednicy. Do badañ baropedometrycznych wykorzystano platformê PDM firmy ZEBRIS wraz z oprogramowaniem FootPrint [10]. Oceniano rozk³ad rzutu masy cia³a na p³aszczyznê podparcia w uk³adzie prawa i lewa strona oraz przód i ty³, a ca³oœæ badania uzupe³niono zdjêciami cyfrowymi sylwetek ka¿dego dziecka. RESULTS The initial review of individual findings demonstrated the presence of lateral curvatures of the spine, ranging from 13° to 42° (x=16.39° + 8.71) and a degree of asymmetry of shoulders and pelvis (x=13.06°, x=12.78°, respectively) in all study participants. S-type curvatures were found in 72% of the patients (including 50% primary thoracic curvatures and 50% primary lumbar curvatures), while C-type deviations were seen in 28% of the subjects. Wartoœci k¹ta skrêcenia miednicy siêga³y tu nawet 40° (x – 18,78°). Najmniejsze nieprawid³owoœci dotyczy³y natomiast p³aszczyzny strza³kowej, gdzie œrednie wartoœci wskaŸnika kompensacji, tj. parametru okreœlaj¹cego przewagê k¹towej wartoœci kifozy piersiowej lub lordozy lêdŸwiowej oscylowa³y w zakresie od 5° do 22°, x – 16,06°. U wszystkich badanych odnotowano tak¿e nierównomierny rozk³ad si³ nacisku stóp na pod³o¿e w swobodnej pozycji stoj¹cej, gdzie ró¿nica wynosi³a u 2 badanych nawet 50%, przy rozk³adzie obci¹¿eñ po stronie prawej i lewej 25% / 75%. Jednak obraz zaobserwowanych tu asymetrii wskazywa³, ¿e u po³owy badanych koñczyn¹ nadmiernie obci¹¿on¹ by³a koñczyna niedow³adna, a u po³owy koñczyna zdrowa. The least abnormalities were seen in the sagittal plane, with mean values of compensation index (a parameter reflecting the dominance, in angular values, of dorsal kyphosis or lumbar lordosis), ranged from 5° to 22°, x= 16.06°. An uneven distribution of pressure forces of the feet on the support base in the habitual standing position was also observed, the difference reaching 50% in two subjects who demonstrated a load distribution of 25% and 75% on the right and left sides, respectively. The overall profile of the asymmetries suggested that the overloaded extremity was the paretic one in half of the subjects, and the healthy one in the other half. Thus, the computed mean values did not show those apparent asymmetries. eo nly -d istr ibu tio np roh ibit ed . It was found that, as regards both the results of photograramometric and baroresistive investigations, it was the direction and nature of asymmetries that were characteristic rather than their values. Two kinds of compensatory postural patterns were observed in the habitual (free) standing position, differing mainly in terms of habitual overloading or release of one of the lower extremities, the direction of the body mass centre projection on the support quadrangle, a characteristic spatial relation of the shoulder and pelvic lines as well as the direction of the lateral curve of spine. Two main compensational postural patterns were distinguished on this basis in hemiparetic children, called antigravitational and progravitational posturing. The shoulder and pelvic lines were more or less parallel in the children who presented with antigravitional posturing, but were situated at an angle in the progravitational type. The two postural types differed also in the direction of spinal curvatures as well as the direction of pelvic rotation. The lateral curve of the spine in both types was directed towards the healthy body side, but in the antigravitational type the healthy side was the overloaded one, whereas in the progravitational type it was the unweighted one. In antigravitational type children, the pelvis tended to rotate backwards, while in progravitational type children frontal rotation to the paretic side was observed. In all subjects with the progravitational postural pattern, the abnormal curvatures were of the S-type and the primary curve was mainly in the dorsal spine. C-type curvatures predominated in antigravitational