STRESZCZENIE Wstęp. W czasie całego życia człowieka tkanka kostna ulega nieustannej przebudowie. W trakcie mineralizacji tkanki kostnej możliwe jest włączenie dodatkowych pierwiastków, także toksycznych, w przebudowywaną kość, co wpływa na zachodzące w niej procesy metaboliczne. Zmiany zwyrodnieniowe oraz czynniki środowiskowe mogą zaburzać te procesy. Materiał i metody. Celem pracy było ustalenie zawartości i korelacji pierwiastków strukturalnych (Ca, Mg, P, Na, K), pierwiastków śladowych (Zn, Cu, Fe, Mo, Cr, Ni), metali potencjalnie toksycznych (Ba, Sr) i metali toksycznych (Pb) w tkance kostnej bliższego końca kości udowej, wykorzystując metodę absorpcyjnej spektrometrii atomowej. Materiał pochodził od 96 chorych z Wielkopolski poddanych zabiegowi endoprotezoplastyki całkowitej stawu biodrowego z powodu zmian zwyrodnieniowych. Dodatkowo określono różnice między stężeniem pierwiastków i ich wzajemnych korelacji w zależności od wieku, płci, miejsca zamieszkania, nałogu palenia tytoniu, spożycia alkoholu, narażenia na zanieczyszczenie środowiska, aktywności fizycznej, rodzaju zmian zwyrodnieniowych, oceny klinicznej oraz obrazu radiologicznego. Dokonano podziału pobieranej kości udowej na głowę kości udowej (kość gąbczastą) oraz szyjkę kości udowej (kość zbitą), który pozwalał precyzyjnie wskazać obszary większej liczby interakcji oraz gromadzenia się metali toksycznych (Pb). Wyniki. Nie stwierdzono istotnych statystycznie różnic stężeń pierwiastków w głowie i szyjce kości udowej w zależności od miejsca zamieszkania, od aktywności fizycznej oraz od rodzaju zmian zwyrodnieniowych. Stężenie Zn, Cu, Ni i Pb w głowie kości udowej było istotnie wyższe u mężczyzn. W szyjce kości udowej wyłącznie u mężczyzn stwierdzono dodatnią korelację Pb z Ca, P, Mg, Na, Zn oraz Sr. Stężenie Ca, P, Mg i Zn w głowie kości udowej było istotnie niższe, a stężenie Pb i Cr wyższe u osób palących. W szyjce kości udowej palaczy Cr silnie dodatnio korelował z Ni (0,74). Stężenie Ni w kości gąbczastej było istotnie wyższe u osób, które miały kontakt z zanieczyszczeniami chemicznymi w środowisku. Korelacje Ca/Pb, P/Pb i Mg/Pb u osób z kontaktem z substancjami chemicznymi w szyjce kości udowej były ujemne (odpowiednio -0,39, -0,43, -0,43), a dodatnie w głowie kości udowej (0,43, 0,43, 0,49). U pacjentów spożywających alkohol stwierdzono znacznie wyższe stężenie Cu, Ni i Pb w porównaniu do abstynentów w głowie kości udowej. U osób spożywających alkohol stwierdzono dodatnie korelacje Pb z Ca, P, Mg, Zn, Na oraz Sr w szyjce kości udowej. Stwierdzono istotne zmniejszenie stężenia Ca, P, Mg z wiekiem w głowie kości udowej. Wykazano także istotnie statystycznie zmniejszenie stężenia Sr z wiekiem wyłącznie w szyjce kości udowej. W szyjce kości udowej stężenie Zn, Mg, P, Ca zmniejszało się z wiekiem tylko u mieszkańców miast. Stężenie Pb nieznacznie zwiększało się z wiekiem. Wykazano ujemną korelację wieku z Ca, P, Mg, Zn, Na, Sr w szyjce kości udowej u osób nieaktywnych fizycznie. W szyjce kości udowej chorych z wtórnymi zwyrodnieniami stwierdzono ujemne korelacje Fe/Ca (-0,31), Fe/P (-0,32) i Fe/Mg (-0,31) oraz Fe/Pb (-0,32), natomiast u chorych ze pierwotnymi zmianami zwyrodnieniowymi stwierdzono korelacje: Pb/Ca (0,32), Pb/P (0,34), Pb/Mg (0,37) i Pb/Zn (0,35). Wnioski. Potwierdzono istotne różnice w metabolizmie metali między kością gąbczastą i korową. Czynnikami, które w największym stopniu wpływają na zawartość i wzajemne interakcje pierwiastków w głowie i szyjce kości udowej są palenie tytoniu (większa zawartość Cr i Ni u palaczy), spożycie alkoholu (wyższe stężenie Ni i Cu u osób spożywających alkohol), a także płeć (wyższe stężenie Cu, Zn, Ni u mężczyzn). Czynnikami sprzyjającymi gromadzeniu się Pb w tkance kostnej są palenie tytoniu, spożywanie alkoholu, płeć męska oraz wiek. Wykazano zmniejszenie stężenia pierwiastków Ca, P, Mg i Sr wraz z wiekiem, do którego częściej dochodzi u mieszkańców miast oraz u osób nieaktywnych fizycznie. W pierwotnych i wtórnych zmianach zwyrodnieniowych stawów biodrowych występuje różne stężenie pierwiastków, wykazano różnice we wzajemnych interakcjach głównie Fe i Pb z innymi metalami. Słowa kluczowe: choroba zwyrodnieniowa stawu biodrowego, tkanka kostna, poziom