Utratę świadomości można określić jako obniżenie reagowania na bodźce zewnętrzne, skojarzone z niepamięcią; śpiączkę można traktować jako utratę świadomości połączoną z określonymi objawami neurologicznymi; niedrgawkowy stan padaczkowy (NDSP) może być określony jako trwający stan padaczkowy bez większych drgawek. Koncepcja i klasyfikacja. U chorego w stanie śpiączki rozpoznanie NDSP oparte jest jedynie na badaniu EEG. W zaawansowanych stanach śpiączki, EEG wykazuje ciągłą lub okresową patologię. Związek tych padaczkokształtnych wzorów EEG ze śpiączką pozostaje nieokreślony. W oparciu o badania EEG, śpiączkowy NDSP można określić jako uogólniony lub połowiczy. Śpiączka z uogólnionymi padaczkokształtnymi zmianami EEG. Ciężkie encefalopatie (niedotlenieniowe, toksyczne, metaboliczne, zwyrodnieniowe, naczyniowe, w stanach krytycznych) są określone jako NDSP. Również guzy z objawami wgłobienia mogą być traktowane jako NDSP. Opisano wiele uogólnionych padaczkokształtnych wzorów EEG. Śpiączka z połowiczymi padaczkokształtnymi zmianami EEG. W większości zmiany EEG z połowiczymi śpiączkowymi NDSP tworzą okresowe połowicze padaczkokształtne wyładowania (PLED). Różne kształty tego wzoru są omawiane. Wśród innych, uszkodzenie naczyniowe, opryszczkowe zapalenie mózgu, ostre objawowe napady z mnogimi ogniskowymi uszkodzeniami mózgu i rzadko rozlanymi nieprawidłowościami mogą prowadzić do PLED. Rokowanie i Leczenie. Na ogół, rokowanie w śpiączkowym NDSP jest złe, ale dla śpiączki w wyniku zatrucia lekami lub z PLED rokowanie jest lepsze. Śpiączkowy NDSP powinien być leczony w oddziałach intensywnej opieki. Brak jest ogól- Definitions. Unconsciousness can be defined as diminished reactivity to exogenous stimuli associated with amnesia, coma can be regarded as unconsciousness combined with distinct neurological signs, and nonconvulsive status epilepticus (NCSE) may be defined as an enduring epileptic condition interrupted at times by major convulsions. Concepts and Classification. In a comatous patient NCSE represents a pure EEG diagnosis. In advanced coma states, the EEG exhibits continuous or periodic EEG abnormalities. The relation of these epileptiform EEG patterns to coma remains undetermined. On the basis of EEG, comatose NCSE may be classified as generalized or lateralized. Coma with generalized epileptiform EEG activities. Severe encephalopathies (hypoxic, intoxication, metabolic, degenerative, vascular, critically ill) have been termed NCSE. Furthermore, space occupying lesions with signs of herniations might present as NCSE. A number of generalized epileptiform EEG pattern are described. Coma with lateralized epileptiform EEG activities. In the majority, EEG signs with lateralized comatose NCSE constitute periodic lateralized epileptiform discharges (PLEDs). The diverse shapes of this pattern and the relation to seizures and NCSE are discussed. Among others, vascular lesions, Herpes encephalitis, acute symptomatic seizures with several different focal brain injuries and rarely diffuse abnormalities can lead to PLEDs. Prognosis and treatment. In general, the prognosis for comatous NCSE is dismal, for coma due to drug intoxications or with PLEDs prognosis is better. Comatose NCSE has to be treated in an intensive care unit. No general agreement exists about the need and the results of aggressive antiepileptic drug treatment. Praca wpłynęła 8.08.2006 r. Wnioski. Nie ma pilnej potrzeby nazywania śpiączki z towarzyszącą ciągłą lub okresową padaczkokształtną czynnością NSDP. Brak jest uzgodnionej klasyfikacji stanów padaczkowych, którą należy stworzyć. Conclusion. There is no reason to term coma accompanied by continuous or periodic epileptiform discharges as NCSE. There is no agreed classification of status epilepticus and such a classification should be established. In a pragmatic way, coma can be defined as an unarousable psychologic unresponsiveness in which the subject lies with eyes closed (Plum and Posner, 1980). The diagnosis of coma is based on neurological signs of disturbed brain stem functions (in cases of transtentorial herniations or infratentorial lesions) or disturbed brain functions down to the midbrain level (in encephalopathies) (Bauer, 2005). Although unconsciousness represents an essential feature or coma, the terms are not interchangeable. Unconsciousness might be diagnosed in cases where no or only inadequate reaction to external stimuli is combined with abolished remembrance. It has to be distinguished from aphasia, amnesia, severe deafferented states like locked-in syndrome and psychiatric disturbances, among several other conditions. Unconsciousness but not coma can be observed with focal complex and absence seizures and status epilepticus of these seizure types. Furthermore, patients might be unconscious in the postictal phase after a generalized tonic-clonic seizure (GTCS). Coma is observed during GTCS, in grand mal status regardless whether primary or secondary generalized or subtle and in cases of NCSE. The definition of NCSE is not straightforward. The current classifications (Commission on Classification and Terminology 1982; 1989) do not encompass status epilepticus. The proposed diagnostic scheme for people with epileptic seizures and with epilepsy (Engel, 2001) includes a section with “continuous seizure types” but avoids a separation into convulsive and nonconvulsive forms. However, a huge number of publications use these terms. In general, NCSE denotes an enduring epileptic condition with reduced or altered consciousness, behavioral abnormalities or merely subjective symptoms. Major convulsive movements have to be absent (Drislane, 2000). The inclusion of coma states has led to a major conceptional enlargement of the definition of status epilepticus (Kaplan, 2003). CONCEPTIONAL OVERVIEW AND CLASSIFICATION T he epileptic pathophysiology of coma tose states can only be suspected by the observation of accompanying continuous epileptiform EEG patterns.