posturing, and the remaining S-type curves were primary lumbar curvatures. Scoliosis angles in children with progravitational pareses were about 11o higher than in those with antigravitational pareses (x = 22° +9.06, x =10.78°+2.86), and the observed difference was statistically significant (t = 3.48, p< 0.008). The pelvis in children with antigravitational paresis tended to rotate backwards on the paretic side, while in children with the progravitational type a frontal rotation on the paretic side could be seen. The values of the pelvis rotation angle were higher in the antigravitational group (x= 18.12° + 4.32, x = 12.44 +2.45), but did not reach statistical significance. As stated above, the distribution of pressure forces of the feet on the base was uneven in all participants. In antigravitational type children, the pressure was higher from the normal limb, whereas in all subjects representing the progravitational type, the overloaded lower extremity was the paretic one. A detailed analysis of individual profiles of the body mass force distribution on the support plane revealed much bigger load asymmetry between the paretic and intact side in the progravitational paresis group (mean difference of pressures x = 60.22% + 30.5) than in the antigravitational type (mean difference of pressures x = 4.67% + 28.76). It probably resulted from a characteristic distribution of maximal loads on the support plane. W swobodnej pozycji stoj¹cej u badanych zaobserwowano dwojakiego rodzaju kompensacyjne wzorce posturalne, ró¿ni¹ce siê przede wszystkim pod wzglêdem nawykowego przeci¹¿ania lub odci¹¿ania jednej z koñczyn dolnych, kierunku przesuniêcia rzutu ogólnego œrodka ciê¿koœci cia³a na czworoboku podparcia oraz charakterystycznym wzajemnym u³o¿eniem linii barków i miednicy oraz kierunkiem bocznego wygiêcia krêgos³upa. Na tej podstawie wyró¿niono dwa podstawowe kompensacyjne wzorce posturalne u dzieci z niedow³adem po³owiczym: wzorzec posturalny prograwitacyjny oraz antygrawitacyjny, U dzieci, które prezentowa³y antygrawitacyjny typ posturalny wzajemne ustawienie linii barków i miednicy by³o mniej wiêcej równoleg³e, natomiast dla typu prograwitacyjnego charakterystyczne by³o wzajemnie k¹towe ustawienie tych linii. W obu typach posturalnych ró¿ny by³ te¿ kierunek bocznego wygiêcia krêgos³upa oraz kierunek rotacji miednicy. Choæ w obu typach krêgos³up kierowa³ siê sw¹ wypuk³oœci¹ w stronê zdrow¹, to w typie antygrawitacyjnym by³a to strona nadmiernie przeci¹¿ona, a w typie prograwitacyjnym – w stronê przeciwna tj. odci¹¿ona. Miednica z kolei u dzieci o typie antygrawitacyjnym wykazywa³a tendencje do obrotu w ty³ po stronie niedow³adnej, podczas gdy a u dzieci o typie prograwitacyjnym po stronie niedow³adnej obserwowano jej przodoskrêcenie. Choæ w obu typach posturalnych krêgos³up kierowa³ siê w stronê zdrow¹, to w typie antygrawitacyjnym by³a to strona przeci¹¿ona, a w typie prograwitacyjnym – strona odci¹¿ona. U wszystkich badanych z prograwitacyjnym wzorcem posturalnym by³y to dwu³ukowe boczne skrzywienia krêgos³upa, a wygiêcie pierwotne, w przewa¿aj¹cej mierze dotyczy³o odcinka piersiowego. W typie antygrawitacyjnym przewa¿a³y natomiast skrzywienia jedno³ukowe, a pozosta³e dwu³ukowe to skrzywienia pierwotnie lêdŸwiowe. Wartoœci k¹towe skolioz w grupie niedow³adów prograwitacyjnych by³y œrednio o 11° wiêksze ni¿ analogiczne w grupie niedow³adów antygrawitacyjnych (x±SD-22,00° 9,06; x±SD-10,78° 2,86), a zaobserwowana ró¿nica by³a istotna statystycznie (t = 3,48, p< 0,008). W grupie dzieci z niedow³adem antygrawitacyjnym miednica wykazywa³a tendencje do obrotu w ty³ po stronie niedow³adnej, podczas gdy u dzieci o typie prograwitacyjnym po stronie niedow³adnej obserwowano jej przodoskrêcenie. Wartoœci kata skrêcenia miednicy by³y wy¿sze w grupie antygrawitacyjnej (x±SD-18,12° 4,32; x±SD-12,44° 2,45), nie uzyska³o to jednak