metali. ABSTRACT Background. During the whole of human life bone tissue undergoes constant remodeling. Mineralization of bone tissue allows additional elements, including toxics metals, to get incorporated in bone remodeling and affects metabolic processes as well. Diseases, such as osteoarthritis, and environmental factors may affect these processes. Methods. The main aim of this study was to determine the content of structural elements (Ca, Mg, P, Na, K), trace elements (Zn, Cu, Fe, Mo, Cr, Ni), potentially toxic metals (Ba, Sr) and a toxic metal (Pb) in bone tissue of the proximal femur of 96 patients undergoing total hip replacement due to osteoarthritis with the use of atomic absorption spectrometry. The other aim was to determine the differences between the concentration of elements and their correlations depending on age, gender, place of residence, tobacco, alcohol consumption, exposure to environmental pollution, physical activity, type of osteoarthritis, clinical evaluation and radiological parameters. The division of the femoral bone into two types: the femoral head (cancellous bone) and the femoral neck (compacted bone) allowed to indicate more accurately the position of a larger number of interactions and the accumulation of toxic metals (Pb). Results. There were no significant differences in the concentrations of elements in the femoral head and neck, depending on the residence, physical activity and the type of degenerative changes. The concentration of Zn, Cu, Ni and Pb in the femoral head was significantly higher in male patients. In the femoral neck in males there was only a positive correlation between Pb and Ca, P, Mg, Na, Sr and Zn. The concentration of Ca, P, Mg and Zn in the head of the femur was significantly decreased, and the concentration of Pb and Cr higher in smokers. In the femoral neck in smokers Cr strongly positively correlated with Ni (0.74). The concentration of Ni in the head of the femur was significantly higher in people who have had contact with chemical contaminants in the environment. Correlations Ca/Pb, P/Pb and Mg/Pb in the femoral neck were negative (respectively -0.39, -0.43, -0.43) in individuals with exposure to chemicals, and positive in the femoral head (0.43, 0.43, 0.49). In patients who consumed alcohol there was a significantly higher concentration of Cu, Ni and Pb in the femoral head in comparison to abstainers. In the femoral neck of drinkers there was Pb positively correlated with other elements: Ca, P, Mg, Zn, Na and Sr. With age there was a significant decrease in the concentration of Ca, P, Mg in the femoral head. In addition, there was noticed a statistically significant decrease in the concentration of Sr with age only in the femoral neck. The concentration of the femoral neck Zn, Mg, P, Ca decreased with age only in urban residents. The concentration of lead increased with age. There was noticed a negative correlation between the age and Ca, P, Mg, Zn, Na and Sr in the femoral neck of physically inactive patients. In the femoral neck of patients with secondary osteoarthritis there were found negative correlations Fe/Ca (-0.31), Fe/P (-0.32) and Fe/Mg (-0.31) and Fe/Pb (-0.32), while in patients with primary osteoarthritis the found correlations were: Pb/Ca (0.32), Pb/P (0.34), Pb/Mg (0.37) and Pb/Zn (0.35). Conclusions. The study confirmed significant differences in the concentration of metals in cancellous and cortical bones. The factors which have the greatest influence on the content and interactions of elements in the femoral head and neck are: tobacco smoking (higher content of Cr and Ni in smokers), alcohol consumption (higher concentrations of Ni and Cu in people who consume alcohol), and sex (higher concentrations of Cu, Zn, Ni in men). The factors contributing to the accumulation of Pb in bone tissue are tobacco, alcohol, sex, and age. It has been noticed that the concentration of elements Ca, P, Mg and Sr decreases with age, which often occurs in urban population and in individuals physically inactive. In primary and secondary osteoarthritis of the hip the content and interactions of elements are different (mainly of Fe and Pb). Keywords: osteoarthritis of the hip, bone tissue, metal content.