potwierdzenia statystycznego. Jak ju¿ wspomniano, u wszystkich badanych odnotowano nierównomierny rozk³ad si³ nacisku stóp na pod³o¿e. U dzieci o typie antygrawitacyjnym wiêksze obci¹¿enie dotyczy³o koñczyny zdrowej, podczas gdy u wszystkich badanych zakwalifikowanych do typu prograwitacyjnego przeci¹¿on¹ koñczyn¹ doln¹ by³a niedow³adna. For the progravitational pattern the maximal pressure forces were most often located along the medial edge of the paretic foot. Statistical analysis only confirmed differences in pressure on the intact side (progravitational x = 38.44% + 8.02, antigravitational x = 62.44% + 7.72) between the two postural patterns. Measurements in subsequent positions also showed clear signs of postural asymmetry as well as an uneven distribution of body mass. Individual findings in the sitting position were also characteristic of the two types of postural patterns. Overloading of the paretic side was observed in the children with the progravitational pattern, as in the upright position, while the relation of the shoulders and pelvis changed from angular in the vertical position to parallel (Fig. 1). In the antigravitational paresis group, the posturing pattern did not change in the upright position, but overloading of the opposite side than in the sitting position, i. e. the paretic side, was observed (Fig. 1). Lateral spinal curves deepened to some extent in both groups, but the differences were not statistically significant. Considerable variations in spinal position in the sagittal plane were, however, found in both groups (Fig. 1). Statistical analysis revealed a significant deepening of the dorsal kyphosis in the progravitational paresis group and a decrease of the lumbar lordosis in the antigravitational subgroup. The postural and baroresistive parameters measured in right-angled kneeling in the progravitational group indicated the same tendencies as in the free standing position, i. e. overloading of the paretic side and lateral curving of the spine with the concavity towards the paresis (Fig. 2). The distribution of those parameters in the antigravitational group was, on the other hand, more similar to the results seen in the sitting position, i. e. overloading of the paretic side and a lateral spinal curve with the convexity towards the paretic side (Fig. 3). There were, however, clear differences regarding anteroposterior spinal curves. A considerable deepening of the lumbar lordosis in kneeling in comparison to the sitting position or even to the upright position, was recorded (t=2.57; 2.35, both p< 0,05). Unfortunately, posturometric and baroreceptive measurements in the one-leg positions: one-leg standing and one-leg kneeling were not obtained in all patients. The task was difficult for many children and required assistance, and many children could not maintain the positions for the time required for the measurements. This was mainly the case in children with antigravitational paresis and a fixed equine or equinovarus position of the foot. Those children were unable to maintain the standing position on the paretic lower extremity (44.5%) and the kneeling position on the intact knee (33.0%). Sustaining the one-leg standing position in the remaining patients was associated with changes of the position of the joints in the lower extremity (knee joint hyperextension and hyperflexion of the hip joint) rather than changes in the positions of trunk segments. However, assessment of those parameters would go beyond the methodological framework of the study. Wynika³o to zapewne z charakterystycznego rozk³adu obci¹¿eñ maksymalnych na p³aszczyŸnie podparcia. Dla wzorców antygrawitacyjnych by³o to przodostopie po stronie niedow³adnej oraz piêta po stronie zdrowej. Dla wzorców prograwitacyjnych maksymalne si³y nacisku zlokalizowane by³y najczêœciej na przyœrodkowej krawêdzi stopy niedow³adnej. Analiza statystyczna potwierdzi³a jedynie ró¿nice w zakresie nacisku po stronie zdrowej (typ prograwitacyjny x±SD 38,44% 8,02, typ antygrawitacyjny x±SD 62,44% 7,72) pomiêdzy omawianymi wzorcami posturalnymi. Równie wyraŸne cechy zarówno asymetrii posturalnej, jak i nierównomiernego rozk³adu masy cia³a na p³aszczyznê podparcia zanotowano podczas pomiarów w kolejnych pozycjach. Indywidualne wyniki badañ w pozycji siedz¹cej uk³ada³y siê tak¿e charakterystycznie w obrêbie obu typów wzorców posturalnych. U dzieci z wzorcem prograwitacyjnym zanotowano przeci¹¿anie strony niedow³adnej, tj. tej samej co w pozycji stoj¹cej, uleg³ tu jednak zmianie wzajemny uk³ad linii barków i miednicy, z k¹towego – w pozycji stoj¹cej zmieni³ siê na równoleg³y (Ryc. 1). W grupie niedow³adów antygrawitacyjnych uk³ad posturalny nie zmieni³ siê w pozycji siedz¹cej, zaobserwowano natomiast przeci¹¿enie strony przeciwnej ni¿ w pozycji stoj¹cej, tj. niedow³adnej (Ryc. 1). W obu grupach nieznacznie, w stosunku do pozycji stoj¹cej pog³êbi³y siê wygiêcia boczne krêgos³upa, ró¿nice te nie by³y jednak istotne statystycznie. W obu grupach zanotowano jednak znaczne zmiany w zakresie ustawienia krêgos³upa w p³aszczyŸnie strza³kowej (Ryc. 1). Analiza statystyczna wykaza³a istotne pog³êbienie kifozy piersiowej w podgrupie niedow³adów prograwitacyjnych oraz sp³ycenie lordozy lêdŸwiowej w podgrupie antygrawitacyjnej. Analizuj¹c parametry uzyskane w klêku prostym mo¿na by³o zaobserwowaæ w grupie dzieci niedow³adem prograwitacyjnym, zarówno w obrazie posturalnym, jak i barorezystywnym te same tendencje jak w pozycji swobodnej stoj¹cej, tj. przeci¹¿anie strony niedow³adnej oraz boczne wygiêcie krêgos³upa skierowane wklês³oœci¹ w stronê niedow³adu (Ryc. 2). W grupie dzieci z niedow³adem antygrawitacyjnym rozk³ad tych parametrów by³ z kolei zbli¿ony do uzyskanych w pozycji siedz¹cej, tj. przeci¹¿enie strony niedow³adnej oraz boczne wygiêcie krêgos³upa skierowane wypuk³oœci¹ w stronê niedow³adu (Ryc. 3). Zdecydowane ró¿nice dotyczy³y tu jednak krzywizn przednio – tylnych krêgos³upa. W pozycji klêcznej zanotowano znaczne pog³êbienie lordozy lêdŸwiowej w stosunku do pozycji siedz¹cej, a nawet do pozycji stoj¹cej (t=2,57; 2,35, oba p< 0,05). Niestety nie uda³o siê dokonaæ kompletnych pomiarów posturometrycznych oraz barorezystywnych w pozycjach jednono¿nych – staniu jednonó¿ i klêku jednonó¿. Dla wielu dzieci by³o to trudne i wymaga³y pomocy, dla niektórych pozycje te by³y niemo¿liwe do utrzymania w czasie koniecznym do dokonania pomiaru. Dotyczy³o to g³ównie dzieci z niedow³adem antygrawitacyjnym z utrwalonym koñskim lub koñko-szpotwym ustawieniem stopy. The overall profile of posturometric parameters did not change considerably, but alterations were observed in the position of the supporting lower extremity. They presented as a deepening of the knee and tarsus valgity, but were not part of the study. A characteristic arrangement of particular body segments and body mass pressure force distribution on the support plane seen in the other positions was thus not observed in either one-leg standing or one-leg kneeling. for pe rs on al us oraz klêcznej na zdrowym kolanie (33%). Ocena tych parametrów wykracza³a jednak ponad za³o¿enia metodyczne opracowania. Wœród dzieci z niedow³adem prograwitacyjnym najwiêksze trudnoœci równie¿ dotyczy³y stania na koñczynie niedow³adnej. Choæ uk³ad ocenianych parametrów posturometrycznych nie uleg³ tu wyraŸnej zmianie to zaobserwowano zmiany ustawienia stawów dolnej koñczyny podporowej. Manifestowa³y siê one pog³êbieniem koœlawoœci kolana oraz stêpu, nie by³y jednak przedmiotem prowadzonych badañ. The stabilisation of the body in a particular position requires much more effort and involves additional body segments, e.g. lower or upper extremities, which, in a way, excludes from the very beginning the possibility of normal development of motor functions, mainly reaching, manipulation and locomotion (development set) [2,7,12]. Such abnormalities observed in CP children as scoliosis, positional asymmetry of individual body segments, hyperkyphosis and hyperlordosis should therefore be viewed as postural consequences of muscle tone disorders [4,5,6]. The postures of hemiparetic children may seem identical or very similar at first sight, but closer observation will find some differences. The majority of those children present with asymmetry in the arrangement of individual body segments and plantar hyperflexion, but the range of observed postural and motor disorders is actually far wider. A lateral curvature of the spine and considerable asymmetry of shoulders and pelvis may be the most conspicuous features in some children, accompanied by a significant limitation of upper extremity function, while other children present with an asymmetry of the pelvis position, a fixed equine foot or equinovarus position of the foot and a major asymmetry of gait. Those two patterns are rarely seen in the same child or in other configurations. The above observations were confirmed by the study. On the basis of the findings, two major types of compensatory postural patterns in children with hemiparesis were recognized. In the children with progravitational posturing, postural asymmetry was chiefly manifested as asymmetry of the spine and body segment arrangement. "True" S-type scolioses, with a primary curvature in the thoracic spine and high angular values occurred in that group. Primary convexities towards the "healthy" body side were observed in both groups, but that side was the unweighted one in children with the antigravitational pattern, and the overloaded one in children with the progravitational pattern. An overload of the concavity of the curvature can explain higher angular values of the "progravitational" scolioses. The asymmetrical position of the pelvis and, above all, backward flexion of the pelvis on the paretic side were typical findings in the children with the antigravitational postural pattern. This was not followed by clear postural consequences in the trunk, though. Pronounced abnormalities in knee and ankle position were observed, the analysis of which was, however, beyond the scope of the study. The children with antigravitational posturing demonstrated far greater problems with assuming and maintaining one-leg positions, despite comparatively less severe postural consequences in those subjects. The difficulties were observed primarily in children with a fixed equine or equinovarus position of the foot, which was also demonstrated by observation of gait in those subjects. Sustaining the body mass on the paretic extremity during the support phase of gait does not pose considerable difficulty, but problems with maintaining support in children with antigravitational posturing necessitate a prompt step forwards with the intact extremity. The opposite direction of pelvic rotation during the support phase of the paretic extremity (backwards in the children with antigravitational posturing and forwards in those with the progravitational pattern) differentiates, in turn, their pattern of walking as well. Choæ wydawaæ siê mo¿e na pierwszy rzut oka, ¿e sylwetki dzieci z niedow³adem po³owiczym s¹ jednakowe lub bardzo podobne, to ju¿ dok³adniejsza obserwacja pozwoli zauwa¿yæ pewne ich zró¿nicowanie. Pomimo tego, ¿e wiêkszoœæ tych dzieci prezentuje asymetriê ustawienia poszczególnych segmentów cia³a oraz nadmierne zgiêcie podeszwowe stopy, to gama obserwowanych tu nieprawid³owoœci posturalnych i motorycznych jest znacznie szersza. U jednych dzieci zwraca bowiem uwagê boczne skrzywienie krêgos³upa oraz znaczna asymetria ustawienia barków ³opatek i towarzyszy im znaczne ograniczenie funkcji koñczyny górnej. U innych jest to asymetria ustawienia miednicy, utrwalone koñkostopie lub koñsko-szpotawa pozycja stopy oraz znaczna asymetria chodu. Rzadko natomiast problemy te wystêpuj¹ ³¹cznie lub w innych konfiguracjach. Przeprowadzone badania potwierdzi³y powy¿sze obserwacje. Na podstawie uzyskanych wyników wyró¿niono dwa podstawowe typy kompensacyjnych wzorców posturalnych u dzieci z niedow³adem po³owiczym. U dzieci z prograwitacyjnym wzorcem posturalnym cechy asymetrii posturalnej w g³ównej mierze koncentrowa³y siê wokó³ asymetrii krêgos³upa oraz ustawienia segmentów tu³owia. W tej grupie te¿ wystêpowa³y „prawdziwe” dwu³ukowe skoliozy, z pierwotnym piersiowym ³ukiem skrzywienia, o znacznych wartoœciach k¹towych. Chocia¿ w obu typach posturalnych obserwowano pierwotne skrzywienie krêgos³upa w kierunku „zdrowej” strony cia³a, to dla dzieci ze wzorcem antygrawitacyjnym by³a to strona odci¹¿ona, a dla dzieci z wzorcem prograwitacyjnym przeci¹¿ona. Nadmierne obci¹¿enie wklês³oœci skrzywienia mo¿e t³umaczyæ wiêksze wartoœci k¹towe skolioz „prograwitacyjnych”. Dla dzieci z antygrawitacyjnycm wzorcem posturalnym charakterystyczna by³a z kolei asymetria ustawienia miednicy, a przede wszystkim skrêcenie miednicy do ty³u po stronie niedow³adu. Nie dawa³o to jednak a¿ tak wyraŸnych konsekwencji posturalnych w obrêbie tu³owia i krêgos³upa. Obserwowano natomiast du¿e nieprawid³owoœci ustawienia stawu kolanowego i skokowego. Nie by³o to jednak przedmiotem przeprowadzonych badañ. Pomimo stosunkowo mniejszych konsekwencji posturalnych obserwowanych u dzieci z antygrawitacyjnym typem wzorca postawy, to w³aœnie te dzieci mia³y zdecydowanie wiêksze problemy z przyjêciem i utrzymaniem pozycji jednono¿nych. Dotyczy³o to g³ównie dzieci z utrwalonym koñskim lub koñsko-szpotawym ustawieniem stopy. Znajduje to potwierdzenia w obserwacji tych dzieci w czasie chodu. O ile bowiem utrzymanie ciê¿aru cia³a na koñczynie niedow³adnej w fazie podporu dla dzieci z prograwitacyjnym wzorcem posturalnym nie nastrêcza wiêkszych problemów, to trudnoœci z utrzymaniem podporu u dzieci ze wzorcem antygrawitacyjnym powoduj¹ szybki wykrok koñczyn¹ zdrow¹. As the attainment of completely correct postural and motor patterns is not possible in cerebral palsy, the major goal of the therapy is the development of postural patterns that can be regarded as optimal for the particular child as well as of optimal functional solutions. Poniewa¿ w m. p. dz. nie ma mo¿liwoœci uzyskania w pe³ni prawid³owych wzorców posturalnych ani motorycznych, podstawowym celem leczenia jest tu kszta³towanie optymalnych dla danego dziecka wzorców posturalnych oraz najlepszych rozwi¹zañ funkcjonalnych. Z pewnoœci¹ rozpoznanie kompensacyjnych wzorców posturalnych i lokomocyjnych na wczeœniejszych ni¿ pozycja stoj¹ca etapach rozwoju psychomotorycznego pozwoli na racjonalne sterowanie kompensacj¹ w procesie usprawniania dziecka z m. p. dz. [7,8,13]. CONCLUSIONS 1. Postural patterns serving "supplementary" stabilisation in children with cerebral palsy can be described in terms of basic posturometric parameters, but always in conjunction with baroresistive parameters determining the distribution of pressure forces on the support plane and the behaviour of the projection of the body mass centre. 2. Wzorce posturalne wykorzystywane do „zastêpczej” stabilizacji u dzieci z m. p. dz. mo¿na scharakteryzowaæ za pomoc¹ podstawowych parametrów posturometrycznych, lecz jedynie w po³¹czeniu z parametrami barorezystywnymi okreœlaj¹cymi rozk³ad si³ nacisku na p³aszczyŸnie podparcia oraz zachowania siê rzutu œrodka ciê¿koœci. 2. Na ich podstawie u dzieci z niedow³adem po³owiczym mo¿na wyró¿niæ dwa podstawowe typy kompensacyjnych wzorców posturalnych: • typ antygrawitacyjny, • typ prograwitacyjny. 3. Wczesne rozpoznanie rozwoju okreœlonego typu mechanizmu kompensacyjnego pozwala na racjonalne sterowanie kompensacj¹ w procesie usprawniania w kierunku optymalnych wzorców posturalnych.
